Preparation of Injectable Double-Layer Microspheres for the Long-Term Treatment of Osteoarthritis

Abstract:Local injection of anti-inflammatory drugs for osteoarthritis has been widely used in clinic, and sustained-release dosage forms of anti-inflammatory drugs also show great application prospects. The current treatment of osteoarthritis is still focused on addressing the inflammatory response of patients in the acute stage of inflammation, and less treatment for the long-term prognosis of cartilage damage after inflammation attack. To solve the acute pain of osteoarthritis patients and improve the long-term prognosis of treatment, we designed a two-pronged approach: An injectable bilayer microsphere containing 'nonsteroidal anti-inflammatory drug-macrophage polarizing factor' was constructed using double emulsification. The microspheres inhibit local inflammatory cytokine production to regulate the intra-articular environment, promote macrophage polarization to the M2 phenotype and increase the expression of cartilage repair factors. The two PLGA copolymer materials maintain the biocompatibility of the microspheres and control the release sequence of the two drugs. The effect of preserving the biological activity of cytokines in the inner layer was achieved. The double-layer microspheres were delivered to the degenerative articular cavity of rats, which resisted inflammation and finally effectively promoted cartilage regeneration.
