Abstract:Background: Subjective cognitive decline (SCD) might occur at the early stages of dementia. Individuals with SCD have an increased risk of subsequent objective cognitive decline and greater rates of progression to dementia. Objective: We aimed to explore the associations between SCD and cerebrospinal fluid (CSF) biomarkers of Alzheimer's disease (AD) pathology in cognitively normal individuals. Methods: A total of 1,099 cognitively normal elders with available data on CSF biomarkers of AD pathology (A beta(42), P-tau, and T-tau) were included in our analysis. Linear regression was used to examine the associations of SCD status and SCD severity with CSF biomarkers. Additionally, a review was conducted to discuss the associations between SCD and CSF biomarkers of AD pathology. Results: After adjustments for covariates, SCD and SCD severity showed significant associations with CSF A beta(42) (SCD: beta = -0.0003, p = 0.0263; SCD severity: beta = -0.0004, p = 0.0046), CSF T-tau/A beta(42) ratio (SCD: beta = 0.1080, p = 0.0064; SCD severity: beta = 0.1129, p = 0.0009) and CSF P-tau/A beta(42) ratio (SCD: beta = 0.0167, p = 0.0103; SCD severity: beta = 0.0193, p = 0.0006) rather than T-tau and P-tau compared with cognitively normal individuals. In the review, a total of 28 studies were finally included after reviewing 174 articles. CSF A beta(42) was lower in SCD than cognitively normal (CN) individuals, but higher than those with objective cognitive decline. However, CSF tau pathology showed no difference between SCD and CN. Conclusion: The results indicated that pathophysiological changes in CSF A beta pathology occurred in individuals with SCD, which provide new insights into early intervention of AD.

Abstract:Recent studies have shown that in the preclinical phase of Alzheimer's disease (AD), subtle cognitive changes can be detected using sensitive neuropsychological measures, and have proposed the concept of objectively-defined subtle cognitive decline (Obj-SCD). We aimed to assess the functional alteration of hippocampal subfields in individuals with Obj-SCD and its association with cognition and pathological biomarkers. Forty-two participants with cognitively normal (CN), 29 with Obj-SCD, and 55 with mild cognitive impairment (MCI) were retrospectively collected from the ADNI database. Neuropsychological performance, functional MRI, and cerebrospinal fluid (CSF) data were obtained. We calculated the seed-based functional connectivity (FC) of hippocampal subfields (cornu ammonis1 [CA1], CA2/3/dentate gyrus [DG], and subiculum) with whole-brain voxels. Additionally, we analyzed the correlation between FC values of significantly altered regions and neuropsychological performance and CSF biomarkers. The Obj-SCD group showed lower FC between left CA1-CA2/3/DG and right thalamus and higher FC between right subiculum and right superior parietal gyrus (SPG) compared with the CN and MCI groups. In the Obj-SCD group, FC values between left CA2/3/DG and right thalamus were positively associated with Auditory Verbal Learning Test (AVLT) recognition (r = 0.395, p = 0.046) and CSF A beta(1-42) levels (r = 0.466, p = 0.019), and FC values between left CA1 and right thalamus were positively correlated with CSF A beta(1-42) levels (r = 0.530, p = 0.006). Taken together, dysfunction in CA1-CA2/3/DG subregions suggests subtle cognitive impairment and AD-specific pathological changes in individuals with Obj-SCD. Additionally, increased subiculum connectivity may indicate early functional compensation for subtle cognitive changes.
Risk factors for subjective cognitive decline: the CABLE study

Abstract:Increasing evidences supported that subjective cognitive decline (SCD) might be a potential first symptomatic manifestation of Alzheimer’s disease (AD). The rapidly growing number of SCD individuals who seek medical help and advice also makes it urgent to develop more precise strategy for SCD. Therefore, this study aimed to explore the risk factors for SCD. Logistics and linear regression models were performed to investigate 41 factors for SCD in 1165 participants without objective cognitive impairment. Cochran-Armitage trend test was used to confirm the constant trend toward higher prevalence of SCD with an increasing number of risk factors. A high overall prevalence of SCD was found in total participants (42%). Eight factors were eventually identified as risk factors for SCD, including four stable factors associated with both SCD statues and severity (older age, thyroid diseases, minimal anxiety symptoms, and day time dysfunction; odds ratio (OR) ranging from 1.74 to 2.29) as well as four suggestive factors associated with either SCD statues or severity (female sex, anemia, lack of physical exercises, and living alone; OR ranging from 1.30 to 2.29). The prevalence of SCD gradually increased with the number of risk factors clustering increased in individuals ( p for trend <0.001). Five of these eight factors were further proved among individuals with SCD-plus features. These findings revealed several risk factors for SCD, providing some new clues for formulating priority strategies for early prevention of SCD.

Abstract:Introduction: The evidence for characteristics of persons with subjective cognitive decline (SCD) associated with amyloid positivity is limited. Methods: In 1640 persons with SCD from 20 Amyloid Biomarker Study cohort, we investigated the associations of SCD-specific characteristics (informant confirmation, domain-specific complaints, concerns, feelings of worse performance) demographics, setting, apolipoprotein E gene (APOE) ε4 carriership, and neuropsychiatric symptoms with amyloid positivity. Results: Between cohorts, amyloid positivity in 70-year-olds varied from 10% to 76%. Only older age, clinical setting, and APOE ε4 carriership showed univariate associations with increased amyloid positivity. After adjusting for these, lower education was also associated with increased amyloid positivity. Only within a research setting, informant-confirmed complaints, memory complaints, attention/concentration complaints, and no depressive symptoms were associated with increased amyloid positivity. Feelings of worse performance were associated with less amyloid positivity at younger ages and more at older ages. Discussion: Next to age, setting, and APOE ε4 carriership, SCD-specific characteristics may facilitate the identification of amyloid-positive individuals.

Abstract:Subjective cognitive decline (SCD) and objective subtle cognitive difficulties (Obj-SCD) are considered the initial stages of aberrant cognition prior to mild cognitive impairment (MCI) due to Alzheimer's disease (AD). We aimed to determine the difference of brain function of SCD and Obj-SCD, furthermore, to figure out which one could be the marker of early AD. One hundred and eighty-five participants were enrolled in this study to determine the amyloid pathology and glucose metabolism changes in SCD and Obj-SCD. The association of amyloid deposition and glucose metabolism with cognitive domains were also investigated. Obj-SCD displayed significantly increased amyloid deposition in frontal and temporal lobes compared to SCD and normal cognitive control (NCC). No difference of amyloid deposition between SCD and NCC, and no difference of glucose metabolism among the three groups were observed. Amyloid deposition was associated with function of memory, language and executive domains, and glucose metabolism was only associated with executive function in Obj-SCD. Amyloid deposition was only associated with executive function in SCD. Obj-SCD could be the early stage of AD, which displayed significant increased amyloid deposition, and the increased amyloid deposition was associated with cognitive function in different domains.
Aberrant functional connectivity network in subjective memory complaint individuals relates to pathological biomarkers

Abstract:Background Individuals with subjective memory complaints (SMC) feature a higher risk of cognitive decline and clinical progression of Alzheimer’s disease (AD). However, the pathological mechanism underlying SMC remains unclear. We aimed to assess the intrinsic connectivity network and its relationship with AD-related pathologies in SMC individuals. Methods We included 44 SMC individuals and 40 normal controls who underwent both resting-state functional MRI and positron emission tomography (PET). Based on graph theory approaches, we detected local and global functional connectivity across the whole brain by using degree centrality (DC) and eigenvector centrality (EC) respectively. Additionally, we analyzed amyloid deposition and tauopathy via florbetapir-PET imaging and cerebrospinal fluid (CSF) data. The voxel-wise two-sample T-test analysis was used to examine between-group differences in the intrinsic functional network and cerebral amyloid deposition. Then, we correlated these network metrics with pathological results. Results The SMC individuals showed higher DC in the bilateral hippocampus (HP) and left fusiform gyrus and lower DC in the inferior parietal region than controls. Across all subjects, the DC of the bilateral HP and left fusiform gyrus was positively associated with total tau and phosphorylated tau 181 . However, no significant between-group difference existed in EC and cerebral amyloid deposition. Conclusion We found impaired local, but not global, intrinsic connectivity networks in SMC individuals. Given the relationships between DC value and tau level, we hypothesized that functional changes in SMC individuals might relate to pathological biomarkers.
Influence of Social Networks on Alzheimer’s Disease Biomarkers and Subjective Cognitive Decline among Elders Without Dementia: A CABLE Study

Abstract:AbstractBackgroundThe social networks has been recommended as a moderator of incident Alzheimer’s disease (AD) and aggravated cognitive deficits. Still few data are available about the effect of social networks on cerebrospinal fluid (CSF) AD biomarkers.MethodParticipants without dementia were recruited from the Chinese Alzheimer’s disease Biomarker and Lifestyle (CABLE) study. Multiple‐adjusted analyses were performed to explore the association of social networks with CSF biomarkers, pre‐AD stage and subjective cognitive decline (SCD).ResultTotal eight hundred and twenty‐two individuals without dementia were included in current study. Moderate to high social network scores were significantly associated with CSF low levels of β‐amyloid (Aβ)40 (moderate, β=‐0.198; close, β=‐0.127), Aβ42/Aβ40 (moderate, β=0.227; close, β=0.184) and phosphorylated tau (p‐tau)/Aβ42 (moderate, β=‐0.106; close, β=‐0.1). In addition, moderate and high scores associated with low p‐tau (β=‐0.076) and total‐tau(t‐tau)/Aβ42 (β=‐0.111) respectively. Mostly significant associations of social network scores with biomarkers persisted in advanced age group, males and SCD group after stratified analyses. Moderate to high social network scores decreased risk for A+ (moderate:odds ratio (OR)=0.519, 95% confidence intervals (CI)=0.302‐0.890; high: OR=0.516, 95%CI=0.305‐0.874), A+T+ (moderate: OR=0.208, 95% CI=0.089‐0.488; high: OR=0.228, 95%CI=0.098‐0.528) and A+T+N+ (moderate: OR=0.144, 95% CI=0.055‐0.378; high: OR=0.141, 95%CI=0.054‐0.370)). In addition, close social ties and high social network scores decreased the risk of SCD (p < 0.05). There were no significant associations of moderate social ties and social network scores with SCD (p > 0.05).ConclusionSocial networks weaken the deleterious effect of AD neurodegenerative biomarkers. Furthermore, favorable influences of social networks on early neurobiological alterations of preclinical stage of AD and self‐reported SCD were suggested.
Changes in Multiparametric Magnetic Resonance Imaging and Plasma Amyloid-Beta Protein in Subjective Cognitive Decline

Abstract:The association between plasma amyloid-beta protein (Aβ) and subjective cognitive decline (SCD) remains controversial. We aimed to explore the correlation between neuroimaging findings, plasma Aβ, and neuropsychological scales using data from 53 SCD patients and 46 age- and sex-matched healthy controls (HCs). Magnetic resonance imaging (MRI) was used to obtain neuroimaging data for a whole-brain voxel-based morphometry analysis and cortical functional network topological features. The SCD group had slightly lower Montreal Cognitive Assessment (MoCA) scores than the HC group. The Aβ42 levels were significantly higher in the SCD group than in the HC group (p < 0.05). The SCD patients demonstrated reduced volumes in the left hippocampus, right rectal gyrus (REC.R), and right precentral gyrus (PreCG.R); an increased percentage fluctuation in the left thalamus (PerAF); and lower average small-world coefficient (aSigma) and average global efficiency (aEg) values. Correlation analyses with Aβ and neuropsychological scales revealed significant positive correlations between the volumes of the HIP.L, REC.R, PreCG.R, and MoCA scores. The HIP.L volume and Aβ42 were negatively correlated, as were the REC.R volume and Aβ42/40. PerAF and aSigma were negatively and positively correlated with the MoCA scores, respectively. The aEg was positively correlated with Aβ42/40. SCD patients may exhibit alterations in plasma biomarkers and multi-parameter MRI that resemble those observed in Alzheimer's disease, offering a theoretical foundation for early clinical intervention in SCD.

Abstract:BackgroundRecently, subjective cognitive decline (SCD) has been described as the earliest at-risk state of Alzheimer’s disease (AD), and drawn attention of investigators. Studies suggested that SCD-community individuals may constitute a more vulnerable population than SCD-clinic patients, therefore, to investigate the early changes of the brain may provide guidance for treatment of the disease. We sought to investigate the changes of structure and functional connectivity alternation of the hippocampus in individuals with SCD recruited from the community using structural and resting-state functional MRI (fMRI).MethodsThirty-five SCD patients and 32 healthy controls were recruited. Resting-state fMRI data and high-resolution T1-weighted images were collected. Whole-brain voxel-based morphometry was used to examine the brain structural changes. We also used the hippocampal tail and the whole hippocampus as seeds to investigate functional connectivity alternation in SCD.ResultsIndividuals with SCD showed significant gray matter volume decreases in the bilateral hippocampal tails and enlargement of the bilateral paracentral lobules. We also found that individuals with SCD showed decreased hippocampal tail resting-state functional connectivity (rsFC) with the right medial prefrontal cortex (mPFC) and the left temporoparietal junction (TPJ), and decreased whole hippocampus rsFC with the bilateral mPFC and TPJ. These brain region and FC showing significant differences also showed significantly correlation with Montreal Cognitive Assessment (MoCA) scores.ConclusionIndividuals with SCD recruited from the community is associated with structural and functional changes of the hippocampus, and these changes may serve as potential biomarkers of SCD.Clinical Trial RegistrationThe Declaration of Helsinki, and the study was registered in http://www.chictr.org.cn. Abstract:Back ground Subjective cognitive decline (SCD) may be the first clinical sign of Alzheimer’s disease (AD). The possible neural mechanisms of SCD are not well known. This study aimed to compare percent amplitude of fluctuation (PerAF) and structural covariance patterns in patients with SCD and healthy controls (HCs). Methods We enrolled 53 patients with SCD and 65 HCs. Resting-state functional magnetic resonance imaging (MRI) data and T1-weighted anatomical brain 3.0-T MRI scans were collected. The PerAF approach was applied to distinguish altered brain functions between the two groups. A whole-brain voxel-based morphometry analysis was performed, and all significant regions were selected as regions of interest (ROIs) for the structural covariance analysis. Statistical analysis was performed using two-sample t -tests, and multiple regressions were applied to examine the relationships between neuroimaging findings and clinical symptoms. Results Functional MRI results revealed significantly increased PerAF including the right hippocampus (HIPP) and right thalamus (THA) in patients with SCD relative to HCs. Gray matter volume (GMV) results demonstrated decreased GMV in the bilateral ventrolateral prefrontal cortex (vlPFC) and right insula in patients with SCD relative to HCs. Taking these three areas including the bilateral vlPFC and right insula as ROIs, differences were observed in the structural covariance of the ROIs with several regions between the two groups. Additionally, significant correlations were observed between neuroimaging findings and clinical symptoms. Conclusion Our study investigated the abnormal PerAF and structural covariance patterns in patients with SCD, which might provide new insights into the pathological mechanisms of SCD.

Abstract:Background Subjective cognitive decline (SCD) is considered as an independent risk factor for objective cognitive impairment, such as dementia and mild cognitive impairment (MCI), but the mechanism is unclear. Methods The current study consisted of two parts, the first of which included 1,010 older adults with SCD and 535 normal controls and was followed for 1 year. The second cross-sectional study included 94 older adults with SCD and 64 healthy controls. Unlike the first cohort, subjects in the second study underwent magnetic resonance imaging and had more detailed neuropsychological tests, such as Mini- mental State Examination (MMSE), Montreal Cognitive Assessment (MoCA), Digit Span, Auditory Verbal Learning Test (AVLT), Associative Learning Test (ALT), Verbal Fluency (VF), Wechsler's filling and Wechsler's building blocks. Results In cohort 1, we found that SCD had a higher risk of objective cognitive impairment compared to normal controls (X2 = 20.354, p = 0.002), and the results of Cox Regression analysis also suggest that SCD was a risk factor for objective cognitive decline (p < 0.001, HR = 2.608, 95%CI: 2.213–3.075). In study 2, we found that the scores of MoCA, digit span, verbal fluency, and Wechsler's filling of SCD elderly were significantly lower than those of normal controls, but the cortical thickness of the rostral middle frontal gyrus (RMFG) was significantly higher than that of normal controls (p < 0.05). Conclusions SCD is a cognition-related disease with multi-cognitive domain impairment, which is associated with a higher risk of objective cognitive impairment. Moreover, the increased cortical thickness of the left rostral middle frontal gyrus (RMFG) might be an important mechanism of cognitive decline in SCD.

Abstract:Introduction: This study tested the self-reported sleep characteristics associated with cerebrospinal fluid (CSF) Alzheimer's disease (AD) biomarkers in cognitively intact older adults. Methods: The linear and non-linear regression analyses were conducted in 736 cognitively normal participants (mean [standard deviation; SD] age, 62.3 [10.5] years, range 40 to 88 years, 59% female) who had measurements of cerebrospinal fluid (CSF) amyloid beta (A beta) and tTau proteins and sleep characteristics, after adjusting for age, gender, education, apolipoprotein E gene (APOE) epsilon 4 status, and general cognition. Results: Greater daytime sleepiness was associated with higher CSF indicators of amyloid deposition in female patients. No significant associations were revealed for CSF tTau proteins after Bonferroni correction. A U-shaped relationship was revealed for nocturnal sleep habits, such that those with insufficient or excessive nocturnal sleep duration had greater CSF biomarkers of amyloid deposition (the reflection range: bedtime: around 10:00 p.m. and sleep duration: 6.0 to 6.5 hours). Discussion: These findings consolidated the close relationship between sleep and AD.

Dyadic SCD patterns, or understanding the self/patient’s in relation to an informant/loved one’s endorsement of SCD, may more accurately reflect underlying Alzheimer’s disease (AD) pathology in particular. We related dyadic discrepancies in SCD to markers of core AD pathology, including amyloid‐β 42 (Aβ 42 ) and phosphorylated tau (p‐tau). Method Vanderbilt Memory and Aging Project participants free of clinical dementia [n = 150, 72±7years, 37% mild cognitive impairment (MCI)] and their informants completed the Everyday Cognition questionnaire. Dyadic discrepancy (self‐reported score minus informant‐reported score) was calculated. Positive scores indicate self>informant SCD and negative scores indicate informant>self SCD. Participants also underwent fasting lumbar puncture to obtain cerebrospinal fluid (CSF) for quantification of Aβ 42 and p‐tau. Linear mixed‐effects regression models related dyadic score to individual CSF biomarkers adjusting for age, sex, self‐reported race/ethnicity, cognitive status, apolipoprotein E‐ ε4 status (positive, negative), and depression symptoms (Geriatric Depression Scale) with false discovery rate (FDR) corrections for multiple comparisons. Models were repeated with a diagnosis x biomarker interaction. Result SCD discrepancy in the negative direction (informant>self‐SCD) was associated with higher CSF p‐tau (β = ‐0.64, pFDRself‐SCD) had lower Aβ 42 levels (β = 5.592, pFDR = 0.02), indicating more cerebral amyloid deposition. The opposite pattern was observed in cognitively unimpaired participants, though findings did not survive FDR correction (β = 1.16, p = 0.03, pFDR = 0.24). Additionally, in MCI participants, negative SCD discrepancy was associated with higher p‐tau levels (β = ‐0.7223, pFDR = 0.02). All significant results survived outlier exclusion. See Figure for details. Conclusion Dyadic SCD discrepancy correlates with AD biomarkers differently in preclinical and prodromal states. In MCI, greater informant‐SCD related to more amyloid and p‐tau deposition, likely due to patient anosognosia and increasing functional changes noted by others. Conversely, in an earlier, preclinical stage before the onset of overt problems, more self‐SCD appears indicative of greater amyloid pathology. Results are consistent with the purported pathological cascade of AD and highlight the relevance of considering dyadic SCD to inform underlying disease.

Abstract:Background: Subjective cognitive decline (SCD) may be the first clinical sign of Alzheimer's disease (AD). SCD individuals with normal cognition may already have significant medial temporal lobe atrophy. However, few studies have been devoted to exploring the alteration of left-right asymmetry with hippocampus and amygdala in SCD. The aim of this study was to compare SCD individuals with amnestic mild cognitive impairment (MCI) patients and the normal population for volume and asymmetry of hippocampus, amygdala and temporal horn, and to assess their relationship with cognitive function in elderly population living in China. Methods: 111 SCD, 30 MCI, and 67 healthy controls (HC) underwent a standard T1-weighted MRI, from which the volumes of the hippocampus and amygdala were calculated and compared. Then we evaluated the pattern and extent of asymmetry in hippocampus and amygdala of these samples. Furthermore, we also investigated the relationship between the altered brain regions and cognitive function. Results: Among the three groups, SCD showed more depressive symptoms (p < 0.001) and higher percentage of heart disease (16.4% vs. 35.1%, p = 0.007) than controls. In terms of brain data, significant differences were found in the volume and asymmetry of both hippocampus and amygdala among the three groups (P < 0.05). In logistic analysis controlled by age, gender, education level, depression symptoms, anxiety symptom, somatic disease and lifestyle in terms of smoking, both SCD and MCI individuals showed significant decreased right hippocampal and amygdala volume than controls. For asymmetry pattern, a ladder-shaped difference of left-larger-than-right asymmetry was found in amygdala with MCI>SCD>HC, and an opposite asymmetry of left-less-than-right pattern was found with HC>SCD>MCI in hippocampus. Furthermore, correlation was shown between the volume of right hippocampus and right amygdala with MMSE and MoCA in SCD group. Conclusion: Our results supported that SCD individuals are biologically distinguishable from HC, and this may relate to cognitive impairment, although more longitudinal studies are need to investigate this further. Moreover, different levels of asymmetry in hippocampus and amygdala might be a potential dividing factor to differentiate clinical diagnosis.

Abstract:INTRODUCTION This study delineated the interrelationships between subclinical alterations in the left heart, cerebrospinal fluid (CSF), Alzheimer's disease (AD) biomarkers, and cognition. METHODS Multiple linear regressions were conducted in 1244 cognitively normal participants (mean age = 65.5; 43% female) who underwent echocardiography (left atrial [LA] and left ventricular [LV] morphologic or functional parameters) and CSF AD biomarkers measurements. Mediating effects of AD pathologies were examined. Differences in cardiac parameters across ATN categories were tested using analysis of variance (ANOVA) and logistic regressions. RESULTS LA or LV enlargement (characterized by increased diameters and volumes) and LV hypertrophy (increased interventricular septal or posterior wall thickness and ventricular mass) were associated with higher CSF phosphorylated (p)‐tau and total (t)‐tau levels, and poorer cognition. Tau pathologies mediated the heart‐cognition relationships. Cardiac parameters were higher in stage 2 and suspected non‐Alzheimer's pathology groups than controls. DISCUSSION These findings suggested close associations of subclinical cardiac changes with tau pathologies and cognition. Highlights Various subclinical alterations in the left heart related to poorer cognition. Subclinical cardiac changes related to tau pathologies in cognitively normal adults. Tau pathologies mediated the heart‐cognition relationships. Subclinical cardiac changes related to the AD continuum, especially to stage 2. The accumulation of cardiac alterations magnified their damage to the brain.

Abstract:IntroductionIt is uncertain whether subjective cognitive decline (SCD) in individuals who seek medical help serves the identification of the initial symptomatic stage 2 of the Alzheimer's disease (AD) continuum.MethodsCross‐sectional and longitudinal data from the multicenter, memory clinic–based DELCODE study.ResultsThe SCD group showed slightly worse cognition as well as more subtle functional and behavioral symptoms than the control group (CO). SCD–A+ cases (39.3% of all SCD) showed greater hippocampal atrophy, lower cognitive and functional performance, and more behavioral symptoms than CO–A+. Amyloid concentration in the CSF had a greater effect on longitudinal cognitive decline in SCD than in the CO group.DiscussionOur data suggests that SCD serves the identification of stage 2 of the AD continuum and that stage 2, operationalized as SCD‐A+, is associated with subtle, but extended impact of AD pathology in terms of neurodegeneration, symptoms and clinical progression.
Tau Pathologies Mediate the Associations of Vascular Risk Burden with Cognitive Impairments in Non-demented Elders: the CABLE Study.

Abstract:BACKGROUND:Studies suggested that vascular dysfunction might increase the risk of developing Alzheimer's disease (AD), but the underlying mechanisms still remain obscure.OBJECTIVE:To evaluate the associations of vascular risk burden with AD core pathologies and investigate the effects of AD core pathologies on relationships between vascular risk burden and cognitive impairments.DESIGN:The Chinese Alzheimer's Biomarker and LifestyLE (CABLE) study was principally focusing on aging, as well as the risk factors and biomarkers of AD initiated in 2017.SETTING:The CABLE study was a large cohort study established in Qingdao, China.PARTICIPANTS:A total of 618 non-demented elders were obtained from CABLE study.MEASUREMENTS:The general vascular risk burden was assessed by the Framingham General Cardiovascular Risk Score (FGCRS). Multivariate linear regression analyses were performed to evaluate the associations of FGCRS with cerebrospinal fluid (CSF) AD biomarkers and cognition. Casual mediation analyses were performed to investigate the mediating effects of AD biomarkers on cognition.RESULTS:Increased FGCRS was related to higher levels of CSF total tau (t-tau, p < 0.001), phosphorylated tau (p-tau, p < 0.001) as well as the ratio of t-tau and amyloid-β 42 (t-tau/Aβ42, p = 0.010), and lower Chinese-Modified Mini-Mental State Examination (CM-MMSE, p = 0.010) score. Stratified analysis indicated that age modified the associations, with FGCRS being significantly associated with tau pathology (p < 0.001 for t-tau and p-tau) in middle-aged group (<65 years old), instead of older group. The influences of FGCRS on cognitive impairments were partially mediated by tau pathologies (a maximum proportion of 20.9%).CONCLUSIONS:Tau pathology might be a pivotal mediator for effects of vascular risk on cognitive decline. Early and comprehensive intervention for vascular risk factors might be a potential approach to delaying or preventing cognitive impairment and AD.

Abstract:Subjective memory decline (SCD) might be the earliest stage of AD, but the association between subjective and subsequent objective function has been heterogeneous. 94 SCD underwent two neuropsychological evaluations (baseline and 12 months follow-up) and a MRI-T1 examination (only baseline). Based on their cognitive performance, we defined the SCD participants as stable (sSCD) and deteriorating subtype (dSCD), separately. We compared subcortical structures (hippocampus and amygdala) and cortical volumes (entorhinal and parahippocampal cortex) between the different cognitive outcome groups. Logistic regression analyses were used to assess the relationship between the significant brain regions (predictors) and dSCD. Furthermore, linear regressions were performed to estimate the associations between the change of cognitive function and the brain morphology at baseline. Of the 94 SCD, 29 participants showed subtle cognitive decline compared with their cognitive test after 1 year (dSCD). Between the two groups, sSCD was younger (68.88±7.28 vs 72.40±5.95, p=0.022) and more percentage of male (30.0% vs 54.7%, p=0.025), but no difference was found in educational level, Geriatric depression scale (GDS) and MoCA in baseline. At follow-up, dSCD showed significant decreased in MoCA (20.13±4.88 vs 25.17±3.07, p＜0.001). Significant lower gray matter volume in bilaterally hippocampus, right amygdala and right parahippocamal gyrus was found in dSCD compared to sSCD. Post hoc ANOVA showed significant reduced right hippocampal and right amygdala was found only in subgroup of age <75 years. Logistic regression showed dSCD was most associated with asymmetry of amygdala (more to left, OR:1.12, 95%CI 1.02-1.23, p=0.02). Linear regression revealed that the delta of MoCA was associated with asymmetry of amygdala (p=0.02), hippocampus(p=0.011) and entorhinal cortex (p=0.001) separately. These associations were independent of all other covariates, including age, gender, education, depression, anxiety and chronic disease.

Abstract:Introduction: Subjective cognitive decline (SCD) in amyloid-positive (Aβ+) individuals was proposed as a clinical indicator of Stage 2 in the Alzheimer's disease (AD) continuum, but this requires further validation across cultures, measures, and recruitment strategies. Methods: Eight hundred twenty-one participants from SILCODE and DELCODE cohorts, including normal controls (NC) and individuals with SCD recruited from the community or from memory clinics, underwent neuropsychological assessments over up to 6 years. Amyloid positivity was derived from positron emission tomography or plasma biomarkers. Global cognitive change was analyzed using linear mixed-effects models. Results: In the combined and stratified cohorts, Aβ+ participants with SCD showed steeper cognitive decline or diminished practice effects compared with NC or Aβ- participants with SCD. These findings were confirmed using different operationalizations of SCD and amyloid positivity, and across different SCD recruitment settings. Discussion: Aβ+ individuals with SCD in German and Chinese populations showed greater global cognitive decline and could be targeted for interventional trials. Highlights: SCD in amyloid-positive (Aβ+) participants predicts a steeper cognitive decline. This finding does not rely on specific SCD or amyloid operationalization. This finding is not specific to SCD patients recruited from memory clinics. This finding is valid in both German and Chinese populations. Aβ+ older adults with SCD could be a target population for interventional trials.

Abstract:Background: Although social networks are deemed as moderators of incident Alzheimer's disease (AD), few data are available on the mechanism relevant to AD pathology. Objective: We aimed to investigate whether social networks affect metabolism of cerebrospinal fluid (CSF) AD biomarkers during early stage and identify modification effects of genetic factor and subjective cognitive decline (SCD). Methods: We studied participants from the Chinese Alzheimer's disease Biomarker and Lifestyle (CABLE) database who received cognition assessments and CSF amyloid-beta (A beta(1-42) and A beta(1-40)) and tau proteins (total-tau [T-tau] and phosphorylated-tau [P-tau]) measurements. The social networks were measured using self-reported questionnaires about social ties. Linear regression models were used. Results: Data were analyzed from 886 cognitively intact individuals aged 61.91 years (SD = 10.51), including 295 preclinical AD participants and 591 healthy controls. Social networks were mostly associated with CSF indicators of AD multi-pathologies (low P-tau/A beta(1-42) and T-tau/A beta(1-42) and high A beta 1-42/A beta(1-40)). Significant differences of genetic and cognitive status were observed for CSF indicators, in which associations of social network scores with CSF P-tau and indicators of multi-pathologies appeared stronger in APOE 4 carriers (versus non-carriers) and participants with SCD (versus controls), respectively. Alternatively, more pronounced associations for CSF T-tau (beta = -0.005, p < 0.001), A beta(1-42)/A beta(1-40) (beta = 0.481, p = 0.001), and T-tau/A beta(1-42) (beta = -0.047, p < 0.001) were noted in preclinical AD stage than controls. Conclusion: These findings consolidated strong links between social networks and AD risks. Social networks as a modifiable lifestyle probably affected metabolisms of multiple AD pathologies, especially among at-risk populations.
