Abstract:Objective: To characterize differences in the prenatal diagnosis of birth defects (BDs) associated with singleton and multiple pregnancies. Design: An observational study conducted using data from a BDs surveillance system. Setting: Eastern China. Population: A total of 54,572 babies with BDs, born in 90 hospitals located in 30 regions of Zhejiang Province between 2012 and 2018. Methods: BDs were diagnosed by obstetricians using ultra sonographic, genetic tests et al., on the basis of the ICD 10th Revision (Q00–Q99). Main outcome measures: Differences in incidence and characteristics of BDs associated with singleton and multiple pregnancies. Multivariate logistic regression models were constructed to characterize relationships between the prenatal diagnosis of BDs and multiple pregnancies, with adjustment for covariates. Results: Totals of 49,872 singletons and 3,324 multiple pregnancies with BDs were analyzed. The mean incidences of BD for single and multiple pregnancies were 26.29 and 109.99 per 1,000 births, respectively (χ2=7600, P<0.001). After adjustment for covariates, BDs associated with multiple pregnancies were less likely to be diagnosed prenatally (adjusted OR: 0.36, 95% CI: 0.32–0.40); as were congenital heart defects, congenital hydrocephalus, cleft lip with cleft palate, congenital talipes equinovarus, cleft lip without cleft palate, limb reduction defects, congenital diaphragmatic hernia, trisomy 21 syndrome, congenital malformation of the urinary system, and other chromosomal malformation, compared with singletons with BDs. Conclusions: Multiple pregnancy is associated with a significantly higher risk of BD, but a significantly lower prenatal diagnosis rate. Therefore, the healthcare of women with multiple pregnancy and their fetuses should be strengthened.
Perinatal Outcomes and Risk Factors for Preterm Birth in Twin Pregnancies in a Chinese Population: A Multi-center Retrospective Study

Abstract:Background: Twin pregnancies are associated with an increased risk of adverse maternal and neonatal outcomes, mainly owing to prematurity. Few studies have evaluated the risk factors for preterm birth (PTB) in Chinese population. The objective of this study is to present the short-term maternal-neonatal outcomes, investigating the potential risk factors associated with preterm birth in Chinese twin pregnancies.Methods: A multi-center retrospective study of women pregnant with twins ≥28 weeks of gestation was conducted. Maternal and neonatal outcomes were analyzed. Logistic regression was used to identify potential risk factors for PTB before 37, 34, and 32 weeks, respectively.Results: A total of 3,288 twin pregnancies and 6,576 neonates were included in 99,585 pregnancies. The rate of twin pregnancy was 3.3%, while the PTB rate before 37, 34, and 32 weeks among this population were 62.1, 18.8, and 10.4%, respectively. Logistic regression revealed that monochorionicity [Odds ratio (OR) 3.028, 95% confident interval (CI) 2.489–3.683, P < 0.001], gestational weight gain (GWG) <10 kg (OR 2.285, 95% CI 1.563–3.339, P < 0.001) and GWG between 10 and 15 kg (OR 1.478, 95% CI 1.188–1.839, P < 0.001), preeclampsia (PE) (OR 3.067, 95% CI 2.142–4.390, P < 0.001), and intrahepatic cholestasis of pregnancy (ICP) (OR 3.122, 95% CI 2.121–4.596, P < 0.001) were the risk factors for PTB before 37 weeks. Monochorionicity (OR 2.865, 95% CI 2.344–3.501, P < 0.001), age < 25 years (OR 1.888, 95% CI 1.307–2.728, P = 0.001), and GWG <10 kg (OR 3.100, 95% CI 2.198–4.372, P < 0.001) were risk factors for PTB before 34 weeks. Monochorionicity (OR 2.566, 95% CI 1.991–3.307, P < 0.001), age younger than 25 years (OR 1.964, 95% CI 1.265–3.048, P = 0.003), and GWG <10 kg (OR 4.319, 95% CI 2.931–6.364, P < 0.001) were the risk factors for PTB before 32 weeks.Conclusions: Monochorionicity and GWG <10 kg were two major risk factors for PTB before 32, 34, and 37 weeks, whereas maternal age, PE, and ICP were also risk factors for PTB in specific gestational age.
Association Between Postterm Pregnancy and Adverse Growth Outcomes in Preschool-Age Children.

Abstract:Background Postterm pregnancy has been associated with higher risk of perinatal mortality and morbidity, but its long-term health effects on offspring are poorly understood. Objectives The aim of the study was to investigate the prospective associations between maternal postterm pregnancy and adverse growth outcomes in children. Methods The Jiaxing Birth Cohort is part of a large population-based health surveillance system in China and recruited pregnant females resident in the Jiaxing area between 1999 and 2013; newborns were followed up for a median duration of 5.8 y until they went to school. Mother-child pairs with maternal gestational information and offspring's anthropometric data at 4-7 y old were included. Postterm pregnancy was defined as maternal gestational age >= 42 and <47 wk, and its associations with offspring obesity, overweight/obesity, and thinness during childhood were determined by using Poisson regression models. Results Of the 101,505 included mother-child pairs, 2369 (2.3%) children were born at postterm. Children born at postterm had significantly lower BMI-for-age z score, weight-for-age z score, and height-for-age z score than those born at term; the mean difference (95% CI) was -0.11 (-0.15, -0.06), -0.17 (-0.21, -0.13), and -0.16 (-0.20, -0.12), respectively. When comparing postterm with term pregnancy, the multivariable-adjusted RRs and 95% CIs among preschool-age children were 0.87 (0.68, 1.11) for obesity, 0.82 (0.72, 0.94) for overweight/obesity, and 1.18 (1.09, 1.28) for thinness, respectively. These risk estimates were robust in sensitivity analyses, but were attenuated in several subgroups stratified by age, sex, mode of delivery, and fetal distress. Conclusions Postterm pregnancy was associated with a higher risk of thinness, and a lower risk of overweight/obesity, as well as lower growth parameters in preschool-age children. These findings imply that postterm pregnancy may impede the long-term growth of offspring.
Association of Maternal Pre-Pregnancy Diabetes and Gestational Diabetes with Congenital Anomalies of the Newborn

Abstract:Objective: To examine the association of maternal pre-pregnancy diabetes, gestational diabetes mellitus (GDM), and 12 subtypes of congenital anomalies of the newborn. Methods: We included 29,211,974 live births with maternal age ranged from 18 to 49 years old documented in the National Vital Statistics System in the United States, 2011-2018. Information on pre-pregnancy diabetes, GDM, and congenital anomalies was retrieved from birth certificates. Log-binomial regression was used to estimate risk ratios (RRs) and 95% confidence intervals (CIs) for congenital anomalies overall and by subtypes. Results: Of the 29,211,974 births, there were 90,061 infants who had congenital anomalies identified at birth. The adjusted RRs of congenital anomalies at birth were 2.44 (95% CI, 2.33-2.55) for pre-pregnancy diabetes and 1.28 (95% CI, 1.24-1.31) for GDM. The association were generally consistent across subgroups by maternal age, race/ethnicity, pre-pregnancy obesity status, and infant sex. For specific subtypes of congenital anomalies, maternal pre-pregnancy diabetes or GDM was associated with an increased risk of most subtypes. For example, the adjusted RRs of cyanotic congenital heart disease were 4.61 (95% CI, 4.28-4.96) for pre-pregnancy diabetes and 1.50 (95% CI, 1.43-1.58) for GDM; the adjusted RRs of hypospadias were 1.88 (95% CI, 1.67-2.12) for pre-pregnancy diabetes and 1.29 (95% CI, 1.21-1.36) for GDM. Conclusion: Pre-pregnancy diabetes and, to a lesser extent, GDM were associated with several subtypes of congenital anomalies of the newborn. These findings suggest potential benefits of preconception counseling in women with pre-existing diabetes or at risk of GDM for the prevention of congenital anomalies.
Changes in Maternal Age and Prevalence of Congenital Anomalies During the Enactment of China's Universal Two-Child Policy (2013-2017) in Zhejiang Province, China: an Observational Study.

Abstract:BACKGROUND:China implemented a partial two-child policy (2013) followed by a universal two-child policy (2015), replacing the former one-child policy mandated by the government. The changes affect many aspects of China's population as well as maternal and infant health, but their potential impact on birth defects (BDs) remains unknown. In this study, we investigated the associations of these policy changes with BDs in Zhejiang Province, China. METHODS AND FINDINGS:We used data from the BD surveillance system in Zhejiang Province, China, which covers 90 hospitals in 30 urban districts and rural counties, capturing one-third of the total births in this province. To fully consider the time interval between conception and delivery, we defined the one-child policy period as data from 2013 (births from October 2012 to September 2013), the partial two-child policy period as data from 2015 (births from October 2014 to September 2015), and the universal two-child policy period as data from 2017 (births from October 2016 to September 2017). Data from 2009 and 2011 were also used to show the changes in the proportion of births to women with advanced maternal age (35 years and older) prior to the policy changes. Main outcome measures were changes in the proportion of mothers with advanced maternal age, prevalence of BDs, rankings of BD subtypes by prevalence, prenatal diagnosis rate, and live birth rate of BDs over time. A total of 1,260,684 births (including live births, early fetal losses, stillbirths, and early neonatal deaths) were included in the analyses. Of these, 644,973 (51.16%) births were to women from urban areas, and 615,711 (48.84%) births were to women from rural areas. In total, 135,543 (10.75%) births were to women with advanced maternal age. The proportion increased by 85.68%, from 8.52% in 2013 to 15.82% in 2017. However, it had remained stable prior to policy changes. Overall, 23,095 BDs were identified over the policy changes (2013-2017). The prevalence of BDs during 2013, 2015, and 2017 was 245.95, 264.86, and 304.36 per 10,000 births, respectively. Trisomy 21 and other chromosomal defects increased in both risk and ranking from 2013 to 2017 (crude odds ratio [95% confidence interval] 2.13 [1.75-2.60], from ranking 10th to 5th, and 3.63 [2.84-4.69], from ranking 16th to 6th, respectively). The prenatal diagnosis rate increased by 3.63 (2.2-5.1) percentage points (P < 0.001), from 31.10% to 34.72%, and identification of BDs occurred 1.88 (1.81-1.95) weeks earlier (P < 0.001). The live birth rate for infants with BDs born before 28 gestational weeks increased from 1.29% to 11.45%. The major limitations of this observational study include an inability to establish causality and the possible existence of unknown confounding factors, some of which could contribute to BDs. CONCLUSIONS:In this study, we observed significant increases in maternal age and the prevalence of total and age-related anomalies following China's new two-child policy. Increases in live birth rate for infants with BDs born before 28 gestational weeks suggest that healthcare for very preterm births with BDs may be warranted in the future, as well as updating the definition of perinatal period.
Associations Between Maternal Abortion History and Neonatal Outcome among Very Preterm Infants:A Multicenter Cohort Study

Abstract:Background It is unclear whether there is an association between maternal abortion history and neonatal outcomes of singleton very preterm infants (VPIs). We assess the association between maternal abortion history and neonatal outcome of VPIs in China. Methods All first parity singleton VPIs born at < 32 weeks gestational age (GA) who were admitted to neonatal intensive care units (NICU) participating in the Chinese Neonatal Network (CHNN) from 2019 to 2021 were included in the study. Multivariable logistic regression models were constructed to compare neonatal outcomes among infants with different maternal abortion histories after adjusting for confounders. Results A total of 7256 VPIs were included in this analysis. Overall, 3133 (43.2%) infants had positive maternal abortion history. The incidence of mortality or any major morbidity including respiratory distress syndrome (RDS), patent ductus arteriosus (PDA), intraventricular haemorrhage (IVH), IVH stage 3 or 4, moderate or severe bronchopulmonary dysplasia (BPD), retinopathy of prematurity (ROP), ROP stage 3, 4, or 5 tended to increase significantly with increasing number of maternal abortion histories from non-abortion group to ≥2 abortions group. Multivariable analysis showed that maternal abortion history was significantly associated with higher risks of mortality or any major morbidity (adjusted odds ratio [aOR] 1.15, 95% confidence interval [CI] 1.03–1.29), RDS (aOR 1.22, 95%CI 1.08–1.38), IVH (aOR 1.17, 95% CI 1.05–1.31), and IVH stage 3 or 4 (aOR 1.27, 95% CI 1.01–1.59). Risk increased with the number of abortions. VPIs of mothers with two or more abortions had the highest risk of mortality or any major morbidity (aOR 1.18, 95% CI 1.02–1.36), RDS (aOR 1.35, 95% CI 1.18–1.56), IVH (aOR 1.23, 95% CI 1.07–1.43) and moderate or severe BPD (aOR 1.17, 95% CI 1.01–1.36), compared to the non-abortion group. Conclusions In China, VPIs born to mothers with previous abortion history may have a greater risk of adverse outcomes. Based on the potential impact of maternal abortion history on newborns, it is necessary to provide additional information on related risks during antenatal counseling for these mothers.

Abstract:Objective: To analyze the incidence of preterm delivery among single live neonates and the association between maternal reproductive history and preterm birth. Methods: A questionnaire survey was conducted on reproductive history among women at childbearing age who were selected through multi-stage stratified random sampling method in Shaanxi, during 2010-2013. Samples would include women at childbearing age and in pregnancy or having had definite pregnancy outcomes. Results: A total of 29 608 women at childbearing age with their infants, were studied. The overall incidence of premature delivery among the single live birth neonates under this study, was 2.7% during 2010-2013. Results from the logistic regression model showed that factors as: having had history with preterm delivery (OR=7.99, 95%CI: 5.59-11.43), age of the mothers, older than 35 (OR= 2.03, 95% CI: 1.59-2.59) and with history of birth defects (OR=1.54, 95% CI: 1.01-2.34) were at higher risks for premature delivery in neonates. Intervals on pregnancies between 3-4 years (compared with ≤2 years, OR=0.74, 95% CI: 0.58-0.93), between 5-6 years (compared with ≤2 years, OR=0.66, 95%CI: 0.52-0.82), or> 6 years (compared with ≤2 years, OR=0.48, 95%CI:0.37-0.61) together with numbers of parity as 1 (compared with primiparas, OR=0.80, 95%CI: 0.67-0.95), as ≥2 (compared with primiparas, OR=0.62, 95% CI: 0.39-0.97) etc. were protective factors to preterm delivery. Factors as: history of preterm delivery, mothers age (older than 35 years) and intervals of pregnancy, appeared influential to the age of gestation, under the ordinal polytomous logistic regression analysis. Conclusion: The incidence of preterm births among single live birth neonates in Shaanxi was lower than the average national level. Programs related to health care services prior to conception and during pregnancy, together with increasing the self-care consciousness of childbearing aged women etc, should all be strengthened in order to reduce the occurrence of preterm birth, in Shaanxi province.

Association of maternal preconception blood pressure with preterm birth: a population-based cohort study

Abstract:The association between maternal preconception blood pressure (BP) and preterm birth (PTB) is still unclear. The purpose of this study was to investigate the association between maternal preconception BP and PTB. This population-based cohort study included 715 984 Chinese women aged 20–49 years who participated in the National Free Preconception Health Examination Project and successfully had a singleton livebirth during 2014–2019 in Guangdong Province, China. Maternal preconception BP were measured by trained health workers. Multivariate logistic regression models and restricted cubic spline regressions were used to examine the association and dose–response relationship between maternal preconception BP and PTB, respectively. Maternal preconception hypertension was associated with the increased risk of PTB (adjusted odds ratios (aOR): 1.24; 95% CI: 1.14–1.34). Compared to women with normal preconception BP, the aORs for PTB were 1.09 (95% CI: 1.06–1.12), 1.24 (95% CI: 1.13–1.36), and 1.43 (95% CI: 1.15–1.79) for women with preconception elevated BP (120–139/ 80–89 mmHg, stage-1 hypertension (140–159/ 90–99 mmHg, and stage-2 hypertension (160–179/100–109 mmHg), respectively. According to the 2017 American College of Cardiology/American Heart Association criteria, maternal preconception elevated BP and hypertension were also significantly associated with an increased risk of PTB. Preconception systolic and diastolic BP showed a U-shaped ( χ 2 = 40.54; nonlinear P < 0.001) and linear ( χ 2 = 6.62; nonlinear P = 0.085) dose–response relationship with PTB, respectively. The association was modified by maternal age and preconception body mass index. These findings identify maternal preconception elevated BP and hypertension as a modifiable risk factor for PTB, providing evidence for future research studies, public health and clinical interventions.

Impact of Maternal Age on Neonatal Outcomes among Very Preterm Infants Admitted to Chinese Neonatal Intensive Care Units: a Multi-Center Cohort Study

Abstract:Background: The percentage of advanced maternal age (aged over 35 years) mothers has been rising across the world, the evidence of maternal age on neonatal outcomes from low- and middle-income countries is scarce. Our objective was to evaluate the effect of maternal age on mortality and major morbidity among very preterm infants admitted to Chinese neonatal intensive care units. Methods: Data from a retrospective multi-center cohort of all complete care very preterm infants admitted to 57 neonatal intensive care units that participated in the Chinese Neonatal Network from January 1st to December 31st, 2019 were analyzed. Neonatal outcomes including mortality or any major morbidity, defined as necrotizing enterocolitis stage 2 or 3, moderate & severe bronchopulmonary dysplasia, severe intraventricular hemorrhage, cystic periventricular leukomalacia, severe retinopathy of prematurity, or sepsis. A multiple logistic regression model was constructed to analyze the independent association between maternal age and neonatal outcome. Results: Among 7,698 eligible newborns, 80.5% of very preterm infants were born to mothers between the ages of 21 and 35 years, with 18.0% born to mothers >35 years and 1.5% born to mothers <21 years. Higher rates of maternal hypertension, maternal diabetes, cesarean deliveries, antenatal steroid usage were noted as maternal age increased. The proportion of prenatal care, cesarean section, antenatal steroid usage and inborn for very preterm infants born to mothers <21 years was lower than those of mothers of other ages. Compared to the ages of 21-35 years group, the odds of severe intraventricular hemorrhage (adjusted odd ratio: 2.00, 95% CI: 1.08-3.71) was significantly higher in the ages of 15-20 years group. Increasing maternal age was associated with higher rates of small for gestational age and lower birth weight of very preterm infants, but no correlation between advanced maternal age and very preterm infants mortality or major morbidity. Conclusions: Among very preterm infants, increasing maternal age was associated with higher rates of small for gestational age but not neonatal mortality or major morbidity. Young maternal age may increase the risk of severe intraventricular hemorrhage of very preterm infants.

Abstract:Background: Maternal exposure to ambient heat may be associated with congenital anomalies, but evidence is still limited. Objectives: We aimed to estimate the association between maternal exposure to ambient heat during the 3-12 weeks post-conception (critical window of organogenesis) and risk of total and various diagnostic categories of major struc-tural anomalies among live singleton births in the contiguous United States (US).Methods: We included data on 2,352,529 births with the first day of critical developmental windows falling within months of May through August from 2000 to 2004 across 525 US counties. We used a validated spatial-temporal model to estimate daily county-level population-weighted temperature. We used logistic regression to estimate the association between ambient temperature and risk of diagnostic categories of anomalies during the critical window after adjusting for individual and county-level factors. We conducted subgroup analysis to identify potential suscepti-ble subpopulations. Results: A total of 29,188 anomalies (12.4 per 1000 births) were recorded during the study period. Maternal exposure to extreme heat (> 95th percentile) was associated with higher risk of total anomalies, central nervous system anom-alies, and other uncategorized anomalies with an odds ratio (OR) of 1.05 (95 % CI: 1.00, 1.11), 1.17 (95 % CI: 1.01, 1.37), and 1.16 (95 % CI: 1.04, 1.29) compared with minimum morbidity temperature, respectively. The associations were homogeneous across subgroups defined by maternal age, maternal race/ethnicity, marital status, educational at-tainment, and parity, but were more pronounced among mothers residing in more socially vulnerable counties and births with multiple anomalies.Conclusions: Among US live singleton births, maternal exposure to ambient heat may be associated with higher risk of total anomalies, central nervous system anomalies, and other uncategorized anomalies. We suggest additional research is carried out to better understand the relations between maternal heat exposure and congenital anomalies in the pres-ence of global warming.
Association between maternal prepregnancy body mass index and risk of preterm birth in more than 1 million Asian American mothers

Abstract:<section class="article-section__content"><h3 class="article-section__sub-title section1"> Background</h3><p>Asian Americans are among the fastest growing subpopulations in the United States. However, evidence about maternal pre‐pregnancy body mass index (BMI) and preterm birth among Asian Americans is lacking.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Methods</h3><p>This population‐based cohort study used nationwide birth certificate data from the US National Vital Statistics System 2014‐2018. All Asian American mothers who had a singleton live birth were included. According to Asian‐specific cutoffs, maternal pre‐pregnancy BMI was classified into underweight (BMI<18.5 kg/m<sup>2</sup>), normal weight (BMI 18.5‐22.9 kg/m<sup>2</sup>), overweight (BMI 23.0‐27.4 kg/m<sup>2</sup>), Class I Obesity (BMI 27.5‐32.4 kg/m<sup>2</sup>), Class II Obesity (BMI 32.5‐37.4 kg/m<sup>2</sup>), and Class III Obesity (BMI ≥ 37.5 kg/m<sup>2</sup>). Preterm birth was defined as gestational age less than 37 weeks. Multivariable logistic regression models were used to estimate the odds ratio (OR) of preterm birth. </p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Results</h3><p>We included 1,081,341 Asian American mother‐infant pairs. The rate of preterm birth was 6.51% (n=70,434). The rate of maternal pre‐pregnancy overweight and obesity was 46.80% (n=506,042). Compared with mothers with normal weight, the adjusted OR was 1.04 (95% CI 1.01‐1.07) for underweight mothers, 1.18 (95% CI 1.16‐1.20) for overweight mothers, 1.41 (95% CI 1.37‐1.44) for mothers with Class I Obesity, 1.69 (95% CI 1.63‐1.76) for mothers with Class II Obesity, and 1.78 (95% CI 1.66‐1.90) for mothers with Class III obesity. Similar patterns of associations were observed in Asian American mothers across different country origins.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Conclusions</h3><p>Among Asian American mothers, maternal pre‐pregnancy overweight or obesity, defined by Asian‐specific, lower BMI cutoffs, was significantly associated with an increased risk of preterm birth. The risk of preterm birth increased with increasing obesity severity. These findings highlight the importance of using Asian‐specific BMI cutoffs in assessing risk of preterm birth among Asian American mothers.</p></section>

Preterm Morbidity and Mortality by Gestational Age. A Birth Population-Based Study

Abstract:Abstract Background: Despite 15-17 million of annual births in China, there is a paucity of information on preterm morbidity and mortality. We characterized the outcome of preterm births and hospitalized preterm infants by gestational age (GA) in Huai’an in 2015, an emerging prefectural region of China.Methods: Of 59,245 regional total births, clinical data on 2,651 preterm births and 1,941 hospitalized preterm neonates were extracted from Huai’an Women and Children’s Hospital (HWCH) and non-HWCH hospitals in 2018-2020. Preterm morbidity and mortality rates were characterized and compared by hospital categories and GA spectra. Death risks of preterm births and hospitalized preterm infants in the whole region were analyzed with multivariable logistic regression.Results: The incidences of extreme, very, moderate, late and total preterm of the regional total births were 1.4, 5.3, 7.2, 30.8 and 44.7‰, with all-death rates being 1.0, 1.6, 0.6, 1.1 and 4.3‰, respectively, of the regional total births. There were 1,025 (52.8% of whole region) preterm admissions in HWCH, with significantly lower in-hospital death rate of inborn (33/802) than out-born (23/223) infants. Compared to non-HWCH, four-fold more neonates in HWCH were under critical care with higher death rate, including most extremely preterm infants. Significant all-death risks were found for the total preterm births in BW < 1,000g, II-III degree of amniotic fluid contamination, Apgar-5 min < 7, and birth defects (BD). For the hospitalized preterm infants, significant in-hospital death risks were found in out-born of HWCH, GA < 32 weeks, Apgar-5 min < 7, BD, necrotizing enterocolitis and ventilation, whereas born in HWCH, antenatal glucocorticoids, cesarean delivery and surfactant use were protective factors against death.Conclusions: The integrated data revealed GA-specific morbidity and mortality on the basis of total preterm births and their hospitalization, demonstrating the efficacy of leading referral center and whole regional perinatal-neonatal network in China. The concept and protocol of our current study should be extended to gain comprehensive understanding in the world-wide campaign for prevention of preterm birth.

Abstract:Summary It is recognized that prenatal care plays an important role in reducing adverse birth. Chinese pregnant women with medical condition were required to seek additional health care based on the recommended at least 5 times health care visits. This study was to estimate the association between prenatal care utilization (PCU) and preterm birth (PTB), and to investigate if medical conditions during pregnancy modified the association. This population-based case control study sampled women with PTB as cases; one control for each case was randomly selected from women with term births. The Electronic Perinatal Health Care Information System (EPHCIS) and a questionnaire were used for data collection. The PCU was measured by a renewed Prenatal Care Utilization (APNCU) index. Logistic regression models were used to estimate odds ratios (OR) and the 95% confidence interval (95% CI). Totally, 2393 women with PTBs and 4263 women with term births were collected. In this study, 695 (10.5%) women experienced inadequate prenatal care, and 5131 (77.1%) received adequate plus prenatal care. Inadequate PCU was associated with PTB (adjusted OR: 1.41, 95% CI: 1.32–1.84); the similar positive association was found between adequate plus PCU and PTB. Among women with medical conditions, these associations still existed; but among women without medical conditions, the association between inadequate PCU and PTB disappeared. Our data suggests that women receiving inappropriate PCU are at an increased risk of having PTB, but it does depend on whether the woman has a medical condition during pregnancy.
Maternal Abortion History and the Risk of Congenital Heart Defects. A Case-Control Study.

Abstract:OBJECTIVE:To explore the association between maternal abortion history and congenital heart defect (CHD) risk in subsequent pregnancies.STUDY DESIGN:A multihospital-based case-control study was conducted. The cases included 370 women whose fetuses were diagnosed with CHDs. The controls were 413 women with fetuses without an apparent malformation in the same hospital. All of the participants were investigated by trained interviewers. Univariate analysis was performed, followed by multivariate logistic regression analysis to calculate odds ratio and 95% confidence interval to evaluate the risk of maternal abortion history on CHD risk.RESULTS:There were no statistically significant positive associations between maternal abortion (induced abortion and spontaneous abortion) occurrence and the risk of CHDs. There were no statistically significant positive associations between the number of maternal abortions (induced and spontaneous) and the risk of congenital heart defects.CONCLUSION:Maternal abortion history may not be associated with fetal CHDs.

Abstract:Background: Both high and low prepregnancy body mass index (BMI) has been associated with small for gestational age births (SGA; birthweight below the population specific 10th centile for the gestational age), but results remain inconsistent. We examined the association between maternal BMI and SGA, and evaluated if the associations were modified by preterm birth (being born prior to 37 weeks) status. Methods: A population-based cohort study was conducted in Wuhan, China from June 2011, to June 2013. Women who delivered a non-malformed livebirth (n = 76 695) were included using the Wuhan Maternal and Child Health Management Information System. Log-binomial regression models were used to analyse the associations between prepregnancy BMI, categorized using thresholds adapted to the Chinese population, and SGA. Stratified analyses were used to examine the relationship of prepregnancy BMI to preterm-SGA and term-SGA. Results: Of the 76 695 live births, 3058 (4.0%) were delivered preterm. For babies born at term, prepregnancy underweight (<18.5 kg/m2 ) was associated with an increased risk of SGA, the adjusted risk ratio (RR) was 1.41 (95% confidence interval (CI) 1.33, 1.49), whereas, being overweight (24.0-27.9 kg/m2 ) was associated with a decreased risk (RR 0.84, 95% CI 0.74, 0.94). For babies born preterm, prepregnancy underweight was not associated with risk of SGA, but being overweight was associated with an increased risk (RR 1.57, 95% CI 1.04, 2.35). Conclusions: These data suggest that the association of overweight and underweight prepregnancy BMI and SGA differs depending on whether the baby is full term or preterm.

Abstract:Previous research has explored theories regarding the vertical transmission of human papillomavirus (HPV) infection and its association with adverse pregnancy and perinatal outcomes. However, the impact of maternal HPV infection on congenital anomalies (CAs) in offspring remains relatively understudied. We conducted a population‐based cohort study linking the Taiwan Birth Registry, Taiwan Death Registry, and National Health Insurance Research Database, in which newborns born in Taiwan between 2009 and 2015 were included. We established a maternal HPV infection cohort comprising 37 807 newborns and matched them with a comparison group of 151 228 newborns at a 1:4 ratio based on index year, age, and sex. The study examined a composite outcome and subgroups of different types of congenital malformations. Differences in cumulative incidence of CAs were assessed using Kaplan–Meier curves and log‐rank tests. Adjusted hazard ratios (aHRs) were estimated using Cox proportional hazard regressions. No significant association was found between HPV infection and the broad spectrum of CAs (aHR: 1.04, 95% confidence interval [CI]: 0.98–1.10; log‐rank test p = 0.14). However, we observed a 19% increased risk of musculoskeletal CAs in the maternal HPV infection group (aHR: 1.19; 95% CI: 1.05–1.34) compared to those without maternal HPV exposure. Other factors, including the type of HPV (aHR: 0.65; 95% CI: 0.16–2.63), the timing of exposure (during or before pregnancy), and maternal age (aHR for <30 years: 1.02, 95% CI: 0.94–1.1; aHR for 30–39 years: 1.05, 95% CI: 0.99–1.11; aHR for ≥40 years: 0.88, 95% CI: 0.67–1.17), did not significantly affect the risk for any CA. In conclusion, gestation detection of HPV infection was associated with musculoskeletal CAs but not other major CAs. Prospective studies are warranted to elucidate the necessity of prenatal screening in populations at risk.
