Abstract:Introduction Recent studies in animal models indicate that recombinant human erythropoietin (rHuEPO) is very effective in enhancing neurological recovery after spinal cord injury (SCI). We described a protocol aimed at evaluating the efficacy of rHuEPO plus methylprednisolone (MP) compared with MP alone in improving neurological function of patients with SCI in randomised controlled trials (RCTs). Methods and analysis This study aims to explore the effect of rHuEPO combined with MP on neurological function in patients with SCI through a meta-analysis. To this end, the authors will search eight research databases for data retrieval: MEDLINE, China National Knowledge Infrastructure, Wan Fang, China Biology Medicine dis, Web of Science, PubMed, Cochrane and Embase for RCTs on SCI in any language. The primary outcome will be the American Spinal Injury Association score at the time of follow-up. The secondary outcomes will be the WHOQOL-100 instrument score, neurophysiological state and related factors. Two authors will independently search literature records, scan titles, abstracts and full texts, collect data, and assess materials for risk of bias. Stata V.14.0 will be used for statistical analysis. Ethics and dissemination This research is exempt from ethics approval because the work is carried out on published documents. We will disseminate this protocol in scientific conferences and a peer-reviewed journal. PROSPERO registration number CRD42021260688.

Abstract:Spinal cord injury (SCI) is a common disease and a major cause of paralysis, carrying much burden around the world. Despite the progress made with growth factors therapy, the response rate of acute SCI treatment still remains unsatisfactory, due largely to complex and severe inflammatory reactions. Herein, we prepare a MFG-E8-loaded copolymer system-based anti-inflammation therapy for SCI treatment. It is shown that the MFG-E8-loaded copolymer system can decrease pro-inflammatory cytokine expression and neuron death. In a rat model of crush-caused SCI, the copolymer system shows significant therapeutic efficacy by ameliorating inflammation, decreasing fibrotic scar, promoting myelin regeneration and suppressing overall SCI severity.
Observation of Clinical Effects of Recombinant Human Erythropoietin Combined with Methylprednisolone Sodium Succinate in the Treatment of Acute Spinal Cord Injury

Abstract:Objective To evaluate the effect of recombinant human erythropoietin (rhEPO) combined with methylprednisolone sodium succinate (MPSS),compared to MPSS alone,in the treatment of neurological function of patients with acute spinal cord injury (SCI).Methods Twenty-one patients presenting in hospital within less than 8 hours after acute SCI were randomly divided into two groups,the control group (10 cases) and the intervention group (11 cases).The control group was treated by MPSS combined with placebo,while the intervention group received MPSS with rhEPO.Both groups received MPSS 30 mg/kg within the first hour,and if the patient was admitted within 4 hours,MPSS would be applied in the treatment with 5.4 mg/kg per hour in the subsequent 23 hours and till 47 hours if the patient was admitted within 4-8 hours after injury.The intervention group received 500 U/kg rhEPO on admission and another 500 U/kg in the next 24 hours,compared with the control group where placebo was used.The evaluation on neurologic function recovery was made on admission,24 h,72 h,one week,2 months and 6 months later,and statistical analysis was performed.Results The change in ASIA score: in the control group,the increase was seen from admission to 6 months after injury in terms of exercise,algesia and tactile sensation ((31.2±6.6) points vs.(57.8±9.8) points,(41.4±9.5) points vs.(64.3±10.6) points, (39.2±6.8) points vs,(61.5±11.3) points),the increase also took place in the intervention group ((29.5±7.2) points vs.(77.4±10.3) points,(39.7±7.2) points vs.(82.3±12.1) points,(37.4±6.2) points vs.(78.6±12.4) points).As time went on,the increase range in the intervention group became larger,compared with the control group.The difference between the two groups in ASLA score was statistically significant (P<0.05).There was no significant difference in red blood cell count and hemoglobin content of routine blood test between the two groups (P>0.05).Conclusion The application of MPSS combined with rhEPO within 8 hours after acute spinal injury may be more effective than MPSS with placebo in the neurologic dysfunction recovery.
The Effect Of Erythropoietin In The Treatment Of Acute Spinal Cord Injury

Abstract:Objectives: The aim of this study was to evaluate the recovery of motor and sensory function in patients with acute spinal cord injury (ASCI) after recombinant human erythropoietin (EPO) treatment. The safety and efficacy of EPO for ASCI was discussed as well. Methods: The clinical data of 60 ASCI cases were analyzed in a retrospective manner, in which EPO-treated therapy group (n=30) and the EPO-untreated group (n=30) were included. The neurological function of each patient at admission, twelve and twenty- fourth months were reviewed. The adverse events were recorded as well. Results: All of the 60 patients were followed up for 2 similar to 3 years (average 2.4 years) after treatment. The follow-up of both groups showed that the patients in the therapy group had a better improvement in the AIS and ASIA motor score and more sensation of touch than the control group (P<0.05). No statistically significant difference regarding the red blood cell count and hemoglobin (Hb) concentration between therapy group and control group was found (P>0.05). Conclusion: EPO is a safe and effective drug for treating ASCI. Early application of EPO can promote the recovery of the motor and sensory function in patients with ASCI.

Abstract:Objective To investigate the efficacy of combined or single drugs of methylprednisolone (MP) and nimodipine in treatment of early spinal cord injury in rats.Methods Eighty rats were completely paralyzed by aneurysm clip method and randomized into four groups,in which intravenously MP,nimodipine,MP combined with nimodipine and no treatment were administrated respectively.Tarlov scale and inclined plane test were performed at the end of 1,2,3 and 4 weeks after injury,and the histopathologic and immunohistochemical examinations,image analysis,CSEP and SCEP examinations were also performed. Results The Tarlov scale and inclined plane test showed significant differences between three treatment groups and control group and no significant differences among three treatment groups.The results of image analysis showed significant differences between treatment groups and control group,and the combined treatment group showed significant difference with single MP and nimodipine group.The CSEP remained no recovery and the rate of SCEP recovery showed significant differences between treatment groups and control group, meanwhile, no significant difference among treatment groups.Immunohistochemical results showed negative 5-HT fibers in ventral horn of control group but that was positive in treatment groups.Conclusion MP and nimodipine were effective in treatment of early spinal cord injury and both showed neuron protective effects, but there was no significant difference between combined therapy and single-drug therapy.

Abstract:This systematic review aims to summarize the findings from all clinical randomized trials assessing the efficacy of potential neuroprotective agents in influencing the outcomes of acute spinal cord injuries (SCI). Following the PRISMA guidelines, we conducted comprehensive searches in four electronic databases (PubMed, Scopus, Cochrane Library, and Web of Science) up to September 5th, 2023. Our analysis included a total of 30 studies. We examined the effects of 15 substances/drugs: methylprednisolone, tirilazad mesylate, erythropoietin, nimodipine, naloxone, Sygen, Rho protein antagonist, granulocyte colony-stimulating factor, autologous macrophages, autologous bone marrow cells, vitamin D, progesterone, riluzole, minocycline, and blood alcohol concentration. Notable improvements in neurological outcomes were observed with progesterone plus vitamin D and granulocyte colony-stimulating factor. In contrast, results for methylprednisolone, erythropoietin, Sygen, Rho Protein, and Riluzole were inconclusive, primarily due to insufficient sample size or outdated evidence. No significant differences were found in the remaining evaluated drugs. Progesterone plus vitamin D, granulocyte colony-stimulating factor, methylprednisolone, Sygen, Rho Protein, and Riluzole may enhance neurological outcomes in acute SCI cases. It is worth noting that different endpoints or additional subgroup analyses may potentially alter the conclusions of individual trials. Therefore, certain SCI grades may benefit more from these treatments than others, while the overall results may remain inconclusive.

Abstract:OBJECTIVE Erythropoietin (EPO) exerts a neuroprotective effect in animal models of traumatic brain injury (TBI). However, its effectiveness in human patients with TBI is unclear. In this study, the authors conducted the first meta-analysis to assess the effectiveness and safety of EPO in patients with TBI. METHODS In December 2015, a systematic search was performed of PubMed, Web of Science, MEDLINE, Embase, the Cochrane Library databases, and Google Scholar. Only English-language publications of randomized controlled trials (RCTs) using EPO in patients with TBI were selected for analysis. The assessed outcomes included mortality, favorable neurological outcome, hospital stay, and associated adverse effects. Continuous variables were presented as mean difference (MD) with a 95% confidence interval (CI). Dichotomous variables were presented as risk ratio (RR) or risk difference (RD) with a 95% CI. Statistical heterogeneity was examined using both I2 and chi-square tests. RESULTS Of the 346 studies identified in the search, 5 RCTs involving 915 patients met the inclusion criteria. The overall results demonstrated that EPO significantly reduced mortality (RR 0.69, 95% CI 0.49-0.96, p = 0.03) and shortened the hospitalization time (MD -7.59, 95% CI -9.71 to -5.46, p < 0.0001) for patients with TBI. Pooled results of favorable outcome (RR 1.00, 95% CI 0.88-1.15, p = 0.97) and deep vein thrombosis (DVT; RD 0.00, 95% CI -0.05 to 0.05, p = 1.00) did not show a significant difference. CONCLUSIONS The authors suggested that EPO is beneficial for patients with TBI in terms of reducing mortality and shortening hospitalization time without increasing the risk of DVT. However, its effect on improving favorable neurological outcomes did not reach statistical significance. Therefore, more well-designed RCTs are necessary to ascertain the optimum dosage and time window of EPO treatment for patients with TBI.

Abstract:In addition to imperiling an individual's daily life, spinal cord injury (SCI), a catastrophic medical damage, can permanently impair an individual's body function. Methylprednisolone (MP), a medically accepted therapeutic drug for SCI, is highly controversial for the lack of consensus on its true therapeutic effect. In recent years, curcumin has served as a potential and novel therapeutic drug in SCI. Our study was intended to investigate the precise effect of MP and curcumin in SCI. We examined the function of MP and curcumin in a SCI model rat, both in vivo and in vitro, and found that there was a momentous improvement in Basso-Beattie-Bresnahan scores in the MP-treated group when compared with Cur-treated group within 14 days. Results obtained from the histological, immunohistochemistry and ultrastructural examinations evidenced the curative effect of MP was better than curcumin before Day 14. Nonetheless, there was a significant variation in the treatment effect between the MP-treated and Cur-treated groups after 14 days. The curcumin's effectiveness was more obvious than MP after 14 days following SCI. Abstract:Spinal cord injury (SCI) is catastrophic and can culminate in disability and death. The routine therapy employed in early stages of SCI currently entails surgical procedures combined with high doses of methylprednisolone (MP). MP is highly controversial for the lack of consensus on its true therapeutic effects. Resveratrol (RES) has recently been recognized as a potential and novel therapeutic drug in SCI. Herein, we investigated the effect of RES in a SCI rat-model and found significant improvement in Basso-Beattie-Bresnahan scores. Results obtained from histological, immunohistochemistry, and ultra-structural examinations evidenced the tremendous treatment effect of RES. On the basis of our experimental results, we hypothesize that RES could serve as an effective SCI therapeutic with prolong treatment time following injury.

Abstract:Dear Sir, Following publication of our article highlighting the importance of scratch tests for the diagnosis of contact allergy caused by Dermabond® (1), we report another case showing their usefulness. Our second patient was a 28-year-old female without an atopic history who presented with localized pruritic eczema on the anterior neck region, 24 h after surgery for thyroidectomy, in which Dermabond® had been used for wound closure. The patient was treated with antihistamines and corticosteroids. Following the same procedure as previously described (1), we performed patch tests, which gave negative results. Scratch tests were then performed with Dermabond®, a mixture of Dermabond® and povidone iodine (1% aq.), and a mixture of Dermabond® and chlorhexidine (0.2% aq.). The scratch test with the mixture of Dermabond® and chlorhexidine gave a positive result (Fig. 1). Scratch tests with Dermabond® and povidone iodine both gave negative results. Abstract:Methylprednisolone (MP) is the drug of choice for treating spinal cord injury (SCI), but the aggressive dosage regimen used often results in adverse side effects. Therefore, MP should be combined with other drugs to lower the required dose. Melatonin is effective in alleviating nerve damage and inhibiting axonal degeneration. The combination of melatonin and half-dose methylprednisolone (HMP) for spinal cord injury treatment has never been reported. In this study, we established a rat model of T9 spinal cord injury by the Allen’s method and assessed the synergistic therapeutic effects of melatonin and HMP by factorial design. Our results demonstrated that melatonin could synergize with HMP to ameliorate acute SCI through PI3K-AKT1 pathway. Combining melatonin with HMP significantly reduced the standard-dose of methylprednisolone and limited its adverse reactions, representing a promising option for treating acute SCI.

Abstract:Background: Spinal cord injury (SCI) has devastating physical and social consequences for patients. Systemic administration of methylprednisolone (MP) at a higher dosage though can reduce neurological deficits following acute SCI. Still, this treatment regimen is controversial, owing to the apparent dose-related side effects and relatively minor improvement in neurological function. Therefore, this study aimed at the bibliometric analysis of published literature related to SCI treatment, which may lead to future research trends. Methods: The literature published relating to SCI and using glucocorticoids for its treatment between 1982 and 2022 was collected and scanned in the Web of Science collection database using the keywords glucocorticoid, dexamethasone, MP, corticosteroids, and SCI, followed by using VOSviewer for bibliometric analysis of these articles. Results: A total of 1,848 published articles and 7,448 authors on SCI and glucocorticoid usage were identified. The SCI total link strength accounts for 1,341, and MP for 762 has a strong link to neuroprotection and inflammation. The mean citation count for the top 20 most-cited articles was 682 (range: 358–1,828), where most of these were descriptive studies having focused on clinical features. The Journal of Neurotrauma was the highest-ranked journal with 6,010 citations. A total of 69 articles were published by Michael G Fehlings from the University of Toronto with 6,092 citations. The University of Toronto has published 90-related manuscripts with 7,632 citations. In contrast, 800 articles were published in the United States, with 39,633 citations and total link strength of 5,714. The second-ranked country was China, with 241 published articles and 3,403 citations. Conclusions: The research published on applying MP in treating SCI has increased with time. Although the United States has made a significant global contribution to this important field of research, it requires rigorous clinical trials designed to verify the therapeutic role of MP in SCI and its appropriate dosage to find solutions for neurological recovery.

Abstract:[Objective]To study the therapeutic effect of recombinant erythropoietin for contusion injury of spinal cord.[Method]Contusion injury of spinal cord was caused by weight dropping in 24 New Zealand rabbits.Twelve hours after injury,the rabbits in control group were given natural saline intravenously and rabbits in low,mediate and large-dose group were given rh-EPO 100 IU/kg,500 IU/kg and 1000 IU/kg respectively.Neurological status of lower limbs were scored at 24 hours,48 hours and one week after spinal cord injury.All rabbits were killed one week after injury and spinal cords were stained by HE and caspase-3 method.Electronic microscopy was used to evaluate ultrastructural injury.[Result]The neurological scores of EPO treated groups were significantly higher than that of control group.HE and Caspase-3 immunohistochemistry showed that histological and ultrastructural damage of EPO treated groups were less severe than that of control group.The caspase-3 positive neurons were significantly fewer than that of saline treated group.There was no significant difference of therapeutic effect between mediate and large-dose EPO treated groups.[Conclusion]Rh-EPO administered 12 hours after contusion injury of spinal cord could lessen histological and ultrastructural damage,prevent apoptosis of neurons and promote neurological recovery of spinal cord.Mediate dose of rh-EPO is the appropriate treatment choice for spinal cord injury.

Potential Efficacy of Erythropoietin on Reducing the Risk of Mortality in Patients with Traumatic Brain Injury: A Systematic Review and Meta-Analysis

Abstract:The objective of this study is to assess the effectiveness of erythropoietin (EPO) on mortality, neurological outcomes, and adverse event in the treatment of traumatic brain injury (TBI).We searched databases including PubMed, OVID, and the Cochrane Library from inception until October 18, 2019 for randomized controlled trials (RCTs) to compare EPO treatment group and placebo in patients with TBI. Two authors independently processed the data and evaluated the quality of inclusion studies. Statistical analysis was performed with heterogeneity test with I2 and chi-square tests. We summarized the mortality, prognosis of neurological function, and deep vein thrombosis (DVT) outcomes and presented as risk ratio (RR) or risk difference (RD) with a 95% CI.Seven RCTs accounting for 1180 patients were included after meeting the inclusion criteria. Compared with placebo, the overall mortality of EPO-treated patients was significantly reduced (RR 0.68 [95% CI 0.50-0.93]; p = 0.02). EPO therapy did not improve neurological prognosis (RR 1.21 [95% CI 0.93-1.15]; p = 0.16) or increase the occurrence of DVT (RR 0.83 [95% CI 0.61-1.13]; p = 0.242), which showed no significant difference.The results showed that the administration of EPO may reduce the risk of mortality without enhancing the occurrence of DVT in TBI patients. However, the effect of EPO on neurological outcome remains indistinct. Through subgroup analysis, we demonstrated that the dose of EPO may be a potential factor affecting the heterogeneity in neurological function and that the follow-up duration may influence the stability of the result.
Targeted-delivery of nanomedicine-enabled methylprednisolone to injured spinal cord promotes neuroprotection and functional recovery after acute spinal cord injury in rats

Abstract:To date, no therapy has been proven to be efficacious in fully restoring neurological functions after spinal cord injury (SCI). Systemic high-dose methylprednisolone (MP) improves neurological recovery after acute SCI in both animal and human. MP therapy remains controversial due to its modest effect on functional recovery and significant adverse effects. To overcome the limitation of MP therapy, we have developed a N-(2-hydroxypropyl) methacrylamide copolymer-based MP prodrug nanomedicine (Nano-MP) that can selectively deliver MP to the SCI lesion when administered systemically in a rat model of acute SCI. Our in vivo data reveal that Nano-MP is significantly more effective than free MP in attenuating secondary injuries and neuronal apoptosis. Nano-MP is superior to free MP in improving functional recovery after acute SCI in rats. These data support Nano-MP as a promising neurotherapeutic candidate, which may provide potent neuroprotection and accelerate functional recovery with improved safety for patients with acute SCI.

Clinical application of methylprednisolone on recovery of thoracic spinal stenosis symptom

Abstract:Objective:To investigate the effect of methylprednisolone on thoracic spinal stenosis symptom.Methods: 82 patients adopt of simply posterior thoracic vertebral canal decompression.8 cases occurred acute spinal shock symptom after(2～4 h) surgical decompression.Considering this kind of symptom is spinal cord ischemic reperfusion(IR) injury.All cases infusion methylprednisolone 1 000 mg before decompression in 30 min.After operation,77 cases were infused methylprednisolone extenuation day by day and 3 cases were spinal cord IR injury;5 cases of spinal cord IR injury were treated with stoss and maintenance dose of methylprednisolone therapy.Results:No death was noted,and 1 case occurred stress ulcer.Among cases treated with methylprednisolone extenuation therapy,74 cases recovered well;3 cases of spinal cord IR injury motor function recovered slowly.After 1 year follow-up,1 case completely recovered and the other 2 cases were not well.5 cases treated with stoss and maintenance dose of methylprednisolone therapy,4 cases motor function remarkably recovered,1 case recovered slowly.After 1 year follow-up,5 cases completely recovered.Conclusion:Methylprednisolone has significantly effect on treatment of thoracic stenosis symptom in extenuation spinal cord swelling,inhibit vasoactive substance,stabilize cellular and lysosomal membrane.
Anti-inflammatory and Anti-apoptotic Effect of Combined Treatment with Methylprednisolone and Amniotic Membrane Mesenchymal Stem Cells After Spinal Cord Injury in Rats

Abstract:This study was undertaken to investigate the synergistic effects of methylprednisolone (MP) administration and transplantation of amniotic membrane mesenchymal stem cells (AM-MSCs) following T11 spinal cord clip compressive injury in rats. The combination treatment with MP (50 mg/kg) and delayed transplantation of AM-MSCs after rat spinal cord injury, significantly reduced (1) myeloperoxidase activity, (2) the proinflammatory cytokines: tumor necrosis factor-α, interleukin (IL)-1β, IL-6, IL-17, interferon-γ and (3) the cell apoptosis [terminal deoxynucleotidyl transferase, dUTP nick end labeling (TUNEL) staining, and caspase-3, Bax and Bcl-2 expressions]; increased: (1) the levels of the anti-inflammatory cytokines (IL-10 and transforming growth factor-β1) and (2) the survival rate of AM-MSCs in the injury site. The combination therapy significantly ameliorated the recovery of limb function (evaluated by Basso, Beattie and Bresnahan score). Taken together, our results demonstrate that MP in combination with AM-MSCs transplantation is a potential strategy for reducing secondary damage and promoting functional recovery following spinal cord injury.

Abstract:Objective To study the possibility of the prevention of spinal cor d injury in rat by using Methylprednisolone(MP) before operation of spine,to fin d new treatment in clinic to improve the functional recovery of nerve,also to pr ovide the theoretical guide to apply the drug prevention for spinal cord injury. Methods Build acute spinal cord injury model in rats according to rate by modi fied Allen's method. 114 healthy adult Sprague-Dawley rats were randomly divided into three groups:normal group,control group and Methylprednisolone group. The function of spinal cord was evaluated by Xu shaoding's 6 graded rate and Rivlin' s rate of slope. The changes of the motor evoked potential(MEP)were studied. Nor mal HE and electronmicroscope would be used to observe the pathological and ultr astructural changes of spinal cord. Results The disabled rate of hindlimb locom otor function of Methylprednisolone group is far below that of control group (P 0.05), but higher than that for normal group(P0.05). The latent period and ampl itude of MEP for Methylprednisolone group is significantly lower than that for c ontrol group (P0.05),but the difference is not significant between those two gr oups(P0.05). Observed under microscope,the spinal cord injury for Methylprednis olone group is not serious. Conclusion Methylprednisolone in prophylaxis can im prove the neurological function of spinal cord.
