Analysis of death risk factors of pulmonary tuberculosis complicated with severe pneumonia

Abstract:Objective:To explore the clinical characteristics and death-related risk factors of patients with pulmonary tuberculosis complicated with severe pneumonia in tuberculosis intensive care unit(TBICU), so as to provide references for clinical diagnosis and treatment of pulmonary tuberculosis complicated with severe pneumonia.Methods:The clinical data of 212 patients with pulmonary tuberculosis complicated with severe pneumonia who were admitted to the TBICU of Public Health Clinical Center of Chengdu from 2019 to 2020 were retrospectively analyzed. According to 28-day prognosis of the disease, the patients were divided into death group(94 cases) and survival group (118 cases). The risk factors for death in patients with tuberculosis complicated with severe pneumonia were analyzed by multivariate Logistic regression model.Results:Of the 212 patients with pulmonary tuberculosis complicated with severe pneumonia, 94 died (44.34%) among whom 43 died within 7 days after admission to TBICU, accounting for 20.28% of all patients. Multivariate Logistic regression analysis showed that living in rural areas ( OR=3.210, 95% CI: 1.565-6.585), without anti-tuberculosis drug treatment before admission to TBICU( OR=4.662, 95% CI:2.292-9.484), APACHE Ⅱ score≥15 ( OR=3.143,95% CI:1.481-6.673), and need to use vasoactive drugs( OR=17.195, 95% CI:5.407-54.677) were independent risk factors for death in patients with pulmonary tuberculosis complicated with severe pneumonia. Conclusions:The prognosis of patients with pulmonary tuberculosis complicated with severe pneumonia admitted to TBICU is poor, especially patients who live in rural areas, do not receive anti-tuberculosis treatment before admission to TBICU, have APACHE Ⅱ score ≥ 15 and need to use vasoactive drugs. These patients need early attention and early intervention to improve the prognosis.
