Treatment of 15 patients with severe burn and trauma in explosion of a tanker

Abstract:Objective:To retrospectively analyze the condition and treatment of patients with burn wounds caused by a tanker truck explosion, and to explore the treatment strategies for burn wound patients after explosion.Methods:We temporarily set up a burn intensive care unit for the rescue, and recruited a rescue service team. This study included 15 patients with severe burn injury, and the data including gender, age, burn area, and burn score were recorded in detail. A retrospective analysis was conducted on various systems of patients at admission and after admission, and the infection status and relevant operations of patients in the ICU were classifies and counts.Results:The average burn area of patients at admission was (85.1±14.3)%, the simple burn index ABSI was 14.7±1.8, the revised Baux score was 168.0±22.4, and the Sequential Organ Failure Assessment score was 10.7±2.4. After active treatment, the patient's overall inflammation indicators gradually decreased, and the respiratory, circulation, internal environment, and renal function gradually improved and tended to be stable. In the first week after admission, there were 113 consultations and discussions, 29 operations, and 14 bedside bronchoscopy. Later on, the condition of some patients gradually stabilized and the related operations became less accordingly. In the end, the 90-day survival rate reached 60.0%.Conclusions:After this explosion, the establishment of a treatment service team led by critical care medicine has achieved great efficacy, which can provide a reference for dealing with future public emergencies.
