Abstract:Background. Anemia is associated with increased mortality risk. The impact of mildly low hemoglobin concentration (Hb) on risk for mortality remains unclear, especially among blacks. We examined the racial differences between Hb and mortality.Methods. This was a population-based study conducted from 1993 through 2006, in a geographically defined community of Chicago, Illinois. A stratified, random sample of 1806 participants 65 years old or older and 50% black, who were participating in the Chicago Health Aging Project and underwent clinical evaluation. Mortality was ascertained using the National Death Index. Cox proportional hazard models were used to assess the independent relation of Hb to mortality risk.Results. The proportion of participants with anemia by World Health Organization (WHO) criteria (Hb < 13.0 g/dL for men and < 12.0 g/dL for women) was 39% among blacks, and 17% among whites. Blacks had lower mean Hb (12.6 +/- 1.5 g/dL) than did whites (13.5 +/- 1.5 g/dL). In multivariable analysis, anemia was associated with increased mortality risk in blacks (hazard ratio [HR], 1.90; 95% confidence interval [CI], 1.43-2.53) and in whites (HR, 1.85: 95% CI, 1.32-2.59). Among blacks, Hb 0-0.9 g/dL below the anemia threshold is associated with increased mortality risk compared to Hb 0-0.9 g/dL above the anemia cutoff (HR, 1.84; 95% CI, 1.21-2.79), Hb 1.1-2.0 g/dL above the anemia cutoff (HR, 1.35: 95% CI, 0.88-2.05) and Hb 2.1-3.0 g/dL above the anemia cutoff (HR, 2.24: 95% CI, 1.12-4.47). The terms for interaction between black ethnicity/race and anemia suggested that blacks did not have a statistically significant difference in mortality risk compared to whites. Subgroup analyses of interaction terms suggested that Hb 0.1-1.0 g/dL above anemia cutoff group, blacks may have lower mortality risk compared to whites in the mildly low normal ranges of Hb (p = .02).Conclusion. Both anemia by WHO criteria and mild reductions in Hb were related to increased risk of mortality in older blacks and whites.

Abstract:Anemia is a global public health problem affecting both developing and developed countries at all ages. According to the World Health Organization (WHO), anemia is defined as hemoglobin (Hb) levels <12.0 g/dL in women and <13.0 g/dL in men. However, normal Hb distribution varies not only with sex but also with ethnicity and physiological status. New lower limits of normal Hb values have been proposed, according to ethnicity, gender, and age. Anemia is often multifactorial and is not an independent phenomenon. For the classification and diagnosis the hematologic parameters, the underlying pathological mechanism and patient history should be taken into account. The aging of population, especially in Western countries, causes an increase of anemia in elderly people. In this population, anemia, recently defined by levels of Hb <12 g/dL in both sexes, is mostly of mild degree (10-12 g/dL). Understanding the pathophysiology of anemia in this population is important because it contributes to morbidity and mortality. In one third of the patients, anemia is due to nutritional deficiency, including iron, folate, or vitamin B12 deficiency; moreover, anemia of chronic disease accounts for about another third of the cases. However, in one third of patients anemia cannot be explained by an underlying disease or by a specific pathological process, and for this reason it is defined "unexplained anemia". Unexplained anemia might be due to the progressive resistance of bone marrow erythroid progenitors to erythropoietin, and a chronic subclinical pro-inflammatory state.

Abstract:Abstract Iron is unique among all minerals in that humans have no regulatable excretory pathway to eliminate excess iron after it is absorbed. Iron deficiency anemia occurs when absorbed iron is not sufficient to meet body iron demands, whereas iron overload and subsequent deposition of iron in key organs occur when absorbed iron exceeds body iron demands. Over time, iron accumulation in the body can increase risk of chronic diseases, including cirrhosis, diabetes, and heart failure. To date, only ∼30% of the interindividual variability in iron absorption can be captured by iron status biomarkers or iron regulatory hormones. Much of the regulation of iron absorption may be under genetic control, but these pathways have yet to be fully elucidated. Genome-wide and candidate gene association studies have identified several genetic variants that are associated with variations in iron status, but the majority of these data were generated in European populations. The purpose of this review is to summarize genetic variants that have been associated with alterations in iron status and to highlight the influence of ethnicity on the risk of iron deficiency or overload. Using extant data in the literature, linear mixed-effects models were constructed to explore ethnic differences in iron status biomarkers. This approach found that East Asians had significantly higher concentrations of iron status indicators (serum ferritin, transferrin saturation, and hemoglobin) than Europeans, African Americans, or South Asians. African Americans exhibited significantly lower hemoglobin concentrations compared with other ethnic groups. Further studies of the genetic basis for ethnic differences in iron metabolism and on how it affects disease susceptibility among different ethnic groups are needed to inform population-specific recommendations and personalized nutrition interventions for iron-related disorders.

Race, Social Determinants of Health, and Comorbidity Patterns Among Participants with Heart Failure in the REasons for Geographic and Racial Differences in Stroke (REGARDS) Study

Abstract:Background: Among individuals with heart failure (HF), racial differences in comorbidities may be mediated by social determinants of health (SDOH). Methods: Black and White US community-dwelling participants in the REasons for Geographic and Racial Differences in Stroke (REGARDS) study aged ≥ 45 years with an adjudicated HF hospitalization between 2003 and 2017 were included in this cross-sectional analysis. We assessed whether higher prevalence of comorbidities in Black participants compared to White participants were mediated by SDOH in socioeconomic, environment/housing, social support, and healthcare access domains, using the inverse odds weighting method. Results: Black (n = 240) compared to White (n = 293) participants with HF with preserved ejection fraction (HFpEF) had higher prevalence of diabetes [1.38 (95% CI: 1.18 - 1.61)], chronic kidney disease [1.21 (95% CI: 1.01 - 1.45)], and anemia [1.33 (95% CI: 1.02 - 1.75)] and lower prevalence of atrial fibrillation [0.80 (95% CI: (0.65 - 0.98)]. Black (n = 314) compared to White (n = 367) participants with HF with reduced ejection fraction (HFrEF) had higher prevalence of hypertension [1.04 (95% CI: 1.02 - 1.07)] and diabetes [1.26 (95% CI: 1.09 - 1.45)] and lower prevalence of coronary artery disease [0.86 (95% CI: 0.78 - 0.94)] and atrial fibrillation [0.70 (95% CI: 0.58 - 0.83)]. Socioeconomic status explained 14.5%, 26.5% and 40% of excess diabetes, anemia, and chronic kidney disease among Black adults with HFpEF; however; mediation was not statistically significant and no other SDOH substantially mediated differences in comorbidity prevalence. Conclusions: Socioeconomic status partially mediated excess diabetes, anemia, and chronic kidney disease experienced by Black adults with HFpEF, but differences in other comorbidities were not explained by other SDOH examined.

Abstract:Background Although anemia is common in patients with myocardial infarction (MI), management remains controversial. We quantified the association of anemia with in-hospital outcomes and resource utilization in patients admitted with MI using a large national database. Methods All hospitalizations with a primary diagnosis code for acute MI in the National Inpatient Sample (NIS) between 2014 and 2018 were identified. Among these hospitalizations, patients with anemia were identified using a secondary diagnosis code. Data on demographic and clinical variables were collected. Outcomes of interest included in-hospital adverse events, length of stay (LOS), and total cost. Multivariable logistic regression and generalized linear models were used to evaluate the relationship between anemia and outcomes. Results Among 1,113,181 MI hospitalizations, 254,816 (22.8%) included concomitant anemia. Anemic patients were older and more likely to be women. After adjustment for demographics and comorbidities, anemia was associated with higher mortality (7.1 vs. 4.3%; odds ratio 1.09; 95% confidence interval [CI] 1.07–1.12, p < 0.001). Anemia was also associated with a mean of 2.71 days longer LOS (average marginal effects [AME] 2.71; 95% CI 2.68–2.73, p < 0.05), and 9703, 95% CI 9829, p < 0.05). Anemic patients who received blood transfusions had higher mortality as compared with those who did not (8.2% vs. 7.0, p < 0.001). Conclusion In MI patients, anemia was associated with higher in-hospital mortality, adverse events, total cost, and length of stay. Transfusion was associated with increased mortality, and its role in MI requires further research.

Abstract:Introduction Environmental, socioeconomic and medical risk factors’ among patients with heart failure (HF) with reduced ejection fraction (HFrEF) may cause clinical and prognostic differences. We studied the racial and ethnic prognostic implications of HFrEF medical management among outpatients treated in our inner-city practice. Methods In a retrospective protocol, we studied patients referred to the HF clinic at our institution from October 2020 to October 2021. Clinical, hemodynamic, echocardiographic, socio-economic data and the use of guideline-directed medical therapy (GDMT) were collected. Patients were followed for a median of 16.7 months for HF hospitalizations, mortality as well as their composite. Results In the study period, 506 patients were included (67.4±14 years, 46% women, LVEF: 32.9±13%). 69(13%) patients were of white race, 99(20%) were Hispanics, 267(53%) were African Americans (AA), and 71(14%) patients were of other races. During follow-up, 155(31%) patients had HF admissions, 60(12%) died and 180(36%) had the combined outcomes. Figure 1A shows basic differences stratified by race. Regarding GDMT, AA and Hispanic patients showed the least use of individual GDMT and the least use of the full set of GDMT. Importantly, AA and Hispanics had more medication non-adherence, while AA showed the most clinic visits no shows. Interestingly, apart from more chronic kidney disease (CKD) in AA and Hispanics, other common medication side effects such as hypotension, bradycardia, hyperkalemia were not different between races. Importantly, more deaths, HF hospitalization, and combined outcomes (Figure 1 A to D) occurred in Hispanics and AA compared to other groups (Figure 1A). Survival analyses for the risk of combined outcomes showed that whites did not have different risks (HR: 1.7, 95% CI: 0.79 to 3.7, Figure), while Hispanics (HR: 2.1, 95% CI: 1.1 to 4.3) and AA (HR: 3.7, 95% CI: 2 to 6.9) had higher risk compared to other races. After adjusting for covariates (age, sex, individual GDMT, CRT, CKD, GFR, EF, medication non-adherence, and clinic no-shows), only AA continued to have elevated risk for combined outcomes (HR: 2.9, 95% CI: 1.5 to 5.6), while whites and Hispanics lost statistical significance. Conclusions Racial and ethnic disparities had prognostic significance in the outpatient management of HFrEF in terms of worse hospitalizations and mortality in the Hispanic and AA populations. Such findings may be attributed to patient factors (less medication and clinic visits non-adherance), and physician factors (stopping medications due to renal dysfunction). AA had worse outcomes even after adjusting for covariates suggesting further studies are needed to determine a more effective therapeutic approach in this population.

Abstract:Objectives To determine the trends in anemia prevalence, treatment rates, and their associated demographic and socioeconomic status factors in the U.S. nationally representative population. Methods We extracted data from individuals >= 20-year-old from the NHANES from 2005 to 2018. The trends of anemia prevalence and treatment were assessed by Cochran-Armitage test. The association between two dependent variables (anemia status and treatment) and demographic were examined by multiple logistic regression. Results The general anemia prevalence has a significant increasing trend from 5.71% to 6.86% (P trend<.001) from 2005 to 2018, whereas the trend in general anemia treatment rate was not significant (p = .148). Logistic regression showed that age groups (OR: 2.15for 60 or older vs. 20-39 years old), sex (0.47 for male vs. female), race (0.22 for NHW vs. NHB), education (0.80for some college or AA degree vs. less than high school), PIR (0.60 for >= 3.5 vs.<= 1.3), and birthplace (0.88 for U.S. vs. other) were significantly associated with anemia. The factors significantly associated with anemia treatment were age group (1.51 for 60 years and older vs. 20-39 years old), sex (0.53 male vs. female), birthplace (1.88 U.S. vs. other), and insurance (1.80 for Yes vs. No). Conclusion While there was no significant increase in anemia treatment between 2005 and 2018, the anemia prevalence increased significantly, particularly among individuals with lower income, less education, and born outside the U.S. Moreover, the high anemia prevalence among NHB compared to other races indicated that more resources are needed to reduce disparities.

Anemia in elderly rheumatoid arthritis patients: a cohort study

Abstract:Introduction Anemia is common in elderly patients with rheumatoid arthritis (RA). This study aimed to evaluate the current status and influencing factors of anemia in RA patients, to provide insights for clinical RA treatment and care. Material and methods This study included elderly patients with high RA activity treated in our hospital from June 1, 2021 to December 31, 2022 for analysis. The characteristics of RA patients were analyzed. Pearson correlation and logistic regression analysis were conducted to analyze the influencing factors of anemia in elderly patients with RA. Results A total of 285 RA patients were included. The incidence of anemia in high RA activity patients was 62.46%. There were significant differences in the course of RA, hemoglobin (Hb), low density lipoprotein cholesterol (LDL-C), platelet/lymphocyte rate (PLR) and albumin (Alb) between RA patients with anemia and without anemia (all p < 0.05). Pearson correlation analysis indicated that course of RA (r = 0.522), Hb (r = 0.797), LDL-C (r = 0.558), PLR (r = 0.615) and Alb (r = 0.604) were correlated with anemia in patients with high RA activity (all p < 0.05). Logistic regression analysis indicated that course of RA ≥ 8 years (OR = 2.584, 95% CI: 1.822–3.647), LDL-C ≤ 2.8 mmol/l (OR = 3.202, 95% CI: 2.804–3.431), PLR ≥ 8 (OR = 2.183, 95% CI: 1.744–2.457), Alb ≤ 35 g/l (OR = 1.716, 95% CI: 1.401–2.006) were the risk factors of anemia in elderly patients with high RA activity (all p < 0.05). Conclusions Anemia in elderly patients with high RA activity is closely related to the course of RA, LDL-C, PLR and Alb. Close attention should be paid to the monitoring of those indicators to take early intervention measures to improve the prognosis of RA patients.

Relationship between Anemia and Academic Performance in Chinese Primary School Students: Evidence from a Large National Survey

Abstract:Anemia is a global public health problem, especially common among children in developing countries, which affects their physical and mental health development. However, there is currently a lack of research on the relationship between anemia and academic performance. The objective of this study was to explore the association between anemia and academic performance, and the possible factors mediating this association among Chinese children. The data for this study came from the baseline survey of the Chinese Education Panel Survey Elementary School Cohort. The cohort was conducted from September 2018 to June 2019 in 160 elementary schools, covering 20 provinces and 40 counties/districts throughout China. Paper-based questionnaires were used, completed by 4th grade students, parents, head teachers, main teachers, and principals. The data used included questionnaire responses, physical measurements, and academic performance of 17,695 students. Based on students’ hemoglobin levels and school altitude data, we grouped them into anemia and nonanemia categories using WHO criteria. The anemia group had 1,154 individuals, while the nonanemia group had 16,541 individuals. An ordinary least squares regression and mediation effect analysis were conducted. Our findings found the prevalence of anemia was 6.52% among Chinese Grade 4 students. Students without anemia had a higher average test score for three academic subjects than students with anemia ( P < 0.001 ); their test scores for Chinese, Mathematics, and English were also higher ( P < 0.05 ). Multivariate regression analysis showed a negative association between anemia and average test scores as well as individual test scores for the three subjects. Mediation analysis found that anemia affected academic performance directly ( P < 0.05 ), and indirectly by decreasing the cognition score ( P < 0.05 ). The indirect effect was 19.9% of the total effect. Findings highlighted anemia affected academic performance both directly and indirectly. Nutrition-related interventions should be implemented to prevent a decrease in academic performance among students with anemia.

Abstract:Anemia in children impairs physical growth and cognitive development, reducing their overall human capital accumulation. While much research has been conducted on anemia prevalence in the primarily poor and rural western provinces in China, little is known about anemia in the more developed provinces of central China. The overall goal of this study is to assess the extent of anemia in central China and determine the effect of anemia on the academic performance of students. Using data collected from fourth grade students in 25 primary schools, we find that 16–27% of sample children are anemic. Female students and students with mothers who have not migrated for work are more likely to be anemic. Importantly, using both regression analysis and matching methods, we find that students with anemia (and those with low hemoglobin levels) are more likely to perform poorly on standardized mathematics exams. These findings suggest that, over the long term, untreated anemia will perpetuate poverty by restricting the human capital development of affected children.
Association Between Anemia and Outcome in Patients Hospitalized for Acute Heart Failure Syndromes: Findings from Beijing Acute Heart Failure Registry (beijing AHF Registry)

Abstract:Whether the anemia increases the risk of mortality in patients with acute heart failure (AHF) remains unclear. This study aims to explore the relationship between anemia and outcomes in patients with AHF including subgroup analysis. This study included 3279 patients with hemoglobin available from the Beijing Acute Heart Failure Registry (Beijing AHF Registry) study. The primary endpoint was all-cause mortality in 1 year, and the secondary endpoint was 1-year all-cause events including all-cause death and readmission. Logistic regression models were applied to describe related variables of anemia in patients with AHF. Multivariate Cox proportional hazards models described associations of anemia with clinical outcomes in the overall cohort and subgroups. 45.4% of the patients were found anemic. They were older and had more comorbidities than non-anemic patients. Variables including older age, female, chronic kidney dysfunction (CKD), lower hematocrit, lower albumin, with loop diuretics applied, without beta-blockers, angiotensin-converting enzyme inhibitors /angiotensin receptor blockers (ACEIs/ARBs) and spironolactone applied in the emergency department (ED) were associated with anemia in AHF patients. Anemic patients had higher 1-year mortality (38.4% vs. 27.2%, p < 0.0001) and 1-year events rates (63.2% vs. 56.7%, p < 0.0001). After adjusted for covariates, anemia was associated with the increase of 1-year mortality (hazard ratio [HR] 1.278; 95% confidence interval [CI] 1.114–1.465; p = 0.0005) and 1-year events (HR 1.136; 95% CI 1.025–1.259; p = 0.0154). The severer anemia patients had higher risks both of 1-year mortality and events. In the subgroup analysis, the independent associations of anemia with 1-year mortality were shown in the subgroups including age < 75 years, male, body mass index < 25 kg/m2 and BMI ≥ 25 kg/m2, New York Heart Association (NYHA) functional class I–II and NYHA functional class III–IV, with and without cardiovascular ischemia, heart rate (HR) < 100 bpm and HR ≥ 100 bpm, systolic blood pressure (SBP) < 120 mmHg and SBP ≥ 120 mmHg, left ventricular ejection fraction (LVEF) < 40% and LVEF ≥ 40%, serum creatinine (Scr) < 133 umol/l, and with diuretics use, with and without beta-blockers use, without ACEIs/ARBs use in the ED. Anemia is associated with older age, female, CKD, volume overload, malnutrition, with loop diuretics, without beta-blockers, ACEIs/ARBs and spironolactone administration, and higher mortality and readmission in AHF. The risk associations are particular significantly obvious in younger, male, overweight, preserved LVEF, lower Scr, with diuretics and beta-blockers, without ACEIs/ARBs administration subgroups. Anemia, hemoglobin concentration and cognitive function in the Longitudinal Ageing Study in India-Harmonized Diagnostic Assessment of Dementia (LASI-DAD) and the Health and Retirement Study

Abstract:Background: In India, anemia is widely researched in children and women of reproductive age, however, studies in older populations are lacking. Given the adverse effect of anemia on cognitive function and dementia this older population group warrants further study. The Longitudinal Ageing Study in India - Harmonized Diagnostic Assessment of Dementia (LASI-DAD) dataset contains detailed measures to allow a better understanding of anaemia as a potential risk factor for dementia. Method: 2,758 respondents from the LASI-DAD cohort, aged 60 or older, had a complete blood count measured from venous blood as well as cognitive function tests including episodic memory, executive function and verbal fluency. Linear regression was used to test the associations between blood measures (including anemia and hemoglobin concentration (g/dL)) with 11 cognitive domains. All models were adjusted for age and gender with the full model containing adjustments for rural location, years of education, smoking, region, BMI and population weights.Results from LASI-DAD were validated using the USA-based Health and Retirement Study (HRS) cohort (n=5720) to replicate associations between blood cell measures and global cognition. Results: In LASI-DAD, we showed an association between anemia and poor memory (p=0.0054). We found a positive association between hemoglobin concentration and ten cognitive domains tested (β=0.041-0.071, p<0.05). The strongest association with hemoglobin was identified for memory-based tests (immediate episodic, delayed episodic and broad domain memory, β=0.061-0.071, p<0.005). Positive associations were also shown between the general cognitive score and the other red blood count tests including mean corpuscular hemoglobin concentration (MCHC, β=0.06, p=0.0001) and red cell distribution width (RDW, β =-0.11, p<0.0001). In the HRS cohort, positive associations were replicated between general cognitive score and other blood count tests (Red Blood Cell, MCHC and RDW, p<0.05). Conclusion: We have established in a large South Asian population that low hemoglobin and anaemia are associated with low cognitive function, therefore indicating that anaemia could be an important modifiable risk factor. We have validated this result in an external cohort demonstrating both the variability of this risk factor cross-nationally and its generalizable association with cognitive outcomes.

Abstract:Background Potential differences in the phenotype of heart failure with preserved ejection fraction (HFpEF) by race or ethnicity has not been assessed in detail. We aimed to characterize racial differences in clinical correlates, cardiac structures, functions, and prognostic importance in HFpEF among Asian, White, and Black populations. Methods and Results A total of 360 Asian, 787 White, and 171 Black patients with HFpEF underwent transthoracic echocardiography were evaluated and followed up for all-cause mortality. The Asian HFpEF group was the oldest and the Black HFpEF group youngest (75±12 vs. 73±13 vs. 62±12 years, p<0.0001) (Figure A). Women constituted the lowest proportion of patients with HFpEF among Asians and but were the highest among Black individuals with HFpEF (49% vs. 56% vs. 73%, p<0.0001) (Figure B). Body mass and obesity prevalence were lower in Asian HFpEF patients and highest in Black HFpEF patients (Figure C). Black individuals with HFpEF had greater LV wall thickening, smaller LV chamber size, and greater LV concentricity (Figure D), with smaller LA volumes (Figure E), but more RV dilatation. Asian individuals with HFpEF had greater LV and LA dilation, and higher LA/LV volume ratio. The Asian HFpEF group had the highest arterial afterload, as assessed by the aortic elastance and systemic vascular resistance indices (Figure F). There were no racial differences in mortality after adjusting for age, sex, and BMI, but multiple cardiac structure/functional changes were differentially associated with increased risk of death. Conclusions Patients with HFpEF of Asian, White, and Black race display differences in age, sex, and body mass, as well as differing changes in cardiac structure and function. While overall survival did not differ, there were distinct relationships between cardiac abnormalities and outcome by race. Consideration of race-related biological differences may be important to individualize risk stratification and potentially treatment strategies in HFpEF.

Association Between Anemia and Frailty in 13,175 Community-Dwelling Adults Aged 50 Years and Older in China

Abstract:Background: Anemia and frailty contribute to poor health outcomes in older adults; however, most current research in lower income countries has concentrated on anemia or frailty alone rather than in combination. The aim of the present study was to investigate the association between anemia and frailty in community-dwelling adults aged 50 years and older in China. Methods: The study population was sourced from the 2007/10 SAGE China Wave 1. Anemia was defined as hemoglobin less than 13g/dL for men and less than 12g/dL for women. A Frailty Index (FI) was compiled to assess frailty. The association between anemia and frailty was evaluated using a 2-level hierarchical logistic model. Results: The prevalence of anemia was 31.0% (95%CI: 28.4%, 33.8%) and frailty 14.7% (95%CI: 13.5%, 16.0%). In the univariate regression model, presence of anemia was significantly associated with frailty (OR=1.62, 95% CI: 1.39, 1.90) and the effect remained consistent after adjusting for various potential confounding factors including age, gender, residence, education, household wealth, fruit and vegetable intake, tobacco use, alcohol comsumption and physical activity (adjusted OR =1.31, 95% CI:1.09, 1.57). Each 1 g/dL increase in hemoglobin concentration was associated with 4% decrease in the odds of frailty after adjusting for several confounding variables (adjusted OR =0.96, 95% CI: 0.93, 0.99). Conclusion: Anemia and low hemoglobin concentrations were significantly associated with frailty. Therefore, health care professionals caring for older adults should increase screening, assessment of causes and treatment of anemia as one method of avoiding, delaying or even reversing frailty.
Relationships of hematocrit concentration with dementia from a multiethnic population-based study

Abstract:Objective: Red blood cell (RBC) concentration impacts cerebrovascular disease, yet it is unclear whether RBC concentrations relate to dementia risk, particularly in racially/ethnically diverse cohorts. We investigated whether RBC concentrations associate with incident dementia risk in a diverse population of stroke-free individuals and explored whether cerebral small vessel disease (CSVD) mediates this relationship. Methods: A longitudinal observational analysis was performed using a population-based cohort of stroke-free, older adult participants (>50 years) from the Northern Manhattan Study (NOMAS) enrolled between 2003-2008. Participants received baseline hematocrit testing, MRI neuroimaging, and cognitive assessments at baseline and long-term follow-up. Associations of baseline hematocrit as a categorical variable (low, normal [reference], and high based on laboratory reference levels) with incident dementia were assessed using Cox models adjusting for relevant covariates. Separate analyses investigated whether MRI CSVD mediated these relationships. Results: We studied 1207 NOMAS participants (mean age 71±9 years, 60% female, 66% Hispanic). Mean hematocrit was 41.2% (±3.8) with 16% of participants developing incident dementia. Lower hematocrit associated with increased dementia risk (adjusted hazard ratio 1.81 [1.01-3.23]) after adjusting for age, sex, race/ethnicity, education, APOE status, and comorbidities. High hematocrit was not associated with dementia risk. No interactions by sex or race/ethnicity were seen and baseline CSVD did not mediate relationships between hematocrit and dementia. Conclusions: Low hematocrit associated with dementia risk in our diverse population cohort. Further work is needed to assess mechanisms behind anemia's relationship with dementia to assess whether this can serve as a trackable, preventable/treatable risk factor for dementia.
Anemia prevalence among ethnic minority schoolchildren in an undeveloped city in southwest China

Abstract:Objectives: Anemia is one of the most important nutritional deficiency diseases among schoolchildren worldwide. More attention needs to be focused on anemia prevalence in ethnic minorities in the multiethnic country of China. This study was conducted in Mengzi City, Yunnan Province, which has the largest variety of ethnic minorities in China, to investigate the anemia prevalence among ethnic minority schoolchildren ages 6 to 15 y. Methods: The plasma hemoglobin concentration of all participants was examined once a year from 2014 to 2020. Anemia was defined as a blood hemoglobin level below 11g/dL, 12g/dL, 13g/dL and 12g/dL for schoolchildren ages 5 to 11y, 12 to 14 y, ages 15 to 19 y males and females, respectively. Results: The overall anemia prevalence (7.3%) was higher than the national level (6.1%). The prevalence among ethnic minorities (8.2%) was higher than among Han Chinese (6.8%), and the prevalence was higher among 6-year-olds (12.2%), females (8.0%), those of Hani Nationality (8.7%), and underweight students (8.5%). The overall prevalence trend of ethnic minorities rose continuously from 2014 to 2020. Ethnic minorities had a higher risk of anemia than did Han Chinese. Overweight schoolchildren (odds ratio [OR], 0.816) and those with obesity (OR, 0.710) had a lower risk of anemia than did students with normal-weight status. Conclusions: Although the prevalence of anemia among schoolchildren in Mengzi is a mild public health problem, the rate among ethnic minorities is consistently higher than among Han Chinese and keeps rising. Measures to prevent and control anemia need to be prioritized in ethnic minorities, particularly those of Hani Nationality, females, and underweight schoolchildren.
