Abstract:It has been suggested that the field of biomedical engineering has been instrumental in creating new therapeutic modalities in rehabilitation of neurological disorders as well as motor dysfunction. Much of this can be attributed to advancements in the scientific research of the cranial nerves. This paper has an objective which is to combine all the efforts including and not limited to the use of electrical stimulation, imaging technique and various cranial imaging techniques in the repair of the cranial nerves. The narrative of this article will look at the synergistic effects of the use of such technology including how it may enhance certain working parameters in the patient through electrical stimulation, autologous nerve and bio-mimic engineered/derived recell scaffolds and Brain-Computer interfaces. Multidisciplinary approaches other than comprehensive examination and analysis of peer reviewed articles, clinical experiments, tests etc. which were relevant to seizures were also used for seaching of the topics of study. Qualitative and Quantitative methods were deployed to evaluate the outcome of applying electrical stimuli, BCIs as well as tissue-engineered scaffolds in the improvement of patient performances. The results consequence of the studies state that for instance electrical stimulation of cranial nerves can improve and restore cognitive abilities and also can be used for treatment of the neurologic disorders that are in existence. Neuromodulation is one of the applications that has some effects of improvement in the brain of the individual and direct treatment for disorders of neurology. There are thus many therapies that are directed for treatment of neurologicTissue engineering methods showed an effective strategy for restoring function in motor cranial nerves; however, the challenges of therapy are due to problems such as misdirection of reinnervation. Brain computer interfaces on the other hand offer a solution for the restoration of lost motor function with significant improvements. It is possible to achieve an increase in the recovery rate by up to 25% using the techniques of motor imagery and neurofeedback. Although the said positive attributes are present in reality, the document credences that more work still needs to be done in the investigation of new technologies, free from the bias on these existing technologies.

Abstract:Self-powered neurostimulation via biomaterials and bioelectronics innovation has emerged as a compelling approach to explore, repair, and modulate neural systems. This review examines the application of self-powered bioelectronics for electrical stimulation of both the central and peripheral nervous systems, as well as isolated neurons. Contemporary research has adeptly harnessed biomechanical and biochemical energy from the human body, through various mechanisms such as triboelectricity, piezoelectricity, magnetoelasticity, and biofuel cells, to power these advanced bioelectronics. Notably, these self-powered bioelectronics hold substantial potential for delivering neural stimulations that are customized for the treatment of neurological diseases, facilitation of neural regeneration, and the development of neuroprosthetics. Looking ahead, we expect that the ongoing advancements in biomaterials and bioelectronics will drive the field of self-powered neurostimulation toward the realization of more advanced, closed-loop therapeutic solutions, paving the way for personalized and adaptable neurostimulators in the coming decades.

Abstract:Abstract Bioelectronic stimulation of the spinal cord has demonstrated significant progress in the restoration of motor function in spinal cord injury (SCI). The proximal, uninjured spinal cord presents a viable target for the recording and generation of control signals to drive targeted stimulation. Signals have been directly recorded from the spinal cord in behaving animals and correlated with limb kinematics. Advances in flexible materials, electrode impedance and signal analysis will allow spinal cord recording (SCR) to be used in next-generation neuroprosthetics. In this review, we summarize the technological advances enabling progress in SCR and describe systematically the clinical challenges facing spinal cord bioelectronic interfaces and potential solutions, from device manufacture, surgical implantation to chronic effects of foreign body reaction and stress–strain mismatches between electrodes and neural tissue. Finally, we establish our vision of bi-directional closed-loop spinal cord bioelectronic bypass interfaces that enable the communication of disrupted sensory signals and restoration of motor function in SCI.

Abstract:Introduction: Nervous system’s cells, particularly neurons, communicate through neurotransmitters and ionic exchanges that generate electrical currents when receiving an external stimulus or when the system itself transmits information through nerve impulses. Considering the principle that this information can be captured, decoded, and used by devices to restore motor and sensory functions, the field of neuroengineering has significantly advanced in recent years. As a multidisciplinary study area, its development requires aligning knowledge of the electrical functioning of the nervous system with engineering and circuits in order to optimize neuroprosthetics to be increasingly efficient, durable, and safe. Objectives: To map the state of the art in neuroengineering and its nuances based on scientific literature. To identify the main developments, challenges, and opportunities in the future of the field. Methods: A systematic review of literature on the combination of engineering and neuroscience in therapeutic applications was conducted. English texts published between 2012 and 2022, that met pre-determined inclusion criteria, were considered/accepted using the following terms for the research: Robotic prosthesis, Neuroengineering, Electrophysiology, Robotic movement, Neural decodification, Nervous system engineering, Neurophysiology, Neural prosthesis, and Neuroanatomy. Results: The review demonstrated that there are established therapeutic approaches based on neuroengineering, such as deep brain stimulation for alleviating Parkinson's symptoms. However, for some neurodegenerative diseases and nervous system injuries, therapeutic-focused neuroprostheses are still in experimental phases or require adjustments to meet user demands and thus achieving greater acceptance and accuracy. Conclusion: Despite the numerous challenges faced in this early stage of the field’s development, advances in research are already observable due to technological developments allowing the implementation of artificial intelligence, more modern microelectrodes, and a better understanding of the system and adaptation between organism and machine.

Abstract:Bidirectional interfacing with the nervous system enables neuroscience research, diagnosis, and therapy. This two-way communication allows us to monitor the state of the brain and its composite networks and cells as well as to influence them to treat disease or repair/restore sensory or motor function. To provide the most stable and effective interface, the tools of the trade must bridge the soft, ion-rich, and evolving nature of neural tissue with the largely rigid, static realm of microelectronics and medical instruments that allow for readout, analysis, and/or control. In this Review, we describe how the understanding of neural signaling and material-tissue interactions has fueled the expansion of the available tool set. New probe architectures and materials, nanoparticles, dyes, and designer genetically encoded proteins push the limits of recording and stimulation lifetime, localization, and specificity, blurring the boundary between living tissue and engineered tools. Understanding these approaches, their modality, and the role of cross-disciplinary development will support new neurotherapies and prostheses and provide neuroscientists and neurologists with unprecedented access to the brain.

Abstract:Limb amputation is a major cause of disability in our community, for which motorised prosthetic devices offer a return to function and independence. With the commercialisation and increasing availability of advanced motorised prosthetic technologies, there is a consumer need and clinical drive for intuitive user control. In this context, rapid additive fabrication/prototyping capacities and biofabrication protocols embrace a highly-personalised medicine doctrine that marries specific patient biology and anatomy to high-end prosthetic design, manufacture and functionality. Commercially-available prosthetic models utilise surface electrodes that are limited by their disconnect between mind and device. As such, alternative strategies of mind–prosthetic interfacing have been explored to purposefully drive the prosthetic limb. This review investigates mind to machine interfacing strategies, with a focus on the biological challenges of long-term harnessing of the user’s cerebral commands to drive actuation/movement in electronic prostheses. It covers the limitations of skin, peripheral nerve and brain interfacing electrodes, and in particular the challenges of minimising the foreign-body response, as well as a new strategy of grafting muscle onto residual peripheral nerves. In conjunction, this review also investigates the applicability of additive tissue engineering at the nerve-electrode boundary, which has led to pioneering work in neural regeneration and bioelectrode development for applications at the neuroprosthetic interface.

Abstract:Technologies capable of establishing intimate, long‐lived optical/electrical interfaces to neural systems will play critical roles in neuroscience research and in the development of nonpharmacological treatments for neurological disorders. The development of high‐density interfaces to 3D populations of neurons across entire tissue systems in living animals, including human subjects, represents a grand challenge for the field, where advanced biocompatible materials and engineered structures for electrodes and light emitters will be essential. This review summarizes recent progress in these directions, with an emphasis on the most promising demonstrated concepts, materials, devices, and systems. The article begins with an overview of electrode materials with enhanced electrical and/or mechanical performance, in forms ranging from planar films, to micro/nanostructured surfaces, to 3D porous frameworks and soft composites. Subsequent sections highlight integration with active materials and components for multiplexed addressing, local amplification, wireless data transmission, and power harvesting, with multimodal operation in soft, shape‐conformal systems. These advances establish the foundations for scalable architectures in optical/electrical neural interfaces of the future, where a blurring of the lines between biotic and abiotic systems will catalyze profound progress in neuroscience research and in human health/well‐being.

Abstract:This review delves into the emerging field of endovascular stent electrodes in the realm of brain-computer interfaces, highlighting advancements in their fabrication methods, signal acquisition performance, electrical stimulation effects, and research progress in electrochemical characteristics, mechanical stability, and biocompatibility. In comparison to traditional invasive procedures, endovascular stent electrodes, implanted through minimally invasive means, mitigate the risks associated with extensive trauma and tissue inflammation, demonstrating commendable signal acquisition capabilities. While exhibiting a slight deficiency in visual evoked potentials, they showcase accuracy in the acquisition and decoding of motor perception signals, rivaling or surpassing traditional surgical methods. Additionally, the technology shows potential therapeutic efficacy in electrical stimulation applications. Nevertheless, challenges such as electrical stimulation thresholds and visibility issues warrant further research and refinement. In summary, endovascular stent electrodes, as an emerging avenue in brain-computer interfaces, hold promise for significant breakthroughs in future research, opening up new possibilities in the field of neuroscience.

Abstract:Abstract Objective . While prosthetic hands with independently actuated digits have become commercially available, state-of-the-art human-machine interfaces (HMI) only permit control over a limited set of grasp patterns, which does not enable amputees to experience sufficient improvement in their daily activities to make an active prosthesis useful. Approach . Here we present a technology platform combining fully-integrated bioelectronics, implantable intrafascicular microelectrodes and deep learning-based artificial intelligence (AI) to facilitate this missing bridge by tapping into the intricate motor control signals of peripheral nerves. The bioelectric neural interface includes an ultra-low-noise neural recording system to sense electroneurography (ENG) signals from microelectrode arrays implanted in the residual nerves, and AI models employing the recurrent neural network (RNN) architecture to decode the subject’s motor intention. Main results . A pilot human study has been carried out on a transradial amputee. We demonstrate that the information channel established by the proposed neural interface is sufficient to provide high accuracy control of a prosthetic hand up to 15 degrees of freedom (DOF). The interface is intuitive as it directly maps complex prosthesis movements to the patient’s true intention. Significance . Our study layouts the foundation towards not only a robust and dexterous control strategy for modern neuroprostheses at a near-natural level approaching that of the able hand, but also an intuitive conduit for connecting human minds and machines through the peripheral neural pathways. Abstract:The development of brain-machine interface technology is a logical next step in the overall direction of neuro-prosthetics. Many of the required technological advances that will be required for clinical translation of brain-machine interfaces are already under development, including a new generation of recording electrodes, the decoding and interpretation of signals underlying intention and planning, actuators for implementation of mental plans in virtual or real contexts, direct somatosensory feedback to the nervous system to refine actions, and training to encourage plasticity in neural circuits. Although pre-clinical studies in nonhuman primates demonstrate high efficacy in a realistic motor task with motor cortical recordings, there are many challenges in the clinical translation of even simple tasks and devices. Foremost among these challenges is the development of biocompatible electrodes capable of long-term, stable recording of brain activity and implantable amplifiers and signal processors that are sufficiently resistant to noise and artifact to faithfully transmit recorded signals to the external environment. Whether there is a suitable market for such new technology depends on its efficacy in restoring and enhancing neural function, its risks of implantation, and its long-term efficacy and usefulness. Now is a critical time in brain-machine interface development because most ongoing studies are science-based and noncommercial, allowing new approaches to be included in commercial schemes under development.

Abstract:In vivo implantation of microelectrodes opens the door to studying neural circuits and restoring damaged neural pathways through direct electrical stimulation and recording. Although some neuroprostheses have achieved clinical success, electrode material properties, inflammatory response, and glial scar formation at the electrode-tissue interfaces affect performance and sustainability. Those challenges can be addressed by improving some of the materials' mechanical, physical, chemical, and electrical properties. This paper reviews materials and designs of current microelectrodes and discusses perspectives to advance neuroprosthetics performance.

Abstract:In the past few decades, driven by the increasing demands in the biomedical field aiming to cure neurological diseases and improve the quality of daily lives of the patients, researchers began to take advantage of the semiconductor technology to develop miniaturized and power-efficient chips for implantable applications. The emergence of the integrated circuits for neural prosthesis improves the treatment process of epilepsy, hearing loss, retinal damage, and other neurological diseases, which brings benefits to many patients. However, considering the safety and accuracy in the neural prosthesis process, there are many research directions. In the process of chip design, designers need to carefully analyze various parameters, and investigate different design techniques. This article presents the advances in neural recording and stimulation integrated circuits, including (1) a brief introduction of the basics of neural prosthesis circuits and the repair process in the bionic neural link, (2) a systematic introduction of the basic architecture and the latest technology of neural recording and stimulation integrated circuits, (3) a summary of the key issues of neural recording and stimulation integrated circuits, and (4) a discussion about the considerations of neural recording and stimulation circuit architecture selection and a discussion of future trends. The overview would help the designers to understand the latest performances in many aspects and to meet the design requirements better.

Abstract:There has been a remarkable convergence in recent years between the biomedical fields of neuroscience and rehabilitation and engineering fields of robotics and prosthetics. This “union” of complementary disciplines has resulted in rapid advances in the robotics and rehabilitation technologies on one hand, and utilization of brain interfaces and brain signals for targeting suitable clinical applications on the other hand. This review will address the slow progress in past, explosive recent developments, and the pending major future challenges to take the science and technology to the patient. I \ review the broad field for neuroprosthesis, from sensory (e.g. tactile) to motor (upper and lower limb) as well as rehabilitation technologies in these fields. I conclude with recent remarkable developments in the field of brain machine interface, how these are impacting the development of neutrally controlled prosthesis and also discuss the challenges of taking the technology to the market and eventually to the patient.
