Abstract:OBJECTIVE: To review the advances in functional reconstruction and rehabilitation of brachial plexus injuries on the basis of the latest research of treatment and rehabilitation for the brachial plexus injuries in recent years at home and abroad. DATA SOURCES: The literatures of reconstruction and rehabilitation of brachial plexus injuries were collected from the Medline database published from January 1990 to July 2005 with the Keywords of "brachial plexus injury,reconstruction and rehabilitation" in English. At the same time,the literatures of reconstruction and rehabilitation of brachial plexus injuries were collected from the CBM database with the key words of "brachial plexus injury,functional reconstruction and rehabilitation" in Chinese published from January 1998 to July 2005. STUDY SELECTION: The data were checked primarily,and the literatures of functional reconstruction and rehabilitation of brachial plexus injuries were selected and the full-texts were retrieved. Inclusive criteria: ①functional reconstruction of brachial plexus injuries,and ②rehabilitation of brachial plexus injuries. Exclusive criteria: ①the tumor and obstetrical brachial plexus palsy,and ②reviews,repeated study and Meta analytical papers. DATA EXTRACTION: The literatures of functional reconstruction of brachial plexus injuries were 46 papers,and 27 papers accorded with criteria were included. Nine articles were about the method and material of salivary gland tissue engineering. DATA SYNTHESIS: Brachial plexus injury could induce the complete or partial palsy of upper extremity muscle. In the treatment some relatively unimportant nerves were moved to important nervous distal end,which was called nerve aversion or nerve transfusion. It was an important therapeutic method of brachial plexus injury. Post-operative comprehensive rehabilitative treatment included sensation training and motor function training. Detailed method of rehabilitation contained ①facilitation of nerve regeneration,②prevention and treatment of muscle atrophy and articular capsule contraction,and ③conscious training of active contraction. To keep the active training was very important. Function of peripheral nerve was closely associated with activity of cerebral cortex. Active activity of cerebral cortex was the important factor of functional restoration of peripheral nerve except recovering succession of peripheral nerve and comprehensive treatment by operation and acceleration of nerve regeneration. CONCLUSION: The therapeutic efficacy depends on the precise pre-operative diagnosis,the interval between the injury and the operation time,choice of aversion dynamic nerve and post-operative active suitable functional training in an early stage. Post-operative conscious active functional training is benefit to prevent muscular atrophy,form cerebral plasticity,produce automatic action,re-educate the re-dominant muscle,and significantly elevate post-operative curative effect.
