Epicardial application of atrial radiofrequency catheter ablation for atrial fibrillation in dogs

Abstract:Objective:To understand the electrophysiological characteristics of atrial fibrillation (AF) and assess the feasibility of radiofrequency catheter ablation (RFCA) and choose the best ablation lines,Methods:RFCA of the atrial epicardium was performed after reproducible induction of sustained AF using burst stimulation during IV infusion of methacholine.Results:In 5 dogs, the first left-side RFCA did not suppress AF. then created linear lesions on the right atria and Bachman's bundle (BB), sustained AF were eliminated in 4 dogs. In another 8 dogs, right atrial lesions limited to the internal junction of the right atrial appendage and right atria, abolished sustained AF in 4 dogs and BB lesions followed by AF termination in the other 4 dogs. Atrial dectrogram types during AF showed mainly type I in atria and left pulmonary veins orifice and the atrial effective refractory period (ERP) of those sites were significantly prolonged after RFCA (P<0.05 OR 0.01).Conclusion:These results suggest that it is possible for RFCA to treat AF. In this model, RFCA at the important sites of the right atria may suppress AF and ablation at BB can increase success rate while unnecessary to place lesions on the left atria, it also indicates RFCA may mainly make ERP prolonged and produce atrial lines of conduction block to prevent the induction of AF.
