Abstract:Objective: Calcific aortic valve disease (CAVD) is a prevalent type of valvular heart disease, its association with dyslipidemia remains controversial. Methods: Of 449 CAVD patients who underwent aortic valve replacement, 228 formed the aortic valve calcification (AVC) group, and 221 were the non-calcification group. We retrospectively reviewed the preoperative and one-year postoperative plasma lipoprotein levels of both and performed a logistic regression to evaluate the factors associated with AVC. Results: Preoperatively, AVC patients had significantly higher coronary heart disease (43.0% vs 24.9%, p< 0.001), peripheral vascular disease (41.7% vs 26.2%, p< 0.001), and heart failure rates (63.6% vs 47.1%, p< 0.001), and a higher level of total cholesterol (4.1 +/- 0.9 vs 3.9 +/- 0.8 mmol/L, p=0.032) and very low-density cholesterol (0.6 (0.4-0.7) vs 0.5 (0.3-0.7) mmol/L, p=0.054). Echocardiography revealed a significant difference of aortic stenosis in both AVC and non-AVC groups (p< 0.05), and also identified aortic regurgitation (AR) with a significant difference between these two groups (p=0.003). The peak transaortic jet velocity, peak transaortic gradient, and mean transaortic gradient were significantly higher in the calcification group (all p< 0.001), but the aortic valve area (0.7 (0.5-1.0) vs 4 (0.9-4.5) cm(2); p< 0.001) was smaller. Age (OR=1.023), total cholesterol (OR=1.272), and mean transaortic gradient (OR=1.182) were AVC risk factors. A larger aortic valve area (OR=0.010) were protective factors. The one-year mortality and perivalvular leakage rates were significantly higher in the calcification group. Conclusion: Total cholesterol was significantly higher in AVC patients and may be an AVC risk factor along with age and mean transaortic gradient. AVC patients had a relatively poorer outcome within one year.

Abstract:Objective To evaluate the incidence of aortic valve calcification,and the correlation between valve function and commonly encountered disease in the aged patients.Methods Totally 996 patients who underwent ultrasonic cardiography (UCG) in our hospital were included.They were divide into elderly group and non elderly group,and the elderly group was divided into calcification subgroup and non calcification subgroup.The calcification,stenosis and regurgitation of aortic valve were detected by UCG,and risk factors of calcification were evaluated by Logistic regression analysis.Results The incidence of calcification was higher in elderly group than in non elderly group [71.8％ (526/733) vs.14.6％ (34/233),x2＝237.10,P＜0.01].In elderly group,the incidence of aortic valve stenosis was 2.1％ (11/526) in calcification subgroup and 1.9％ (4/207) in non calcification subgroup (x2＝0.81,P＞0.05).In elderly group,the incidence of aortic valve regurgitation was 63.3％ (333/526) in calcification subgroup and 19.3％ (40/207) in non calcification subgroup (x2＝116.10,P＜0.01).The hazard ratio of aortic valve calcification in different diseases were as follows:hypertension (OR＝2.06,95％CI:1.400-3.031),coronary heart disease (OR＝3.46,95％CI:2.217-5.384),diabetes mellitus (OR ＝ 2.66,95％CI:1.652-4.278),renal dysfunction (OR＝ 2.34,95％ CI:1.415-3.869),osteoporosis (OR＝ 2.33,95％CI:1.119-4.838).Conclusions The incidence of calcification,mainly causing aortic valve regurgitation,is high in elderly patients.Patients with hypertension,coronary heart disease,diabetes mellitus,renal dysfunction and osteoporosis are prone to the development of aortic valve calcification.
SERUM URIC ACID LEVELS WERE INDEPENDENTLY ASSOCIATED WITH AORTIC ARCH CALCIFICATION IN MIDDLE- AGED AND ELDERLY CHINESE POPULATION

Abstract:Background: Aorta calcification are considered markers of subclinical atherosclerotic disease that are independent predictors of subsequent cardiovascular events. A growing number of studies have shown that serum uric acid is associated with the development of cardiovascular diseases. Objectives: The main purpose of this study was to investigate the prevalence of aorta calcification and to determine the relationship between the levels of serum uric acid and aorta calcification in the middle-aged and elderly population. Methods: From Jan 2018 to Dec 2018, totally 6152 consecutive participants aged >50 years old underwent annual health survey were included in this study. Detailed physical examination was performed as well as a thorough review from structured questionnaires, which in baseline demographics and medical history including alcohol consumption, smoking and physical activity status, BMI, blood pressure, blood glucose and serum lipid were all collected and recorded. Aortic calcification was analyzed by chest X-ray. All data were analyzed retrospectively. Results: The prevalence of aortic arch calcification was 13.9% in these 6152 participants with age >50 years and was equally common in men and women. Aortic arch calcification prevalence was significantly increased in populations with hyperuricemia (defined as serum uric acid >420 umol/L), as compared with populations with normal serum uric acid levels (defined as serum uric acid ≤ 420 umol/L) (83.39% vs. 16.61%, p < 0.001). Participants were divided into three groups according to their uric acid levels.A more prevalent aortic arch calcification was identified in high uric acid level group than those in middle or low uric acid level group (p < 0.05). On logistic regression analysis, serum uric acid levels were found to be independently associated with aortic arch calcification (OR=1.469, 95%CI: 1.199∼1.800, P<0.01) in these participants. Conclusion: Serum uric acid levels were independently associated with aortic arch calcification in middle-aged and elderly Chinese population. These results may suggest the need for more aggressive actions for urate lowering therapy in middle-aged and elderly hyperuricemia patients, especially in patients coexist with other cardiovascular disease risks. Abstract:Although scientists have made a significant breakthrough in transcatheter aortic valve implantation (TAVI), however, this technique is now just limited to application in patients with calcific aortic valve stenosis (CAS). Non-calcific aortic stenosis (AS) is currently not an indication for TAVI. Therefore, investigating the prevalence of CAS, especially severe CAS, can give us useful information about the number of candidates indicated to TAVI. Association of Serum Lipoprotein(a) Level with the Severity and Prognosis of Calcific Aortic Valve Stenosis: a Chinese Cohort Study.

Abstract:BACKGROUND:There was a causal relationship between elevated lipoprotein(a) [Lp(a)] levels and increased risk of calcific aortic valve stenosis (CAVS) in whites and blacks. The present study aimed to investigate whether Lp(a) levels were associated with aortic stenosis (AS) severity and clinical events in Chinese patients.METHODS:Levels of serum Lp(a) were measured in 652 patients with CAVS, whom all underwent baseline echocardiographic examination. The clinical endpoint was defined as a composite of aortic valve replacement (AVR) and cardiac death.RESULTS:Patients in the tertile 3 of Lp(a) had a higher percentage of severe AS compared with those in the tertile 1 and 2 of Lp(a) (46.2% vs. 33.9%, P = 0.005). Moreover, the top tertile of Lp(a) was an independent predictor of severe AS (OR = 1.78, 95% CI: 1.18-2.66, P = 0.006). However, there was no significant association between tertile 3 of Lp(a) and clinical events (hazard ratio: 0.73; 95% CI: 0.43-1.24; P = 0.239) in the multivariate Cox regression analysis during a mean follow-up time of 3.16 ± 2.74 years.CONCLUSIONS:Elevated Lp(a) level was an independent predictor of severe AS by echocardiography in the Chinese population, but was not associated with the increased risk of AVR and cardiac death, suggesting that Lp(a) levels might be helpful in the risk stratification of patients with CAVS.

The HR of aortic valve calcification in different diseases were as follows: Hypertension (HR 2.060, 95% CI 1.400 to 3.031, p<0.001), Coronary heart disease(HR 3.455, 95% CI 2.217 to 5.384, p<0.001), Diabetes mellitus (HR 2.659, 95% CI 1.652 to 4.278, p<0.001), Renal dysfunction (HR 2.339, 95% CI 1.415 to 3.869, p=0.001), Osteoporosis (HR 2.327, 95% CI 1.119 to 4.838, p<0.05). Conclusions In older-age patients the incidence of calcification was high, and aortic valve regurgitation was seen frequently in these patients. Patients with hypertension, coronary heart disease, diabetes mellitus, renal dysfunction and osteoporosis were prone to the development of aortic valve calcification.
Aortic arch calcification and vascular disease: the Guangzhou Biobank Cohort Study.

Abstract:Objective: To examine the association between aortic arch calcification (AAC) and vascular disease in an older Chinese sample. Methods: For this study, 30,203 Chinese aged 50-85 years were recruited with baseline information on socioeconomic position, lifestyle and vascular risk factors. The presence and severity of AAC were diagnosed independently from chest X-ray by two radiologists. Results: The age-adjusted prevalence of AAC was significantly higher in women than men [34.6% (95% Cl 33.9-35.3) vs. 27.9% (95% Cl 26.8-28.8), p < 0.001]. Severity of AAC was significantly associated with physician-diagnosed ischemic heart disease (adjusted OR = 1.55, 95% Cl 1.35-1.79) and combined vascular disease (OR = 1.48,95% Cl 1.30-1.69) after adjusting for multiple potential confounders. Increasing severity of AAC was associated with increased risk for ischemic heart disease and vascular disease (p for trend = 0.02 to <0.001). No association between AAC and stroke was found. Conclusions: AAC was strongly and independently associated with vascular disease, suggesting that assessment of AAC from chest X-ray, which is noninvasive and relatively inexpensive, can provide useful information for risk stratification of vascular disease, and should be routinely incorporated in chest X-ray examination. Serum Uric Acid is Independently Associated with Aortic Arch Calcification in a Cross-Sectional Study of Middle-Aged and Elderly Women

Abstract:Background and aims: The increased serum uric acid (SUA) level is associated with the prevalence of cardiovascular disease (CVD) risks. Aortic arch calcification (AAC) reflects subclinical coronary atherosclerosis and is linked to subsequent cardiovascular morbidity and mortality risks closely. To better understand the role of SUA on arteriosclerosis and CVD, we aim to determine the association between SUA and the presence of AAC. Methods and results: A total of 5920 individuals aged >45 years old without prior CVD disease were included. The prevalence rate of AAC was 14.4% in all participants and a significantly increasing trend for AAC prevalence rate was found across the SUA tertiles (p <0.001 for trend). Subsequent subgroup analyses revealed that this positive association trend was only significant in female subjects. After adjusting for confounders, SUA is an independent predictor for the presence of AAC in overall participants and in women. Conclusion: SUA is independently associated with AAC in middle-aged and elderly population, especially in the women. More research needs to determine whether lower thresholds for CVD risk screening for those middle-aged and elderly women with higher SUA tertile even without hyperuricemia are warranted. Relationship between aortic valve calcification and the severity of coronary atherosclerotic disease.

Abstract:Background and aim of the study: Aortic valve calcification (AVC), which has been confirmed to be associated with various risk factors of cardiac disease, is common in the elderly and associated with increased cardiovascular mortality. It has been hypothesized that AVC is associated with coronary atherosclerotic disease, and its severity. Methods: Between July 2007 and November 2007, a total of 235 patients with chest pain or chest distress were admitted to the authors' institution for coronary angiography. The severity of coronary atherosclerotic disease (CAD) was evaluated by the Gensini score, the number of stenosed vessels, and the prevalence of total occlusion. All patients underwent transthoracic echocardiography to detect AVC. Results: Patients with CAD had a higher prevalence of AVC than those without CAD (44% versus 26%, p = 0.005). Likewise, the prevalence of AVC was significantly higher in patients with a higher Gensini score than in those with a lower score. Patients with AVC had a higher prevalence of CAD, and higher Gensini scores and numbers of stenosed coronary arteries, even after stratification by age (65 years). On multivariable logistic regression analysis for CAD, the odds ratio (OR) of AVC was 2.315 (95% confidence interval (CI): 1.158-4.629, p = 0.018); this value was higher than that for total cholesterol (OR = 1.637, p = 0.008), lipoprotein-a (OR = 1.003, p = 0.015) and fibrinogen (OR = 1.009, p = 0.006), and marginally less than that for male gender (OR = 2.665, p = 0.005). Conclusion: Patients with AVC had a higher prevalence and greater severity of CAD.
Clinical Risk Factors of Aortic Valve Sclerosis

Abstract:Aim To investigate the prevalence of aortic valve sclerosis(AVS) in the high-risk group of cardio-cerebral vascular events,and to evaluate the clinical risk factors of AVS. Methods 1058 individuals were enrolled in the cohort of the community of the Capital Steel Corporation in 2005.Participants were excluded if they had a history of any of the following diseases:rheumatic valve disease,bicuspid aortic valve malformation,prosthetic valve replacement,aortic valve stenosis.Risk factor status(including smoking,hypertension and diabetes etc) were adjudicated based on a review of data collected from hospitalizations and outpatient records.Total cholesterol and high density lipoprotein(HDL) cholesterol,triglycerides were measured from blood samples.Transthoracic echocardiography was performed to access the aortic valve sclerosis.And according to the results of the echocardiography,population was divided into two groups: AVS group and non-AVS group. Results 1039 individuals were included in this study finally with mean age 62.47±8.99 years old.The prevalence of hypertension of 1039 individuals was 68.72% and of diabetes mellitus was 25.89%.AVS was present in 401 individuals(38.59%).The risk factors of AVS included age(OR=1.589,P<0.001),male(OR=2.263,P<0.001),hypertension(OR=1.296,P=0.063),diabetes mellitus(OR=1.794,P<0.001).However after multivariate Logistic regression analysis,the independent risk factors of AVS included age(OR=1.507,P<0.001),male(OR=1.974,P=0.003),hypertension(OR=1.428,P=0.015),diabetes mellitus(OR=1.719,P<0.001). Conclusion Multiple risk factors may involve in the progress of AVS.AVS and arteriosclerosis have some same risk factors which indicates that some factors for atherosclerosis may also play an important role in the prevalence of aortic valve sclerosis.

Abstract:ObjectivesWe sought to describe the morphological characteristics of aortic valve disease in a Chinese population presenting for transcatheter aortic valve replacement (TAVR).BackgroundRacial and regional differences are known to exist in aortic stenosis (AS), but limited data exist comparing the specific anatomical variations between populations.MethodsPatients were prospectively evaluated in a Chinese population presenting for TAVR in the Venus A‐Valve trial, the first trial evaluating TAVR in China. A systematic anatomical assessment protocol employed contrast‐enhanced computed tomography (CT) in all the cases.ResultsA total of 120 consecutive patients were studied. Of these, 61 (50.7%) were patients with tricuspid, 57 (47.5%) bicuspid, and 2 (1.7%) unicuspid valve morphologies. Of the 57 cases with bicuspid valve, 31 (54.4%) had no raphe (Sievers classification, type 0) and 26 (45.6%) were of raphe type. Although the incidence of bicuspid valve morphology was more than a third in the northern Chinese population, this was lower than the eastern Chinese population (P = 0.035), in whom the incidence was more than half. A comparison of tricuspid morphologies in China versus a Western series of 229 consecutive patients undergoing TAVR assessed with CT showed a threefold excess of leaflet calcium burden in China, with a leaflet calcium volume of 421 mm3 (IQR, 188–688 mm3) versus 142 mm3 (IQR, 58–267 mm3).ConclusionsPatients presenting for TAVR in China have a very high frequency of bicuspid valve morphology. Even in tricuspid disease, there are clear differences to Western patients, with a high calcium burden, which presents challenges for TAVR in this population (ClinicalTrials.gov NCT01683474). © 2015 Wiley Periodicals, Inc.

Abstract:Objectives In patients with coronary artery calcification (CAC), a predictor of adverse cardiovascular events, coronary computed tomography angiography (CCTA) also shows valvular calcification. In this study, we evaluated common clinical indicators in CAC patients with aortic (AoVC) and mitral valve (MVC) calcification. Methods CAC and valvular calcification were quantified using the Agatston score in 636 hospitalised patients with CAC who underwent CCTA. Results Valvular calcification was found in 30.5% of patients, with 25.2% (160 patients) showing AoVC. Age was an independent predictor of AoVC in both men (odds ratio (OR), 1.086; 95% confidence interval (CI), [1.054-1.119]; p < 0.001) and women (OR, 1.109; CI, [1.066-1.154]; p < 0.001). In men, we also found that a history of cerebral infarction was an independent predictor of AoVC (OR, 2.402; CI, [1.177-4.902]; p < 0.05). The independent predictors of AoVC in the 60- to 69-years age group were BMI (OR, 1.181; CI, [1.061-1.316]; p < 0.01) and history of cerebral infarction (OR, 3.187; CI, [1.283-7.919]; p < 0.05). Conclusions Age is a key independent predictor of AoVC in CAC patients. History of cerebrovascular disease was also an independent predictor of AoVC, but only in men and patients aged 60-69 years. Our results indicate that a history of cerebral infarction may be used as a risk factor when identifying AoVC in patients with CAC.
Higher prevalence of elevated LDL-C than non-HDL-C and low statin treatment rate in elderly community-dwelling Chinese with high cardiovascular risk

Abstract:Lipid levels are increasing in all age groups in the Chinese population, but the use of statin treatment in the elderly is not well documented. We examined serum lipids, statin usage and achievement of lipid goals in 3950 subjects aged ≥65 years. Established CVD was present in 7.77% of participants and increased CVD risk was common. Elevated LDL-C according to CVD risk level was present in 46.70% of all subjects and was more frequent (p < 0.01) than elevated non-HDL-C at 32.58%. With increasing age, LDL-C was unchanged but triglycerides and non-HDL-C decreased and HDL-C increased. Individuals at moderate risk for CVD had higher TC, LDL-C, and non-HDL-C than low-risk subjects, but the values were lower in high- and very-high-risk individuals, probably because of the use of statin which was 28.57% in high-risk subjects with established CVD and 37.60% in very-high-risk individuals, but only 2.62% in those with estimated high-risk and 3.75% in those with high-risk from diabetes. More subjects in each risk group reached the non-HDL-C goal than the LDL-C goal because of the relatively low triglycerides and VLDL-C levels. These findings demonstrate a high prevalence of elevated LDL-C but low rate of statin treatment in elderly community-dwelling Chinese.
A Hospital-Based Survey of Patients with Severe Valvular Heart Disease in China

Abstract:Objectives: Due to increasing aging, the epidemiology of VHD may have changed in China. This study aimed to provide contemporary information on the prevalence, distribution patterns, and etiology of severe VHD in China.Methods: This was a retrospective survey at Second Affiliated Hospital of Zhejiang University, which included all consecutive patients between 2010 and 2015.Results: In all, 139,496 patients were enrolled. Among severe valve diseases, MR was the most frequent (n = 946, 0.68%) followed by MS (n = 524, 0.38%), AS (n = 392, 0.28%), and AR (n = 371, 0.27%). Severe MR and AS prevalence rates increased strikingly with age. Rheumatic heart disease had an prevalence of 1.56% (n = 2179), and remained one of the most common causes of severe VHD in patients younger than 65 years old (99.5% of MS with rheumatic; 27.6% of MR with rheumatic; 25.7% of AS with rheumatic; 31.6% of AR with rheumatic). Aortic valve calcification was the predominant AS etiology, and its prevalence greatly increased with age. In severe AR, rheumatic fever was the most common etiology in patients below 65; in those above 65, etiology was mostly degenerative. In severe primary MR, mitral valve prolapse was the most common cause. Prevalence of secondary MR increased with age, from 16.4% in 18-44 years old to 51.7% in individuals >= 75.Conclusions: Severe valvular diseases are very common; rheumatic fever and degenerative valvular changes remain predominant causes in patients below 65 and older ones, respectively. Abstract:Objectives: To make the clinical determination of abnormal left ventricular structure and function in older Chinese community population. Methods: Shanghai Elderly Cardiovascular Health Study (SHECHS) recruited 3891 elderly residents aged 65 years and over with 593 very elderly age over 80 years. Demographic, laboratory echocardiography and tissue Doppler data were obtained and follow-up 3 years for mortality. Results: We defined a subset of low risk participants free of prevalent cardiovascular disease or risk factors referred to as the ‘low risk reference subgroup’ and use quantile regression to calculate 10th, 50th, and 90th percentile limits with associated 95% confidence limits in the low risk reference subgroup overall and stratified by sex with the resulting 10th and 90th percentile limits considered as reference limits for these measures in the overall SHECHS subjects. Therefore, the reference limit and guideline cutoff value in men and women of LVM, LVM index to BSA, LVEDD, LVEDD index to BSA, LAD, LAD index to BSA, e’, E/e’, EF were determined. And the prevalence of LV hypertrophy, LV enlargement, LA enlargement, abnormal EF and E/e’ were determined according to the SHECHS reference limit. Further multivariate logistic regression analysis shows significance of LAD and E/e’ adjusted by age, sex, BMI, SBP, hemoglobin, fast glucose, LDL-C, homocysteine, UA, creatinine, CK, hsCRP and AF (see Table). Conclusion: Our finding suggested that the use of age-based SHECHS reference limit in Older Community-Dwelling Chinese Population improves the accuracy for identify the subjects with LV structural and functional abnormality. LAD and E/e’ might be more associated with age, sex and cardiovascular risks by multivariate logistic regression analysis, while LV enlargement might be not.
Global Burden of Calcific Aortic Valve Disease and Attributable Risk Factors from 1990 to 2019.

Abstract:BackgroundCalcific aortic valve disease (CAVD) was highly prevalent among developed countries and caused numerous deaths. Based on the Global Burden of Disease 2019, this study was designed to present comprehensive epidemiological information, attributable risks, and relevant factors.MethodsAll data were available online via the Global Health Data Exchange (GHDx). In this study, we analyzed the global incidence, prevalence, deaths, and disability-adjusted life years (DALYs) of CAVD across different regions from 1990 to 2019. We applied the estimated annual percentage changes (EAPCs) to evaluate the change trends and their attributable risks. In addition, we explored several relevant factors.ResultsFrom 1990 to 2019, the incidence cases, prevalence cases, CAVD-related deaths, and DALYs of CAVD gradually increased globally. However, the age-standardized death rate (ASDR) was relatively stable, and the age-standardized DALYs rate gradually declined during the past 30 years. Males and elderly individuals were more likely to suffer from CAVD. High systolic blood pressure (SBP) was the predominant attributable risk of disease burden that presented a global downward trend (death: EAPC = −0.68, 95% CI −0.77~−0.59, P < 0.001; DALYs: EAPC = −0.99, 95% CI −1.09 to −0.89, P < 0.001). Alcohol consumption (R = 0.79, P < 0.001), smoking prevalence (R = 0.75, P < 0.001), and calcium (R = 0.72, P < 0.001) showed a positive correlation with the age-standardized incidence rate (ASIR), whereas classic monsoon region (R = −0.68, P < 0.001) and mean temperature (R = −0.7, P < 0.001) showed a negative correlation with age-standardized incidence rate (ASIR). Besides, medical and healthcare resources presented a positive correlation with ASIR. Meanwhile, similar relationships were found in age-standardized prevalence rate (ASPR), ASDR, and age-standardized DALY rate (ASDALYR).ConclusionCAVD displays widely varied spatial distribution around the world, of which high SDI regions have the highest burdens. Age is a powerful factor and hypertension a predominant attributable risk factor. Moreover, controlling blood pressure, avoiding smoking, reducing alcohol consumption, and so on, could effectively reduce the burden of CAVD.
Traditional and novel risk factors for incident aortic stenosis in community-dwelling older adults

Abstract:Objectives: Calcific aortic stenosis (AS) is the most common valvular disease in older adults, yet its risk factors remain insufficiently studied in this population. Such studies are necessary to enhance understanding of mechanisms, disease management and therapeutics. Methods: The Cardiovascular Health Study is a population-based investigation of older adults that completed adjudication of incident AS over long-term follow-up. We evaluated traditional cardiovascular risk factors or disease, as well as novel risk factors from lipid, inflammatory and mineral metabolism pathways, in relation to incident moderate or severe AS (including AS procedures) and clinically significant AS (severe AS, including procedures). Results: Of 5390 participants (age 72.9±5.6 years, 57.6% female, 12.5% black), 287 developed moderate or severe AS, and 175 clinically significant AS, during median follow-up of 13.1 years. After full adjustment, age (HR=1.66 per SD (95% CI=1.45, 1.91)), male sex (HR=1.41 (1.06, 1.87)), diabetes (HR=1.53 (1.10, 2.13)), coronary heart disease (CHD, HR=1.36 (1.01, 1.84)), lipoprotein-associated phospholipase-A2 (LpPLA2) activity (HR=1.21 per SD (1.07, 1.37)) and sCD14 (HR=1.16 per SD (1.01, 1.34)) were associated with incident moderate/severe AS, while black race demonstrated an inverse association (HR=0.40 (0.24, 0.65)), and creatinine-based estimated glomerular filtration rate (eGFRcr) showed a U-shaped relationship. Findings were similar for clinically significant AS, although CHD and sCD14 fell short of significance, but interleukin-(IL) 6 showed a positive association. Conclusion: This comprehensive evaluation of risk factors for long-term incidence of AS identified associations for diabetes and prevalent CHD, LpPLA2 activity, sCD14 and IL-6, and eGFRcr. These factors may hold clues to biology, preventive efforts and potential therapeutics for those at highest risk.

Abstract:Calcific aortic valve disease (CAVD) is highly prevalent in our aging world and has no effective pharmaceutical treatment. Intense efforts have been made but the underlying molecular mechanisms of CAVD are still unclear.This study was designed to identify the critical genes and pathways in CAVD by bioinformatics analysis. Microarray datasets of GSE12644, GSE51472, and GSE83453 were obtained from Gene Expression Omnibus database. Differentially expressed genes (DEGs) were identified and functional and pathway enrichment analysis was performed. Subsequently, the protein-protein interaction network (PPI) was constructed with Search Tool for the Retrieval of Interacting Genes and was visualized with Cytoscape to identify the most significant module. Hub genes were identified by Cytoscape plugin cytoHubba.A total of 179 DEGs, including 101 upregulated genes and 78 downregulated genes, were identified. The enriched functions and pathways of the DEGs include inflammatory and immune response, chemotaxis, extracellular matrix (ECM) organization, complement and coagulation cascades, ECM receptor interaction, and focal adhesion. The most significant module in the PPI network was analyzed and genes among it were mainly enriched in chemotaxis, locomotory behavior, immune response, chemokine signaling pathway, and extracellular space. In addition, DEGs, with degrees ≥ 10 and the top 10 highest Maximal Chique Centrality (MCC) score, were identified as hub genes. CCR1, MMP9, VCAM1, and ITGAX, which were of the highest degree or MCC score, were manually reviewed.The DEGs and hub genes identified in the present study help us understand the molecular mechanisms underlying the pathogenesis of CAVD and might serve as candidate therapeutic targets for CAVD.
Arterial Stiffness Evaluation by Cardio-Ankle Vascular Index and Its Related Factors Analysis in Ethic She Population of China

Abstract:Objectives Arterial stiffness is an independent predictor for vascular diseases. Cardio-ankle vascular index (CAVI) is a new index of the overall stiffness of the artery from the origin of the aorta to the ankle. In the present study, we investigated the possible risk factors involving CAVI in Ethic She Population, one of ethnic groups of China. Methods 408 (age: 47.3±12.8 years, male: 167) natural persons from Ethic She group were enrolled into our study. Pulse wave velocity (PWV) and CAVI were measured by Complior apparatus. Multivariate analysis was performed to detect independent predictors of PWV among age, sex, body mass index (BMI), systolic blood pressure (SBP), diastolic blood pressure (DBP), low-density lipoprotein cholesterol (LDL-C), uric acid (UA), total cholesterol (TC), high-density lipoprotein cholesterol (HDL-C), hypersensitivity C-reactive protein (hs-CRP) and so on. Results The incidences of coronary artery disease, hypertension, diabetes, hyperlipidaemia, stroke were 1.7%, 10.3%, 3.2%, 3.4%, 2.2% in the entire group. The value of CAVI was higher in male group than in female group (7.48±1.33 vs 7.10±1.22, p=0.003). CAVI was positively correlated with age, SBP, DBP, glucose, triglyceride, LDL-C, UA, TC in entire group (r=0.570, 0.514, 0.375, 0.184, 0.150, 0.123, 0.101, 0.142, respectively, all p<0.05). There was negative trend between CAVI and BMI, HDL-C respectively, but without significant difference. We found that CAVI was significantly positive correlated with hs-CRP (r=0.191, p=0.004), a marker of inflammation, which indicated there was relationship between arterial stiffness and inflammation. Our result showed that CAVI was positively correlated with PWV (r=0.556, p<0.001), a standard index for arterial stiffness. Multivariate analysis showed that age, SBP, glucose were significant independent predictors of CAVI in all subjects. However, the independent predictors of CAVI were different between male and female groups (age, SBP in male; age, DBP, UA, glucose in female, respectively). Conclusions CAVI was a reliable evaluation index of arterial stiffness. The related factors of CAVI were different in different gender. CAVI was correlated with hs-CRP, which provided new theory basis for the mechanism of arterial stiffness. Our research might provide new recommendation for precaution of vascular disease and standard value of CAVI in different ethnic population groups.
