MUTATION IN MECHANOTRANSDUCTION GENE, PIEZO2 LEADS TO MULTIPLE ORGAN DYSFUNCTION-A CASE REPORT

Abstract:Introduction: Through mechanotransduction, cells translate mechanical forces into biochemical signals, which are critical for organ development, homeostasis, and functioning. Here, we present a case report with gain of function mutation in mechanotransductionrelated gene, PIEZO2 that resulted in multiple organ dysfunction. Material and method: We herein reported a 27-year-old female was diagnosed with ankylosis of tarsometatarsal joint, scoliosis, bilateral hyperopia, strabismus and amblyopia, and premature ovarian insufficiency. Result: She suffered from repeated pneumonia and pneumothorax. Her chest CT scan showed fibrotic change with cicatricial atelectasis. Cardiac catheterization showed impaired right ventricular systolic function and pulmonary hypertension. Sildenafil, diuretics, and digoxin were prescribed. Panendoscopy revealed reflux esophagitis and oropharynx stenosis. Due to unusual symptoms of multiple organ dysfunction, whole exome sequencing was performed revealing (p.Glu2727del) PIEZO2 mutation. She was diagnosed with mechanotransduction gene mutation-related multiple organ dysfunction of eyes, lung, heart, gastrointestinal tract, skeletal system, and genial system. Conclusion: Mechanotransduction mediates diverse biological pathways, and regulates cellular structure and function. This case revealed events disrupting the mechanotransduction pathway that can result in multiple organ dysfunction.
