Screening for prognostic genes associated with mismatch repair defects in colorectal cancer patients

Abstract:Objective:To explore key prognostic genes associated with microsatellite instability-high (MSI-H) in colorectal cancer patients.Methods:Totally, 375 colorectal cancer patients were included from the the cancer genome atlas (TCGA) database, screened for differentially expressed genes in MSI-H and mismatch repair stable (MSS) patients (threshold for false discovery rate <0.05 and fold difference >2), and subjected to enrichment analysis (false discovery rate <0.05). Gene co-expression networks were constructed using Pearson correlation analysis (R>0.4 and P<0.05), and pivotal genes were screened. Key prognostic genes with high expression in tumor tissues were further screened by Cox regression ( P<0.05), and single-cell sequencing analysis was used to verify the expression of key genes and the relationship with MSI-H. Differences in gene expression between different patient groups were verified by t test. Results:A total of 558 MSI-H-related genes and 811 MSS-related genes were screened. Enrichment analysis showed that MSI-H-related genes were mainly involved in inflammation and immune response. By network analysis and Cox regression, three MSI-H significantly associated prognostic key genes were identified, namely GJB5, TNNT1 and KRT16, which were highly expressed in colorectal cancer tumor tissues and had risk ratios of 1.4 ( P<0.05), 1.5 ( P<0.01) and 1.7 ( P<0.01) for overall survival in metastatic patients, respectively. In single-cell sequencing data, GJB5, TNNT1 and KRT16 were similarly found to be highly expressed in tumor cells and MSI-H patient cells. Conclusion:GJB5, TNNT1 and KRT16 were significantly poor prognostic key genes that were highly expressed in MSI-H colorectal cancer patients.
