Abstract:Objective To investigate application value of narrow-band imaging(NBI)flexible ureteroscopy in the diagnosis, treatment and follow-up of upper urinary tract transitional carcinomas (UTUC). Methods From June 2015 to June 2017, 16 patients of UTUC were treated by flexible ureteroscopy with white light(WLI)and NBI at Department of Urology,Peking University Third Hospital, including 10 females and 6 males. The age of patients ranged from 43 to 84 years(average 68.9 years). There were 54 cases,including 16 cases with first-suspicion of cancer,and 38 cases with known of UTUC as follow-up. Full renal collecting system examination was performed first under WLI and then under NBI by a single urologist,using the URF-V digital flexible ureteroscope. Then number of tumors visualized by WLI and NBI flexible ureteroscopy were imaged,recorded,and then biopsied or subsequently treated by holmium laser resection with pathological examination. The diagnosis results of NBI flexible ureteroscopy were compared with WLI flexible ureteroscopy results. All the patients underwent 2nd-look ureteroscopy after 4 to 6 weeks since the first ureteroscopy, and after that follow-up flexible ureteroscopy was every 6 months. Results All the operations of 54 cases were successful. One case was treated with flexible ureteroscopy and percutaneous nephroscopy to treat the renal pelvis tumors, duo to the multiple and much larger tumors. Subjectively, NBI significantly provided a much clearer view of the tumors, especially their limits and vascular architecture. Objectively,4 additional tumors(11.5%), as well the extended limits of 3 tumors (8.5%)were detected by NBI when findings by WLI were considered normal. The rate of diagnosis of tumors raised 20.0%. Conclusions Compared with WLI, NBI flexible ureteroscopy provided even more image of UTUC especially their border between tumor tissue and normal tissue. NBI improved the detection of UTUC over standard WLI with higher rate of diagnosis or lower rate of missed diagnosis.

Abstract:Objective To evaluate the effect of narrow band imaging (NBI) in the early diagnosis and postoperative recurrent monitoring of bladder cancer.Methods 64 consecutive patients were included in this group.Among them 16 had undergone transurethral resection of superficial transitional cell carcinoma and had been followed up and 48 had hematuria.These patients were detected with conventional white light imaging(WLI) cystoscopy and NBI,using the same video-cystoscope.Then all the suspected lesions were biopsied.Results Bladder urothelial carcinoma was confirmed pathologically in 46 cases.34 were detected by both WLI and NBI and another 12 only by NBI cystoscopy.The sensitivity of WLI vs NBI was 78.8% vs 100%(P=0.038);The false positive rate was 27.3% vs 23.3%(not significant).Conclusions Sensitivity of NBI was higher than WLI,and NBI could improve sensitivity in the early diagnosis of the bladder cancer.

Abstract:Objective To study the sensitivity and specifity for detection of bladder tumor by Narrow-band imaging flexible cystoscopy compared with WLI flexible systoscopy. Methods Between February 2009 and July 2009, NBI flexible cystoscopy and conventional WLI flexible cystoscopy with the same instrument (Olympus Exera Ⅱ endoscopy system) were both performed on 31 patients highly suspect of bladder neoplasm with same observed time and in a randomized sequenced paradigm. Every suspect mucosa lesion was biopsied in both NBI and WLI image to compare the diagnostic accuracy between them. Results Twenty-eight patients(90%) were pathologically bladder urothelial cell carcinoma (UCC). Of 28 patients 3 were Tis, 15 were Ta, 7 were T1, and 3 were T2. Twenty were low grade carcinom, 8 were high grade carcinoma and 16 had multiple tumors, 12 had a single tumor.Of 73 biopsied lesions, 61 were diagnosed UCC under WLI image with 84% sensitivity, while 80 of 91 diagnosed under NBI image with 88% sensitivity. WLI detected 23 patients with bladder UCC while NBI detected all 28 patients. NBI detected 19 additional UCC lesions in 15 of 28 patients, as compared with WLI(P＜0. 05). Conclusion NBI flexible cystoscopy can detect more bladder urothelial cell carcinoma than WLI flexible cystoscopy.

Abstract:【Objective】To investigate narrow-band imaging(NBI) cystoscopy in the detection of bladder cancer.【Methods】Patients received white light imaging(WLI) cystoscopy first then NBI cystoscopy. Biopsy was done in narrow band imaging mode and rechecked in white light and NBI mode. 【Results】468 biopsies by narrow-band imaging cystoscopy were carried out in 243 cases. A total of 468 lesions were harvested. 113 patients with 214 lesions were confirmed bladder transitional cell carcinoma by pathologist.For primary bladder tumors, 100 patient with193 lesions were founded, 153 lesions were confirmed as bladder transitional cell carcinoma. For recurrent bladder tumors, 27 patients with 80 lesions were founded, 61 lesions were bladder transitional cell carcinoma. NBI cystoscopy detected additional 10 tumor lesions in 6 patients compared with WLI cystoscopy. No lesions could be found by WLI cystoscopy while overlooked by NBI cystoscopy. 【Conclusion】Compared with WLI cystoscopy, NBI cystoscopy increases the detection rate of recurrent bladder cancer.

Abstract:INTRODUCTION:The objective of this study was to investigate the value of narrow-band imaging (NBI) cystoscopy in the detection of patients with positive voided urine cytology (VUC) who have no evidence of disease after standard initial investigations.PATIENTS AND METHODS:Between February 2009 and December 2010, 12 patients with positive or suspicious VUC but no regular endoscopic evidence of cancer were investigated with NBI flexible cystoscopy. All the specimens were biopsied both under NBI and white light imaging (WLI). Random biopsies of bladder and prostatic urethra were performed in cases without suspect lesions.RESULTS:Fourteen NBI cystoscopies were carried out in 12 patients. Non-muscle-invasive bladder cancer was diagnosed in 5 of 12 (42%) patients on the first NBI. One patient had carcinoma in situ diagnosed on repeat NBI 3 months later. The sensitivity and specificity in diagnosing unconfirmed positive VUC was 78 and 91% for NBI vs. 50 and 80% for WLI.CONCLUSIONS:NBI cystoscopy significantly improves detection of unconfirmed positive VUC over WLI. It should be carried out early in the investigation of such patients before random biopsies and ureteroscopy.

Abstract:Background To investigate the diagnostic value of narrow-band imaging for flat bladder lesions. Methods Forty-nine patients with flat bladder lesions diagnosed by white light cystoscopy + narrow-band imaging followed by transurethral resection were included. The diagnostic value of narrow-band imaging was compared with postoperative pathological results. Results A total of 59 flat lesions were identified, in which 8 were normal urothelium, 3 were chronic inflammation, 1 was papillary urothelial neoplasm of low malignant potential, 2 were mild dysplasia, 1 was moderate dysplasia, 1 was severe dysplasia, 3 were carcinoma in situ, 16 were low-grade papillary urothelial carcinoma, 16 were high-grade papillary urothelial carcinoma, and 8 were invasive papillary urothelial carcinoma. For narrow-band imaging, the sensitivity was 86.7% (39/45), specificity was 57.1% (8/14), diagnostic accuracy was 79.7% (47/59), false-positive rate was 42.9% (6/14), positive predictive value was 86.7% (39/45), negative predictive value was 57.1% (8/14), area under receiver operating characteristic (ROC) curve was 0.719. Among these lesions, the sensitivity and specificity for postoperative recurrent lesions were 100% (3/3) and 40% (2/5), respectively, and those for erythematous patch-like lesions were 90% (9/10) and 100% (4/4), respectively. Conclusions Narrow-band imaging can improve the detection rate for flat bladder tumor lesions, and reduce the risk for missed diagnosis under white light cystoscopy, especially for otherwise indistinguishable erythematous patch-like lesions.

Abstract:Bladder cancer is characterized by its frequent recurrence and progression. Effective treatment strategies need to be based on an accurate risk stratification, in which muscle invasiveness and tumor grade represent the two most important factors. Traditional imaging techniques provide preliminary information about muscle invasiveness but are lacking in terms of accuracy. Although as the gold standard, pathological biopsy is only available after the surgery and cannot be performed longitudinally for long-term surveillance. In this work, we developed a microfluidic approach that interrogates circulating tumor cells (CTCs) in the peripheral blood of bladder cancer patients to reflect the risk stratification of the disease. In a cohort of 48 bladder cancer patients comprising 33 non-muscle invasive bladder cancer (NMIBC) cases and 15 muscle invasive bladder cancer (MIBC) cases, the CTC count was found to be considerably higher in the MIBC group compared with the NMIBC group (4.67 vs. 1.88 CTCs/3 mL, P=0.019), and was significantly higher in high-grade bladder cancer patients verses low-grade bladder cancer patients (3.69 vs. 1.18 CTCs/3mL, P=0.024). This microfluidic assay of CTCs is believed to be a promising complementary tool for the risk stratification of bladder cancer.
Narrow-band imaging and white light imaging for non-muscle invasive bladder cancer: a meta-analysis

Abstract:Objective:To systematically evaluate the value of cystoscopy by narrow-band imaging (NBI) for non-muscle-invasive bladder caner.Methods:We searched through the major medical databases such as Pub Med,EMBASE,Medline,Science Citation Index,Web of Science and Chinese National Knowledge Infrastructure Database (CNKI) and Wangfang (Database of Chinese Ministry of Science & Technology) for all published studies from 2007 until July 2016.The following search terms were used:"bladder cancer","White-Light Imaging","Narrow Band Imaging".Furthermore,additional related studies were manually searched in the reference lists of all published reviews and retrieved articles.We used the Review Manager software (RevMan 5.3,Cochrane Collaboration) to carry out the meta-analysis.Results:There were 20 eligible studies included.We found the detection rate on bladder cancer patients was significantly higher in NBI group than in WLI group (RR=1.17,[95% CI:1.11,1.23],P<0.000 01,I2=0%),and the detection rate of carcinoma in situ also was higher in NBI group than in WLI group (RR=1.47,[95% CI:1.27,1.69],P<0.000 01,I2=66%).Meanwhile,the recurrence rate of TURBT was significantly lower in NBI group than in WLI group (RR=0.67,[95% CI:0.50,0.89],P=0.006,I2=52%).Conclusions:Cystoscopy by NBI was considered to detect more patients of non-muscle-invasive bladder cancer and carcinoma in situ than WLI,and it might reduce the recurrence rate after TURBT.

Abstract:OBJECTIVE:To investigate the effect of NBI assisted white light transurethral resection of bladder tumor (TURBT) in the treatment of bladder urothelial carcinoma and to summarize the experience of narrow band imaging (NBI) operation.METHODS:Patients with bladder urothelial carcinoma were selected, and TURBT was performed after anesthesia. First of all, the bladder tumor was found and resected under white light. Then we replaced with NBI, looked for suspicious lesions and resected them, The specimens excised under white light and NBI were collected separately. The number, location and pathological results of the lesions under white light were recorded, and the residual lesions under NBI were also recorded. To evaluate the effect of NBI, the ratio of residual bladder tumor was calculated. The cases were divided into three groups according to the time sequence. The clinical data of each group were collected and the learning curve of TURBT under NBI assisted white light was observed.RESULTS:A prospective study of 45 patients with bladder tumor from April 2018 to January 2020, including 32 males and 13 females, aged from 23 to 89 years, with an average age of 65.2 years. All the operations were successfully completed, without obvious complications after operation. Nine cases were single and 36 cases were multiple. The maximum diameter of the tumors was 0.5 to 4.0 cm, with an average of 2.2 cm. The histopathology of the resected tissue under white light was urothelial carcinoma, and 19 cases (42.2%) were pathologically positive by NBI resection. The 45 cases were divided into three groups according to the time sequence, 15 cases in each group. The true positive rate of NBI was 33.3%, 46.7% and 46.7%, respectively, and the false positive rate was 60.0%, 46.7% and 26.7%, respectively in the three groups.CONCLUSION:TURBT is an effective way to treat bladder urothelial cancer, NBI is an effective supplement of white light, which can increase the detection rate of bladder cancer and reduce post-operative recurrence. The NBI light source has a certain learning curve. With the increase of cases, the false-positive rate of NBI is gradually reduced. After the NBI operator has rich experience, the recognition degree of flat tumor is gradually improved under white light, and the residual rate of NBI is reduced after the removal under white light.

Abstract:Objectives To explore the application value of narrow band imaging (NBI) guided transurethral resection of bladder tumor(TURBT) in the prevention and reduction of postoperative recurrence of bladder tumor.Methods A total of 120 cases of non muscle invasive bladder cancer were selected from January 2012 to June 2015 in our hospital.The patients were divided into observation group and control group, the control group of 63 patients with conventional white surgery in the treatment of transurethral resection, the observation group of 57 patients were treated with the NBI guide type by transurethral bladder tumor resection, two groups of patients with operation related index and clinical prognosis outcome were compared.Results All patients were successfully completed surgery, no medical accident, bladder perforation and other malignant events occurred.Catheter indwelling time, hospitalization time and postoperative complication rate in the two groups had no significant difference (P>0.05).Postoperative follow-up results showed that postoperative recurrence rate of patients with bladder tumor in the control group was significantly higher than that of the observation group (P<0.05) and observe group mean recurrence survival time was significantly higher than that of control group (P<0.05).Conclusions Curative effect of NBI-TURBT in the treatment of bladder tumor is accurate, and it can effectively reduce the postoperative recurrence rate, with clinical application and generalization.

Abstract:Objective To investigate the value of narrow band imaging in the diagnosis for multiple primary cancer of upper gastrointestinal tract.Methods From June 2010 to June 2011,2 452 patients underwent upper gastrointestinal endoscopy were enrolled into this study.The suspected lesions were screened with both white light and narrow band imaging(NBI)system,and the superficial glandular structure and microvascular pattern were observed by using magnifying endoscopy with NBI.The T staging of carcinoma diagnosed by biopsy sample was evaluated by endoscopic ultrasound,abdominal CT or surgical resection specimen.ResultsThirteen patients(0.53%)had synchronous multiple cancers.Of these 13 cases,4(30.8%)had early cancer,including 2 carcinoma in situ of the esophagus and 2 intramucosal gastric carcinoma.Conclusion Early cancer is not rare in multiple primary cancer of upper gastrointestinal tract,and it is difficult to detect the lesion by using conventional white light endoscopy.NBI can improve the diagnostic yield.
Narrow-band Imaging Flexible Cystoscopy in the Detection of Primary Non-Muscle Invasive Bladder Cancer: a "second Look" Matters?

Abstract:Objective We investigated whether narrow-band imaging (NBI) was superior to white light imaging (WLI) for detecting primary non-muscle invasive bladder cancer (NMIBC) in a randomized imaging sequence modality, as the increased detection rate by NBI maybe result from the "second look'' inspection of the bladder. Methods Between February 2009 and May 2010, NBI and WLI flexible cystoscopy were prospectively performed on 78 patients with primary suspected NMIBC. According to randomization protocol, the bladder was mapped using WLI then NBI or vice versa within the same observation time. Suspicious lesions together with controlled normal-appearing mucosa were biopsied and examined by a pathologist blinded to the imaging sequence. The tumors detected on patient and tumor level, sensitivity, specificity, and accuracy were compared in both imaging. Results Sixty-nine of 78 (88.5%) patients were diagnosed with bladder cancer including 211 tumors totally. Of these, 36 (17.1%) tumors in 13 patients were detected by NBI only, while 4 (1.9%) tumors in 3 patients found by WLI only. NBI identified significantly more additional tumors than WLI (P < 0.01). The sensitivity of WLI versus NBI for detecting primary bladder tumors was 77.7 versus 92.9% (P < 0.0001), the specificity 82.7 versus 73.5% (P > 0.05), and the accuracy 79.3 versus 86.7% (P < 0.05). The sensitivity of WLI versus NBI for detecting carcinoma in situ (CIS) was 68.3 versus 87.8% (P < 0.05), the specificity 82.9 versus 77.1% (P > 0.05), and the accuracy 75 versus 82.9% (P > 0.05). Conclusions The "second look'' did not compromise the superiority of NBI over standard WLI flexible cystoscopy for detecting primary NMIBC including CIS lesions.
Diagnosis Of Narrow-Band Imaging In Non-Muscle-Invasive Bladder Cancer: A Systematic Review And Meta-Analysis

Abstract:Objectives The objective was to evaluate the diagnostic accuracy of cystoscopy assisted by narrow-band imaging compared with white-light imaging for non-muscle-invasive bladder cancer. Methods An electronic database search of PubMed, Embase, the Cochrane Library, Ovid and Web of Science was carried out for all articles comparing narrow-band imaging with white-light imaging cystoscopy in the detection of non-muscle-invasive bladder cancer. The review process followed the guidelines of the Cochrane Collaboration. Results Seven studies with prospectively collected data including a total of 1040 patients were identified, and 611 patients with 1476 tumors were detected by biopsy. In the patient- and tumor-level analysis, an additional 17% of patients (95% confidence interval, 10-25%) and an additional 24% of tumors (95% confidence interval, 17-31%) were detected by narrow-band imaging, respectively. In the patient- and tumor-level analysis, significantly higher detection rates using narrow-band imaging (rate difference 11%; 95% confidence interval 5-17%; P<0.001; and rate difference 19%; 95% confidence interval 12-26%; P<0.001, respectively) rather than white-light imaging were found. On the tumor level, an additional 28% of carcinoma insitu was detected (95% confidence interval 14-45%) by narrow-band imaging, and a significantly higher detection rate (rate difference 11%; 95% confidence interval 1-21%; P=0.03) was found. The false-positive detection rate of tumor level did not differ significantly between the two techniques. Conclusions Cystoscopy assisted by narrow-band imaging detects more patients and tumors of non-muscle-invasive bladder cancer than white-light imaging, and it might be an additional or alternative diagnostic technique for non-muscle-invasive bladder cancer.

Abstract:To explore the clinical application value of narrow band imaging (NBI) in the early screening of colorectal cancer. 200 patients were selected as the research objects, and were divided into test group and control group by the randomization method, within 100 cases in each group. The patients in the test group were examined by NBI, while the patients in the control group were examined by ordinary endoscopy, and then the lesion detection rate, image definition and pathological examination results of the two groups were compared. After the patients in the test group received NBI examination, the lesion detection rate, image definition and lesion detection coincidence rate were significantly higher than those of the control group, and the comparative difference between the two groups was statistically significant (P < 0.05). NBI examination of colorectal mucosa can clearly show the pit pattern and the morphological structure of capillaries of the early colorectal cancer and its precancerous lesions, thus effectively improving the diagnosis rate of early colorectal cancer and its precancerous lesions, playing a good clinical application value in the early screening of colorectal cancer, and guiding the later treatment, so it is suitable for clinical promotion.

Abstract:OBJECTIVE:To compare the real-time diagnostic accuracy of conventional white-light imaging (WLI) endoscopy with that of narrow-band imaging (NBI) endoscopy in patients at high risk for nasopharyngeal carcinoma (NPC). DESIGN:Prospective study. SETTING:A university tertiary care center. PATIENTS:From July 28 through October 27, 2009, a total of 211 consecutive patients at high risk for NPC were enrolled. A high-performance endoscopic system equipped with WLI and NBI modes was used for a detailed examination of the nasopharynx during the same endoscopy. MAIN OUTCOME MEASURES:Diagnostic efficacies of WLI and NBI were compared with pathologic findings. Lesions were classified according to the detailed morphologic epithelial microvessel observations during NBI. RESULTS:A total of 285 lesions were detected, including 66 cancerous lesions. The sensitivity and negative predictive values of NBI in NPC screening were significantly higher than those of WLI (93.9% vs 71.2%, P = .001; and 98.1% vs 91.7%, P = .003; respectively); specificity and positive predictive value were not significantly different. During NBI, the presence of superficial, distorted, irregularly shaped microvessels indicated malignant lesions; 53 of 55 lesions (96.4%) with type IV intrapapillary capillary loops were confirmed on histologic testing as malignant. The false-negative and false-positive rates for NBI were 4.5% and 3.6%, respectively. CONCLUSIONS:Narrow-band imaging endoscopy is a promising tool to differentiate nonmalignant from malignant nasopharyngeal lesions on the basis of the morphologic findings of mucosal capillary vessels in vivo. In addition, NBI may increase the diagnostic value of endoscopy in populations at high risk for NPC.
Application of narrow-band imaging endoscopy in diagnosis of nasopharyngeal carcinoma

Abstract:OBJECTIVE To study the application of narrow-band imaging in diagnosis of the nasopharyngeal malignant lesions. METHODS A total of 52 cases of nasopharyngeal malignant lesions were examined and diagnosed by white and NBI endoscopy between June 2014 and August 2015, and their diagnosis efficacies were evaluated based on pathological diagnosis as a gold standard. RESULTS Of 52 cases, 46 cases were malignant lesion(88.46%) and 6 were non malignant lesion(11.54%). The diagnostic accuracy by NBI for the nasopharyngeal malignant lesions was 86.54%(45/52), which was significantly higher than that of white light 69.23%(36/52, χ2=4.522, P<0.05). The sensitivity of NBI and white light to detect nasopharyngeal carcinoma was 97.83%(45/46) and 78.26%(36/46) respectively(χ2=8.364, P<0.05). CONCLUSION NBI has high application value in diagnosis of nasopharyngeal carcinoma, and is expected to become one of the important means to diagnosis of the nasopharyngeal carcinoma.

Abstract:Background: White light (WL) is the established imaging modality for transurethral resection of bladder tumour (TURBT). Narrow band imaging (NBI) is a promising addition. Objectives: To compare 12-mo recurrence rates following TURBT using NBI versus WL guidance. Design, setting, and participants: The Clinical Research Office of the Endourological Society (CROES) conducted a prospective randomised single-blind multicentre study. Patients with primary non-muscle-invasive bladder cancer (NMIBC) were randomly assigned 1: 1 to TURBT guided by NBI or WL. Intervention: TURBT for NMIBC using NBI or WL. Outcome measurements and statistical analysis: Twelve-month recurrence rates were compared by chi-square tests and survival analyses. Results and limitations: Of the 965 patients enrolled in the study, 481 patients underwent WL-assisted TURBT and 484 patients received NBI-assisted TURBT. Of these, 294 and 303 patients, respectively, completed 12-mo follow-up, with recurrence rates of 27.1% and 25.4%, respectively (p = 0.585, intention-to-treat [ITT] analysis). In patients at low risk for disease recurrence, recurrence rates at 12 mo were significantly higher in the WL group compared with the NBI group (27.3% vs 5.6%; p = 0.002, ITT analysis). Although TURBT took longer on average with NBI plus WL compared with WL alone (38.1 vs 35.0 min, p = 0.039, ITT; 39.1 vs 35.7 min, p = 0.047, per protocol [PP] analysis), lesions were significantly more often visible with NBI than with WL (p = 0.033). Frequency and severity of adverse events were similar in both treatment groups. Possible limitations were lack of uniformity of surgical resection, data on smoking status, central pathology review, and specific data regarding adjuvant intravesical instillation therapy. Conclusions: NBI and WL guidance achieved similar overall recurrence rates 12 mo after TURBT in patients with NMIBC. NBI-assisted TURBT significantly reduced the likelihood of disease recurrence in low-risk patients. Patient summary: Use of a narrow band imaging technique might provide greater detection of bladder tumours and subsequent treatment leading to reduced recurrence in low-risk patients. Although it is now frequently used in gastrointestinal endoscopy, there are still some controversies about its benefit in the detection of urinary bladder cancer. One of the limitations of previous study is the possible observer bias as white-light imaging (WLI) and NBI were performed subsequently by the same urologist. This study was designed to investigate the detection rate of new non-muscle invasive bladder cancer(NIMBC) between NBI and WLI flexible cystoscopy with a randomized image sequential paradigm. METHODS Between February 2009 and October 2009, NBI and WLI flexible cystoscopy with the same instrument (Olympus Lucera sequential endoscopy system)were both performed on 45 patients with suspected new NIMBC in a randomized image sequential paradigm with the same observing time to avoid the observer bias. Biopsies obtained by NBI and WLI were examined separately for pathology. After biopsy, a standard TUR with resected samples was performed to show presence of tumors or not. RESULTS 41 patients (91.1%) were pathologically NIMBC. Of these 22 were Ta, 14 were T1, 5 were Tis. 89 Of 110 biopsied lesions were found tumor under WLI while 113 of 142 were under NBI. The sensitivity of WLI vs NBI for detecting bladder tumors in all 45 patients was 80.5% vs 97.6% (P<0.05), the specificity 50% vs 75% (P<0.05), the accuracy 77.8% vs 95.6% (P<0.05), and the AUC was 0.652 vs 0.863(P>0.05). All 5 Tis were detected by NBI while 2 missed by WLI. NBI detected 24 additional cancer lesions in 15 of 41 patients, as compared with WLI (P<0.05). CONCLUSIONS NBI flexible cystoscopy improved the detection of NIMBC over standard WLI cystoscopy, contributing to its high value in the clinical practice of bladder cancer. Application of Narrow Band Imaging Endoscopy in the Diagnosis of Laryngeal Cancer

Abstract:OBJECTIVE:To investigate the value of narrow band imaging (NBI) endoscopy in the detection of laryngeal cancer and its precursor lesions.METHODS:Between December 2008 and July 2009, a total of 122 consecutive patients with suspected precancerous or cancerous lesions of the larynx were enrolled in this study. High performance endoscopic system equipped with the white light mode and NBI mode was introduced in the examination of larynx. Full pharyngeal and laryngeal examination was performed first under white light endoscope and then under NBI. Each lesion was observed by NBI endoscopy and classified according to the detailed morphologic findings of epithelial microvessels. Histological results were then compared to the corresponding white light and NBI images.RESULTS:Among the 122 patients, a total of 149 lesions were detected by white light and NBI modes. The histological diagnoses of these lesions were as follows: 16 laryngeal polyp, 7 laryngitis, 24 simple hyperplasia, 17 mild dysplasia, 6 moderate dysplasia, 6 severe dysplasia, 9 carcinoma in situ, 64 invasive cancer. The diagnostic accuracy by NBI for the laryngeal lesions was 90.6% (135/149), which was significantly higher than that of white light (75.2%, 112/149, chi² = 12.514, P < 0.001). The sensitivity of NBI and white light to detect laryngeal carcinoma was 93.2% (68/73) and 68.5% (50/73), respectively (chi² = 14.317, P < 0.001).CONCLUSIONS:NBI endoscopy was a promising tool for the differentiation of non-malignant from malignant laryngeal lesions in vivo by detection of the morphology of mucosal capillary vessels. NBI endoscopy was easy to application in the routine pharyngolaryngeal examination with high sensitivity, and facilitated to improve the abilities of preoperative diagnosis and postoperative surveillance.

Abstract:OBJECTIVE:To evaluate the value of narrow band imaging (NBI) endoscopy in the diagnosis of nasopharyngeal lesions.METHODS:Between December 2009 and April 2010, a total of 124 patients with nasopharyngeal lesions were examined with electronic nasopharyngolaryngoscope equipped with the white light mode and NBI mode. The biopsies of nasopharyngeal lesions were done in all patients. The characteristics of morphologies of mucosa and mucosal superficial vessels of each lesion under NBI mode were evaluated.RESULTS:Of all cases, there were 1 of papilloma, 87 of lymphoid follicular hyperplasia and chronic inflammation, 11 of nasopharyngeal cysts, and 25 of nasopharyngeal carcinoma. According to the pathological results, the morphologies of nasopharyngeal lesions under NBI mode were quite different. The color depth of the mucosa could be divided into four types: light red (+), dark red (++), prunosus (+++), and blue or blue black (++++). Under NBI, the color depths were (+) in papilloma, (++) in nasopharyngeal cysts, and (+++) in lymphoid follicular hyperplasia and chronic inflammation, without abnormal vessels. The color depths were (+++)-(++++) in nasopharyngeal cancer, with abnormal vessels.CONCLUSION:NBI has a potential ability to predict pathological results of nasopharyngeal lesions.
