Abstract:BACKGROUND AND IMPORTANCE: Cervical foraminal schwannomas commonly originate from spinal nerves that pass through the intervertebral foramen of the cervical vertebrae. Because of the proximity of this type of tumor to the vertebral artery and spinal nerves, surgical management remains a major challenge. Conventional open spine surgery usually requires the removal of the articular process and is supplemented by a simultaneous posterolateral spine fusion surgery. To decrease the associated risks of surgical complications by further reducing invasiveness, percutaneous spinal endoscopy may be used for resection of foraminal spinal neoplasm. C LINICAL PRESENTATION: A 52-yr-old female who presented with neck pains with duration of 1 yr was admitted to our hospital. Physical examination revealed moderate rigidity in the neck and grade 5 muscle strength in both upper and lower limbs. Preoperative magnetic resonance imaging (MRI) scans demonstrated a left-sided lesion at the C3-C4 intervertebral foraminal area. Under C-arm fluoroscopy navigation and neuromonitoring, the endoscope was properly positioned on the same side of the tumor, and a small part of the left C3 inferior and C4 superior lamina were first removed by an endoscopic drill to enlarge the interlaminar space. Next, through an endoscopic working canal, the left intervertebral ligamentum flavum was removed to fully expose the tumor. The tumor mass was finally resected in a piecemeal approach. Postoperative MRI confirmed complete tumor resection. CONCLUSION: This is the first case report of a total removal of a cervical foraminal schwannoma with a percutaneous spinal endoscopic procedure.

Abstract:Objective: To present our classification for peripheral nerve schwannomas as well as explore the surgical strategies and operative management of peripheral nerve schwannomas based on the intraoperative neurophysiological monitoring (INM) technique and to decrease the risk of postoperative neurological deficits in the management of these schwannomas. Materials and methods: A retrospective study was conducted on 92 cases of peripheral nerve microsurgery performed, using the INM technique. We also made the classification for peripheral nerve schwannomas into two types according to operative findings and proceeded corresponding surgical strategies. Results: All tumors were removed completely under microscopy and INM. Three patients developed residual neurological deficits at final follow-up. There were different results about temporary (18/92, 19.6%) and permanent (3/92, 3.3%) neurological deficits. The incidence of temporary and permanent neurological deficits in type II group was significantly higher than that in type I group (p < 0.01). The incidence of permanent neurological deficits in larger size tumors was significantly higher than that of smaller size (p < 0.01). Conclusions: We made the classification for peripheral nerve schwannomas according to operative findings based on INM that is helpful to our surgical strategies. Intracapsular enucleation was the preferred strategy with satisfactory results and low risk of nerve injury. The size and location of tumors seem to be related to the risk of fascicular injury. Flexible Endoscope Visualization to Assist in the Removal of a String of 10 Schwannomas at the Cauda Equina: Technical Case Report.

Abstract:Bead-like schwannomas at the cauda equina are rare but benign intraspinal tumors. They can involve multiple nerve roots and spread within the spinal canal, and open resection would cause significant trauma. The authors have successfully applied a novel minimally invasive technique for the total removal of such schwannomas. A 68-year-old woman presented with a 1-month history of left waist and leg pain. MRI demonstrated multiple intraspinal lesions located from L1 to S1. The diagnosis was bead-like schwannomas at the cauda equina. Two incisions were made at the T12 and L5 levels. A flexible endoscope was introduced into the spinal canal following hemisemilaminectomy under a microscope to identify the relationship between the tumors and the carrying nerves. After dissecting both cranial and caudal ends of the carrying nerve, the string of bead-like tumors was gently pulled out from the caudal end as a whole. The endoscope was reintroduced into the spinal canal to ensure complete tumor removal. The patient recovered quickly, and no tumor residual was found at postoperative MRI. Flexible endoscope-assisted visualization plus microscopic hemisemilaminectomy via 2 incisions is a feasible minimally invasive approach for selected patients with bead-like schwannomas at the cauda equina.

Abstract:BACKGROUND:The incidence of spinal epidural hematoma (SEH) is estimated to be 1 per 1,000,000 patients per year; SEH can be classified as idiopathic, spontaneous, and secondary. The cause of spontaneous SEH is uncertain but it may be associated with minor trauma. SEH can compress surrounding structures, shown by clinical symptoms and signs that affect the spinal cord or nerve roots. Surgical treatment may be considered if medical treatment fails. CASE DESCRIPTION:A 26-year-old man presented with lower back pain and significant radicular symptoms on the left side for a week. He denied previous lumbar trauma or the use of anticoagulation drugs. We used the full-endoscopic transforaminal approach (extraforaminal technique) to remove the SEH under local anesthesia. The patient was discharged home 2 days after surgery and the radicular pain disappeared completely. Three months later, follow-up magnetic resonance imaging showed that the dark-brown lesion had been totally removed. CONCLUSIONS:Spontaneous SEHs are uncommon. Although lumbar laminectomy is the mainstream treatment in those with neurologic deficits caused by epidural hematomas, the percutaneous full-endoscopic transforaminal approach may be an option for certain SEHs.

Abstract:BACKGROUND: Although percutaneous endoscopic technique has been routinely used in the treatment of disk herniation, there are few reports on its application in the management of intraspinal tumors. We present a case report of lumbar epidural angiolipoma that was totally removed by percutaneous endoscopic technique. CASE DESCRIPTION: A 63-year-old man presented with a 4-month history of progressively worsening low back pain. No abnormal neurologic finding was noted on physical examination, and magnetic resonance imaging demonstrated a dorsally located L2-3 epidural lesion, suggestive of a noninfiltrating angiolipoma. During the operation, an 8-mm skin incision was made, and a dilator was bluntly inserted toward the interlaminar space, followed by insertion of a working cannula onto the ligamentum flavum and placement of the endoscope. The interlaminar space was enlarged by resection of part of the lower rim of the right L2 vertebral laminae, and the right side of the interlaminar ligamentum flavum was removed to expose the tumor. The tumor was totally removed piecemeal under endoscopic guidance, and pathologic examination confirmed the diagnosis of angiolipoma. CONCLUSIONS: This report supports the application of percutaneous endoscopic technique in the surgical resection of noninfiltrating extradural lumbar angiolipoma.

Abstract:BACKGROUND:There are currently no high-quality studies on the optimal therapeutic approach for juxtafacet cyst, as treatment guidelines have not been developed. Herein, a novel technique in which we used an endoscopic transfacet approach to treat a patient with symptomatic lumbar synovial cyst is presented.CASE DESCRIPTION:An 87-year-old man presented with severe dull pain in the right anterior thigh. Lumbar magnetic resonance imaging revealed disc extrusion over the central canal zone at the L2-L3 and L4-L5 levels and an ovoid lesion with a hyperintense center plus a hypointense rim on the T2-weighted image. The lesion was located over the medial side of the right juxtafacet region at the L2-L3 level, causing thecal sac compression. After the operation, the visual analog pain scale improved with a value of 0-1/10, and straight leg raise test was negative. Microscopically, cystic fibrous tissue with focal myxoid degeneration, fibrin exudate, and scant synovial-like lining was observed. These findings were consistent with clinical synovial cyst. Three months later, lumbar magnetic resonance imaging was performed, and no evidence of cyst was disclosed. Lumbar computed tomography revealed the upper part of left L2-L3 facet joint was removed. The patient did not report any radicular pain during the 6-month follow-up period.CONCLUSIONS:Percutaneous endoscopic lumbar surgery could be a new option for the management of lumbar synovial cysts, especially when general anesthesia is not appropriate for the patient.
The Surgical Management of Nasal Skull Base Schwannoma under Endonasal Endoscope: a Retrospective Review of 52 Cases

Abstract:Objective: To discuss the surgical technique and outcome of nasal skull base schwannoma treated under endonasal endoscope. Methods: Fifty-two patients with nasal skull base nonvestibular schwannoma were treated under endonasal endoscope from May 2006 to June 2017 in Shanghai E&ENT Hospital. Of the patients, there were 21 men and 31 women. The age of the patients ranged from 33 to 71 years.Schwannoma mainly came from trigeminal nerve.Clinical symptoms included facial numbness, facial pain, nasal obstruction, headache, hypopsia, diplopia and tinnitus. Surgical approaches included pure endonasal endoscope approach (18 cases) and endoscopic endonasal resection asissted with sublabial transmaxillary approach (34 cases). Results: Total tumor resection was achieved in 42 patients(80.8%), subtotal resection in 8 patients, and partial resection in 2 patients. The relief rate of facial numbness, facial pain, nasal obstruction, headache, hypopsia, diplopia and tinnitus were 68.8%, 45.5%, 100.0%, 90.0%, 60.0%, 40.0% and 100.0%, respectively. During surgery, cerebrospinal fluid leakage was observed in 5 cases. All cases were successfully repaired with a nasoseptal flap or autologous fascia lata and fat obtained from thigh. Four cases with recurrence were observed in the follow-up period (6-134 months). Conclusion: Endonasal endoscopic approach is a safe and feasible procedure for schwannoma in nasal cavity, paranasal sinus, orbital apex, pterygopalatine fossa, infratomporal fossa, cavernous sinus and Meckel cave.

Abstract:Background: Schwannomas originating from the intravertebral canal rarely extend into the paravertebral region or form large masses. There are few reports on such medical cases, and their clinical diagnosis and management are poorly understood. Here, we report a case of an intraspinal schwannoma with a giant extraspinal mass in a middle-aged Chinese woman and the clinical implications of the symptoms, diagnosis, and treatment of thoracic vertebral schwannoma. Case presentation: A 59-year-old female patient who presented with sudden lower limb numbness and difficulty in walking was referred to our department. Magnetic resonance imaging (MRI) revealed a tumor compressing the T11-T12 spinal canal and extensively invading the left thoracic cavity to form a giant paravertebral mass. Schwannoma was diagnosed after intraoperative pathological examination, in which tumor cells were immunoreactive to S100 and Sox10 but negative for smooth muscle actin (SMH). Intraspinal mass resection through the posterior approach and extraspinal chest mass resection under video-assisted thoracoscopy through the left lateral approach were performed simultaneously, and instrumentation was used to maintain thoracic spinal stability. Conclusion: In patients with large schwannomas, customized treatment plans are crucial. Tailoring the approach to an individual's specific condition by considering factors such as tumor size and location is essential for optimal outcomes.
Analysis of surgical treatment for schwannomas of thoracic and lumbar spines

Abstract:Objective To study retrospectively the efficacy and complications of complete tumor resection via a posterior midline approach used to treat primary spinal schwannomas.Methods The patients' clinical presentations,operative findings and follow-up were reviewed in 39 patients,over the period from 2001 to 2010.There were 25 males and 14 females with a mean age of 41.5 years(range 14-72 years).Affected segments included thoracic vertebral canal in 17 cases,thoracolumbar vertebral canal in 4 case,lumbar vertebral canal in 15 cases,and lumbosacral vertebral canal in 3 cases.A posterior midline incision approach with laminectomy and facetectomy for single-stage complete resection of intraspinal and paraspinal components of tumor were used.Spine reconstruction with pedicle screw system was also carried out in relevant cases.Results Total excision was achieved in all patients.The patients were followed up for a mean of 35 months(range 10-69 months).Local recurrence was observed in one case of benign schwannoma.One patient with malignant nerve sheath tumor had local recurrent and lung metastasis,who died in 17 months after operation.ASIA sensory scores increased 1.6 in average after operation in 22 patients with paresthesia and ASIA motor scores increased 2.5 in average in 7 of 10 patients with paraparesis.Segmental instability occurred in 2 of 6 cases without spinal instrumentation after surgery.Conclusion Optimizing operative exposure is an important prerequisite for a safe and complete resection of thoracic and lumbar spinal schwannoma.This surgical exposure allows for facetectomy and costotransversectomy when necessary.Leaving the affected nerve root behind can result in a potential recurrence with the attendant difficult problems of reoperation.Spinal instrumentation was required except for patinets with the tumor mass located in midthoracic region.

Abstract:Objective To study the diagnostic and therapeutic value of endoscopy for gastrointestinal (GI) schwannoma.Methods We retrospectively studied data of 13 patients who underwent endoscopy and were confirmed as having GI schwannoma by pathology in our hospital during January 2006 and July 2011.Results Schwannoma of 8 patiets located in stomach,3 in esophagus and 2 in colon.Endoscopic ultrasonography showed schwannoma originated from muscularis propria,characterized by hypoecho.Twelve patients received endoscopic treatment,and 11 were successfully removed.The en bloc resection rate was 91.7％.Among them,6 were treated by endoscopic submucosal excavation,4 by endoscopic full-thickness resection and one by submucosal tunnel endoscopic resection.Another patient with schwannoma located in esophagus with a maximum diameter of 4.7 cm was transferred to thoracic surgery since the tumor had a wide base even after partial resection.No bleeding,inflammation or abscess occurred.The average follow-up time was 23.3 ± 10.8 months ( range,6-36 months).No metastasis or recurrence was found during follow-up.Conclusion Endoscopy and endoscopic ultrasonography,safe and efficacious,are valuable for the diagnosis and treatment of GI schwannoma.
The Surgical Management of Intraspinal Enterogenous Cysts

Abstract:Objective To analyze and summarize the method and technique of surgical management of intraspinal enterogenous cysts.Methods The surgical approach and manipulation,surgical results of 20cases of intraspinal enterogenous cysts were summarized retrospectively,which were treated in Department of Neurosurgery,Huashan hospital from Jan.2006 to Nov.2010.Results Of all 20 cases of intraspinal enterogenous cysts,total removal was achieved in 18cases,partial removal was achieved in 2 cases;Symptoms and signs improved in 19 cases,ipsilateral lower extremity strength decreased in one case.Seventeen cases were followed up clinically with average interval 31.2 months. Sixteen of the followed patients have got somewhat improved in symptom and sign.No recurrence was found on MRI image in fifteen cases with lesion total removal,whereas 2 recurrence in 2 cases with partial removal.Conclusions It is proper surgical approach and correct manipulation that are crucial to total removal and heal intraspinal enterogenous cysts.

Abstract:Background and Introduction: Minimally invasive spine surgery (MISS) is a set of techniques and procedures that reduces the local tissue injury while achieving the same results through traditional open surgery. The techniques for the treatment of lumbar disc herniation and degenerative canal stenosis have evolved from tubular discectomy to endoscopic discectomy. This helps in preserving spine function. Good clinical outcomes have been reported with the use of an endoscope for degenerative lumbar disc disease. Objective: In this video abstract, we present a case lumbar foraminal stenosis in an elderly man that was treated successfully using unilateral biportal endoscopic spine surgery. (UBESS). Surgical Technique: A 70-year-old man presented with progressive neurogenic claudication and left L4 numbness and paresthesias. His claudication distance was 200 m. His MRI Lumbosacral spine showed severe left L4-5 foraminal stenosis due to a disc prolapse. The patient underwent a left-sided unilateral biportal endoscopic transforaminal decompression and discectomy at L4-5 successfully. Postoperative period was uneventful and the patient had significant relief in his symptoms. Postoperative MRI Lumbosacral spine showed no muscle damage with adequate decompression at left L4-5 level. Results: The patient had an uneventful recovery. Conclusion: Biportal percutaneous transforaminal endoscopic discectomy/decompression is an ideal alternative to the uniportal endoscopic and minimal invasive microsurgery for foraminal and paracentral disc herniations and lateral recess and foraminal stenosis.

Full Endoscopic Resection of Intralabyrinthine Schwannomas: A Case Series.

Abstract:Objective: To investigate the technique and efficacy of fully endoscope resection of intralabyrinthine schwannomas (ILS) by transcanal transpromontorial endoscopic approach (TTEA). Study Design: Retrospective case review. Setting: Hospital. Patients: All patients who were affected by ILS, without extension to the internal auditory canal and underwent surgery with TTEA in our hospital in 2020. Intervention(s): Therapeutic. Main Outcome Measure(s): Recovery status, postoperative complications and remaining symptoms after surgery. Results: Three patients were included, all of which underwent gross total resections. The follow-up period was from 10 months to 2 years. No intraoperative and postoperative major complications were observed. There was no facial paralysis or cerebrospinal fluid leakage postoperatively. The hospitalization time of TTEA was 5 days. Three patients' vertigo was relieved after 1 week without receiving vestibular therapy. Only 1 patient complained of transient episodes of vertigo when climbing or holding heavy objects. Conclusions: TTEA has the advantages of clear vision to identify the anatomical structure, enabling complete tumor resection, reduced operation time, and faster postoperative recovery.Level of Evidence: IV

Abstract:Background Cervical dumbbell-shaped schwannomas are often challenging in terms of complete resection. There are few reports on the use of endoscope-assisted surgery for foraminal schwannomas. Method We used the wide-angle endoscope to assist in resecting a large cervical 3-4 dumbbell-shaped schwannoma. The well-designed set-up allowed the surgeon to achieve easier manipulation ins a straight surgical trajectory with minimal intraoperative adjustment. The endoscope allowed surgeons to visualize and operate in regions beyond the straight line of sight, without the need for facetectomy and subdural inspection. Conclusion We believe that this case demonstrates the feasibility of applying endoscope-assisted surgery for large cervical dumbbell-shaped schwannomas.

Abstract:Science Progress, Volume 107, Issue 4, October-December 2024. Rectal Schwannomas are exceptionally uncommon tumors that arise from the peripheral nerve plexus. These neoplasms are incidentally discovered as submucosal tumor-like lesions during routine colonoscopy in individuals without symptoms. Surgical resection accompanied by biopsy is considered the standard approach for diagnosis and treatment. We describe here the first successful utilization of submucosal tunneling endoscopic resection (STER) technique for removal of a rare rectal Schwannoma. A 59-year-old asymptomatic female was found to have a 15 × 12 mm submucosal tumor during colonoscopy. Submucosal tunneling endoscopic resection achieved en bloc resection and pathology confirmed Schwannoma. The patient was discharged uneventfully with no adverse events. This study highlights the efficacy and safety of STER for removing rare rectal Schwannomas.

Abstract:Background and importance: To describe the first case of a thoracic perineural cyst successfully treated using a direct thoracic transforaminal endoscopic approach. Methods: Case report. Clinical presentation: A 66-year-old male presented with right-sided radicular pain in a T4 distribution. MRI of the thoracic spine revealed a right T4 perineural cyst caudally displacing the root in the T4-5 foramen. He had failed attempts at nonoperative management. The patient underwent an all endoscopic transforaminal perineural cyst decompression and resection as a same-day surgical procedure. Postoperatively, the patient noted near complete resolution of the preoperative radicular pain. A thoracic MRI with and without contrast was performed 3 months after surgery and showed no evidence of the preoperative perineural cyst and no symptom recurrence was noted by the patient. Conclusion: This case report presents the first safe and successful report of an all endoscopic transforaminal decompression and resection of a perineural cyst in the thoracic spine.
Infratemporal Fossa Schwannoma Surgery via a Combined Prelacrimal Recess, Caldwell-Luc, and Distal Intraoral Approach

Abstract:Background: The deep location of infratemporal fossa (ITF) combined with the abundant vascular plexus in it increased the difficulty of removing the mass in ITF through endoscope surgery approach. However, under appropriate circumstances, the excision of ITF tumors through a combined prelacrimal recess, Caldwell-Luc, and distal intraoral approach can be safely performed with minimal impact on the surrounding tissues. Case presentation: The Department of Neurology received a 69-year-old male patient who had been experiencing headache, dizziness, and numbness from the mastoid region of his left ear to the corner of his mouth for a duration of 22 days. Cranial magnetic resonance imaging revealed the presence of a tumor located in the ITF. Following transfer to our department, surgical intervention was performed using a combined approach involving the prelacrimal recess, the anterior wall of maxillary sinus, and lateral ITF to successfully remove the tumor. Postoperative pathologic examination confirmed schwannoma as its nature. The patient was discharged in excellent condition without any functional impairment. Conclusions: On the basis of this case, the authors believe that this combined approach can offer a distinct endoscopic perspective and adequate surgical workspace, which is crucial for tumor removal while preserving the integrity of surrounding normal tissues. Moreover, the utilization of multiple small incisions has minimal impact on postoperative recovery.

Surgical Treatment of Intradural-Extramedullary Schwannomas and Management of Cerebrospinai Fluid Leakage after Operation

Abstract:Objective:To evaluate the effect of surgical treatment of intradural-extramedullary Schwannomas and the management of cerebrospinal fluid(CSF) leakage after operation.Method:A total 29 cases(17 males,12 females) with intraspinal Schwannomas from January 2000 to June 2006 underwent surgical treatment via posterior approach.The average age of the patients was 44.6 years(range,24-65 years).Of all cases,there were 10 cases in cervical spine,5 cases in the thoracic,14 cases in the lumbar or sacral.The involved nerve roots were sacrificed in all cases,and spinal internal fixation and bone graft were performed in 18 cases.The tumor resection and the postoperative CSF leakage were observed.Result:The complete resection was achieved in 27 cases,subtotal resection was done in 2 cases.The CSF leakage was found in all patients.The CSF leakage in the patients without internal fixation was drainaged for 3-5 days,and then the tube was closed,8 cases healed,3 cases healed after suturing drainage orifice.The CSF leakage in the patients with internal fixation could continue for longer time,healing could be obtained by extubation and suture of drainage orifice after 11-14 days drainage.The average follow-up time was 27 months,complete or partial symptom relief was found in 26 cases,worse in 3 cases,of which two recovered to the preoperative condition,one could not recovered finally.The tumor recurrence was not found in the patients with complete resection,and the residual tumor did not become larger in patients with subtotal resection.Conclusion:The surgical resection for intradural-extramedullary schwannomas could gain better result.The spinal internal fixation can be used if the spinal stability was destructed,but it may lead to the longer CSF leakage and increase the risk of the infection.
