Abstract:ObjectiveTo analyze a case of neglected cervicovesical fistula with intrauterine adhesions caused by cesarean section.MethodsA 36-year-old female patient with a history of two previous cesarean sections complained of the absence of menstruation for the last 18 months. The diagnosis of the cervicovesical fistula was made through hysteroscopy and cystoscopy. The reconstruction of the uterus and bladder was achieved by a laparoscopic repair technique.ResultsThe patient resumed normal menstruation postoperatively without complaining of any complications. Uterine continuity and cavity had been restored to normal at the second look of hysteroscopy.ConclusionsCervicovesical fistula with intrauterine adhesions is very rare in our clinical work. Hysteroscopy might play an essential role in diagnosing cervicovesical fistula and IUA. In our literature review, a surgical approach was the mainstay and definitive management of the cervicovesical fistula following a cesarean section.

Abstract:Periclitoral abscess . Given the sensitive location of the abscess, gynecology was consulted to guide management and establish outpatient follow-up. Incision and drainage were not advised, and the patient was discharged with a treatment of cephalexin, topical anesthetic, ibuprofen, and warm compresses. She reported spontaneous drainage of the abscess associated with immediate relief of her pain within 2 days of her ED visit at outpatient gynecology follow-up. She had no recurrence after 2 months.

Abstract:Introduction: Pelvic inflammatory disease (PID) is defined as an acute and subclinical infection of the upper genital system, including the uterus, fallopian tubes, ovaries, and ligaments. It can lead to various complications, such as salpingitis, endometritis, oophoritis, perihepatitis, peritonitis, and tubo-ovarian abscess (TOA). Case Presentation: In our case, a 22-year-old Afghan woman presented with generalized pain in the abdomen and pelvis, accompanied by vaginal discharge and fever, following 17 days of normal vaginal delivery (NVD) and septic shock. She had no previous history of PID. Abdominal ultrasonography revealed a significant amount of free fluid in the paracolic grooves and pelvis, along with a perforated mass on the right adnexa. As a result, she underwent laparotomy and right adnexectomy. A 10-day course of meropenem and vancomycin was prescribed, and the woman was discharged with oral antibiotics. Conclusions: Tubo-ovarian abscess following NVD is a rare condition that can occur in patients with preterm premature rupture of membranes (PPROM) and episiotomy. Therefore, it is essential to prescribe antibiotics in cases involving episiotomy and rupture of the membranous.
Pelvic inflammatory disease presenting 16 months after vaginal hysterectomy: A case report and literature review

Abstract:Pelvic inflammatory disease after hysterectomy is rare and the underlying route of infection is highly heterogeneous. We report the case of a 52-year-old woman with a history of vaginal hysterectomy for uterine prolapse admitted to the emergency department with acute abdominal pain and fever. Vaginal discharge and pelvic tenderness were evident in the clinical examination. Ultrasound and computed tomography scans showed a cystic pelvic mass in contact with the vaginal cuff, suggesting the diagnosis of pelvic inflammatory disease. Laparoscopic examination revealed a bilateral tubo-ovarian abscess firmly attached and fistulized to the vaginal cuff, and after tubal removal and antibiotic coverage the patient had an optimal recovery. We performed a review of the case reports published on this subject, and concluded that pelvic inflammatory disease should not be excluded in patients with a history of hysterectomy when symptoms and findings are compatible.
Catheter Displacement and Penetration of a Vaginal Cuff Following Laparoscopic Adjustable Gastric Banding: A Case Report

Abstract:This case report describes a rare complication following laparoscopic adjustable gastric banding (LAGB) in a 47-year-old woman. The patient, who had a history of obesity and a previous hysterectomy, presented with dyspareunia. Upon examination, a catheter was visualized in the upper vaginal canal, which penetrated the right vaginal cuff and caused significant pain. Imaging revealed that a catheter from the LAGB device had penetrated the vaginal cuff. This unexpected migration of the catheter necessitated surgical intervention for removal. The case underscores the importance of monitoring for unusual symptoms in patients with a history of LAGB, as this procedure, while minimally invasive and generally safe, can have serious long-term complications. These complications may include gastric erosion, perforation, band migration, and, in this rare instance, vaginal cuff penetration. The report emphasizes the need for healthcare providers to maintain a high level of suspicion for such complications, particularly in patients presenting with atypical symptoms post LAGB. It also highlights the interdisciplinary approach required to manage these complex cases, involving both general surgery and radiology teams. To the best of our knowledge, this is one of the first cases with a history of LAGB, which was associated with the complication of penetration of the vaginal cuff.
Scar abscess six years after cesarean section: Laparoscopic and hysteroscopic management

Abstract:Long-term infection of pelvic organs after cesarean section is a rare event still associated with maternal morbidity. Undrained collection of pus is associated with persistent fever. Hysterectomy has been classically suggested as a safe option to treat this kind of lesion. We present the case of a patient who presented an abscess in the cesarean section scar tissue 6 years after the surgery. The myometrium covering the abscess was almost absent. This lesion was first managed with antibiotic therapy and ultrasound-guided aspiration. After failure of the initial therapy, this situation was managed by hysteroscopic fulguration of the capsule under laparoscopic control. For patients with abscesses spreading into the uterine cavity, endoscopic management is a feasible and safe intervention that may be offered to the patient after a correct evaluation.

[Endometriosis in delayed scarring of postpartum eutocic episiorrhaphy. Integral aspects and a case report]

Abstract:Endometriosis remains a mystery because it has not yet been shown why these cells are viable in the abdominal cavity, although it has been thought that the immune system plays a role in implantation outside the intrauterine cavity by abasement in the regulatory capacity of natural killer cells, as well as peripheral and peritoneal immunologic cells. Among methods used to recognize this illness are laparoscopy, laparotomy, ultrasound, antibodies, and the histologic study. It has been observed that surgical scars can present cyclical inflammation and pain when affected with this pathology. We present the case of a patient with a second delivery who arrived at the Gynecology Service due to referring intense pain in the episiorrhaphy scar as well as superficial dyspareunia. One year after performance of the surgical procedure, pain did not allow the patient was unable to sit normally; in addition, during the last 3 months the area of the cicatrix augmented in volume during menstruation. Under peridural block and with surgical spindle excision, the abnormal tissue was dissected without complications; the surgery showed brittle tissue and with abundant new vascularity. The histologic diagnosis reported vulvar-tissue endometriosis. Was initiate complementary treatment was initiated with gestrinon once a week for 4 weeks, as well as danazol daily for 2 months to avoid possible persistence of endometrial tissue. The scar at present is minimal and is observed along the borders of the surgical union line, without an increase in size nor discomfort on digital pressure. We consider it necessary to assure cleaning of the episiotomy before initiating surgical suturing to diminish presence of endometriosis, despite the fact that incidence of this disturbance is low (0.03%). Advancement in knowledge of the physiopathology process will permit elimination of the remaining endometrial tissue with new therapeutic strategies, as well as clearing up the mechanism of ectopic implantation of endometrial cells.

A diagnostic dilemma of an 18-week cervical ectopic pregnancy: A case report

Abstract:Introduction: Cervical ectopic pregnancy (CEP) is characterised by the implantation of trophoblastic tissue within the cervical canal and is associated with a significant risk of maternal morbidity and mortality. This case report addresses a second-trimester CEP with unusual sonographic features suspicious of placenta accreta spectrum (PAS), which was successfully managed with an abdominal hysterectomy. Case presentation: A 27-year-old woman, G6P2, presented to the labour ward of a rural hospital at 18 weeks of gestation with premature rupture of membranes. The index pregnancy was complicated by an absence of any antenatal care, as well as a history of cigarette smoking and cannabis use. An ultrasound scan demonstrated a live pregnancy with the foetal head within the cervical canal. A termination of pregnancy was arranged with misoprostol 200 mg orally followed by an oxytocin induction. However, a repeat ultrasound scan, after 12 h of oxytocin infusion, which failed to terminate the pregnancy, demonstrated a still live foetus as well as increased vascularity, concerning for PAS. The patient underwent an emergency abdominal hysterectomy, with an intraoperative diagnosis of a CEP. The postoperative course was unremarkable, and the patient was discharged home on day 3 post-operatively. Discussion: Appropriate antenatal care and early booking-in would have identified a CEP early in gestation and allowed for minimally invasive management and potential conservation of fertility. When this is not possible in such cases, meticulous pre-operative planning by a gynaecologist with experience in advanced pelvic surgery can minimise the associated morbidity and mortality.

Abstract:This case report describes the presentation, diagnosis, and surgical management of a rare vesical ectopic pregnancy in a 36‐year‐old woman with a history of multiple cesarean sections. The patient presented with symptoms of suprapubic pain, fever, and amenorrhea. An initial ultrasound indicated retained products of conception, leading to a preliminary diagnosis of septic miscarriage. However, subsequent rescanning revealed an empty uterus and a non‐viable fetus within the bladder, connected to the uterine cavity. Cystoscopy confirmed the presence of fetal parts inside the bladder. Finally, a laparotomy was performed and the fetus was removed from the bladder with repair of the underlying uterovesical fistula. An uneventful postoperative period ensued. The literature review revealed only four previously reported cases with similar overall presentations. This case highlights the importance of considering vesical ectopic pregnancies in patients with a history of cesarean sections and unusual symptoms, as prompt surgical intervention is crucial for ensuring successful management of the condition.

Abstract:Advantages of the low transverse Pfannenstiel incision include lower rates of incisional hernia, wound infection, hematoma formation, direct postoperative pain and distinctly improved aesthetic appearance. Relative disadvantages include insufficient intraoperative exposure of the upper abdomen and an increased incidence of superficial nerve entrapment. Incisional hernia following Pfannenstiel incision is a rare event with a reported incidence of between 0 and 0.5% while in contrast midline vertical incisions are considerably more common with a reported incidence of 5 to 30%. Symptomatology of Pfannenstiel incisional hernia has been reported to include: bulging, pain or discomfort, and distention of the surgical area. We report an unusual case of early postpartum unilateral vulvar edema, which led to imaging diagnosis of Pfannenstiel incisional hernia in a 41 year-old woman on Day 2 following Cesarean delivery due to preeclampsia with worsening severe features, at 30 weeks' gestation.
