P69-8 Effectiveness of the Early Palliative Care Intervention Program on Depression and Anxiety: A Randomized Controlled Trial

Abstract:The integration of palliative care into standard cancer treatment during the early phase of the disease has been reported to improve cancer patients' mood. The current study examined the effectiveness of a nurse-led, screening triggered, early specialized palliative care intervention program (NS-EPC) for depression and anxiety of patients (pts) with advanced lung cancer. This is a secondary analysis of a randomized control of NS-EPC (J-SUPPORT1603). The intervention, which was triggered using by a brief, self-administered screening tool, comprised comprehensive need assessments, counseling, and service coordination by advanced-leveled nurses. The rate of ≥10 of depression on the Patient Health Questionnaire (PHQ-9) and the rate of ≥10 of anxiety on the Generalized Anxiety Disorder 7-item (GAD-7) were compared between the intervention group (IG) and the usual care group (UG) with marginal model. A total of 204 pts were included (102 for each group). The median age was 69 y (range, 27-91) and 77.5% were male. Seventy-two pts had extensive disease small-cell lung carcinoma and 132 pts had stage IV non-small cell lung carcinoma. Because there was not a significant time-by-group interaction, we estimated main effects and the IG did not show a significant improvement in PHQ-9 (-0.06; 95% CI, -0.13 to 0.01; P = .083) and GAD-7 (0.01; 95% CI, -0.03 to 0.05; P = .651) score from the baseline at week 12 and 20 compared to the UG. However, when we considered time-by-group interaction effects exploratorily, the IG did show significant improvement in PHQ-9 at week 20 (-0.10; 95% CI, -0.18 to -0.02; P = .019). This trial did not show statistical superiority of NS-EPC intervention over usual care, however, it's possible delayed clinical benefits of improvement in depression. This intervention program has the potential to improve depression without the need for mental health professionals.
