Abstract:Pandemics and political responses to them can bring about short- and long-term economic and environmental effects. This paper aimed to understand the effects of a pandemic and the policy responses to it on Iran’s economy and environmental conditions using a Bayesian dynamic stochastic general equilibrium model. The results suggested that a pandemic can decrease the value of macroeconomic variables. In the health sector, a pandemic reduces health capital stock, thereby increasing quarantine hours and household health expenditures. Following an increase in quarantine hours and a decrease in economic activities, air pollution decreases, causing a short-term improvement in the quality of the environment. Moreover, an increase in public health expenditure during a pandemic leads to an accelerated convergence of economic and environmental variables into a steady state. Considering that the positive environmental effects of the outbreak of an infectious disease are short-term, and since industrialization is necessary for economic growth, it is recommended that in order to achieve sustainable development, we should move toward energy-efficient industries and clean fuels and technologies.

The unequal effects of the health-economy trade-off during the COVID-19 pandemic

Abstract:Despite the global impact of the coronavirus disease 2019 pandemic, the question of whether mandated interventions have similar economic and public health effects as spontaneous behavioural change remains unresolved. Addressing this question, and understanding differential effects across socioeconomic groups, requires building quantitative and fine-grained mechanistic models. Here we introduce a data-driven, granular, agent-based model that simulates epidemic and economic outcomes across industries, occupations and income levels. We validate the model by reproducing key outcomes of the first wave of coronavirus disease 2019 in the New York metropolitan area. The key mechanism coupling the epidemic and economic modules is the reduction in consumption due to fear of infection. In counterfactual experiments, we show that a similar trade-off between epidemic and economic outcomes exists both when individuals change their behaviour due to fear of infection and when non-pharmaceutical interventions are imposed. Low-income workers, who perform in-person occupations in customer-facing industries, face the strongest trade-off.
Neoliberal economics, planetary health, and the COVID-19 pandemic: a Marxist ecofeminist analysis

Abstract:Planetary health sees neoliberal capitalism as a key mediator of socioecological crises, a position that is echoed in much COVID-19 commentary. In this Personal View, I set out an economic theory that emphasises some of the ways in which neoliberal capitalism's conceptualisation of value has mediated responses to COVID-19. Using the intersection of ecological, feminist, and Marxist economics, I develop an analysis of neoliberal capitalism as a specific historical form of the economy. I identify the accumulation of exchange value as a central tendency of neoliberal capitalism and argue that this tendency creates barriers to the production of other forms of value. I then analyse the implications of this tendency in the context of responses to COVID-19. I argue that resources and labour flow to the production of exchange value, at the expense of production of other value forms. Consequently, the global capitalist economy has unprecedented productive capacity but uses little of this capacity to create the conditions that improve and maintain people's health. To be more resilient to coming crises, academics, policy makers, and activists should do theoretical work that enables global economies to recognise multiple forms of value and political work that embeds these theories in societal institutions.

Abstract:Background: A reconceptualised global strategy is key as nations begin to shift from crisis management to medium- and long-term planning to rebuild and strengthen their economic, social and public health systems. Efforts towards measuring, modelling, and forecasting Mental Wealth could serve as the catalyst for this reconceptualization. The Mental Wealth approach builds systemic resilience through investments which promote collective cognitive and emotional wellbeing. This paper presents the theoretical foundations for Mental Wealth. It presents, for the first time, literature across the disciplines of health and social sciences, economics, business, and humanities to underpin the development of an operational metric of Mental Wealth. Discussion: An approach which embeds social and psychological dimensions of prosperity, alongside the economic, is needed to inform the effective allocation of investments in the post-pandemic world. The authors advocate for a transdisciplinary framework of Mental Wealth to be applied in innovating population-level policy interventions to address the growing challenges brought on by COVID-19. Mental Wealth highlights the value generated by the deployment of collective mental assets and supporting social infrastructure. In order to inform this position, a review of the literature on the concepts underpinning Mental Wealth is presented, limitations of current measurement tools of mental and social resources are evaluated, and a framework for development of a Mental Wealth metric is proposed. Conclusion: There are challenges in developing an operational Mental Wealth metric. The breadth of conceptual foundations to be considered is extensive, and there may be a lack of agreement on the appropriate tools for its measurement. While variability across current measurement approaches in social resources, wellbeing and mental assets contributes to the difficulty creating a holistic and generic metric, these variations are now clearer. The operationalisation of the Mental Wealth metric will require comprehensive mapping of the elements to be included against the data available.
COVID-19 epidemic and mitigation policies: Positive and normative analyses in a neoclassical growth model

Abstract:The COVID-19 pandemic is still ravaging the planet, but its (short-, medium-, and long-term) diverse effects on health, economy, and society are far from being understood. This article investigates the potential impact of a deadly epidemic and its main nonpharmaceutical control interventions (social distancing vs. testing-tracing-isolation, TTI) on capital accumulation and economic development at different time scales. This is done by integrating an epidemiological susceptible-infectious-recovered model with a Solow-type growth model including public expenditure, as a parsimonious setting to offer insights on the trade-off between protecting human lives and the economy and society. The work clarifies (i) the long-term interactions amongst a deadly infection, demography, and capital accumulation, (ii) the lack of viability of persistent social distancing measures also using an analytical characterization, and the threat of policy-enhanced COVID-19 endemicity, (iii) the potentially high return on investments in TTI activities to avoid future lockdowns and related capital disruption. It also quantifies the welfare effects of a range of policies, confirming a counterintuitive role for tax-funded preventive investments aimed at strengthening TTI as more desirable interventions than generalized lockdowns.

Abstract:The COVID-19 pandemic has highlighted the need for priority setting in health financing and resource allocation, spotlighting the limitations of traditional health financing strategies. This commentary explores the relevance of mathematical modeling in enhancing allocative efficiency within the health sector, especially in the aftermath of the pandemic. We draw from the World Bank's experiences in supporting over 20 countries to employ mathematical optimization models for priority setting, aiming to achieve optimal health outcomes within constrained budgets. The pandemic's impact on economic growth, revenue collection, debt stress, and the overall fiscal space available for health financing has necessitated a paradigm shift toward prioritizing efficiency improvements in health service delivery. We outline lessons learned from such modeling and chart future directions to enhance efficiency gains, including for integrated, patient-centered approaches to health service delivery. We advocate for flexible and effective localized priority-setting, leveraging data-driven insights to navigate the complexities of health financing in a post-COVID era.

Abstract:A global crisis such as a pandemic causes a decrease in the global trade of medical supplies. One of the most significant issues healthcare workers and people face is the shortage of personal protective equipment (PPE) items. This study constructs the first international trade model to link infectious disease dynamics and global trade networks, considering the important relationship between government preparedness, domestic manufacturers, and consumers. We examine social welfare measures here in the presence of quantity controls and taxes on the global trade flows. An equilibrium coverage among countries is investigated that integrates net government revenue, purchasing cost, transportation cost, and the health cost caused by the shortage of PPE supply. We develop an optimisation model that balances domestic firms and the global trade network to satisfy the total demand for each traded PPE product. The proportional change in value-added on domestic production is also studied by considering the marginal manufacturing costs of a face mask. The results obtained from testing our model show that the average quantity coverage by the global trade networks among four countries decreased by up to using the proposed trade policy. Hence, a large amount of demand is met by relying on domestic production.

Abstract:As of July, 2020, acute respiratory syndrome caused by coronavirus COVID-19 is spreading over the world and causing severe economic damages. While minimizing human contact is effective in managing the outbreak, it causes severe economic losses. Strategies solving this dilemma by considering interrelation between the spread of the virus and economic activities are in urgent needs for mitigating the health and economic damage. Here we propose an abstract agent-based model for the outbreak of COVID-19 in which economic activities are taken into account. The computational simulation of the model recapitulated the trade-off between health and economic damage associated with lockdown measures. Based on the simulation results, we discuss how macroscopic dynamics of infection and economy emerge from the individuals' behaviours. We believe our model can serve as a platform for discussing solutions to the abovementioned dilemma.

Abstract:During the COVID-19 pandemic of 2019/2020, authorities have used temporary ad-hoc policy measures, such as lockdowns and mass quarantines, to slow its transmission. However, the consequences of widespread use of these unprecedented measures are poorly understood. To contribute to the understanding of the economic and human consequences of such policy measures, we therefore construct a mathematical model of an economy under the impact of a pandemic, select parameter values to represent the global economy under the impact of COVID-19, and perform numerical experiments by simulating a large number of possible policy responses. By varying the starting date of the policy intervention in the simulated scenarios, we find that the most effective policy intervention occurs around the time when the number of active infections is growing at its highest rate -- that is, the results suggest that the most severe measures should only be implemented when the disease is sufficiently spread. The intensity of the intervention, above a certain threshold, does not appear to have a great impact on the outcomes in our simulations, due to the strongly concave relationship that we identify between production shortfall and infection rate reductions. Our experiments further suggest that the intervention should last until after the peak established by the reduced infection rate, which implies that stricter policies should last longer. The model and its implementation, along with the general insights from our policy experiments, may help policymakers design effective emergency policy responses in the face of a serious pandemic, and contribute to our understanding of the relationship between the economic growth and the spread of infectious diseases.

Abstract:The potential tradeoff between health outcomes and economic impact has been a major challenge in the policy making process during the COVID-19 pandemic. Epidemic-economic models designed to address this issue are either too aggregate to consider heterogeneous outcomes across socio-economic groups, or, when sufficiently fine-grained, not well grounded by empirical data. To fill this gap, we introduce a data-driven, granular, agent-based model that simulates epidemic and economic outcomes across industries, occupations, and income levels with geographic realism. The key mechanism coupling the epidemic and economic modules is the reduction in consumption demand due to fear of infection. We calibrate the model to the first wave of COVID-19 in the New York metropolitan area, showing that it reproduces key epidemic and economic statistics, and then examine counterfactual scenarios. We find that: (a) both high fear of infection and strict restrictions similarly harm the economy but reduce infections; (b) low-income workers bear the brunt of both the economic and epidemic harm; (c) closing non-customer-facing industries such as manufacturing and construction only marginally reduces the death toll while considerably increasing unemployment; and (d) delaying the start of protective measures does little to help the economy and worsens epidemic outcomes in all scenarios. We anticipate that our model will help designing effective and equitable non-pharmaceutical interventions that minimize disruptions in the face of a novel pandemic.

The economics of the COVID-19 pandemic: economic evaluation of government mitigation and suppression policies, health system innovations, and models of care

Abstract:Background: The COVID-19 pandemic has impacted the scope of health economics literature, which will increasingly examine value beyond health care interventions such as government policy and broad health system innovations. Aim: The study analyzes economic evaluations and methodologies evaluating government policies suppressing or mitigating transmission and reducing COVID-19, broad health system innovations, and models of care. This can facilitate future economic evaluations and assist government and public health policy decisions during pandemics. Methods: The Preferred Reporting Items for Systematic reviews and Meta-Analyses extension for Scoping Reviews (PRISMA-ScR) was used. Methodological quality was quantified using the scoring criteria in European Journal of Health Economics, Consolidated Health Economic Evaluation Reporting Standards (CHEERS) 2022 Checklist and the National Institute for Health and Care Excellence's (NICE) Cost Benefit Analysis Checklist. PUBMED, Medline, and Google Scholar were searched from 2020-2021. Results: Cost utility analysis (CUA) and cost benefit analysis (CBA) analyzing mortality, morbidity, quality adjusted life year (QALY) gained, national income loss, and value of production effectively evaluate government policies suppressing or mitigating COVID-19 transmission, disease, and impacting national income loss. The WHO's pandemic economic framework facilitates economic evaluations of social and movement restrictions. Social return on investment (SROI) links benefits to health and broader social improvements. Multi-criteria decision analysis (MCDA) can facilitate vaccine prioritization, equitable health access, and technology evaluation. Social welfare function (SWF) can account for social inequalities and population-wide policy impact. It is a generalization of CBA, and operationally, it is equal to an equity-weighted CBA. It can provide governments with a guideline for achieving the optimal distribution of income, which is vital during pandemics. Economic evaluations of broad health system innovations and care models addressing COVID-19 effectively use cost effectiveness analysis (CEA) that utilize decision trees and Monte Carlo models, and CUAs that effectively utilize decision trees and Markov models, respectively. Conclusion: These methodologies are very instructive for governments, in addition to their current use of CBA and the value of a statistical life analytical tool. CUA and CBA effectively evaluate government policies suppressing or mitigating COVID-19 transmission, disease, and impacts on national income loss. CEA and CUA effectively evaluate broad health system innovations and care models addressing COVID-19. The WHO's framework, SROI, MCDA, and SWF can also facilitate government decision-making during pandemics. COVID-Town: An Integrated Economic-Epidemiological Agent-Based Model

Abstract:I develop a novel macroeconomic epidemiological agent-based model to study the impact of the COVID-19 pandemic under varying policy scenarios. Agents differ with regard to their profession, family status and age and interact with other agents at home, work or during leisure activities. The model allows to implement and test actually used or counterfactual policies such as closing schools or the leisure industry explicitly in the model in order to explore their impact on the spread of the virus, and their economic consequences. The model is calibrated with German statistical data on time use, demography, households, firm demography, employment, company profits and wages. I set up a baseline scenario based on the German containment policies and fit the epidemiological parameters of the simulation to the observed German death curve and an estimated infection curve of the first COVID-19 wave. My model suggests that by acting one week later, the death toll of the first wave in Germany would have been 180% higher, whereas it would have been 60% lower, if the policies had been enacted a week earlier. I finally discuss two stylized fiscal policy scenarios: procyclical (zero-deficit) and anticyclical fiscal policy. In the zero-deficit scenario a vicious circle emerges, in which the economic recession spreads from the high-interaction leisure industry to the rest of the economy. Even after eliminating the virus and lifting the restrictions, the economic recovery is incomplete. Anticyclical fiscal policy on the other hand limits the economic losses and allows for a V-shaped recovery, but does not increase the number of deaths. These results suggest that an optimal response to the pandemic aiming at containment or holding out for a vaccine combines early introduction of containment measures to keep the number of infected low with expansionary fiscal policy to keep output in lower risk sectors high.

Behavioral epidemiology: An economic model to evaluate optimal policy in the midst of a pandemic

Abstract:This paper combines a canonical epidemiology model of disease dynamics with government policy of lockdown and testing, and agents' decision to social distance in order to avoid getting infected. The model is calibrated with data on deaths and testing outcomes in the Unites States. It is shown that an intermediate but prolonged lockdown is socially optimal when both mortality and GDP are taken into account. This is because the government wants the economy to keep producing some output and the slack in reducing infection is picked up by social distancing agents. Social distancing best responds to the optimal government policy to keep the effective reproductive number at one and avoid multiple waves through the pandemic. Calibration shows testing to have been effective, but it could have been even more instrumental if it had been aggressively pursued from the beginning of the pandemic. Not having any lockdown or shutting down social distancing would have had extreme consequences. Greater centralized control on social activities would have mitigated further the spread of the pandemic.

Abstract:The quality and access to healthcare systems depend on community health resources, infrastructure, and funding; however, a significant disparity in these resources persists globally. The effectiveness of national health systems depends on a balanced approach to health spending, access to facilities and a skilled local health workforce. What accounts for country‐level differences in those critical community and societal health resources? We proposed and tested a model that leverages political and socioeconomic factors to predict various health resources and services in countries. Data, including community health training, research, and support, universal health coverage, healthcare infrastructure, and per capita health expenditure, were collected and analysed by statistical methods, like bivariate correlations and hierarchical multiple linear regressions from 105 countries. Countries with more grassroots activism, fiscal decentralisation, freedom, and globalisation and less perceived corruption and inequality had more community and societal health resources. In multivariate analyses, stronger community health training and research is associated with the globalisation index, freedom score, government fiscal decentralisation, and income inequality. The strongest predictor of health insurance coverage and hospital beds was the country's population education index, and of nurses and midwives‐per‐capita and health expenditures‐per‐capita was GDP‐per‐capita. These insights could guide policymaking to reduce global health inequalities.

Abstract:With coronavirus disease 2019 (COVID-19) receding, many countries are pondering what a post-pandemic economy should look like. Some advocate a more inclusive stakeholder model of capitalism. Others caution that this would be insufficient to deal with our pre-pandemic crises of income inequality and climate change. Many countries emphasize a 'green recovery' with improved funding for health and social protection. Progressive tax reform and fiscal policy innovations are needed, but there is concern that the world is already tilting towards a new round of austerity. Fundamentally, the capitalist growth economy rests on levels of material consumption that are unsustainable and inequitable. More radical proposals thus urge 'degrowth' policies to reduce consumption levels while redistributing wealth and income to allow the poorer half of humanity to achieve an ethical life expectancy. We have the policy tools to do so. We need an activist public health movement to ensure there is sufficient political will to adopt them.

Abstract:Abstract The COVID-19 pandemic severely disrupted financial markets and the real economy worldwide. These extraordinary events prompted large monetary and fiscal policy interventions. Recognizing the unusual nature of the shock, the academic community has produced an impressive amount of research during the last year. Macro-finance models have been extended to analyze the impact of epidemics. Empirical papers study the origins and consequences of the disruptions and the impact of policy interventions. New research evaluates the ongoing financial fragility and its relation to previous episodes and regulations. This special issue contains early contributions to this important and rapidly developing literature.1

Abstract:The COVID-19 pandemic has caused a massive economic shock across the world due to business interruptions and shutdowns from social-distancing measures. To evaluate the socio-economic impact of COVID-19 on individuals, a micro-economic model is developed to estimate the direct impact of distancing on household income, savings, consumption, and poverty. The model assumes two periods: a crisis period during which some individuals experience a drop in income and can use their savings to maintain consumption; and a recovery period, when households save to replenish their depleted savings to pre-crisis level. The San Francisco Bay Area is used as a case study, and the impacts of a lockdown are quantified, accounting for the effects of unemployment insurance (UI) and the CARES Act federal stimulus. Assuming a shelter-in-place period of three months, the poverty rate would temporarily increase from 17.1% to 25.9% in the Bay Area in the absence of social protection, and the lowest income earners would suffer the most in relative terms. If fully implemented, the combination of UI and CARES could keep the increase in poverty close to zero, and reduce the average recovery time, for individuals who suffer an income loss, from 11.8 to 6.7 months. However, the severity of the economic impact is spatially heterogeneous, and certain communities are more affected than the average and could take more than a year to recover. Overall, this model is a first step in quantifying the household-level impacts of COVID-19 at a regional scale. This study can be extended to explore the impact of indirect macroeconomic effects, the role of uncertainty in households’ decision-making and the potential effect of simultaneous exogenous shocks (e.g., natural disasters).

Abstract:This paper introduces the Susceptible-Infectious-Recovered model into the Bewley-type incomplete market model and uses it to study the impact of the coronavirus disease (COVID-19) pandemic on China's macroeconomics. The calibrated model predicts that the average propensity to consume household wealth will decline, while the demand for money will increase, and these predictions are consistent with the data. Monetary policy is effective because it provides enough liquidity for households to buffer health risks. Monetary stimulus is more effective in an economy with greater health risks and consumption uncertainty. Counterfactual experiments show that abandoning the containment policy too early would avoid a sharp drop in output and employment in the short term, but it would greatly increase mortality and ultimately lead to a decline in social welfare.

In this vein, the motivation behind the present study was to get an understanding of the effect of the rise of the health disaster risk on the dynamics of Iran's macroeconomic variables by using Bayesian Dynamic Stochastic General Equilibrium. As opposed to Computable General Equilibrium models, DSGE models can be evaluated in a stochastic environment. Since the duration of the virus outbreak and its effect on the economy is not known, it is more appropriate to use these models. The results demonstrated that increased health disaster risk has a remarkable negative effect on macroeconomic variables. According to the findings of the research and the significance of public vaccination as an essential solution for improving health status and quality of life, it was suggested that the government pave the path for the thriving of businesses and socio-economic activities as early as possible by employing specific policies such as tax exemption or budget allocation for vaccine manufacturing companies or importers.

Abstract:The current COVID-19 pandemic and subsequent lockdowns have highlighted the close and delicate relationship between a country's public health and economic health. Macroeconomic models that use preexisting epidemic models to calculate the impacts of a disease outbreak are therefore extremely useful for policymakers seeking to evaluate the best course of action in such a crisis. We develop an SIR model of the COVID-19 pandemic that explicitly considers herd immunity, behavior-dependent transmission rates, remote workers, and indirect externalities of lockdown. This model is presented as an exit time control problem where lockdown ends when the population achieves herd immunity, either naturally or via a vaccine. A social planner prescribes separate levels of lockdown for two separate sections of the adult population: low-risk (ages 20-64) and high-risk (ages 65 and over). These levels are determined via optimization of an objective function which assigns a macroeconomic cost to the level of lockdown and the number of deaths. We find that, by ending lockdowns once herd immunity is reached, high-risk individuals are able to leave lockdown significantly before the arrival of a vaccine without causing large increases in mortality. Moreover, if we incorporate a behavior-dependent transmission rate which represents increased personal caution in response to increased infection levels, both output loss and total mortality are lowered. Lockdown efficacy is further increased when there is less interaction between low- and high-risk individuals, and increased remote work decreases output losses. Overall, our model predicts that a lockdown which ends at the arrival of herd immunity, combined with individual actions to slow virus transmission, can reduce total mortality to one-third of the no-lockdown level, while allowing high-risk individuals to leave lockdown well before vaccine arrival.
