Abstract:PURPOSE:To investigate the prevalence, features, and risk factors of macular retinoschisis (MRS) in highly myopic eyes and their morphological features in a Chinese population. DESIGN:Population-based, cross-sectional study. From the Beijing Eye Study 2011, a total of 213 highly myopic eyes from 129 participants were included. METHODS:All participants underwent spectral-domain optical coherence tomography with a macula cube scan of 30°×30°centered in the fovea. High myopia was defined as a refractive error ≤-6 diopters or an axial length ≥26.0 mm. MRS presence and its subtypes were assessed based on location, and affected retinal layers were evaluated. Prevalence, features, and associated factors of MRS and its subtypes in highly myopic population were assessed. RESULTS:Of 213 highly myopic eyes (129 participants), MRS was observed in 48 eyes, with a prevalence of 22.5% (95% CI, 16.9-28.6) per eye, or 36 participants with 27.9% (95% CI, 20.5-35.7) per subject. In addition to well-documented factors such as older age and higher myopia, the prevalence of MRS was found to be related with a higher intraocular pressure (P = .013, odds ratio [OR], 1.25; 95% CI, 1.05-1.48), a thinner subfoveal choroidal thickness (P = .006, OR, 0.86; 95% CI, 0.77-0.96), a wider Gamma zone (P = .003, OR, 1.99; 95% CI, 1.05-3.11), the presence of glaucoma (P = .010, OR, 3.37; 95% CI, 1.34-8.48), and the presence of epiretinal membrane (P = .023, OR, 3.13; 95% CI, 1.17-8.36) after multivariate analysis. Eyes with advanced high myopia (P = .021) and wider gamma zone (P = .005) were more likely to develop foveal MRS. Eyes with glaucoma tended to have a higher prevalence of outer retinal MRS compared with inner retinal MRS (60.9% vs 36.0%), although the difference was not statistically significant (P = .06). MRS located in the foveal region or affecting the outer retina was related with a significantly worse best-corrected visual acuity (BCVA) compared with MRS that was perifoveally located or affected other regions (P < .05). CONCLUSIONS:The prevalence of MRS was 27.9% among the elderly highly myopic population. MRS occurring in the foveal region or involving the outer retina demands increased vigilance due to its significant impact on BCVA. The findings contribute to a deeper understanding of MRS, offering insights into its mechanisms and vision prognosis.

Abstract:AIM: To evaluate the clinical factors related to chronic rhegmatogenous retinal detachment (RRD).METHODS: A retrospective case-control study. A total of 103 consecutive patients (103 eyes) with primary RRD were studied to evaluate the clinical factors related to chronic RRD.RESULTS: Chi-square test was used to sift out the following associated factors with chronic RRD: younger patients(P=0.0028), better preoperative best corrected visual acuity (BCVA, P=0.0316), atrophic retinal break (P<0.0001), inferior retinal break (P<0.0001), smaller break (P=0.0005); then the independent risk factors related to chronic RRD was determined by stepwise logistic regression analysis as following: atrophic retinal break (odds ratio (OR)=7.997, P=0.007), inferior retinal break (OR=14.127, P<0.0001) and better preoperative BCVA (OR=1.636 P<0.0722).CONCLUSION: Atrophic retinal break, inferior retinal break and better preoperative BCVA are the independent risk factors related to chronic RRD.
Study on Macular Retinoschisis by Optic Coherence Tomography in Eyes with High Myopia

Abstract:Objective To observe the the clinical characteristics of images of optic coherence tomography (OCT) in highly myopic eyes with retinoschisis. Methods The clincial data of 158 patients (158 eyes) with high myopia diagnosed by examinations of best corrected visual acuity and refraction, indirect stereoscopic ophthalmoloscopy, A/B-scan ultrasonography, and OCT, were retrospectively analyzed. The patients were divided into retinoschisis group and non-retinoschisis group according to the results of OCT (whether the patients had macular reinoschisis at the posterior pole). There were 53 patients (55 eyes, 34.8%) in the former group, and 101 patients (103 eyes, 65.2%) in the latter group. The age, sex, diopter, visual acuity, ocular axial length, and incidence of posterior scleral staphyloma, vitreous traction, and retinal detachment of the two groups were compared. Results B-scan uhrasonography showed posterior scleral staphloma in all of the 158 eyes. OCT indicated that in the 55 eyes in the retinnoschisis group, 15 (27.3%) had inner, 53 (96.4%)had outer, and 7 (12.7%)had middle retinoschisis. The inner and outer one could exist independently or in the same eye, while the middle one was always accompanied by the outer retinoschisis. Two or more types of schisis coexisted in 13 eyes (23.6%), single outer retinoschisis was found in 40 eyes (72.7 %), and single inner retinoschisis was found in 2 eyes (3.6 %). There were 26 eyes (47.3%)were accompanied with retinal detachment, 13 eyes (23.6%) with macular hole, and 12 eyes (21.8%)with vitreous traction. In the 103 eyes in the non-retinoschisis group, 23 eyes (22.3%)had vitreous traction, 19 eyes (18.4%) had macular hole, and 21 eyes (20.4%)had retinal detachment. The differences of age, diopter and ocular axial length, sex, incidence of macular hole and vitreous traction between the two groups were not statistically significant (P>0.05). The visual acuity in retinoschisis group was much lower than that in the non-retinoschisis group (P<0.05), and the difference of incidence of the retinal detachment between the two groups was significant (P<0.01). Conclusion Macular retinoschisis in eyes with high myopia can exist in inner or middle retina, but most of them locate at outer retina. The patients always have poor visual acuity and are often accompanied by other macular lesions such as retinal detachment.

Abstract:Purpose: To review the clinical features of acute zonal occult outer retinopathy (AZOOR) in Chinese patients. Methods: All patients with AZOOR during 2002–2004 in our hospitals were reviewed retrospectively. Results: Seven consecutive Chinese patients with AZOOR were recruited and followed up for 4–18 months. Their age ranged from 26 to 47 years and all were affected bilaterally. They were from the cities near the Pacific Ocean and were used to eating seafood. The common complaints were slightly reduced visual acuity and photopsia. At least one eye of each patient had a visual field defect or decreased local area sensitivity and one patient had bilateral blind spot enlargement. Ten in 14 eyes showed increased numbers of vitreous cells and 4 eyes had anterior chamber inflammatory cells and a keratic precipitate. In their initial examination, minimal or no fundus changes were found, only yellow-white dots or gray dots presented on the deep retina or outer retinal layer. Fundus fluorescent angiography showed large-area depigmentation and hyperfluorescein spots corresponding to fundus findings. Electroretinogram (ERG) or multifocal ERG was abnormal in all eyes with no changes in their follow-up examination. Not all of the initial diagnoses of these patients were consistent with the final ones. Conclusions: AZOOR is not a common disease in China, but easy to misdiagnose. Female predilection, photopsia, visual field defect, ERG abnormality and minimal ophthalmoscopic changes are the common characteristics of AZOOR in Chinese patients. Living habits may play a role in the development of AZOOR.
Logistic Regression Analysis of Risk Factors for Subretinal Fluid after Rhegmatogenous Retinal Detachment Repair

Abstract:Objective:To identify factors associated with persistent subretinal fluid (SRF) after scleral buckling and to evaluate surgical outcomes of patients with primary rhegmatogenous retinal detachment (RRD).Methods:Cross-sectional study and cohort study. This study included 104 patients (104 eyes) who underwent scleral buckling for repair of RRD between January 2016 and June 2017. Several statistically significant risk factors associated with SRF were screened out with univariate analysis. Then independent risk factors were determined with multivariate stepwise logistic regression analysis. Examinations were taken preoperatively, at 1, 3, 6, and 12 months postoperatively, and thereafter every six months. Patients were divided into two groups depending on the presence or absence of persistent SRF on optical coherence tomography at 1 month.Results:Persistent SRF occurred in 69.2% (72/104) of patients. In multivariate analysis, younger age ( OR=32.860, 95% CI=3.636-296.986, P=0.002), high myopia ( OR=7.229, 95% CI=1.217-42.94, P=0.03) and macula-involving retinal detachment ( OR=357.397, 95% CI=29.761-4292.0, P=0.000) were associated with persistent SRF. Best corrected visual acuity in patients with SRF at 1 month (0.71±0.18 vs. 0.58±0.11; t=4.047, P<0.01) and 3 months (0.55±0.15 vs. 0.43±0.12; t=3.914, P<0.01) was worse than that in patients with absence of SRF. Best corrected visual acuity in patients with SRF was 0.44±0.16 at 6 months, 0.37±0.12 at 12 months and 0.36±0.10 at the last follow-up, with no significant difference from patients without SRF at the three time points (0.39±0.13, 0.38±0.12 and 0.35±0.09; t=1.643, -0.202, 0.464; P>0.05). Conclusions:RRD patients with younger age, high myopia and macular involvement were more likely to develop persistent SRF after scleral buckling. The presence of persistent SRF slowed visual recovery but did not influence the final visual outcomes. (Chin J Ophthalmol, 2021, 57: 426-432)

Abstract:·AIM: To improve our understanding of retinoschisis at the macular area.·METHODS: From January 2003 to January 2005, the patients whose macular retinoschisis was confirmed by optical coherence tomography (OCT) were included in the study. Their data and other results were further analyzed.·RESULTS: During that period, macular retinoschisis was found in 116 eyes which fit the including criteria. Among these, 94 eyes were pathological myopia; 17 were X-linked congenital retinoschisis; and 5 had excavation of optic disk.By analyzing the OCT figures, it was found that retinoschisis could happen at many locations within retinal neural epithelium, including inner, middle and outer part. The retinoschisis caused by different reasons differs in OCT results.·CONCLUSION: Retinoschisis at the macular area was not rare at the clinic, and the retinoschisis may be located at different parts of the retina. OCT was useful in the diagnosis and follow-up of macular retinoschisis.

Abstract:Objective To improve our understanding of the morphological characteristics of macular retinoschisis in highly myopic eyes by optical coherence tomography and to discuss the probably associated factors of it. Methods In the retrospective study, 268 patients(369 eyes)with high myopia(spherical equivalent 3≥-6.00D)were included, from which a diagnosis of retinoschisis was given by optical coherence tomography, and observed the area and morphological characteristics of the retinoschisis. The factors that were associated with the occurrence and the development of the retinoschisis were investigated. Results OCT showed that 68 patients(93 eyes)had retinoschisis. Outer schisis was present in 78 eyes, mixed schisis in 14 eyes and inner schisis in 1 eye. The retinoschisis in 2 eyes located far from the macular area, and the other 91 eyes located in the macular area, which were involved into one to four quadrants, in which foveoschisis were present in 53 eyes, and parafoveoschisis in 38 eyes. There were several kinds of types in foveoschisis, companied with other pathological changes, such as vitreoretinal traction, preretinal membrane, foveal detachment and lamellar holes.We proceeded the comparison of the axial length, spherical equivalent, posterior staphyloma, vitreoretinal interface factors and posterior polar chorioretinal atrophy in retinoschisis and non-retinoschisis in highly myopic eyes, finding that all the differences were significant(P <0.01), but Logistic regression showed that all the factors above were the risk factors associated with the retinoschisis(P =0.002, 0.027, 0.003, 0.011)except the spherical equivalent(P =0.073). Conclusions OCT can display the area and morphological characteristics of retinoschisis clearly, the retinoschisis is mostly present in the fovea, which is usually involved into the whole macular area, and the outer schisis is common. The occurrence and development of the retinoschisis is associated with the axial length, posterior staphyloma, vitreoretinal interface factors and posterior polar chorioretinal atrophy.
Myopic Macular Retinoschisis in Teenagers: Clinical Characteristics and Spectral Domain Optical Coherence Tomography Findings

Abstract:To investigate the morphological characteristics of myopic macular retinoschisis (MRS) in teenagers with high myopia, six male (9 eyes) and 3 female (4 eyes) teenagers with typical MRS identified from chart review were evaluated. All cases underwent complete ophthalmic examinations including best corrected visual acuity (BCVA), indirect ophthalmoscopy, colour fundus photography, B-type ultrasonography, axial length measurement and spectral-domain optical coherence tomography (SD-OCT). The average age was 17.8 ± 1.5 years, average refractive error was −17.04 ± 3.04D, average BCVA was 0.43 ± 0.61 and average axial length was 30.42 ± 1.71 mm. Myopic macular degenerative changes (MDC) by colour fundus photographs revealed Ohno-Matsui Category 1 in 4 eyes and Category 2 in 9 eyes. Posterior staphyloma was found in 9 eyes. SD-OCT showed outer MRS in all 13 eyes, internal limiting membrane detachment in 7 eyes, vascular microfolds in 2 eyes and inner MRS in 1 eye. No premacular structures such as macular epiretinal membrane or partially detached posterior hyaloids were found. Our results showed that MRS rarely occurred in highly myopic teenagers and was not accompanied by premacular structures, severe MDC, or even obvious posterior staphyloma. This finding indicates that posterior scleral expansion is probably the main cause of MRS.

Abstract:Objective: To analyze the possible causes of retinoschisis in highly myopic eyes by observing its features of optical coherence tomography (OCT). Methods: OCT appearances from 22 eyes of 16 patients with retinoschisis secondary to high myopia were examined and analyzed retrospectively during October 2000 to October 2003. Results: Retinoschisis secondary to high myopia was mainly localized in the outer layer of neurosensory retina in the posterior pole. Vitreous traction was found in 18 eyes. The neurosensory retinal thickness at uninvolved fovea was significantly thinner than that of normal control (P = 0.0052). Conclusion: Retinoschisis secondary to high myopia is associated with vitreous traction and the pre-existing retinal degeneration.
Subfoveal scleral thickness is associated with peripheral retinal changes in high myopia in children and adolescents

Abstract:BackgroundThis study aims to identify the risk factors in peripheral retinal changes (PRC) associated with high myopes among children and adolescents.MethodsThis is a cross-sectional study on children and adolescents diagnosed with high myopia. The subjects involved underwent a series of ocular examinations, including the dilated fundus examination for PRC and the swept-source optical coherence tomography for foveal retinal, choroidal and scleral thickness measurement. Then, the variables were compared among the eyes with high risk, low risk, and no PRC. Spearman correlation was applied to evaluate the relationship between the parameters and the extent of PRC. Logistic regression was performed to identify the potential risk factors.ResultsA total of 117 eyes from 117 subjects were recruited. The prevalence of PRC was 57.3% (67 eyes), while that of high-risk PRC was 22.2% (26 eyes). Significant differences were found in the mean subfoveal scleral thickness, spherical equivalent refraction, and axial length among the eyes with high-risk, low-risk, and no PRC (p < 0.01, p < 0.01, p = 0.048, respectively). Compared with spherical equivalent (r = 0.32, p < 0.01) and axial length (r = 0.18, p = 0.05), subfoveal scleral thickness exhibited higher correlation coefficient with PRC (r = − 0.38, p < 0.01). Subfoveal scleral thickness and spherical equivalent refraction were identified as the independent risk factors for PRC and high-risk PRC.ConclusionIt was demonstrated that there was a correlation between subfoveal scleral thickness and PRC. The eyes with thinner subfoveal scleral thickness carried a higher risk of PRC.

Retinoschisis at Macular Area in Highly Myopic Eye by Optic Coherence Tomography

Abstract:PURPOSE:To improve our understanding of macular schisis in highly myopic eyes.METHODS:From 2002.6-2003.12, the highly myopic eyes (> 6D) which has macular schisis found by OCT at our hospital were included in the study. Each eye then received further examinations including visual acuity, refractive error and ultrasonography. Then the results were analyzed to find the characteristic of the entity.RESULTS:During that period, OCT found macular schisis in 38 highly myopic eyes from 36 patients. Their vision was low, and their average axial length and refractive error were 29.6mm and -10.5D respectively. The appearance and the area of the schisis are different among the eyes. But the schisis can be largely divided into two types: inner and outer schisis. They can exit independently or concurrently in the same eye. And the schisis was often accompanied by other macular findings, like retinal detachment, macular hole.CONCLUSION:Macular schisis in highly myopic eyes is not uncommon in the clinic. OCT is useful in its diagnosis.
LONG-TERM EVOLUTION OF MYOPIC RETINOSCHISIS WITH A DOME-SHAPED MACULA AND PREDICTORS OF PROGRESSION AND VISUAL PROGNOSIS

Abstract:PURPOSE: To study the long-term natural course of myopic retinoschisis (MRS) with a dome-shaped macula (DSM) and to identify the factors affecting its development and visual prognosis.METHODS: In this retrospective case series study, we followed 25 MRS eyes with a DSM and 68 MRS eyes without a DSM for at least two years and observed changes in optical coherence tomography morphological features and best-corrected visual acuity (BCVA).RESULTS: During the mean follow-up of 48.3±13.24 months, the difference in the rate of MRS progression between the DSM and non-DSM groups was not significant (P = 0.7462). In the DSM group, the patients whose MRS progressed were older and had a higher refractive error than those whose MRS was stable or improved (P = 0.0301 and 0.0166, respectively). The patients whose DSM was located in the central fovea had a significantly higher progression rate than those whose DSM was located in the parafovea (P = 0.0421). For all DSM eyes, BCVA did not decrease significantly in eyes with extrafoveal retinoschisis (P = 0.2500), patients whose BCVA decreased more than 2 lines had a greater central foveal thickness initially than those whose BCVA decreased less than 2 lines during the follow-up period (P = 0.0478).CONCLUSION: A DSM did not delay the progression of MRS. The development of MRS in DSM eyes was associated with age, myopic degree and DSM location. A higher schisis cavity predicted visual deterioration, and a DSM protected visual function in extrafoveal MRS eyes during the follow-up period.

Epidemiological Investigation of Senile High Myopia in Shanghai Beixinjing Community

Abstract:Objective To investigate the prevalence of high myopia,the prevalence and risk factors of high myopia associated with chorioretinopathy in residents aged 60 years or over in Beixinjing community, Shanghai, China. Methods A cluster stratified random sampling method was used to screen 4153 people aged 60 and over in Beixinjing community. There were 3851participants in total with a 92.73% response rate. Participants were invited to complete a questionnaire and received a comprehensive eye examination including visual acuity, refraction, slit-lamp microscopic examination, direct ophthalmoscopy and fundus photography and so on. Spherical equivalent (SE) was used to determine the degree of refractive errors. The diagnosis of high myopic chorioretinopathy was made if SE>-6.00 D and myopic ehorioretinal atrophy lesions were presented such as posterior seleral staphyloma, lacquer cracks, Fuchs spot and myopic arc spots. The degree of visual acuity impairment was determined according to the World Health Organization (WHO) classification as low vision (the best corrected visual acuity≥0. 05, but <0.3) or blindness (the best corrected visual acuity < 0. 05). Results There were 207/3851 (5.37 %) high myopia patients, in which 183/207 (88.40%) patients were associated with myopic chorioretinopathy. The prevalence of myopic chorioretinopathy decreased while age increased (X2= 19.21, P<0.01), but statistically there was no gender difference (X2= 1.83, P>0.05). Logistic regression analysis showed that there were significant differences in the prevalence of high myopia between people with different age, educational levels and family history (X2= 19.21,32. 08,960. 68; P<0.01). There were 29 cases of bilateral blindness, 96 cases of unilateral blindness, 104 cases of bilateral low vision and 562 cases of unilateral low vision in those participants. In 183 cases of high myopic chorioretinopathy patients, 111(60.65 %)cases had an obvious visual impairment, including 34.48% (10 cases) of bilateral blindness, 11.46% (11 cases) of unilateral blindness, 29.81% (31 cases)of bilateral low vision and 10.50% (59 cases) of unilateral low vision. Conclusions The prevalence of high myopia of residents aged ≥ 60 years in Beixinjing community, Shanghai, China is relatively high. Age, education level and family history are the most important factors affecting the occurrence of chorioretinopathy in high myopia patients.

Abstract:Purpose To evaluate the severity and related factors of macular retinoschisis (MRS) and paravascular retinoschisis (PVRS) in high myopia (HM) using ultra-wide-field optical coherence tomography (UWF-OCT) and a novel Gaussian curvature (K). Design A cross-sectional study. Methods Patients diagnosed with HM in ໿Peking Union Medical College Hospital were recruited between January 2022 and November 2022. The presence and severity of retinoschisis, along with the three highest K values, were assessed using UWF-OCT and en face images. Logistic regressions ໿were employed to identify factors associated with MRS, PVRS, and the severity of retinoschisis in the 24 × 20 mm scan region. Results A total of 108 HM eyes from 55 patients were recruited. The highest Gaussian curvature (K1) was predominantly found in the vascular arcade (43, 40%). Multivariable logistic regression found that age and PVRS were significant risk factors for MRS occurrence (P < 0.05), while MRS and higher K1 were significantly associated with the presence of PVRS in HM patients (P < 0.05). The axial length (AL) and spherical equivalent were associated with the severity of MRS, while AL and K1 values were associated with the severity of retinoschisis in the 24 × 20 mm scan region (P < 0.05). Conclusions An association existed between large Gaussian curvature and the presence of MRS and PVRS, as well as the severity of retinoschisis in a wide field of view. UWF-OCT, which enables visualization of the central and peripheral retinal areas, holds promise as an imaging technique for the early detection of extrafoveal retinoschisis.

Relationship Between Paravascular Abnormalities and Choroidal Thickness in Young Highly Myopic Adults.

Abstract:Purpose:The purpose of this study was to investigate the clinical characteristics of paravascular abnormalities (PVAs) and retinoschisis, and their associations with choroidal thickness (ChT) in young highly myopic (HM) adults.Methods:A total number of 645 eyes were included. Paravascular microfolds (PMs), paravascular cystoid spaces (PCs), paravascular lamellar holes (PLHs), and retinoschisis were detected using swept-source optical coherence tomography. Their associations with macular ChT and risk factors were analyzed.Results:PMs, PCs, and PLHs were detected in 203 (31.5%), 141 (21.9%), and 30 (4.7%) eyes, respectively. Retinoschisis was found in 50 (7.8%) eyes, 43 (86.0%) of which were located around the retinal vessels surrounding the optic disc. A decreasing trend of macular ChT (P < 0.001) was observed in the eyes with PMs only, with both PCs and PMs, and with PLHs, PCs, and PMs. After adjustments for age, sex, and axial length (AL), the presence of PCs, PLHs, or retinoschisis around the optic disc was negatively associated with macular ChT (all P < 0.05). Eyes with longer AL, incomplete posterior vitreous detachment (PVD), and myopic atrophic maculopathy (MAM) were more likely to have PCs (all P < 0.01) and retinoschisis around the optic disc (all P < 0.05).Conclusions:PVAs were observed in approximately one third of the young HM adults in this study. The presence of PCs, PLHs, or retinoschisis around the optic disc was associated with thinner macular ChT. Eyes with longer AL, incomplete PVD, and MAM may be at risk of developing PVAs and retinoschisis around the optic disc.Translational Relevance:PCs, PLHs, and retinoschisis around the optic disc could serve as early indicators for myopia progression.

Abstract:Aim To analyze the possible causes of retinoschisis in highly myopic eyes by evaluating its optical coherence tomographic characteristics Methods A retrospective study was made of 43 eyes of 26 patients, who were admitted into our hospital from October 2000 to March 2003 due to complaints of vision decrease and were examined using optical coherence tomography (OCT). Among them,hospital records of 18 eyes (12 patients) (42%) with retinoschisis were analyzed. Results Retinoschisis in highly myopic eyes occurred mainly in the outer layer of the neurosensory retina in the posterior pole. Vitreoretinal traction was found in 15 out of 18 eyes. The neurosensory retinal thickness at the fovea which had not been involved by retinoschisis was significantly smaller than that of normal control ( P=0.0052 ). Conclusion Retinoschisis is not uncommon in highly myopic eyes and is associated with vitreoretinal traction and the pre-existing retinal degeneration.
Comprehensive analysis of genetic and clinical characteristics of 30 patients with X-linked juvenile retinoschisis in China

Abstract:Purpose: To provides the clinical and genetic characteristics of a series of Chinese patients with X-linked juvenile retinoschisis (XLRS) through multimodal imaging and next-generation sequencing. Methods: Thirty patients (60 eyes) from 29 unrelated families of Chinese origin with XLRS were screened using multigene panel testing, and underwent a complete clinical evaluation. All variants identified in this study and reported in the Human Gene Mutation Database were analysed. Results: Twenty-five distinct variants in the retinoschisin gene were identified, of which eight were novel, and one was de novo. Missense mutations were the most prevalent type, and mutation hot spot was localized in the discoidin domain. The mean Snellen best-corrected visual acuity was 0.28 ± 0.17. Of all eyes presenting with schisis, 92.86% had lamellar schisis and 62.5% had peripheral schisis. Schisis changes mostly involved inner and outer nuclear layers. X-linked juvenile retinoschisis (XLRS) patients had a high incidence of complications, and peripheral schisis was a risk factor for it. No obvious genotype-phenotype association was observed. Conclusion: This study provides comprehensive analyses of the genetic and clinical characteristics of XLRS in a cohort of Chinese patients. The fourth de novo mutation in RS1 was identified. And we show that XLRS has a wide spectrum of clinical characteristics; hence, molecular diagnosis is crucial for its diagnosis, differential diagnosis and genetic counselling. Peripheral schisis is a risk factor for the high incidence of complications, and no clear genotype-phenotype correlations were found.

Abstract:Abstract Background To evaluate alterations in the choroidal volume (CV) of eyes with entire macula-involved myopic retinoschisis (MRS) and to investigate predictive factors for CV and visual acuity. Methods This cross-sectional observational study recruited 32 MRS eyes, 52 non-MRS highly myopic eyes, and 55 emmetropic eyes. Retinal and choroidal morphological findings were obtained using SD-OCT. The CV of the three groups was compared, and factors associated with CV and BCVA were analyzed. Results The macular CV was significantly lower in MRS eyes than in emmetropic eyes ( P < 0.0001) and in non-MRS highly myopic eyes ( P < 0.05). The difference in CV for MRS eyes among the superior (0.34 ± 0.16 mm 3 ), inferior (0.33 ± 0.17 mm 3 ), nasal (0.27 ± 0.12 mm 3 ) and temporal (0.34 ± 0.21 mm 3 ) quadrants was not significant ( P = 0.3755). Multivariate regression analysis showed that the total macular CV in MRS eyes was negatively correlated with axial length ( P = 0.0459), decreasing by 0.19 mm 3 for each millimeter of axial length. Refractive error and defects in the ellipsoid zone (EZ) were associated with BCVA in logMAR units ( P = 0.0075 and 0.0002, respectively); BCVA decreased by 0.027 logMAR units for each diopter of myopia. The total macular retinal volume (RV) in MRS eyes with EZ defects was higher than that in eyes with intact or partially continuous EZs ( P = 0.0460). Conclusion In MRS eyes, the choroid is thinner, the total macular CV deceases as the axial length increases, and the severity of myopia and the integrity of the EZ, associated with the schisic extent are predictors of visual acuity, and the decrease of the macular CV is not the influencing factor of the visual function.

Abstract:Objective The objective is to use optical coherence tomography ( OCT) to observe the features of the retinoschisis in the fundus posterior pole of high myopia with posterior staphyloma, and to analyze the reasons to causthe disease.Methods The research is in the way of non-intervene and observation.212 eyes for more than 6.00D in 126 ca-ses were checked with type-B ultrasonic to find the patients who have posterior staphyloma.Further checks were made to them with split lamp, fundus check, mydriasis funduscopy check, Type-A ultrasonic check and OCT check.Then the OCT imagine with retinoschisises are analyzed.Results 37 eyes among the 212 eyes were found the the retinoschisis in fundus posterior pole of high myopia with posterior staphyloma.The location of the retinoschisis is corresponding to the location of the posterior staphyloma.Among the 37 eyes 26 eyes have single or interior and/or middle-level retinoschisis.29 eyes of the 37 eyes have the phenomenon of having vitreous body pull the retinas, including 22 eyes to have the location of the pos-terior hyaloids membrane be corresponding to the highest points of the retinoschisis or retinal detachment.Four eyes have macular holes and two eyes of them have full-thickness ceasma holes and two have pseudo-macular holes.The 175 eyes which have no retinoschisis shows that the thickness of their macular epiretinal membrance is negative corresponding to the eye axial lengths.Conclusion The retinoschisis has relationship with the vitreous body of the fundus posterior pole of high myopia with posterior staphyloma, pulling force of the internal limiting membrane and the stretching of the posterior staphy-loma.The shrinking of the choroid membrances for the high myopia weakens the closeness of the layers of the retinas to make the retinas be easy to form retinoschisis.The hight myopia people should have a regular check with OCT to have the former check and diagnosis of retinoschisis and/or retinal detachment.

Abstract:Abstract Backgrounds Unilateral high myopia (uHM), commonly observed in patients with retinal diseases or only with high myopia, is frequently associated with amblyopia with poor prognosis. This study aims to reveal the clinical and genetic spectrum of uHM in a large Chinese cohort. Methods A total of 75 probands with simplex uHM were included in our Pediatric and Genetic Eye Clinic. Patients with significant posterior anomalies other than myopic fundus changes were excluded. Variants were detected by exome sequencing and then analyzed through multiple-step bioinformatic and co-segregation analysis and finally confirmed by Sanger sequencing. Genetic findings were correlated with associated clinical data for analysis. Results Among the 75 probands with a mean age of 6.21 ± 4.70 years at the presentation, myopic fundus of C1 and C2 was observed in 73 (97.3%) probands. Surprisingly, specific peripheral changes were identified in 63 eyes involving 36 (48.0%) probands after extensive examination, including peripheral retinal avascular zone (74.6%, 47/63 eyes), neovascularization (54.0%), fluorescein leakage (31.7%), peripheral pigmentary changes (31.7%), and others. Exome sequencing identified 21 potential pathogenic variants of 13 genes in 20 of 75 (26.7%) probands, including genes for Stickler syndrome ( COL11A1 and COL2A1 ; 6/20), FEVR ( FZD4 , LRP5 , and TSPAN12 ; 5/20), and others ( FBN1, GPR179, ZEB2 , PAX6, GPR143 , OPN1LW, FRMD7, and CACNA1F ; 9/20). For the peripheral retinal changes in the 20 probands, variants in Stickler syndrome-related genes were predominantly associated with retinal pigmentary changes, lattice degeneration, and retinal avascular region, while variants in genes related to FEVR were mainly associated with the avascular zone, neovascularization, and fluorescein leakage. Conclusions Genetic defects were identified in about one-fourth of simplex uHM patients in which significant consequences may be hidden under a classic myopic fundus in up to half. To our knowledge, this is the first systematic genetic study on simplex uHM to date. In addition to routine care of strabismus and amblyopia, careful examination of the peripheral retina and genetic screening is warranted for patients with uHM in order to identify signs of risk for retinal detachment and other complications and provide meaningful genetic counseling.
