Methods 73 artificial hip joint replacement patients were divided into routine nursing group and rehabilitation guidance group, compared two groups of postoperative nursing situation and effect. Abstract:Objective:To discuss rehabilitation methods of ipsilateral total hip and total knee replacement Method:From1992to1999,30cases43ipsilateral total hip and total knee replacement were rehabilitated,the rehabilita-tion plan was made according to the characters of individual and operation.Result:Compared with pre-operation,general motion and Harris score of hip were average improved93.7°and45points respectively.Motion and HSS score of knee were average improved44°and46.5points respectively.After5.2years follow-up,flexional contrac-ture was reoccur in one case one knee,a femoral shaft fracture was occurred in on case,which was recovered af-ter operation.Others was satisfied with the results,the pain was released and the distance of ambulation was ex-ceed100m.Conclusion:Detailed plan must be made before operation of ipsilateral total hip and total knee re-placement,different rehabilitation method should be made for different individual.Our principle was following the order and step by step,adjusting at real time.
Application of Comprehensive Rehabilitation Nursing Based on 5E Concept in Patients after SuperPath Minimally Invasive Total Hip Replacement

Abstract:Objective? To explore the effect of rehabilitation nursing based on 5E concept in patients after SuperPath minimally invasive total hip replacemen(t THR). Methods? From February 2016 to February 2018, 194 patients who underwent THR through SuperPath approach in Huzhou Central Hospital of Zhejiang Province were selected as the study subjects. The patients were randomly divided into observation group (n=97) and control group by coin flipping method. The control group was given routine nursing care while the observation group was given the rehabilitation nursing based on 5E concept on the basis of that in the control group. The degree of pain, hip function score, incidence of complications and compliance of functional exercise were compared between the two groups. Results? The Harris score of the patients in the observation group was (65.25±5.29) at 1 month, (87.69±6.47) at 3 months and (93.85±8.66) at 6 months after operation, while that of the control group was (55.84±4.14), (77.48±5.89), (82.45±7.69). There were significant differences between the two groups (t=13.767, 11.511, 9.465;P＜ 0.05). The incidence of postoperative complications was 4.12% (4/97) in the observation group and 27.84% (27/97) in the control group (χ2=20.310, P＜ 0.05). The total compliance rate of functional exercise was 95.88% (93/97) in the observation group and 87.63% (85/97) in the control group, the difference was statistically significant (χ2=4.359,P＜ 0.05). The nursing satisfaction of patients in the observation group was 96.91% (94/97) and 81.44% (79/97) in the control group. The difference was statistically significant (χ2=12.015,P＜0.05). Conclusions? The application of comprehensive rehabilitation nursing based on 5E concept in SuperPath minimally invasive THR can effectively promote the recovery of hip function, reduce the occurrence of complications after operation, enhance the compliance of patients with functional exercise, and significantly improve the satisfaction of patients with nursing.

Abstract:The main purpose of total hip replacement is to relieve hip pain and recover the lost mobility of the joint. The postoperative functionality of the hip endoprosthesis is closely related to proper physiotherapy. A good range of motion, sufficient pain reduction and minimum disruption to the muscle tissues are conditions allowing the patients to perform individualized muscle strengthening exercises and to exercise the newly gained mobility, following practice sessions during the early postoperative period still in hospital. The authors describe the individual fazes of the physiotherapy and summarize the principles, which lead to a successful rehabilitation of the patient and his early recovery as well as return to normal life activities.

Abstract:Objective To observe the influence of early rehabilitating function after total hip arthroplasty(THA) before discharge and its related factors. Methods Thirty patients underwent unilateral THA and were ran-domly divided into two groups: 21 patients had rehabilitation therapy right after operation, the other 9 cases served ascontrol. Active range of motion (AROM) of operated hip and functional independence measure (FIM) at dischargewere compared, and the factors affecting AROM and FIM were analyzed. Results AROM and FIM in the patientswith early rehabilitation were higher than those in the control group (P＜0.01). Age and preoperative bed-riddenwere significantly related to AROM and FIM. Conclusion Early rehabilitation intervention would improve the functionafter THA and would be better for elderly patients or patients with long preoperative bed-ridden. Abstract:Objective:Improving the psychological status and rehabilitative motivation of the elderly patients undergoing total hip re -placement and enhancing the effect of rehabilitation through group therapy .Methods:To divided randomly into collective group and indi-vidual group (30 cases in each group) .Patients In two groups were treated with conventional treatment of rehabilitation .After treatment, according to therapist requirements , Self-help exercises in the collective group was done in form of group .After treatment of one month and three months ,each patients in two groups was evaluated the HARRIS and HAMD -17 .And when the patients had been treated for three months, Questionnaire stressed on how they spend their spare time handed to each participants .Results: After treatment of three months, there are significant differences in spare time distribution between the collective group and individual group .Patients in the col-lective group spent much longer time in doing functional activities and exercises than that of patients in the individual group (P<0.01). The psychological status and physical function of patients in collective group were significantly improved compared with that of patients in the individual group (HARRIS p<0.05;HAMD-17 p<0.05).Conclusion:The group therapy was a helpful measure to improve reha-bilitative motivation and psychological condition of the elderly patients with total hip replacement and then further improved the effects of patient's rehabilitation .

Abstract:Authors present a set of exercises for patients after total hip replacement (THR) treated due to idiopathic hip joint osteoarthritis. Outcome of surgical treatment depends largely on physical therapy conducted after the procedure. Physical therapy following total hip arthroplasty involves restoration of proper physical function. Exercises increase the strength of hip girdle muscles and stabilize the involved hip joint. Total postoperative rehabilitation improves the gait esthetics. Restoring patient's full independence in everyday and professional life after total hip arthroplasty is the best test for properly conducted rehabilitation. A rehabilitation algorithm following hip arthroplasty was established based on the data acquired from literature and authors' own studies. Methods of rehabilitation following total arthroplasty was unified with regard to the type of endoprosthesis (cemented and non-cemented). Rehabilitation after revision and cancer arthroplasties were not taken into consideration. Exercises were divided into those performed in supine and standing positions as well as resistance training (using an elastic TheraBand® tape). At a later stage of rehabilitation, marching and walking as well as cycloergometer training were included. Patient's position during the day and in the sleep for two months following THR was taken into account, including some types of exercises that are contraindicated and pose a threat of endoprosthesis luxation.
Characteristics of rehabilitation therapy for patients with total knee replacement

Abstract:OBJECTIVE: To analyze the influencing factor, as well as rehabilitation method after total knee replacement. METHODS: Chinese Journal Full-text Database and PubMed database were retrieved with key words of artificial knee joint, athroplasty, and rehabilitation, from January 1999 to June 2009. The literature search was restricted to Chinese and English languages. The evaluation indexes were the knee range of movement, function, muscle strength, and joint stability. Accordingly, clinical research concerning rehabilitation treatment of total knee replacement was included, and the animal experiment was excluded. RESULTS: A total of 492 literatures were obtained by initial search with computer. On the basis of inclusion criteria, the related papers were analyzed. Rehabilitation training after total knee replacement includes pain control, joint activity training, strength training, proprioceptive training and walking training. Knee movement range was one of the important indicators in evaluating total knee replacement, which was also the most important component in the knee-rating system. Pain reduced and normal walking function was important to observe the therapeutic effects after total knee replacement. Strength training was an important part of rehabilitation training, early rehabilitation not only can regain the muscle strength, but also can promote blood circulation and prevent deep vein thrombosis of lower limbs. CONCLUSION: Rehabilitation training can restore muscle strength, joint activity and walking ability as quickly as possible, and control pain greatly, which influence therapeutic effects of replacement.
Rehabilitation approach after bilateral total knee replacement in patients with severe osteoporosis

Abstract:Aim: To explore the perioperative rehabilitation methods of bilateral total knee replacement (TKR). Methods: Perioperative rehabilitation was conducted to 77 patients with bilateral TKR, a series of rehabilitation methods were used aiming at the characteristics of patients. Range of motion (ROM), Hospital for Special Surgery Knee Rating Scale (HSS) score system, bone mineral density (BMD) and clinical results were recorded to evaluate the rehabilitation results. Results: All patients gained good rehabilitation results. Compared with before operation, average ROM improved by 52.5 degree, HSS score by 45 points, T value of BMD by 0.65 and muscle strength improved 1 class respectively after operation. No complications occurred during rehabilitation in hospital. After 2.5-year follow-up, most of the patients were satisfied with the rehabilitation results. Conclusion: For the patients with bilateral TKR, individual specialized rehabilitation plan can lead to good rehabilitation results, and provide assurance for the maintenance of operation results.

Abstract:Objective To investigate the effect of preoperative rehabilitation on early function of joints after total hip arthroplasty by comparing with non-preoperative rehabilitation.Methods Between March 2010 and November 2011,98 patients with hip disease were recruited in study.Of 98 patients,49 received preoperative rehabilitation for 2 weeks in trial group,and 49 did not receive preoperative rehabilitation in control group.There was no significant difference in gender,age,disease duration,disease cause,preoperative visual analogue scale(VAS) score,and preoperative Harris score between 2 groups(P > 0.05).Results There was no significant difference in VAS score at each time point after operation between 2 groups(P > 0.05),but significant differences were found in Harris score(P < 0.05).At 3 days after operation,compliance evaluation results showed that 36 cases were of full compliance,and 13 cases were of partial compliance in trial group;27 cases were of full compliance,14 cases were of partial compliance,and 8 cases were of non-compliance in control group;there was significant difference between 2 groups(Z=2.286,P=0.002).The ambulation time was within 7 days in trial group,and was within 7 days in 47 cases and more than 7 days in 2 cases in control group;there was significant difference between 2 groups(Z=3.840,P=0.000).Conclusion Preoperative rehabilitation plays an important role in compliance,decreased bed rest time,and hip function improvement in patients after total hip arthroplasty.

Abstract:Objective To probe into rehabilitation education and kinesiatrics of patients after total knee arthroplasty(TKA).Methods 34 knees of 28 patients were treated with TKA.The patients received convalescent trainings post-operation,including training of joint movement,muscle strength and walking ability of legs.Results The patients were followed up for 5 months to 3 years,the knee joints were examined and evaluated with American Association of Knee Joint Surgery standard,22 cases graded excellent and 8 cases good with a good rate of 100%.Conclusion Convalescent and standardization kinesiatrics is an effective method to improve the effects of TKA.

Abstract:Objective:To introduce rehabilitation after total knee arthroplasty in patients with severe flexion contracture.Methods:80 cases of TKA with knee contractures greater than 60° were instructed postoperatively for rehabilitation for one year.According to HSS knee score scale,the knee function score was 51.25±6.10 preoperative.The program included: muscle strength exercise,muscle resistance,proprioception,range of motion,physiotherapy,TCM therapy,psychotherapy.Through analysis of stability,muscle strength,range of motion,gait.and knee scale score evaluation,the applicability and effectiveness of the nursing program were studied.Results:The tumefaction of the operated knee was regained in all cases three months after surgery.The strength of quadriceps femoris,hamstrings and gastreocnemius all reached Ⅳgrade(MMT) one year after operation.The patients were satisfactory.The total knee score were 87.13±7.43 according to HSS knee score scale.Statistically the difference between the HSS score before and one year after operation has prominent significance.Conclusion:For the patients with severe flexion contracture after TKA this rehabilitation was practical and effective.

Abstract:Objective To observe the application of ointegrated rehabilitation approach and orthopaedic treat －ment during the perioperative period of total knee arthroplasty (TKA).Methods 100 patients with TKA were divided into the treatment group (n＝50) and the control group (n＝50),the treatment group was received the routine postoper－ative treatment and ultra early systematic rehabilitation treatment ( preoperative rehabilitation education ,postoperative systematic rehabilitation therapy , hourly rehabilitation management and comprehensive rehabilitation measures ).The control group was received only routine postoperative treatment.The knee flexion,extension,rest pain,active pain,thigh and calf circumference,and the postoperative drainage were compared between the two groups from 1st to 3rd days after the operation and discharge day.Results There was no significant difference in active flexion ,extension and range of motion between the two groups (P＞0.05).From the 1st to 3rd day and discharge day,the active flexion and ROM in the treatment group were significantly better than those in the control group (P＜0.01) at the discharge day,the active ex－tension in the treatment group were significantly better than those in the control group (P＜0.01).There was no signifi－cant difference in rest and active pain at the preoperation ,1st to 2nd after the operation between the two groups ( P＞0.05).The improvement of active pain in the treatment group was significantly better than that in the control group at the 3rdand discharge day(P＜0.05).There was no significant difference in the healthy and affected side about the thigh and calf circumference at the preoperation ,1st to 3rd day and discharge day between the two groups (P＞0.05).The drain－ age flow of treatment group was significantly less than that in the control group at the 1st day after operation (P＜0.01). Conclusion Integrated rehabilitation approach and orthopaedic treatment can effectively recover the function after TKA .

Abstract:Objective To discuss the effects of early postoperative individuals rehabilitation following total knee replacement(TKR).Method Sixty patients were randomly divided into rehabilitation group and control group,30 subjects each.All patients suffered from knee osteoarthritis were underwent the operation in hospital.All patients got the normal nursing care.In addition,the rehabilitation group got the individual rehabilitation systematically.Patients were evaluated at the 2nd day before operation,the 2nd,the 7th and the 14th day after operation.The evaluation indexes involved the hospital special surgery knee score(HSS),knee range of motion(ROM),and visual analogue scale(VAS).Result All the evaluation indexes had no differences between rehabilitation group and control group before operation and the 2nd day after operation(P>0.05).At the 7th and 14th day after operation,HSS and VAS in rehabilitation group was better than those in control group(P<0.05,P<0.01).The knee ROM in rehabilitation group was much better than that in control group(P<0.01).Conclusion Individual rehabilitation program could facilitate function restoration and significantly improve range of motion for the patients after TKR.

Abstract:In recent years, rehabilitation after total knee arthroplasty ( TKA ) has received widespread attention. There are few rehabilitation therapists who can provide rehabilitation guide for the patients receiving TKA in most hospitals of our country. There are some factors that may inlfuence the effects of rehabilitation after TKA, including operation timing, timing of rehabilitation intervention and postoperative rehabilitation exercise. In early postoperative period, neuromuscular electrical stimulation ( NMES ) on the quadriceps femoris can help to improve the strength of quadriceps femoris and the function of apparatus of extention knee, increase the speed of walking and promote the recovery of knee joint function. A study showed that supra-physiological doses of testosterone before TKA could improve the postoperative muscle strength, increase the functional independence measure ( FIM ) score and shorten the hospitalization time. Based on the Lesquesne questionnaire data, the health status of the patients and quality of rehabilitation after TKA can be assessed.

Abstract:Total joint replacement (e.g.total hip replacement,total knee replacement) is one of the most common and most successful orthopaedic procedures.Infection after total joint arthroplasty is a devastating complication that causes pain and dysfunction,increases the cost.It is important to prevent postoperative infection.There are preoperative,perioperative and postoperative risk factors of total joint replacement,and preventions should base on these risk factors.This article reviews muhiple preoperative,perioperative and postoperative risk factors of postoperative infection and the preventions based on these risk factors.
Application of Whole-course Health Education Pathway in Patients over 85 Years Undergoing Total Hip Replacement

Abstract:Objective To explore the effects of whole-course health education pathway in patients aged over 85 years who underwent total hip replacement.Methods From March 2008 to November 2011,49 patients aged over 85 years who underwent total hip replacement were recruited in this study to receive the whole-course health education.All the patients were followed up for the health status and the incidence of complications.Results The follow-up period was 6 to 36 months,the mean Harris hip score of the initial 3-month follow up was 81.84±5.62(70-93 point);the mean Harris hip score of the final follow up result was 83.86±6.25(71-93 point).Of the interviewees,10 patients were observed with excellent,18 with good and 8 with satisfactory,the excellent and good rate accounted for 77.1%.The mean hospital stay was 14.87±1.05 days(13-17 days).The incidence of postoperative deep venous thrombosis was 18.36%.The knowledge awareness of health education after the implementation of health education pathway was significantly improved than before(P0.01).Conclusion The application of whole-course health education pathway can optimize aged patients' health education pathway,help the responsible nurses regulate their health education methods and enhance the ability of health education for themselves.
