Abstract:Objective To analyze clinical characteristics,sex and age distributions,and treatment of hypertensive patients related obstructive sleep apnea syndrome (OSAHS),to understand the importance of respiratory intervention eombined with OSAHS in the treatment of hypertension.Methods Retrospective study was used to analyze 587 inpatients who were diagnosed with sleep apnea syndrome combined,among these patients,383 with hypertension in our hospital from April 2012 to May 2013.And 144 patients received at least 3 months continuous positive airway pressure (CPAP) therapy.The study aims to analyze the clinical characteristics,sex and age distributions,and respiratory intervention respiratory intervention treatment status of these patients.Results Among the 383 sleep apnea syndrome related hypertensive patients,patients developed hypertension younger than the general population and the patients younger than 45 years old with diagnosis of hypertension accounted for 52.9％.There were 306 males and 77 females among all patients with the ratio of 3.91 to 1.After CPAP treatment,the systolic blood pressures,diastolic blood pressures,BMI were significantly decreased [(130.0 ± 7.6) mmHg vs (150.0 ± 12.2)mmHg,(85.0±2.l)mmHg vs (93.0 ±2.3)mmHg,(24.8 ±2.6)kg/m2 vs (26.9±2.7) kg/m2] (P＜ 0.05).Sleep apnea hypopnea index and night minimum oxygen saturation were significantly improved [(3.5 ± 1.2) vs (48.5±10.6),(89.5 ±5.2)％ vs (75.1 ±8.5)％] (P＜0.01).Some of the patients reduced or stopped antihypertensive medications and only used CPAP to keep the normal blood pressure.Conclusions Sleep apnea syndrome related hypertensive patients the age of onset developed hypertension with young tendency.After CPAP treatment,the sleep respiratory disorder is greatly improved and blood pressure is well controlled,indicating that the screening for OSAHS is essential for hypertensive patients.CPAP treatment is an effective antihypertensive treatment for the hypertension combined with OSAHS patients.

Abstract:Objective To investigate the effects of angiotensin converting enzyme inhibitors (ACEI) on apnea and hypopnea in patients with obstructive sleep apnea-hypopnea syndrome(OSAHS)complicated by hypertension. Methods 63 subjects with OSAHS examined by polysomnography (PSG)were enrolled in this study (male 54, female 9), average age ( 53 ± 12 ) years. The subjects were divided into 3 groups, group A(pure OSAHS group, n = 21), group B(hypertension patients with OSAHS without antihypertensive treatment group, n = 19), group C(hypertension patients with OSAHS with ACEI group, n =23). All subjects underwent 8-hour PSG monitoring and their data were analyzed. The apnea-hypopnea index (AHI) and lowest SaO2 (LSaO2) were compared among the three groups. Results The AHI of group C were lower significantly than that of group B [(21. 32±3. 53) n/h vs (38. 99±2.51) n/h,P ＜0. 05], while LSaO2 of group C were higher significantly than that of group B [(79. 43±7. 54) % vs(72.56±10. 5) %, P ＜0.05]. There was no significant difference in AHI and LSaO2 between group A and group C. Conclusions ACEI may have improving effects on apnea and hypopnea in hypertension patients with OSAHS.

Abstract:OBJECTIVETo evaluate the effects of oral appliance (OA) treatment upon systemic blood pressure (BP) in mild to moderate patients with obstructive sleep apnea-hypopnea syndrome (OSAHS).METHODSForty-six consecutive patients diagnosed with OSAHS on polysomnography were divided into OA treatment group (OA group, 25 patients, 15 patients with hypertension) and non-tolerated OA treatment group (N-OA group, 21 patients, 13 patients with hypertension). Polysomnography and 24-hour ambulatory blood pressure monitoring (ABPM) were performed at baseline in two groups. Polysomnography and ABPM were repeated after a completion of 12 weeks of treatment in OA group and after a cessation of treatment for 12 weeks in N-OA group. Hypertensive patients in two groups continued taking the same kind and the same dose of antihypertensive agents during the period of study.RESULTSThere was no significant difference between the two groups in age, body mass index, Epworth sleepiness score (ESS), apnoea-hypopnoea index (AHI), arousal index (AI) and minimum arterial oxygen saturation (MSaO2) at baseline. After a 12-week treatment, OA group showed significant improvement in AHI [(7.0 +/- 3.8) vs (21.0 +/- 6.5) per hour, P < 0.01], AI [(22.9 +/- 6.3) vs (32.2 +/- 9.3) per hour, P < 0.01] and MSaO2 (86.8% +/- 3.5% vs 80.0% +/- 5.2%, P < 0.01), while nocturnal mean systolic blood pressure (SBP) and diastolic blood pressure (DBP), 24-hour and diurnal SBP, and nocturnal mean artery pressure (MAP) were significantly reduced [(121.3 +/- 7.0) vs (125.3 +/- 9.3), (76.1 +/- 6.1) vs (78.8 +/- 6.8), (127.2 +/- 7.5) vs (129.4 +/- 8.8), (131.5 +/- 6.9) vs (133.6 +/- 8.1), and (91.2 +/- 6.4) vs (94.3 +/- 7.6) mm Hg respectively, all P < 0.01]. The reduction in nocturnal MAP was significantly correlated to improvement in AI(r = 0.37, P = 0.005) and AHI (r = 0.32, P = 0.011), to baseline nocturnal mean blood pressure (SBP: r = 0.39, P = 0.015; DBP: r = 0.30, P = 0.024). The N-OA group showed no differences in blood pressure variables between baseline and after a cessation of treatment for 12 weeks.CONCLUSIONOral appliance treatment for mild to moderate OSAHS may lead to a reduction in systemic blood pressure.

Abstract:Background Obstructive sleep apnea-hypopnea syndrome (OSAHS) is common in people with hypertension and cardiac rhythm disorder. The objectives of this study were to determine the effect of short-term continuous positive airway pressure (CPAP) on blood pressure (BP) and cardiac rhythm in Chinese patients with moderate to severe OSAHS. Patients and methods Eligible patients in the two hospitals were consecutively enrolled into the prospective study. Ambulatory BP monitoring and Holter monitoring were both performed for 24h in 214 patients who previously encountered a full night polysomnography. Ambulatory BP was measured again in the follow-up of 59 patients with OSAHS who underwent home CPAP for 30 days, whereas Holter was repeated within 2-3 days after institution of CPAP therapy in 15 patients with OSAHS. Results Fifty-one patients with OSAHS with hypertension who used CPAP for at least 4h/night received 30 days of CPAP treatment. Added CPAP on usual antihypertension treatment showed that systolic BP, diastolic BP, and mean arterial BP were significantly reduced at night (5.08, 3.05, and 3.73 mmHg, respectively), in the morning (6.31, 4.83, and 5.32 mmHg, respectively), and during the whole a day (3.09, 2.60, and 2.76 mmHg, respectively). There were no significant changes in daytime BP values but did reduce daytime BP by 2.09, 2.37, and 2.28 mmHg, respectively. In addition, CPAP therapy resulted in abolition of most sinus pauses and atrioventricular block in the 15 patients with OSAHS having coexisting pathologically rhythm disturbances, whereas the effect on other types of arrhythmia was not effective enough. Conclusion Short-term CPAP reduced BP modestly in patients with OSAHS with hypertension, especially in the morning and at night-time. Copyright (C) 2018 Wolters Kluwer Health, Inc. Abstract:Objective To explore the influence of continuous positive airway pressure(CPAP) treatment on blood pressure in patients with obstructive sleep apnea hypopnea syndrom(OSAHS) combined hypertension. Methods We searched literature references through foreign language database like Medline, Web of knowledge, EMBASE, the Cochrane Library, Springerlink databases(between January 2000 and July 2013), and domestic database like CNKI, VIP, Wangfang(between January 2000 and July 2013). Nine papers were selected from 290 related literatures. Investigators, using a standardized protocol to collect data including sample sizes, participants characteristics, research design, intervention methods, periods, and the treatment results. Nine tests were analyzed using random effects model. Results Compared with the control group, CPAP group had decreased average net systolic blood pressure by 2.57 mmHg(P0.01), lower average net diastolic blood pressure of 4.91 mmHg(P0.05). Mean arterial pressure was lower 6.07 mmHg than the control(P0.05). Net changes of blood pressure between the daytime and nighttime were not obvious. Conclusion CPAP can reduce blood pressure of OSAHS patients in systolic blood pressure, diastolic pressure and mean arterial pressure.

Abstract:Aims: Arterial hypertension is highly prevalent and difficult to control in patients with obstructive sleep apnea (OSA). High sympathoadrenergic activity is a hallmark physiological phenomenon in OSA. We hypothesized that an antihypertensive drug with inhibitory properties on this activity, such as beta blockers (BBs), may be particularly efficacious in OSA patients. Methods: Hypertensive OSA patients receiving blood pressure-lowing treatment in the European Sleep A pnea Database (ESADA) (n =5818, 69% men, age 58 +/- 11 years, body mass index 33 +/- 7 kg/m(2), apnea hypopnea index 34 +/- 26 events/h) were analyzed. Reported medications [BB, diuretic, renin-angiotensin blocker (RAB), calcium channel blocker (CCB), and centrally acting antihypertensive (CAH)] were classified according to ATC code. Office blood pressure was compared in patients with monotherapy or combination therapy controlling for confounders. Results: Poorly controlled SBP according to the ESC/ESH guidelines was found in 66% of patients. Patients receiving monotherapy with RAB, CCB or CAH had 2.2 (95% CI 1.4-3.0), 3.0 (1.9-4.1) and 3.0 (1.7-4.7) mmHg higher SBP compared with those on BB (adjusted model, P=0.007, 0.008 and 0.017, respectively). In those with a combination of two antihypertensive drugs, SBP was 5.5 (4.0-7.1), 5.1 (3.7-6.6), 4.3 (2.5-6.1) and 3.1 (1.6-4.6) mmHg higher in those on CCB/RAB, BB/RAB, BB/CCB or diuretic/RAB compared with those on BB/diuretic (adjusted model, P< 0.001, <0.001, 0.018 and 0.036, respectively). Conclusion: Poorly controlled blood pressure was common in OSA patients with antihypertensive medication. Treatment with BB alone or BB in combination with a diuretic was associated with the lowest systolic pressure in this large clinical cohort.

Abstract:Objective: In a randomised controlled trial, we investigated the blood pressure (BP) lowering effect of continuous positive airway pressure (CPAP) in patients with moderate-severe obstructive sleep apnoea syndrome (OSAS, an apnoea-hypopnoea index, AHI of 15 or higher) and nocturnal hypertension (night-time systolic/diastolic BP ≥120/70 mmHg).Methods: Sixty patients were randomly assigned to CPAP or sham CPAP, while maintaining their antihypertensive treatment. Ambulatory BP monitoring was performed at baseline (first run-in visit) and the end of follow-up. Clinic and home BP were measured at baseline and each of the monthly follow-up visits.Results: Of the 60 patients, 47 completed the 3-month study. CPAP (n = 26), compared with sham CPAP (n = 21), slightly and non-significantly reduced 24-h systolic/diastolic BP by −2.8/−2.5 mmHg (p ≥ 0.27), with a slightly greater between-group difference in the daytime (−4.0/−2.8 mmHg, p ≥ 0.29) than night-time (−0.2/−1.5 mmHg, p ≥ 0.50). The CPAP treatment did not significantly influence clinic or home BP during follow-up (p ≥ 0.27). Nonetheless, simple and partial correlation analyses showed that the ambulatory BP lowering effect was dependent on the daytime pulse rate at baseline (r ≥ 0.47, p ≤ 0.01). In patients with a daytime pulse rate greater than 85 beats/min, the mean changes in daytime systolic BP were significantly greater in the CPAP (n = 10) than sham CPAP group (n = 11), with a between-group mean difference of −10.1 mmHg (p = 0.048).Conclusions: The CPAP treatment did not show significant ambulatory BP lowering effect in patients with moderate-severe OSAS and nocturnal hypertension. However, it may be effective in lowering daytime BP in patients with a faster pulse rate.

Abstract:Abstract Purpose The goal of blood pressure (BP) control will be lower when hypertensive patients have comorbidities that can affect the risk of cardiovascular diseases. But, the goal of BP control for hypertensive patients coexistent with obstructive sleep apnea (OSA) is not discussed, which is a special population at high risk of cardiovascular diseases. Patients and methods Using data from a retrospective study(Urumqi Research on Sleep Apnea and Hypertension (UROSAH) study, we enrolled 3267 participants who were diagnosed with hypertension and performed polysomnography during 2011–2013 to explore the association between BP control and long-term major adverse cardiovascular and cerebrovascular event (MACCE). Outcomes of interest was the levels of BP control, MACCE, cardiac event and cerebrovascular event. Then we calculated the cumulative incidence of MACCE and performed Cox proportional hazards with stepwise models. Results 379 of 3267 patients experienced MACCE during a median follow-up of 7.0 years. After full risk adjustment, BP control of 120-139/80-89mmHg was associated with the lowest risk of cerebrovascular event (HR: 0.53, 95%CI:0.35–0.82) rather than MACCE and cardiac event in the total cohort. The association did not change much in patients with OSA. When the SBP and DBP were discussed separately, the SBP control of 120-139mmHg or < 120mmHg was associated with the decreased incidence of MACCE and cerebrovascular event. When DBP control < 80 mm Hg, the risk of cerebrovascular event showed 54% decrease [(HR:0.46, 95%CI: 0.25–0.88)] in patients with hypertension and OSA. Conclusion In this retrospective study, antihypertensive-drug-induced office and home BP control at 120-139/80-89mmHg showed possible beneficial effect on incident MACCE. However, current results need to be verified in future studies.

Abstract:Background Obstructive sleep apnea syndrome(OSAS)is a common disease.Prevalence rate of hypertention among patients of OSAS is higher than that without OSAS.Objective To test the hypothesis that in- flammatory factors are involved in the pathogenesis of OSAS with hypertension.Methods Based on aspea index (AHI),136 subjects were straitified into five groups:① OSAS(AHI 5-20 beats/h)with hypertension group(n= 22);② moderate and severe OSAS with hypertension group(n=58):OSAS(AHI>20 beats/h)with hypertension; ③ hypertension alone group(n=20);④ OSAS alone group(n=16):moderate and severe OSAS(AHI>20 beats/ h);⑤ normal control group(n=20):healthy person without OSAS or hypertension.The serum level of IL-6, sCD40L,hsCRP,sICAM-1,VACM-1 are determined by ELISA method.Results The serum level of IL-6, sCD40L,hsCRP,sICAM-1,VCAM-1 in patients of OSAS alone group or HT alone group is higher than that of normal control group;whereas,the serum level of IL-6,sCD40L,hsCRP,sICAM-1,VCAM-1 in patients with hy- pertension co-existed OSAS are furthermore higher than that of OSAS alone group,hypertension alone group(P< 0.05);The serum level of hsCRP,sICAM-1,VCAM-1 are positively related with AHI and decreased SaO_2(r= 0.852,P<0.01).Conclusion Inflammatory factors were associated with pathogenesis of hypertension in patients of obstructive sleep apnea syndrome.OSAS potentially aggravate hypertension via infammatory reaction.

Abstract:Objective: The impact of the co-occurrence of hypertension and obstructive sleep apnea (OSA) on the risk of long-term cardiovascular disease (CVD) outcomes has not been extensively studied in the Asian population, and the residual effect of OSA on CVD in patients under antihypertensive treatment is not clear. The study aimed to explore the impact of OSA on the risk of CVD outcomes in a large-scale Asian cohort under antihypertensive treatment using retrospective design. Design and method: Hypertensive patients who underwent polysomnography (PSG) test in January 2011 to December 2013 were recruited from the UROSAH cohort, which was conducted in Hypertension Center of People's Hospital. OSA was defined as apnea hypopnea index over 5. Outcomes were extended major adverse cardiovascular and cerebrovascular events (MACCE), including the first occurrence of nonfatal myocardial infarction, nonfatal stroke, revascularization, rehospitalization due to unstable angina or heart failure and all-cause death. Cox regression analysis was performed to explore the effect of OSA and hypertension coexistence on MACCE. Results: A total of 3329 hypertension patients were enrolled, of whom 2585 patients (about 77.7%) suffered from OSA. During a median follow-up period of 7.0 years, 415 patients developed extended MACCE. The incidence of extended MACCE was significantly greater in patients with OSA than those without OSA (hazard ratio [HR]: 1.59; 95% confidence interval [CI]: 1.27-1.99; P<0.001). Overall, patients with OSA had an increased risk of cardiac events of 57% compared to those without OSA [HR (95%CI): 1.57 (1.04-2.39), P=0.034] and the association did not change in further sensitivity analysis. Particularly in uncontrolled hypertension, OSA was found to have a 93% increased risk of cardiac events, compared with patients without OSA (P=0.036). Conclusions: Untreated OSA seemed to be a factor affecting the prognosis of cardiac events in hypertensive patients, although the association between OSA and cardiac events would be attenuated by the pharmacological-induced blood pressure control, which highlights the need to treat OSA.

Abstract:Background Obstructive sleep apnea (OSA) and metabolic syndrome (MetS) often coexist, and the causal relationship between them is not yet clear; treatments for OSA include continuous positive airway pressure (CPAP), mandibular advancement device (MAD), surgery, and lifestyle intervention and so on. However, the effects of different treatments on metabolic syndrome in OSA patients are still under debate. Objectives Review the effects of different treatments on metabolic syndrome in OSA patients by meta-analysis. Methods we searched articles in PubMed, Embase, Cochrane Library, CNKI, CBM, and Wanfang data from database construction to Feb. 2024.RevMan5.4 and Stata software were used to conduct a meta-analysis of 22 articles. Results A total of 22 articles were finally included. The results showed that CPAP treatment could reduce the prevalence of metabolic syndrome in OSA patients in randomized controlled trials (RCTs) (RR = 0.82 [95% CI, 0.75 to 0.90]; p < 0.01) and single-arm studies (RR = 0.73 [95% CI, 0.63 to 0.84]; p < 0.01). As for metabolic syndrome components, CPAP treatment reduces blood pressure, fasting glucose (FG), triglycerides (TG), and waist circumference (WC) but can’t affect high-density lipoprotein cholesterol (HDL-C) levels. Lifestyle intervention could significantly reduce the prevalence of metabolic syndrome in OSA patients (RR = 0.60 [95% CI, 0.48 to 0.74]; p < 0.01) and can lower blood pressure, fasting glucose, and waist circumference but can’t affect the lipid metabolism of OSA patients. Upper airway surgery can only reduce TG levels in OSA patients (MD = −0.74 [95% CI, −1.35 to −0.13]; p = 0.02) and does not affect other components of metabolic syndrome. There is currently no report on the impact of upper airway surgery on the prevalence of metabolic syndrome. No study has reported the effect of MAD on metabolic syndrome in OSA patients. Conclusion We confirmed that both CPAP and lifestyle intervention can reduce the prevalence of MetS in OSA patients. CPAP treatment can lower blood pressure, fasting glucose, waist circumference, and triglyceride levels in OSA patients. Lifestyle intervention can lower blood pressure, fasting glucose, and waist circumference in OSA patients. Upper airway surgery can only reduce TG levels in OSA patients. Abstract:Objective: This paper aims at analyzing how continuous positive airway pressure (CPAP) could improve the quality of life of patients with obstructive sleep apnea hypopnea syndromes (OSAHS) associated hypertension with continuous positive airway pressure. Methods: 49 patients with moderate or severe OSAHS associated hypertension were selected and were treated by CPAP. Then 5 measures including 24-hour ambulatory blood pressure monitoring, arterial blood gas analysis, and polysomnogram (PSG), the Epworth sleepiness scale (ESS) and the Berlin sleep assessment were carefully observed before and after the CPAP treatment. Results: After the treatment, 49 patients with CPAP were found that blood pressure control is better than before (P ＜ 0.05). Also, the sleep apnea hypopnea index (AHI) was lower than before, and the difference was statistically significant (P ＜ 0.05). The min Sa O2 was higher than that before (P ＜ 0.05). Besides, the Pa O2 was higher than before and the Pa CO2 was lower than before, and the differences of both were statistically significant (P ＜ 0.05). The Epworth somnolence scale (ESS) score and Berlin sleep quality assessment score were lower than that before treatment, and the difference was also statistically significant (P ＜ 0.05). Conclu-sion: CPAP could significantly improve the quality of life of patients with OSAHS associated hypertension.
CPAP effects on blood pressure in different OSA phenotypes - a meta-analysis

Abstract:Introduction: The causal relationship between obstructive sleep apnea (OSA) and arterial hypertension is well established. In an unselected patient population with OSA, continuous positive airway pressure (CPAP) lowers blood pressure (BP) marginally. Aims and Objectives: To study the different effects of CPAP on office, 24h, diurnal, and nocturnal BP in different OSA phenotypes, including different OSA severities, initial blood pressure, comorbidities, and usage patterns. Methods: We conducted a study-level meta-analysis, including 75 RCTs that compared CPAP treatment to passive controls and performed subgroup analysis and meta-regressions. Results: CPAP treatment reduced office, 24h, diurnal, and nocturnal systolic BP (SBP) by -2.48 (95%CI -3.78, -1.18; n=40), -2.57 (95%CI -3.56, -1.58; n=29), -2.23 (95%CI -3.20, -1.27; n=32), and -3.46 (95%CI -4.44, -2.48; n=32) mmHg, respectively. In a meta-regression, higher CPAP usage hours as higher baseline 24h and diurnal SBP were significantly associated with greater BP reduction (all p<0.05). In subgroup analysis, reduction in 24h, diurnal, and nocturnal SBP was greater in studies with baseline apnea-hypopnea index ≥30/h (–3.18 vs -1.49, -2.38 vs -1.35, -3.64 vs -3.10 mmHg, respectively) and a sleepy patient population (baseline Epworth Sleepiness Scale ≥10) (–2.67 vs -1.87, -3.88 vs -1.59, -4.96 vs -4.02 mmHg, respectively). CPAP usage ≥5h/night had a higher impact on BP lowering for office, 24h, and nocturnal SBP (–4.01 vs -1.44, -4.18 vs -2.10, -4.23 vs -1.58 mmHg, respectively). Conclusions: We displayed that CPAP effects on BP in OSA patients are not only dependent on different aspects of BP but on underlying OSA severity and comorbidities.

Abstract:Obstructive Sleep Apnea (OSA) and hypertension have a high rate of co-occurrence, with OSA being a causative factor for hypertension. Sympathetic activity due to intermittent hypoxia and/or fragmented sleep is the most important mechanisms triggering the elevation in blood pressure in OSA. OSA-related hypertension is characterized by resistant hypertension, nocturnal hypertension, abnormal blood pressure variability, and vascular remodeling. In particular, the prevalence of OSA is high in patients with resistant hypertension, and the mechanism proposed includes vascular remodeling due to the exacerbation of arterial stiffness by OSA. Continuous positive airway pressure therapy is effective at lowering blood pressure, however, the magnitude of the decrease in blood pressure is relatively modest, therefore, patients often need to also take antihypertensive medications to achieve optimal blood pressure control. Antihypertensive medications targeting sympathetic pathways or the renin-angiotensin-aldosterone system have theoretical potential in OSA-related hypertension, Therefore, beta-blockers and renin-angiotensin system inhibitors may be effective in the management of OSA-related hypertension, but current evidence is limited. The characteristics of OSA-related hypertension, such as nocturnal hypertension and obesity-related hypertension, suggests potential for angiotensin receptor-neprilysin inhibitor (ARNI), sodium-glucose cotransporter 2 inhibitors (SGLT2i) and glucose-dependent insulinotropic polypeptide receptor/ glucagon-like peptide-1 receptor agonist (GIP/GLP-1 RA). Recently, OSA has been considered to be caused not only by upper airway anatomy but also by several non-anatomic mechanisms, such as responsiveness of the upper airway response, ventilatory control instability, and reduced sleep arousal threshold. Elucidating the phenotypic mechanisms of OSA may potentially advance more personalized hypertension treatment strategies in the future.

Abstract:Objective:To analyze the screening rates primary aldosteronism in obstructive sleep apnea syndrome with hypertension as the blood pressure classification.Method:Retrospective study was performed based on in-patients with hypertension and OSAS in Sleep monitoring center in department of hypertension.PA was screened by the ARR cut-off≥25(ng·L-1)/(μg·L-1·h-1)in the condition of washout of all interfering antihypertensive medications.Result:①There were 4(21.05%)PA in isolated systolic hypertension with OSAS;there were 9(12. 68%)PA in isolated diastolic hypertension with OSAS;there were 29(16.48%)PA in both systolic and diastolic hypertension with OSAS.②There were 18(15.00%)PA in first grade hypertension with OSAS;there were 18 (18.75%)PA in second grade hypertension with OSAS;there were 6(12%)PA in first third hypertension with OSAS.Conclusion:There are no significant differences about the screening rates of PA as the blood pressure classification in OSAS with hypertensio.

Abstract:Hypertension, Ahead of Print. BACKGROUND:Obstructive sleep apnea is associated with increased blood pressure (BP). Obstructive sleep apnea treatment reduces BP with substantial variability, not explained by the apnea-hypopnea index, partly due to inadequate characterization of obstructive sleep apnea's physiological consequences, such as oxygen desaturation, cardiac autonomic response, and suboptimal treatment efficacy. We sought to examine whether a high baseline heart rate response (ΔHR), a marker of high cardiovascular risk in obstructive sleep apnea, predicts a larger reduction in posttreatment systolic BP (SBP). Furthermore, we aimed to assess the extent to which a reduction in SBP is explained by a treatment-related reduction in hypoxic burden (HB).METHODS:ΔHR and HB were measured from pretreatment and posttreatment polygraphy, followed by a 24-hour BP assessment in 168 participants treated with continuous positive airway pressure or nocturnal supplemental oxygen from the HeartBEAT study (Heart Biomarker Evaluation in Apnea Treatment). Multiple linear regression models assessed whether high versus mid (reference) ΔHR predicted a larger reduction in SBP (primary outcome) and whether there was an association between treatment-related reductions in SBP and HB.RESULTS:A high versus mid ΔHR predicted improvement in SBP (adjusted estimate, 5.8 [95% CI, 1.0–10.5] mm Hg). Independently, a greater treatment-related reduction in HB was significantly associated with larger reductions in SBP (4.2 [95% CI, 0.9–7.5] mm Hg per 2 SD treatment-related reduction in HB). Participants with substantial versus minimal treatment-related reductions in HB had a 6.5 (95% CI, 2.5–10.4) mm Hg drop in SBP.CONCLUSIONS:A high ΔHR predicted a more favorable BP response to therapy. Furthermore, the magnitude of the reduction in BP was partly explained by a greater reduction in HB.
