Repolarization dispersion and life-threatening ventricular tachyarrhythmias in patients with congestive heart failure

Abstract:Objective:To investigate the association between repolarization dispersion and lifethreatening ventricular tachyarrhythmias in congestive heart failure patients.Methods:Dispersion of repolarization interval was evaluated in the 12-lead surface ECG in 25 congestive heart failure (CHF) patients who suffered spontaneous life-threatening ventricular tacharrhythmia (VTA) and 57 matched arrhythmiafree CHF patients. Repolarization dispersions were quantified in terms of maximal-minimal QT/JT interval (dispersion, -d) and the coefficient of variation (cv) of QT and JT intervals.Results:Univariate analyses revealed that 2 standard ECG parameters (QTc, JTc) and 4 dispersion variables (JTcd, JTccv, QTcd and JT cv) were associated with VTA (P<0.05). Multivariat analyses revealed that the combination of the JTc-d, and QTc interval made an independent contribution to the risk of VTA.Conclusion:Increased repolarization dispersion is a marker in CHF patients who are at risk of VTA and sudden cardiac death.
