Hereditary Leiomyomatosis and Renal Cell Cancer: A Rare Case in a 22-Year-old Female

Abstract:A 22-year-old Chinese female, who was unmarried and denied sex history, came to our hospital for further evaluation of abdominal masses, which were found during a routine examination two weeks ago. She did not have any symptoms. Both the abdominal ultrasound and enhanced computed tomography showed large masses in the uterus, and the largest one was a cystic-solid mixed mass (18.8 cm x 6.0 cm x 11.4 cm) in the anterior and upper part of the uterine fundus (Fig. 1). Pertinent laboratory values showed a decrease of hemoglobin (99 g/L) and an elevation of carbohydrate antigen 125 (CA125, 49.3 U/mL). She had undergone an open right nephrectomy for suspected multiple renal cysts 9 years ago, and the histologic examination confirmed stage I renal clear cell carcinoma. Nine years postnephrectomy, she reported no worrisome symptoms. The patient did not have any cutaneous lesions, and denied a family history of renal cancers but reported a family history of uterine leiomyoma. Her mother and 2 aunts underwent hysterectomy due to uterine leiomyoma. The key clinical feature, in this case, is making the diagnosis. HLRCC should be suspected based on the patient's age and clinical history. Instead, surgery is required. Because the patient is very young, uterine myomectomy is more suitable. Meanwhile, she was also recommended for early reproduction, owing to the significant risk of recurrence and, therefore, hysterectomy at a young age. The clinical diagnosis of HLRCC of this patient was established based on the clinical manifestation, the histological examination, and the personal and family medical history.
