Abstract:Abstract Abstract 1901 Regulatory γδ T cells (γδ Tregs) is a novel subset of cells with immunosuppressive function while methods for γδ Treg induction is rarely introduced and its role in graft-versus-host disease (GVHD) prevention remains unkown. Decitabine, a kind of hypomethylating agents, can act synergistically with TGF-β1 to convert a variety of αβ T cells to regulatory αβ T cells with suppressive function but its role in induction and function of γδ Tregs has not been reported. We show here the role of decitabine for the induction of γδ Tregs. Moreover, we provide functional analysis and underlying mechanisms of decitabine-induced γδ Tregs relative to γδ Tregs without decitabine induction as well as in vivo evidences of their preventions on GVHD. Human peripheral blood mononuclear cells (PBMCs) were cultured with IL-2, IL-15, TGF-β1 and zoledronic acid (ZOL). On day 2, 0.5μmol/ml decitabine was added to aliquots of PBMCs. On days 4, 7, and 10, half of the supernatant volume was replaced with media containing cytokines. On day 11, frequencies of γδ Tregs were detected by flow cytometry (FACS). We found the frequency of γδ Tregs was 36.2% in TGF-β1/IL-15/ZOL stimulated group (referred to as common γδ Tregs below) and 59.9% in IL-2/TGF-β1/IL-15/ZOL/decitabine stimulated group (referred to as decitabine-induced γδ Tregs) (p<0.05). In order to compare immunosuppressive function of the two populations, γδ T cells containing γδ Tregs were isolated by magnetic cell sorting system (MACS) and tested for their ability to suppress proliferation of alloreactive PBMCs using CFSE-based assay. After 5 days of in vitro culture, CFSE-labeled PBMCs proliferation was significantly reduced in the presence of enriched γδ Tregs even at 8:1(PBMCs: γδ Tregs) ratio. The inhibition rate was significantly different (decitabine-induced γδ Tregs VS common γδ Tregs at ratio 1:1 is 81.3% VS 68.2%, p<0.05). To clarify the underlying mechanisms we performed ELISA to measure levels of inhibitory cytokines IL-10, IL-4 and TGF-β1 in supernatant of CFSE-based assay. We noted an elevated IL-10 secretion in the decitabine-induced γδ Tregs group compared with common γδ Tregs group (92.7±11pg/ml VS 10.3±2pg/ml at ratio 1:1, p<0.01). We confirmed the result by intracellular IL-10 detection using FACS. Previous reports showed high levels of inducible T-cell costimulator (ICOS) were correlated with IL-10 synthesis. So γδ Tregs were monitored for ICOS expression by FACS. The result revealed that ICOS expression was up-regulated in decitabine-induced γδ Tregs in contrast to common γδ Tregs (MFI: 268 VS 54). Stability of Foxp3 is a critical factor in the immunosuppressive ability of Tregs. Thus we evaluated the frequency of γδ Tregs after 5 days in CFSE-based assay. We observed loss of Foxp3 expression in decitabine-induced γδ Tregs was negligible (<3%) while 15.5% common γδ Tregs lost foxp3 expression. To confirm the results in vitro we tested the functional ability to prevent GVHD in vivo. GVHD was induced in NOD/SCID mice following busulfan and anti-CD122 condition and 1×107 PBMCs transfusion. Animals were co-injected with either decitabine-induced γδ Tregs or common γδ Tregs at a ratio of 1:1. Survival time and GVHD manifestations of the transplanted mice were evaluated. As a result, transplantation of human PBMCs alone induced lethal GVHD with average survival time 25± 8 days while the survival time was 43± 5 days and 58±7 days in mice co-injected with common γδ Tregs and decitabine-induced γδ Tregs, respectively (p<0.05). Clinical manifestations such as hunched back, diarrhea, and body weight loss were statistically different among 3 groups. To investigate the infiltration of human lymphocytes into nonlymphoid tissues in GVHD mice, we performed immunohistochemical analysis of the liver and intestines using anti-human CD45. Remarkably abundant invasion of human CD45+ cells was observed around the veins in the liver and intestines transplanted with PBMCs alone while less invasion in mice co-injected with common γδ Tregs and the lest invasion in mice co-injected with decitabine-induced γδ Tregs. Altogether, our findings reveal that decitabine and the cytokines can efficiently syngenerize to induce γδ Tregs with enhanced immunosuppression on GVHD which are via higher levels of IL-10 production due to ICOS up-regulation as well as stability of Foxp3 expression. Thus γδ Tregs may be potentially exploited therapeutically in a variety of transplantation settings. Disclosures: No relevant conflicts of interest to declare.

Abstract:Abstract Abstract 3281 Regulatory γδT cells (γδTregs) are capable of suppressing T-cell activation and proliferation (Rita et al Journal of immunology 2009). Our previous studies demonstrated the immunosuppressive features of γδTregs and further supported the use of γδTregs as a promising therapeutic strategy for the adoptive immunotherapy of allograft rejection, post-transplantation graft-versus-host disease (GVHD) and autoimmune diseases (Hu et al ASH abstract 2011). To this end, large-scale, efficient expansion of γδTregs represents a fundamentally important prerequisite for their clinical application. Here we explored the efficacy of decitabine (a DNA hypomethylating agent) for the generation of functional γδTregs and its underlying mechanisms. Human peripheral blood mononuclear cells (PBMCs) were cultured with interleukin (IL)-2, IL-15, TGF-β1 and zoledronic acid (ZOL). Decitabine was added to aliquots of PBMCs. Frequency and absolute number of γδTregs were investigated by flow cytometry (FACS) and trypan blue live-cell counts after cultivation. Our results showed a higher frequency (61.9±10.3% versus 38.4±7.8%, p<0.01) and absolute number of γδTregs [(3.1±0.1)×106 versus (5.9±0.2)×106, p<0.01] were seen in cells treated with decitabine plus ZOL/IL-2/IL-15/TGF-β1 (referred to as decitabine-induced γδTregs below) than in cell without decitabine induction (referred to as common γδTregs) as shown in Fig. 1. These data demonstrated that decitabine and ZOL/cytokines synergized to induce stable γδTregs via improved Foxp3 expression. Next we analyzed the underlying mechanisms of decitabine on γδTreg induction. Genomic DNA was isolated from γδTregs sorted by FACS. The Foxp3 gene methyaltion status of upstream enhancer CpG islands from −5995 to −5778 and from −5031 to −4792, proximal promoter CpG island from -139 to -15 and non-coding DNA sequence 3 (CNS3) CpG island from 3748 to 3971 was analyzed by bisulfite sequencing. We found decitabine-induced γδTregs displayed efficient CpG demethylation of the upstream enhancer from −5031 to −4792, and of CNS3 from 3748 to 3971, but no differences were detected in the other CpG islands investigated in contrast to common γδTregs (Fig. 2A). We subsequently investigated if DNA demethylation of CpG islands could influence upstream enhancer or CNS3 activity to promote Foxp3 transcription. The upstream enhancer from −5031 to −4792 and CNS3 from 3748 to 3971 were methylated using SssI (CpG) methylase, and methylated or unmethylated sequences were inserted into demethylated pFoxP3Pro, respectively. Vectors were added to γδT cells and electroporated using the T-023 program of the Nucleofector. Dual fluorescence-based transient reporter assays showed an obvious reduction in luciferase activity after methylation of both CpG islands, but not with the demethylated CpG islands (Fig. 2B), indicating that demethylation of the upstream enhancer and CNS3 can promote Foxp3 expression via increased enhancer activity. Transcription factors (TF) play important roles in gene expression. To test whether decitabine enhanced specific TFs expression to influence γδTreg induction, we performed TF CHIP assays in FACS-sorted common γδTregs versus decitabine-induced γδTregs. The expression levels of 245 TFs were evaluated and the result showed that NF-κb expression in decitabine-induced γδTregs was up-regulated 50 times in contrast to that in common γδTregs. NF-κb up-regulation was further comfirmed by western blot analysis from 6 different healthy volunteers. Finally, We evaluated the immunosuppressive function of the enriched γδTregs on Carboxyfluorescein Diacetate (CFSE)-labeled hPBMC proliferation and activation stimulated with anti-CD3/anti-CD28 monoclonal antibodies for 5 days in vitro. As shown in Fig. 3, both common γδTregs and decitabine-induced γδTregs exerted dose-dependent suppression of proliferation, but decitabine-induced γδTregs had a greater suppressive effect than common γδTregs. In conclusion, these data demonstrate for the first time that decitabine facilitates the generation of functional γδTregs via foxp3 gene demethylation and NF-κb up-regulation. Futhermore, our results lay the foundation for large-scale availability of induced γδTregs for potential clinical applications, and reveal novel targets for regulating Foxp3 expression in γδT cells. Demethylating Agent Decitabine Disrupts Tumor-Induced Immune Tolerance by Depleting Myeloid-Derived Suppressor Cells

Abstract:Purpose The immunoregulatory effect of demethylating agent decitabine (DAC) has been recognized recently. However, little is known about its impact on immune tolerance. In this study, we aimed to determine the impact of DAC on the immune tolerance induced by tumor cells. Methods The effects of DAC on immune cells in vivo were measured by flow cytometry. Myeloid-derived suppressor cells (MDSCs) were sorted using magnetic beads and cultured in vitro. The mixed lymphocyte reaction was used to determine the immunoregulatory effect of DAC in vitro. An adoptive transfusion mouse model was established to evaluate the effect in vivo. Results We found that DAC treatment significantly depleted MDSCs in vivo by inducing MDSCs apoptosis. When given at a low dose, the immune effector cells were less affected by the treatment, except for MDSCs. The mixed lymphocyte reaction in vitro showed that T-cell responses were enhanced when MDSCs were depleted. Supplementation of MDSCs would attenuate this T-cell activation effect. Using an adoptive transfusion mouse model, we further demonstrated in vivo that DAC treatment could induce autologous anti-tumor immune response by depleting MDSCs. Conclusions This study is the first to illustrate DAC’s immunoregulatory effect on immune tolerance. The disruption of immune tolerance is due to MDSCs depletion that induces an autologous immune response in vivo. By depleting MDSCs, DAC treatment removes one of the obstacles affecting anti-tumor immune activation and warrants further experimental and clinical studies to explore its potential utility in combination with various anti-tumor immunotherapies in the future.

Abstract:Background: The hypomethylating agent decitabine is the standard therapy for intermediate or high risk myelodysplastic syndrome (MDS).Methods: In this trial, 191 adult patients with intermediate/high risk MDS (IPSS score = 0.5) randomly received decitabine using a standard regimen (20 mg/m(2)/day for 5 consecutive days; n = 94) or an extended regimen with lower daily dose (12 mg/m(2)/day for 8 consecutive days; n = 97) every 4 weeks, for a total of 4 cycles.Results: The median follow-up was 14 months (range 2-36). The primary end point of overall response rate in the intent-to-treat analysis was 41.5% and 38.1% in the standard and extended dosing arms, respectively (p = 0.660). Complete remission and marrow complete remission also did not differ between the two arms. Cytopenia was the most frequent adverse event (76.4%). The median duration of neutropenia per cycle did not differ between the two arms during the first two cycles, but significantly shorter in the extended dosing arm in the third cycle (8.5 vs. 15.5 days, p = 0.049) and in the fourth cycle (8 vs. 14 days, p = 0.294).Conclusion: The 5-day 20-mg/m(2)/day and 8-day 12-mg/m(2)/day decitabine regimens have similar efficacy and safety in patients with intermediate or high risk MDS.
Low Dose Decitabine Treatment Induces CD80 Expression in Cancer Cells and Stimulates Tumor Specific Cytotoxic T Lymphocyte Responses.

Abstract:Lack of immunogenicity of cancer cells has been considered a major reason for their failure in induction of a tumor specific T cell response. In this paper, we present evidence that decitabine (DAC), a DNA methylation inhibitor that is currently used for the treatment of myelodysplastic syndrome (MDS), acute myeloid leukemia (AML) and other malignant neoplasms, is capable of eliciting an anti-tumor cytotoxic T lymphocyte (CTL) response in mouse EL4 tumor model. C57BL/6 mice with established EL4 tumors were treated with DAC (1.0 mg/kg body weight) once daily for 5 days. We found that DAC treatment resulted in infiltration of IFN-γ producing T lymphocytes into tumors and caused tumor rejection. Depletion of CD8(+), but not CD4(+) T cells resumed tumor growth. DAC-induced CTL response appeared to be elicited by the induction of CD80 expression on tumor cells. Epigenetic evidence suggests that DAC induces CD80 expression in EL4 cells via demethylation of CpG dinucleotide sites in the promoter of CD80 gene. In addition, we also showed that a transient, low-dose DAC treatment can induce CD80 gene expression in a variety of human cancer cells. This study provides the first evidence that epigenetic modulation can induce the expression of a major T cell co-stimulatory molecule on cancer cells, which can overcome immune tolerance, and induce an efficient anti-tumor CTL response. The results have important implications in designing DAC-based cancer immunotherapy.

Abstract:As a component of the innate immune cell population, gamma delta T cells are involved in tumor immunosurveillance and host defense against viral invasion. In this study, we demonstrated a novel function of human gamma delta T cells as regulatory cells by detecting their suppressive effect on the proliferation of autologous naive CD4(+) T cells. These regulatory gamma delta T cells (gamma delta Tregs) could be generated in vitro by stimulating with anti-TCR gamma delta in the presence of TGF-beta and IL-2. Similar to CD4(+)Foxp3(+) Tregs, gamma delta Tregs also expressed Foxp3. Additionally, they primarily belonged to the V delta 1 subset with a CD27(+)CD25(high) phenotype. Furthermore, these gamma delta Tregs showed an immunoregulatory activity mainly through cell-to-cell contact. Importantly, this gamma delta regulatory population decreased in the peripheral blood of systemic lupus erythematosus patients, suggesting a potential mechanism in understanding the pathogenesis of systemic lupus erythematosus. Effect of domestic decitabine influence on morphology, proliferation and CD80 expression in mouse lymphoma/leukemia cell line EL4 cells

Abstract:Objective To compare the influence of cytotoxicity and CD80 expression in EL4 cells, following in vitro treatment with decitabine ( DEC) manufactured internal.Methods EL4 cell line was treated with Dacogen ( DAC, manu-factured by Xian Janssen Pharmaceutical Ltd) or DEC (final concentration 0.25μmol/L) in vitro for 3 days.Morphology was analysised by method of Wright-Gimesa staining.CD80 expression was detected by method of flow cytometry.Cell pro-liferation was detected by method of CCK-8 kit.Results DAC and DEC treatment resulted in similar morphological chan-ges in EL4 cells.Moreover, effects of CD80 expression and inhibited proliferation induced by both DAC and DEC in EL4 cells were no significant differences (P>0.05).Conclusions There was no significant difference between DAC and DEC in the ability of change cellular morphology, inhibit proliferation and stimulate CD80 expression in mouse-lymphoma/leuke-mia cell EL4 cultured in vitro, which indicates the effects of the two drugs were similarity in both cytotoxic effect on tumor cells and stimulation of anti-tumor immune response when administered in vivo.

Abstract:γδ T cells represent a relevant proportion of T lymphocytes that express T cell receptors (TCRs) encoded by the γ and δ T cell receptor genes. Whereas the most circulating γδ T cell type, Vδ2 T cells, has been described and studied intensively, the potential role of Vδ1 T cells remains largely unclear. Here, we identified that expanded peripheral blood Vδ1 T cells stimulated with anti-human TCR Vδ1 monoclonal antibody (mAb) in vitro predominantly expressed forkhead box p3 (Foxp3). In addition, these cells also expressed other regulatory T cell-related molecules, such as CD25, glucocorticoid-induced TNFR family-related protein and cytotoxic T lymphocyte-associated protein-4 (CTLA-4), and held the potent capacity for the production of transforming growth factor beta 1 (TGF-β1). These autocrine and/or paracrine TGF-β1 could bind TGF-β1 receptors on Vδ1 T cells and induce sustained Foxp3 expression. Moreover, Foxp3 expression coincided with high CD25 expression. CD25 high Vδ1 T cells exhibited stronger suppression on CD4 + T cell proliferation compared with TGF-β1-induced CD25 high CD4 + regulatory T cells. Therefore, our phenotypic and functional analyses first demonstrate the potential regulatory property of anti-human TCR Vδ1 mAb-activated Vδ1 T cells. These results will broaden our understanding about the role of peripheral blood Vδ1 T cells under physical and pathological conditions.
Decitabine for myelodysplastic syndromes: dose comparison in a real world clinical setting.

Abstract:We retrospectively studied 133 myelodysplastic syndrome patients receiving decitabine during January 2009 and September 2017. The dose of 15 mg/m2/d (n = 83) and 20 mg/m2/d (n = 50) had comparable overall response rates (ORR) (51.8% vs. 52.00%) and complete remission rate (CRR) (15.66% vs. 22.00%). The 15 mg/m2/d group had a lower incidence of grade 3/4 neutropenia (60.24% vs. 88.00%, p < .05) and thrombocytopenia (65.06% vs. 88.00%, p < .05). The 15 mg/m2/d group had a longer median overall survival (OS) (21.60 months vs. 15.23 months, p = .02). The same results were seen in refractory anemia with excess blasts (RAEB) patients: The 15 mg/m2/d group also had comparable ORR, CRR, decreased hematological toxicities and longer OS. Further analysis suggested that survival benefit of 15 mg/m2/d group was mainly in those patients with lower risk stratification. In conclusion, 15 mg/m2/d decitabine is associated with a lower incidence of hematological toxicities and longer OS and may be more suitable for patients with relatively lower risk.

Abstract:BACKGROUND:Cancer testis antigens (CTAs) are a novel group of tumor associated antigens. Demethylating agent decitabine was reported to be able to up-regulate CTAs through its hypomethylation mechanism, thus enhance the immunogenicity of leukemia cells. However, few researches have ever focused on the questions that whether this immunostimulatory effect of decitabine could induce autologous CTA specific cytotoxic T lymphocytes (CTLs) in vivo, and if so, whether this effect contributes to disease control. In this study, we aimed to show that decitabine could induce specific autologous CTLs against some mouse CTAs in leukemia cells in vitro and in vivo.METHODS:Several mouse CTAs were screened by RT-PCR. CTL specific to one of the CTAs named P1A was detected and sorted by P1A specific dimer by flow cytometry. The activity of specific CTLs was measured by real time RT-PCR.RESULTS:We firstly screened expression of some CTAs in mouse leukemia cells before and after decitabine treatment and found that decitabine treatment did up-regulate expression of many CTAs. Then we measured the CTLs' activity specific to a mouse CTA P1A in vivo and showed that this activity increased after decitabine treatment. Finally, we sorted these in vivo induced P1A specific CTLs by flow cytometry and demonstrated their cytotoxicity against decitabine treated leukemia cells.CONCLUSIONS:Our study showed the autologous immune response induced by decitabine in vivo. And more importantly, we firstly proved that this response may contribute to disease control. We believe that this immunostimulatory effect is another anti-cancer mechanism of decitabine, and this special effect would inspire new applications of decitabine in the field of leukemia treatment in the future.

Abstract:Abstract 3628 PURPOSE: In the preliminary experiment, we investigated the effect of anti-leukemia drugs (idarubicin, daunorubicin cytarabine, thalidomide, homoharringtonine) in combination with decitabine of inhibiting growth of myeloid leukemic cell lines. The result showed notable synergistic effect of sequential combination of decitabine and idarubicin. In searching the associated mechanism, we assessed the effect of sequential combination of decitabine and idarubicin in apoptosis, cell cycle arrest, and depression of wnt pathway. Methods: Myeloid leukemia cells (U937, HEL and SKM-1) were treated with decitabine alone, idarubicin alone, and decitabine sequential with idarubicin to determine their impact on cellular proliferation, cell cycle regulation, apoptosis and the regulation of wnt pathway by detecting DNA methylation status, mRNA level and protein expression. Results: The sequential combination of decitabine and idarubicin produced synergistic inhibition of myeloid leukemia cells growth by apoptosis and cell cycle arrest. Sequential treatment with both agents induced synergistic effect on decreasing methylation status of wnt antagonists (SFRP1, HDPR1 and DKK3) which result in re-expression or up-expression of these genes. Furthermore, we found that the down-steam genes of wnt pathway including c-myc and β-catenin were down-regulated. Conclusion: Taken together, these findings showed that decitabine sequentially combined with idarubicin has synergistic anti-leukemia effect, which suggest clinical potential in the treatment of myeloid leukemias. Anti-γδ TCR Antibody-Expanded Γδ T Cells: a Better Choice for the Adoptive Immunotherapy of Lymphoid Malignancies

Abstract:Cell-based immunotherapy for lymphoid malignancies has gained increasing attention as patients develop resistance to conventional treatments. γδ T cells, which have major histocompatibility complex (MHC)-unrestricted lytic activity, have become a promising candidate population for adoptive cell transfer therapy. We previously established a stable condition for expanding γδ T cells by using anti-γδ T-cell receptor (TCR) antibody. In this study, we found that adoptive transfer of the expanded γδ T cells to Daudi lymphoma-bearing nude mice significantly prolonged the survival time of the mice and improved their living status. We further investigated the characteristics of these antibody-expanded γδ T cells compared to the more commonly used phosphoantigen-expanded γδ T cells and evaluated the feasibility of employing them in the treatment of lymphoid malignancies. Slow but sustained proliferation of human peripheral blood γδ T cells was observed upon stimulation with anti-γδ TCR antibody. Compared to phosphoantigen-stimulated γδ T cells, the antibody-expanded cells manifested similar functional phenotypes and cytotoxic activity towards lymphoma cell lines. It is noteworthy that the anti-γδ TCR antibody could expand both the Vδ1 and Vδ2 subsets of γδ T cells. The in vitro-expanded Vδ1 T cells displayed comparable tumour cell-killing activity to Vδ2 T cells. Importantly, owing to higher C–C chemokine receptor 4 (CCR4) and CCR8 expression, the Vδ1 T cells were more prone to infiltrate CCL17- or CCL22-expressing lymphomas than the Vδ2 T cells. Characterizing the peripheral blood γδ T cells from lymphoma patients further confirmed that the anti-γδ TCR antibody-expanded γδ T cells could be a more efficacious choice for the treatment of lymphoid malignancies than phosphoantigen-expanded γδ T cells.

Abstract:Background CD4 + T cells play multiple roles in controlling tumor growth and increasing IFN-γ + T-helper 1 cell population could promote cell-mediated anti-tumor immune response. We have previously showed that low-dose DNA demethylating agent decitabine therapy promotes CD3 + T-cell proliferation and cytotoxicity; however, direct regulation of purified CD4 + T cells and the underlying mechanisms remain unclear. Methods The effects of low-dose decitabine on sorted CD4 + T cells were detected both in vitro and in vivo . The activation, proliferation, intracellular cytokine production and cytolysis activity of CD4 + T cells were analyzed by FACS and DELFIA time-resolved fluorescence assays. In vivo ubiquitination assay was performed to assess protein degradation. Moreover, phosphor-p65 and IκBα levels were detected in sorted CD4 + T cells from solid tumor patients with decitabine-based therapy. Results Low-dose decitabine treatment promoted the proliferation and activation of sorted CD4 + T cells, with increased frequency of IFN-γ + Th1 subset and enhanced cytolytic activity in vitro and in vivo . NF-κB inhibitor, BAY 11-7082, suppressed decitabine-induced CD4 + T cell proliferation and IFN-γ production. In terms of mechanism, low-dose decitabine augmented the expression of E3 ligase β-TrCP, promoted the ubiquitination and degradation of IκBα and resulted in NF-κB activation. Notably, we observed that in vitro low-dose decitabine treatment induced NF-κB activation in CD4 + T cells from patients with a response to decitabine-primed chemotherapy rather than those without a response. Conclusion These data suggest that low-dose decitabine potentiates CD4 + T cell anti-tumor immunity through enhancing IκBα degradation and therefore NF-κB activation and IFN-γ production.

[A preliminary study on the outcome of lower-risk myelodysplastic syndrome by low-dose decitabine].

Abstract:Objective: To assess the efficiency and safety of low-dose decitabine in patients with lower-risk myelodysplastic syndrome (MDS) to couple with the clinical significance of MDS-related gene mutations. Methods: This study was done in 4 institutions in Zhejiang Province. A total of 62 newly diagnosed patients with lower-risk MDS were assigned to two groups of decitabine (12 mg·m(-2)·d(-1) for 5 consecutive days) and best supportive care (BSC) . Their bone marrow samples were subject to examinations of MDS-related 15 gene mutations. The primary endpoints were the proportion of patients who achieved overall response (ORR) after at least two cycles and progression-free survival (PFS) , and their relevances to the gene mutations. Results: Of 62 enrolled patients, and 51 cases were included in the final analysis. 16 of 24 patients (66.7%) in decitabine group achieved ORR versus 8 of 27 (29.6%) in BSC group (χ(2)=6.996, P=0.008) ; PFS prolongation of decitabine versus BSC was statistically significant (not reached vs 13.7 months, P=0.037) . Among 51 patients, at least one gene mutation was identified in 20 patients (39.2%) , including 4 single SF3B1 mutation. PFS in cases with gene mutations (not including single SF3B1 mutation) was significantly shorter than of no gene mutation (9.2 months vs 18.5 months, P=0.008) , but not for ORR (37.5% vs 58.1%, P=0.181) . Among 16 patients with mutated genes, ORR in decitabine and BSC groups were 75% (6/8) and 0 (0/8) , respectively. The most adverse events in decitabine group were grade 3 to 4 neutropenia (45.8%) and grade 3 to 4 infections (33.3%) . Conclusion: This preliminary study showed that low-dose decitabine produced promising results with an acceptable safety in lower-risk MDS patients, especially for those with mutated genes. Further study targeting poor prognostic lower-risk MDS patients should be warranted.
Low-dose decitabine modulates T-cell homeostasis and restores immune tolerance in immune thrombocytopenia

Abstract:Abstract Our previous clinical study showed that low-dose decitabine exhibited sustained responses in nearly half of patients with refractory immune thrombocytopenia (ITP). The long-term efficacy of decitabine in ITP is not likely due to its simple role in increasing platelet production. Whether decitabine has the potential to restore immune tolerance in ITP is unknown. In this study, we analyzed the effect of decitabine on T-cell subpopulations in ITP in vitro and in vivo. We found that low-dose decitabine promoted the generation and differentiation of regulatory T (Treg) cells and augmented their immunosuppressive function. Splenocytes from CD61 knockout mice immunized with CD61+ platelets were transferred into severe combined immunodeficient mouse recipients to induce a murine model of ITP. Low-dose decitabine alleviated thrombocytopenia and restored the balance between Treg and helper T (Th) cells in active ITP mice. Treg deletion and depletion offset the effect of decitabine in restoring CD4+ T-cell subpopulations in ITP mice. For patients who received low-dose decitabine, the quantity and function of Treg cells were substantially improved, whereas Th1 and Th17 cells were suppressed compared with the pretreatment levels. Next-generation RNA-sequencing and cytokine analysis showed that low-dose decitabine rebalanced T-cell homeostasis, decreased proinflammatory cytokines, and downregulated phosphorylated STAT3 in patients with ITP. STAT3 inhibition analysis suggested that low-dose decitabine might restore Treg cells by inhibiting STAT3 activation. In conclusion, our data indicate that the immunomodulatory effect of decitabine provides one possible mechanistic explanation for the sustained response achieved by low-dose decitabine in ITP.

Abstract:Decitabine treatment improves immunological recognition that increases expression of cancer-testis antigens (CTAs) against solid tumors. The mechanisms of decitabine enhancement of immunogenicity when used for patients with myelodysplastic syndromes (MDS) remain unclear. In the present study, we found relatively low baseline expression of MAGE-A1, MAGE-A3, and SP17 in MDS-derived cell lines. Decitabine treatment significantly improved MAGE-A1, MAGE-A3, and SP17 expression in these cell lines and in MDS patients. Decitabine-treated K562 and SKM-1 target cells with incrementally induced MAGE-A1, MAGE-A3, or SP17 levels up-regulated T lymphocyte function. Decitabine treatment improved CTA-specific cytotoxic T lymphocyte (CTL) recognition of MDS cells via the up-regulation of CTAs. This response was accompanied by enhanced T lymphocyte function and HLA class antigen expression, and increased ICAM-1. These findings suggested that decitabine may have a broad range of therapeutic applications when it is used in association with active adaptive immunity responses against up-regulated CTAs.

Abstract:Gamma delta T cell, expressing gamma delta T cell receptor (gamma delta TCR), is considered as a group of unconventional T cells linking innate and adaptive immunity. Although gamma delta T cells only represent a minor population in human immune system, their functions are very broad. Recent researches have suggested that depending on the microenvironment, gamma delta T cells can assume features reminiscent of Th1, Th2, Th17 and regulatory T cells (Treg) as well as professional antigen present cells. Regulatory gamma delta T cell (gamma delta Treg) is a recently reported subset of gamma delta T cells characterized by both expressions of gamma delta TCR and forkhead/winged-helix family transcriptional repressor p3 (Foxp3), with potential immunosuppressive functions. Researches that focused on gamma delta Treg have showed the regulatory roles it has in preventing autoimmune response and relieving autoimmune diseases such as systemic lupus erythematosus (SLE), and the suppressive activity of breast tumor-derived gamma delta Tregs on innate and adaptive immunity, as well as the protective effects enhanced by gamma delta Tregs against xenogenic graft-versus-host disease in humanized mice found in our previous studies. All these observations suggest the important roles that gamma delta Treg plays in human immune system and its potential clinical applications. However, the further studies of gamma delta Treg are limited mainly due to its low quantities in vivo and the lack of methods to induce gamma delta Treg largely in vitro. It would be much significant if we found efficient induction and expansion methods for functional gamma delta Tregs.

Abstract:Regulatory γδT cell (γδTreg) is a recently reported subset of γδT cells characterized by both expressions of TCRγδ and Foxp3, with potential immunosuppressive functions. However, the further studies of γδTreg are limited mainly due to its low quantities in vivo and the lack of methods to induce γδTreg largely in vitro. Here we show that rapamycin together with TGF-β1, IL-2 and IL-15 can induce and expand γδTregs derived from human peripheral blood mononuclear cells efficiently in vitro. Thus-induced γδTregs express high levels of Foxp3 and CD25, and display significant immunosuppressive activities in vitro.

Abstract:Myeloid-derived suppressor cells (MDSCs) play a crucial role in promoting melanoma metastasis. Reprogramming MDSCs into mature M1 macrophages has emerged as a strategy to inhibit metastasis. Decitabine (Dec) is known to eradicate MDSCs and suppress tumor growth. In this study, we provide evidence that Dec not only reduces the MDSC population by inducing apoptosis, arresting cell cycle, and impairing recruitment, but also suppresses their immunosuppressive function by downregulating related genes and facilitating differentiation into M1 macrophages. Transcriptomic analysis of Dec-treated MDSCs revealed a marked downregulation of immunosuppressive genes including S100a9 , S100a8 , Vegf , Cxcr2 , and Nos2 . Meanwhile, M1 macrophage-associated genes involved in immune activation were upregulated, such as Ddx58 , Isg15 , Tap1 , Ccl5 , Cxcl9 , and Cxcl10 . Further bioinformatic analysis indicated that Dec promotes MDSC-to-M1 macrophage differentiation and activates innate immune pathways including NOD-like signaling to enhance anti-tumor immunity. Time-course studies implied that Dec upregulates myeloid transcription factor Irf7 to initiate MDSC differentiation and orchestrate the anti-tumor immune response. Collectively, our study unveils a novel dual-functional mechanism of Dec as both a cytotoxic agent reducing MDSCs and an inducer of their differentiation into M1 macrophages, thereby alleviating immunosuppression. This highlights Dec's potential for clinical melanoma metastasis suppression.

Abstract:CD8 + exhausted T cells (T ex ) are heterogeneous. PD-1 inhibitors reinvigorate progenitor T ex , which subsequently differentiate into irresponsive terminal T ex . The ability to maintain a capacity for durable proliferation of progenitor T ex is important, but the mechanism remains unclear. Here, we showed CD8 + progenitor T ex pretreated with decitabine, a low-dose DNA demethylating agent, had enhanced proliferation and effector function against tumors after anti–PD-1 treatment in vitro. Treatment with decitabine plus anti–PD-1 promoted the activation and expansion of tumor-infiltrated CD8 + progenitor T ex and efficiently suppressed tumor growth in multiple tumor models. Transcriptional and epigenetic profiling of tumor-infiltrated T cells demonstrated that the combination of decitabine plus anti–PD-1 markedly elevated the clonal expansion and cytolytic activity of progenitor T ex compared with anti–PD-1 monotherapy and restrained CD8 + T cell terminal differentiation. Strikingly, decitabine plus anti–PD-1 sustained the expression and activity of the AP-1 transcription factor JunD, which was reduced following PD-1 blockade therapy. Downregulation of JunD repressed T cell proliferation, and activation of JNK/AP-1 signaling in CD8 + T cells enhanced the antitumor capacity of PD-1 inhibitors. Together, epigenetic agents remodel CD8 + progenitor T ex populations and improve responsiveness to anti–PD-1 therapy.
