IgG4-related cholangiopathy, pancreatopathy and lymphadenopathy

Abstract:A 72-year-old man presented as a transfer to our hospital with abdominal pain and obstructive jaundice. He had elevated total bilirubin (436 μmol/L, reference range, 1.71–21 μmol/L), direct bilirubin (282 μmol/L, reference range, 1.71–13 μmol/L), alkaline phosphatase (AKP, 411 IU/L, normal range: 50–130 IU/L), gamma-glutamyl transpeptidase (γ-GTP, 157 IU/L, normal range: 10–60 IU/L), and CA19–9 (55 IU/mL, reference range, 0–40 IU/mL). Three-dimensional magnetic resonance cholangiopancreatogram (MRCP) revealed severe both intrahepatic and extrahepatic biliary ductal dilatation with tapering and cutoff near the pancreas (Fig. 1A). The contrast-enhanced abdominal computed tomography (CT) found diffuse swelling of the pancreas with sausage-like appearance (Fig. 1B). Subsequently, serological test showed that the patient's serum IgG4 concentration was elevated to 8 times the upper limit of normal (ULN). Ultrasound-Guided biopsies of cervical lymph nodes were done. Immunohistochemistry demonstrated enriched IgG4 lymphoplasmacytic infiltration (IgG4:IgG ratio > 0.4), and large number of IgG4 positive cells >50 per high-power field (Fig. 1C, D, E), which was consistent with IgG4-related lymphadenopathy. Finally, the patient received the diagnosis of IgG4-Related cholangiopathy, pancreatopathy and lymphadenopathy based on the above imaging, serological and histopathological findings.
