Efficacy and safety of clockwise thoracoscopic lobectomy for congenital pulmonary malformation in children

Abstract:Objective:To explore the safety and efficacy of clockwise lobectomy for congenital lung malformations (CLMs) in children.Methods:From January 2015 to January 2019, clinical data were reviewed for 184 CLMs children undergoing lower clockwise lobectomy. There were 113 boys and 71 girls with a median operative age of 6.8 months and a median weight of 9 kg. The involved side was left (n=81) and right (n=103). Preoperative chest enhanced computed tomography (CT) scans were performed for detecting the locations of lesions and associated deformity. And postoperative pathological examinations and post-discharge follow-ups were conducted.Results:The postoperative pathological diagnosis was congenital pulmonary airway malformation (n=133), intralobar isolated lung (n=44), bronchiectasis (n=4) and congenital lobar emphysema (n=3). Three cases were converted into thoracotomy due to thoracic adhesions while the remainders were successfully operated. The average operative duration was 46(35-113) min and the average volume of blood loss 3.5(1-60) ml. Chest tube was not inserted in 88.0%(162/184) and all of them resumed a normal diet at 6h postoperatively. There were low-grade fever (n=23) and postoperative air leakage (n=2). The median postoperative hospital stay was 2 days. And 21 children were lost to follow-ups. No significant complications occurred during a follow-up period of over 1 year.Conclusions:Lower clockwise thoracoscopic lobectomy is both safe and effective for pediatric CLMs.
