Abstract:Factor V deficiency is a rare bleeding disorder. Fresh frozen plasma (FFP) is the only source of factor V because factor V concentrates is not available now. We present here a patient had concomitant gallbladder and common bile (CBD) stones with factor V deficiency. The patient is successfully treated by laparoscopic CBD exploration and cholecystectomy with perioperative fresh frozen plasma transfusion. To best of our knowledge, this is the first report of laparoscopic surgery successfully performed in a factor V deficiency patient. Our result suggest that laparoscopic surgery in a factor V deficient patient can be performed safely if normal coagulation profile is achieved after injection of FFP. Our experience in this case also indicate that the incidence of delayed bleeding after surgery is low once hemostasis is successfully obtained during operation and there is no need to continue FFP infusion beyond day 2 postoperative.

Abstract:Factor V deficiency is a rare bleeding disorder. [ 1 ] Congenital factor V deficiency is expressed in an autosomal recessive manner. This disorder is characterized by various bleeding features; however, life-threatening bleeding has rarely been reported. [ 2 ] The first step in diagnosing factor V deficiency is routine coagulation testing. In addition, genomic analysis can be used to confirm the diagnosis. [ 3 , 4 ] Herein, we describe the case of a child with congenital factor V deficiency due to coagulation dysfunction combined with a joint mass which was mistaken for an abscess, resulting in unnecessary antibiotic and surgical therapy. The factor V gene variant c.2439delC (p.I814Lfs*23) in exon 13 was reported for the first time in this case.

Abstract:rare and have been associated with surgery, malignancy, blood transfusion, systemic infection, use of antibiotics, and exposure to bovine thrombin contaminated with bovine factor V. Three previous cases of factor V inhibitor with lupus-like features have been reported in literature.1-3 All of them had either bleeding or thrombotic complications. We report an unusual case with a factor V inhibitor showing features of lupus anticoagulant without any coagulation complications.

Abstract:Acquired factor V deficiency (AFVD) is a rare haemostatic disorder that is primarily because of the development of factor V inhibitors. Approximately, 200 cases have been reported and the greatest portion of these cases was because of bovine thrombin exposure. We report a case of a man who presented with haematuria followed by multiple haemorrhages associated with an elevated prothrombin time and an activated partial thromboplastin time. A workup revealed reduced factor V activity and a factor V inhibitor level of 1.9BU, which were likely secondary to a urinary tract infection. Using corticosteroids, we successfully eliminated the inhibitor and controlled the bleeding. We review the published literature to identify the conditions that are associated with nonbovine thrombin AFVD. A Rare Case of Hemophilia: Acquired Factor VIII Deficiency

Abstract:Unlike hereditary hemophilia, acquired hemophilia is a rare life-threatening bleeding disorder that occurs in a person who has no personal or family history of bleeding episodes. It usually presents with new-onset subcutaneous/joint/muscle bleeding causing ecchymoses and hematomas, hematuria, GI bleeding, retroperitoneal bleeding, or rarely acute intracranial hemorrhage in elderly individuals. The diagnosis involves assessing prolonged activated partial thromboplastin time (aPTT) and conducting mixing studies after ruling out lupus anticoagulants and interfering substances such as heparins. Management consists of controlling hemostasis using recombinant activated factor VII (rFVIIa) and activated prothrombin complex concentrates (aPCC), along with eradicating autoantibodies against factor VIII from the body system using immunosuppressive therapies. Due to the risk of uncontrolled bleeding in individuals who were previously normal, delayed diagnoses and recurrences are not uncommon, potentially resulting in unfavorable outcomes.

Abstract:Factor V (FV) is a glycoprotein that plays a pivotal role in hemostasis, being involved in coagulant and anticoagulant pathways. Congenital FV deficiency is a rare bleeding disorder with an incidence of 1 per million live births, considering the most severe homozygous form. FV deficiency is diagnosed using routine coagulation tests and FV activity assays. Several mutations, including missense, nonsense, and frameshift, have been detected in the F5 gene. Clinical symptoms are variable, ranging from mild ecchymoses and mucosal bleeding to life-threatening intracranial hemorrhage. The mainstay of treatment includes fresh-frozen plasma, preferentially virus-inactivated. In this narrative review, we provide an update of the main laboratory, molecular, clinical, and therapeutic features of inherited FV deficiency. Article published online: 27 August 2024 © 2024. Background Congenital coagulation factor V (FV) deficiency is a very rare hemorrhagic disease with an incidence of approximately one in a million. The common clinical manifestations of FV deficiency include ecchymosis and mucosal bleeding. Life‐threatening intracranial bleeding is rare. It has been reported in several cases. However, the molecular basis has been established in only a few cases. Methods We reported a 2‐month‐old girl with congenital FV deficiency and intracranial hemorrhage. Coagulation screening combined with clinical manifestations was performed to diagnose congenital FV deficiency. Genetic testing was performed to identify the pathogenic genes. A literature review was included to emphasize the clinical manifestation, diagnosis, and treatment for congenital FV deficiency with intracranial bleeding. Results The coagulation tests revealed a significantly prolonged prothrombin time (PT) of 51 s and an activated partial thromboplastin time (APTT) of 73.7 s. The patient had a plasma FV activity of 0.9%. Genetic testing showed compound heterozygous mutations of the patient's FV gene. A literature review showed that patients with homozygous or compound heterozygous variants of the FV gene were often associated with a severe bleeding phenotype. Conclusion Our study provides a direction for the rapid and accurate diagnosis and treatment for FV deficiency to avoid life‐threatening bleeding. Infants with spontaneous cranial hematoma and intracranial hemorrhage should be investigated for underlying hemostatic defects. Congenital coagulation factor deficiency should be considered. Once congenital FV deficiency is diagnosed, fresh frozen plasma (FFP) should be given on a regular basis. Liver transplantation may be performed in severe cases.

Abstract:Background: Factor VII deficiency is a rare inherited bleeding disorder that has similar clinical presentation to hemophilia. Case report: A 7-year-old male child of African origin experienced recurrent nasal bleeding since 3 years of age and recurrent swelling of the joints that was remarkable at the age of 5-6 years. He received multiple blood transfusions and has been managed as a patient with hemophilia until he presented to our facility. Reviewed evaluation of the patient revealed abnormal prothrombin and normal activated partial thromboplastin time, FVII analysis showed activity level of less than 1%, and the diagnosis of FVII deficiency was made. The patient was treated with fresh frozen plasma, vitamin K injection, and tranexamic tablets. Conclusion: Even though factor VII deficiency is an extremely rare bleeding disorder, it does occur in our setting. This case highlights the need for clinicians to consider this condition when faced with challenging patients presenting with bleeding disorders.

Abstract:Background: Factor XIII (FXIII) deficiency is a rare yet profound coagulopathy. FXIII plays a pivotal role in hemostasis, and deficiencies in this factor can precipitate unchecked or spontaneous hemorrhaging. Immunological assays for detecting FXIII inhibitors are indispensable for diagnosing acquired FXIII deficiency; however, the availability of suitable testing facilities is limited, resulting in prolonged turnaround times for these assays. Case summary: In this case study, a 53-year-old male devoid of significant medical history presented with recurrent intracranial hemorrhages and a hematoma in the right hip. Subsequent genetic analysis revealed a homozygous mutation in the ACE gene, confirming the diagnosis of acquired FXIII deficiency. Conclusion: This case underscores the significance of considering acquired deficiencies in clotting factors when evaluating patients with unexplained bleeding episodes.
A Rare Case of Acquired Factor VIII Deficiency in an Elderly Male With a History of Rheumatoid Arthritis

Abstract:Acquired hemophilia A (AHA) or factor VIII (FVIII) deficiency is caused by autoantibodies targeting FVIII in the blood coagulation pathway; it is a rare condition making it challenging to diagnose. A timely diagnosis is crucial, without which there is a risk of catastrophic bleeding. We report a case of a patient with a history of duodenal arteriovenous malformations, previously on apixaban, who presented with four days of melena. On admission he was found to have a hemoglobin of 5.7 and elevated partial thromboplastin time (PTT), promoting further workup showing FVIII levels of <1%, with a mixing study that failed to correct suggesting the presence of inhibitors against FVIII. Other characteristics of this patient's cases included controlled rheumatoid arthritis without detectable rheumatoid factor or increased erythrocyte sedimentation rate (ESR). The patient was initially treated with prednisone and intravenous immunoglobulins, but an insufficient response prompted the initiation of recombinant factor VII, rituximab, and cyclophosphamide during hospitalization.

Abstract:Objective To understand better the clinical features of acquired hemophilia A with venous thrombosis.Methods The etiology,pathogenesis,symptom,diagnosis,and treatment of acquired hemophilia A with venous thrombosis were discussed after retrospectively analysis of the prestentions of the case report and the literature.Results Acquired hemophilia A is a rare bleeding disease caused by autoantibodies directed against clotting factor Ⅷ.This autoimmune disorder most commonly occurs in the elderly.In approximately 50% of cases,it is idiopathic,while the remaining cases may be associated with autoimmune diseases,underlying hematologic or solid cancers.Hemorrhages in patients with acquired hemophilia A involve most frequently soft tissues.The associate prothrombotic risk factors(both congenital,but mainly acquired)are probably responsible for the occurrence of thrombosis in some of acquired hemophilia A patients.The two treatment priorities are to arrest the acute bleeding with substitution therapy and to eradicate the factor Ⅷ autoantibody with immunosuppressive therapy.Anti-coagulation and anti-thrombosis therapy may enhance the bleeding tendency in acquired hemophilia A patients.The report case was an 66-year-old man with gastric cancer.He had a prolonged APTT pre-operation and soft tissue hematoma post-operation.F Ⅷ inhibitor title was 128 Bethesda Unit.He was treated with prednisone,cyclophosphamide,and high-dose intravenous immunoglobulin to eradicate the inhibitor,factor concentrates(human F Ⅷ concentrates and prothrombin complex concentrate)transfusion to arrest the acute bleeding.Transfusion of prothrombin complex concentrate was discontinued after his right femoral and calf muscular venous thrombosis developed.He was treated with combined chemotherapy containing Paclitaxel for three cycles.The signs of hematoma and thrombosis disappeared and coagulation tests returned to normal with inhibitor undetected.Conclusions Acquired hemophilia A with venous thrombosis is a very rare occurrence of hemorrhagic diathesis and veno-occlusive disorder with treatments incompatible.Correct diagnosis and comprehensive and individual therapies are the key to successive treatment.

Abstract:Introduction: Congenital factor V (FV) deficiency is a rare clotting disorder affecting ∼1 in 1,000,000, with bleeding severity that ranges broadly for poorly understood reasons. Aim: To help understand the molecular basis of the observed phenotype in FV deficient patients, the genetics and biochemistry causing a patient's FV deficiency were evaluated. Methods and results: A 71-year-old female, who had serious life-long bleeding upon provocation and profound menorrhagia that lead to hysterectomy, was found to have 3% of normal plasma FV antigen with normal electrophoretic mobility. Platelet FV was similarly low, although the banding pattern was less fragmented than normal. Plasma clotting activity was <1% of normal. Familial inheritance and DNA sequence analysis from peripheral blood leukocytes were consistent with novel compound heterozygosity with missense mutations in exon XVII, Leu1821 to Ser (L1821S) and exon XXV, Gly2192 to Cys (G2192C). The respective single-mutation variants were expressed and purified. Explaining why the antigen level and activity were inequivalent, thrombin activation of recombinant (r) FV/L1821S was impaired, and rFV/G2192C was unable to bind to a procoagulant phospholipid membrane. Conclusion: These findings are consistent with the observed phenotype, highlighting the importance of understanding FV biochemical function to rationalize clinical bleeding severity when the circulating antigen level is discordant.
Congenital Factor VII Deficiency : A Retrospective Analysis of 14 Cases from One Hospital

Abstract:Objective To explore the clinical characteristics, laboratory findings,diagnosis, treatment, and prognosis of congenital factor VII (FVII) deficiency. Methods Clinical data of 14 patients of congenial FVII deficiency diagnosed from Jan 2019 to Jun 2022 were retrospectively analyzed. Results Of the 14 patients,there were 8 males and 6 females with median age 31.5 (1.3~70) years. Family history was found in 2 cases. There were 7 cases with bleeding symptoms, of which were mucocutaneous bleeding (1 case), gingival bleeding (2 cases), menorrhagia (3 cases), hemoptysis (1 case ), post-surgery hemorrhage (1 case), and 1 case with cerebral infarction. Menorrhagia occurred in 3 cases (75% of female patients who were in fertile age). Laboratory findings were characterized by significantly prolonged prothrombin time (PT) (19.2~58.9s) , of which could be corrected by mixed normal plasma, normal partial thromboplastin time (APTT), and decreased FVII activity (FVII:C) (2.4~24.7%). 1 case and his family members received gene mutation analysis and compound Heterozygous mutation (heterozygous deletion mutation and heterozygous missense mutation) was found in the proband. 3 cases were treated with prothrombin complex concentrates (PCC), 1 case with PCC and human recombinant activated FVII (rhFVIIa). 10 patients with no or mild bleeding symptoms did not receive any replacement therapy. Conclusion Most patients with congenital FVII deficiency have mild or no bleeding symptom, but have a tendency to excessive bleeding after surgery or trauma. There is no significant correlation between FVII:C and the severity of bleeding symptoms. Prophylaxis could be applied in patients with severe bleeding symptom. Gene mutation test is significant for diagnosis, prognosis prediction of the disease.
A Single-Center Study of Patients with Isolated Acquired Clotting Factors Deficiency: Clinical Features, Treatments and Prognosis

Abstract:Introduction Isolated acquired clotting factors deficiency (ACFDs) are rare hemorrhagic disorders mainly caused by antibodies directly neutralizing or accelerating the clearance of factors. Their rarity and heterogeneity may lead to difficulty in diagnosis and management. Method Clinical features of patients with isolated ACFDs in the Blood Disease Hospital, Chinese Academy of Medical Sciences were collected from July 1997 to December 2021. Clinical information, laboratory test findings, therapy strategies and outcomes were analyzed retrospectively. Result A total of 219 patients with isolated ACFDs were enrolled in our study, including 132 females and 87 males. The median age of all patients was 50 (4-90) years. The commonest isolated ACFDs was acquired hemophilia A (AHA) affecting 165 (75.3%) patients. Other types (non-AHA) were Factor (F) V deficiency (9.1%), FX deficiency (7.3%), FXII deficiency (2.3%), FXI deficiency (1.8%), FII deficiency (1.8%), FXIII deficiency (0.9%), von Willebrand factor (vWF) deficiency (0.9%), and FIX deficiency (1.5%). The most frequent comorbidities of patients with isolated ACFDs were malignancies (12.3%) and autoimmune diseases (11.0%). Two hundred and nine patients (95.4%) experienced at least one bleeding event, and two patients (one with FIX deficiency and another with FXII deficiency) presented with thrombotic symptoms. All 4 patients with FXII deficiency had no bleeding diathesis. Subcutaneous bleeding was most common with an incidence of 74.0% among all patients. The median bleeding score of all patients was 4 (0-12). Among all patients, 79.5% received replacement treatment and 91.3% underwent immunosuppressive therapy. Of 179 patients with records of survival outcomes, 81.6% achieved complete remission (CR) and 8.9% died. AHA patients achieved a significantly higher CR rate than non-AHA patients (87.6% vs 48.1%, p<0.0001). Conclusion Clinical characteristics are varied across patients with different kinds of isolated ACFDs. AHA patients are more common and have a better prognosis than non-AHA patients. It is crucial for clinicians to pay more attention to recognizing and managing non-AHA patients to avoid morbidity and mortality. Acquired hemophilia A in the setting of dual anticoagulation therapy and lupus anticoagulant: a case report

Abstract:Background: Acquired hemophilia A is a disorder caused by autoantibodies against coagulation factor VIII that may present with severe bleeding. We report a rare case of acquired hemophilia A presenting with coexisting lupus anticoagulant. Case presentation: An 81-year-old Caucasian female presented with large ecchymoses over the torso and extremities in the setting of an enoxaparin bridge to warfarin. Anticoagulation was held, but she continued to develop bruises with significant anemia and prolonged coagulation studies that failed to correct with mixing. Workup revealed factor VIII activity < 1% and a positive lupus anticoagulant. Initial testing for a factor VIII inhibitor was confounded by the presence of lupus anticoagulant, requiring a chromogenic Bethesda assay to confirm the presence of the inhibitor, establishing the diagnosis of acquired hemophilia A. The patient was initially treated with oral prednisone 80 mg daily and factor VIII inhibitor bypassing activity 25 units/kg twice daily before transitioning to susoctocog alfa 50 units/kg twice daily after placement of a tunneled line for outpatient rituximab infusions. On discharge, the patient's ecchymoses were resolving and factor VIII levels improved. Following completion of rituximab therapy, the patient's factor VIII activity normalized and factor VIII inhibitor was suppressed. Conclusions: Diagnosis of acquired hemophilia A can be confounded by other causes of abnormal coagulation studies and may require specialized testing, such as a chromogenic Bethesda assay, to confirm the presence of a factor VIII inhibitor.

Abstract:An elderly woman with light chain myeloma presented with prolonged epistaxis and extensive cutaneous haematomas: her kappa/lambda ratio was high at 395, her coagulation screen, thrombin and reptilase times were abnormal, her FV and FX were in the low range in the absence of specific inhibitors, her Clauss fibrinogen was low at 0.95 g/l but antigenic FNG was 1.58 g/l. The patient denied treatment and died of progressive renal failure. We wish to describe the unusual association of FX and FV deficiency co-existing with an acquired dysfibrinogenaemia.

Abstract:Acquired hemophilia is caused by acquired autoantibodies to 1 of the factors of the coagulation cascade, usually factor VIII or IX, and is an exceedingly rare phenomenon in children. The finding of an acquired factor VIII inhibitor in a pediatric patient with idiopathic multicentric Castleman disease has never been reported. Patients with acquired hemophilia can have life-threatening bleeds that are refractory to blood product support, requiring bypassing agents to manage bleeding symptoms. We present the novel finding of acquired hemophilia resulting from an autoantibody to factor VIII in a pediatric patient with idiopathic multicentric Castleman disease and discuss the optimal management of bleeding in a patient with acquired hemophilia.

Abstract:Objective To analyze the clinical characteristics of hereditary coagulation factor Ⅴdeficiency (FⅤD) and to improve the diagnosis and treatment ability of hereditary FⅤD.Methods A total of 17 patients with hereditary FⅤD admitted to the Department of Hematology , Peking University People's Hospital from February 2013 to January 2018 were selected, and their clinical characteristics , laboratory examination , treatment and prognosis were retrospectively analyzed .Results There were 9 males and 8 females patients with FⅤD, the median age was 36 (1-72 ) years.The median age of men was 39 (1-72) years, and the median age of women was 33 (8-56) years.There was no significant difference between them (P=0.793).The median prothrombin time(PT) and activated partial thromboplastin time (APTT) values were 21.0(13.0-39.6) s and 54.6(38.2-121.2) s, and the median level of plasma FⅤwas 8.2％(0.9％-39％).Thirteen cases (13/17) were mild, 3 cases (3/17) were moderate, and only 1 case ( 1/17 ) was severe according to the FⅤ level.Five cases ( 5/17 ) had bleeding , including 3 cases with skin ecchymosis , 1 case with vaginal bleeding , and 1 case with gastrointestinal bleeding . According to the severity of bleeding, 1 case (1/17) had severe bleeding, and the other 4 cases (4/17) were mild bleeding.Conclusions Patients with hereditary FⅤD either have or do not have bleeding symptoms, with prolonged PT and APTT and decreased plasma of F Ⅴ activity.There is no need for treatment for those without bleeding symptoms .Fresh frozen plasma is the first choice for patients who have bleeding , and the overall prognosis is good .
Unusual presentation of haemophilia in two paediatric patients

Abstract:Haemophilia A is a rare X-linked recessive bleeding disorder caused by deficiency or functional defects in coagulation factor VIII (FVIII). Here, we report two cases of challenging diagnosis of haemophilia A because of unusual presentation. The first case is a 10-month-old female, admitted to our hospital because a neck mass appeared within the previous 24 h, who had a past medical history consistent with recurrent spontaneous haematomas but no family history of bleeding disorders. Despite several radiological evaluations, only the histology of the mass defined the presence of a haematoma. Chromosomal analysis revealed a normal female karyotype and a de-novo mutation into the FVIII intron 22 associated with a skewed X chromosome inactivation. The second case is a male neonate with a history of seizures who underwent brain MRI that showed a suspicious vascular malformation on the quadrigeminal cistern, causing cerebellum compression and hydrocephalus. The clinical conditions of the child progressively worsened and blood tests revealed a severe deficit of FVIII levels. The radiological images were re-evaluated; vascular anomalies were excluded and the diagnosis of haematoma was made. Family history was negative for coagulation disorders. Molecular studies revealed a rearrangement of the FVIII gene involving intron 22. The haemophilia A diagnosis can be challenging. Lack of family history, difficulties in detecting haematomas by imaging techniques, female sex and neonatal age represent misleading factors that can delay the diagnosis.

Abstract:Congenital factor VII (FVII) deficiency, the most frequent among the recessively inherited disorders of blood coagulation, is characterized by a wide range of symptoms, from mild mucosal bleeds to life-threatening intracranial hemorrhage. Complete FVII deficiency may cause perinatal lethality. Clinically relevant thresholds of plasma levels are still uncertain, and modest differences in low FVII levels are associated with large differences in clinical phenotypes. Activated FVII (FVIIa) expresses its physiological protease activity only in a complex with tissue factor (TF), which triggers clotting at a very low concentration. Knowledge of the FVIIa–TF complex helps to interpret the clinical findings associated with low FVII activity as compared with other rare bleeding disorders and permits effective management, including prophylaxis, with recombinant FVIIa, which, however, displays a short half-life. Newly devised substitutive and nonsubstitutive treatments, characterized by extended half-life properties, may further improve the quality of life of patients. Genetic diagnosis has been performed in thousands of patients with FVII deficiency, and among the heterogeneous F7 mutations, mostly missense changes, several recurrent variants show geographical distribution and identity by descent. In the general population, common F7 polymorphisms explain a large proportion of FVII level variance in plasma through FVII-lowering effects. Their combination with pathogenic variants may impact on the frequent detection of FVII coagulant levels lower than normal, as well as on mild bleeding conditions. In the twenties of this century, 70 years after the first report of FVII deficiency, more than 200 studies/reports about FVII/FVII deficiency have been published, with thousands of FVII-deficient patients characterized all over the world.
