Abstract:Background: Conventional cytogenetics only analyzes metaphase cells and this analysis is affected by chromosome number and quality. A large mass of metaphase or interphase cells can be detected by fluorescence in situ hybridization on chromatosome and bone marrow smears to examine the number and structure of chromatosome. Objective: To test chromosome abnormality by fluorescence in situ hybridization and compare with the results from conventional cytogenetic analysis (CCA) in myelodysplastic syndrome (MDS). Design, Time and Setting: A case analysis was conducted at the Shenzhen Hospital of Peking University from August 2006 to September 2007. Participants: Forty-eight MDS patients, who had been preliminarily diagnosed according to FAB criteria, followed up and received chromatosome examination, were enrolled at the Shenzhen Hospital of Peking University. Simultaneously, five iron deficiency anemia patients were included as controls. Bicolor probe and CEP8 SpectumAqua probe were purchased from Vysis. Methods: Bone marrow samples received CCA by R differential staining technique. Slide was prepared by direct or short-term culture method, at least 20 karyotypes in each sample. Remaining samples received fluorescence in situ hybridization. Using airdrying, fluid was prepared at the ratio of 1:2:7 (probe: water: hybrid buffer solution), dropped in the hybrid region of the slide. Red/green fluorescence hybrid signal of interphase cells were observed under a fluorescent microscope and color filter glass, 200 in each sample. Main Outcome Measures: Abnormal positive rate of bone marrow cells was measured by two techniques. Results: Of 48 MDS patients, 38 survived after 24 months of follow-up. Using CCA, karyotype abnormalities were found in 18 (37.5%) of 48 cases, among which complex karyotypes were found in 4 (8.3%) cases, a deletion of chromosome 5/a deletion of the long arm of chromosome 5 (-5/5q-) in 5 (10.4%) cases, -7/7q- in 5 (10.4%) cases, trisomy 8 in 8 (16.6%) cases, 20q- in 2 (4.6%) cases, unconsistent translocations in 3 (6.2%) cases. Fluorescence in situ hybridization detected 5q- in 2 additional cases, 7q- in 5 additional cases, -7 in 1 additional case, +8 in 12 addition cases in addition to confirming -5/5q-, -7/7q- and +8 found by CCA in 5, 5, 8 cases raising the detection rates of -5/5q-, -7nq- and +8 to 14.5%, 22.9% and 41.7%, respectively. Conclusion: The fluorescence in situ hybridization with panel probes is more sensitive and specific for detection of chromosome 5, 7, 8 abnormalities in MDS patients than that of CCA. But it cannot replace CCA, because it cannot detect the chromosome translocation. This indicates that their combination can increase the positive rate of chromosome abnormality in MDS patients.
