A Case Report of Conduit Obstruction from Redundant Ileum after Esophagectomy with Ileocolon Interposition Via Retrosternal Route

Abstract:Background: Ileocolonic interposition was an alternative method of esophageal substitution for patients for whom gastric tube cannot be used.The migration of anastomosis caused by the redundant ileum was rarely reported.Therefore, this study reported that a patient with esophageal cancer who received esophagectomy and ileocolic interposition via posterior mediastinum developed the complication.Case Description: The patient complained of dyspnea and intermittent low fever 7 days after operation.Computed tomography (CT) scan indicated right pneumothorax and pleural effusions, however, bronchoscopy results were normal.Blood tests revealed the infection and inflammation conditions.Chest tubes were inserted from the second intercostal right side.And the left cervical incision were reopen for drainage.Gastroscopy showed the anastomosis and redundant ileum migrated to the right posterior mediastinum 12 days after operation.Then, redundant ileum was resected through neck incision.End-toend cervical anastomosis was re-performed with T-tube drainage.The patient was discharged 23 days and T tube was removed 2 months after the second operation.Conclusions: Ileocolonic redundancy following anastomotic leakage is an intractable postoperative complication that occurs after colonic interposition.Minimizing the length of substitution with a tensionfree anastomosis and putting a nasointestinal tube are important for patients receiving ileocolic interposition.And the surgery is indicated to relieve the associated symptoms and mitigate the failure of ileocolonic graft construction.
