Abstract:Background: Early diagnosis of portal hypertension is imperative for timely treatment to reduce the mortality rate. However, there is still no adequate method to noninvasively and accurately assess the portal hypertension in routine clinical practice. Purpose: We aimed to evaluate the accuracy of parameters measured using dual energy spectral computed tomography (LightSpeed CT750 HD) in assessing portal venous pressure in patients with liver cirrhosis. Study: Forty-five patients with liver cirrhosis who underwent percutaneous transhepatic portal vein puncture as part of their treatment for liver disease were enrolled in this study. Measurement of direct portal venous pressure was performed preoperatively. All patients underwent dual energy spectral computed tomography within 3 days before their operations. Results: The iodine concentrations of portal vein and hepatic parenchyma during the portal venous phase and the alanine aminotransferase level were found to be independently correlated with the direct portal venous pressure according to stepwise multivariate linear regression analysis (P<0.001, 0.004, and 0.024, respectively). In a receiver operating characteristic analysis, the area under the receiver operating characteristic of iodine concentrations of the portal vein (ICPV) for identifying clinically significant portal hypertension (>= 10 mm Hg) was significantly higher than that of iodine concentrations of hepatic parenchyma (ICliver) and the alanine aminotransferase level (0.944, 0.825, and 0.301, respectively). The threshold ICPV of 58.27 yielded a sensitivity of 93.8%, specificity of 69.2%, positive predictive value of 88.2%, and negative predictive value of 81.8%, respectively. Conclusions: ICPV values may be a useful tool in noninvasively assessing the portal venous pressure and identifying clinically significant portal hypertension in liver cirrhosis.

Abstract:Objective: To investigate the value of spectral CT imaging in the diagnosis and classification of liver cirrhosis during the arterial phase (AP) and portal venous phase (PVP).Materials and Methods: Thirty-eight patients with liver cirrhosis (Child-Pugh class A/B/C: n = 10/14/14), and 43 patients with healthy livers, participated in this study. The researchers used abdominal spectral CT imaging during AP and PVP. Iodine concentration, derived from the iodine-based material-decomposition image and the iodine concentration ratio (ICratio) between AP and PVP, were obtained. Statistical analyses {two-sample t test, One-factor analysis of variance, and area under the receiver operating characteristic curve (A [z])} were performed.Results: The mean normalized iodine concentration (NIC) (0.5 +/- 0.12) during PVP in the control group was significantly higher than that in the study group (0.4 +/- 0.10 on average, 0.4 +/- 0.08 for Class A, 0.4 +/- 0.15 for Class B, and 0.4 +/- 0.06 for Class C).(All p < 0.05). Within the cirrhotic liver group, the mean NIC for Class C during the AP (0.1 +/- 0.05) was significantly higher than NICs for Classes A (0.1 +/- 0.06) and B (0.1 +/- 0.03) (Both p < 0.05). The ICratio in the study group (0.4 +/- 0.15), especially for Class C (0.5 +/- 0.14), was higher than that in the control group (0.3 +/- 0.15) (p < 0.05). The combination of NIC and ICratio showed high sensitivity and specificity for differentiating healthy liver from cirrhotic Liver, especially in Class C cirrhotic liver.Conclusion: Spectral CT Provides a quantitative method with which to analyze the cirrhotic liver, and shows the potential value in the classification of liver cirrhosis.
Contrast‐Enhanced Sonography for Quantitative Assessment of Portal Hypertension in Patients with Liver Cirrhosis

Abstract:ObjectivesThe clinical utility of contrast‐enhanced sonography in portal hypertension remains unclear. We explored the feasibility of using contrast‐enhanced sonography for noninvasive assessment of portal venous pressure.MethodsTwenty healthy individuals (control group; 9 men; mean age, 46.4 years) and 18 patients with portal hypertension (15 men; mean age, 46.2 years) were enrolled in this study. The portal hypertension group included patients who underwent splenectomy and pericardial blood vessel disarticulation at our hospital from October 2010 to March 2011. One week before surgery, patients with portal hypertension underwent preoperative liver contrast‐enhanced sonography. Two‐dimensional, Doppler, and contrast‐enhanced sonographic parameters were compared between the groups. Portal venous pressure was measured intraoperatively by portal vein puncture in the portal hypertension group, and its relationship with the other parameters was analyzed.ResultsThe 2‐dimensional, Doppler, and contrast‐enhanced sonographic parameters differed between the groups (P < .01). Portal venous pressure was inversely correlated with the area under the portal vein/hepatic artery time‐intensity curve ratio (Qp/Qa), portal vein/hepatic artery strength ratio (Ip/Ia), and portal vein/hepatic artery wash‐in perfusion slope ratio (βp/βa), with correlation coefficients of −0.701, −0.625, and −0.494, respectively.ConclusionsMeasurement of the liver contrast‐enhanced sonographic parameters Qp/Qa, Ip/Ia, and βp/βa could be used as a new quantitative method for noninvasively assessing portal venous pressure.
A Non‐invasive Method for Evaluating Cirrhotic Portal Hypertension by Administration of 99mtc‐mibi Per Rectum

Abstract:A study was performed to evaluate radio-isotopic imaging using technetium-99m hexakis 2-methoxyisobutyl isonitrile administered per rectum to assess portal collateral circulation.The heart-liver ratios (H/L; mean +/- standard deviation) in 15 controls, 13 cases of histologically confirmed viral hepatitis and 57 cirrhosis patients were 0.27 +/- 0.11, 0.43 +/- 0.14 and 1.00 +/- 0.28, respectively (P < 0.001). Among the cirrhosis patients those with the Child-Pugh classification A, B and C had H/L of 0.56 +/- 0.14, 1.00 +/- 0.20 and 1.19 +/- 0.26, respectively (P < 0.001). A high value of H/L was associated with a high risk of hepatic encephalopathy (1.25 +/- 0.17, P < 0.01) and oesophageal varices (1.02 +/- 0.20, P < 0.01). There were associations between H/L and serum bilirubin (P < 0.01), albumin (P < 0.05) and prothrombin time (P < 0.05). The results also showed a good correlation between H/L and portal vein pressure measured during operation in 13 patients (P < 0.001, r = 0.87). The regression equation: gamma = 6.77 + 32.5 H/L, allowed portal vein pressure to be estimated.The prognostic value of the test was supported by the fact that good correlations were observed between the H/L ratio and widely accepted prognostic classification (Child-Pugh). It is suggested that this new method could be a reliable non-invasive way to give an indication of the degree of portasystemic shunting to evaluate the prognosis and to follow up the effects of medications for reducing portal hypertension in patients with cirrhotic portal hypertension.
Predicting Risk of Esophageal Variceal Bleeding Caused by Liver Cirrhosis with Spectral CT Imaging in Portal Hypertension Patients

Abstract:Objective To observe the value of spectral CT imaging on predicting the risk of esophageal variceal bleeding(EVB) in portal hypertension patients caused by liver cirrhosis.Methods Spectral CT imaging data of 60 patients with liver cirrhosis and portal hypertension(30 patients complicated with EVB,30 patients without EVB) and 30 patients with normal liver were analyzed retrospectively.ROC curve was drawn to evaluate the diagnosis performance that use iodine concentrations of left,right and caudate lobe of liver in portal phase and diameter of gastric coronary vein(GCV) to predict EVB.The highest sensitivity and the diameter of GCV were selected to evaluate the joint diagnostic value that the iodine concentration of liver lobe.Results When cut-off value of iodine concentrations in left,right and caudate lobe of liver and the diameter of GCV was 17.56 mg/ml,18.29 mg/ml,23.01 mg/ml,5.8 mm,respectively.The area under ROC curve was 0.833,0.874,0.701,0.726,respectively.The sensitivity and specificity was 80.0%,86.7%,70.0%,73.3% and 66.7%,76.7%,60.0%,63.3%,respectively,the positive predictive value and negative predictive value was 76.9%,85.2%,62.1%,66.7% and 70.6%,78.8%,63.6%,69.2%,respectively.The coincidence rate of the sample back was 88.3% that joint the concentration of liver right lobe and GCV diameter.Conclusion Spectral CT imaging can be used to predict the risk of EVB for patients with liver cirrhosis and portal hypertension.

Abstract:Portal hypertension is the most common complication of liver cirrhosis. Noninvasive radiological examinations have important values in evaluating liver parenchyma, collateral circulation, and esophagogastric variceal bleeding in liver cirrhosis. Computed tomography (CT) and magnetic resonance imaging (MRI) can reflect the changes in liver morphology, the density and signals of liver parenchyma, and hemodynamics and have important values in disease diagnosis and prognostic evaluation. CT and functional MRI, such as perfusion-weighted imaging, MR elastography, and MR diffusion-weighted imaging, can provide quantitative information and can be applied in the diagnosis of hepatic fibrosis and early-stage liver cirrhosis in clinical practice. CT angiography and MR angiography, especially unenhanced MR angiography, can show the condition of the branches of portal vein, which helps to evaluate the risk of esophagogastric variceal bleeding and detect the markers for early warning. A combination of CT and MRI and evaluation of their potential in clinical application can give full play to the role of radiological examinations in the diagnosis, evaluation, and prognostic analysis of cirrhotic portal hypertension.
Computed tomography perfusion in differentiating portal hypertension: A correlation study with hepatic venous pressure gradient

Abstract:BACKGROUND: Hepatic venous pressure gradient (HVPG) is the gold standard for diagnosis of portal hypertension (PH), invasiveness and potential risks in the process of measurement limited its widespread use.AIM: To investigate the correlation of computed tomography (CT) perfusion parameters with HVPG in PH, and quantitatively assess the blood supply changes in liver and spleen parenchyma before and after transjugular intrahepatic portosystemic shunt (TIPS).METHODS: Twenty-four PH related gastrointestinal bleeding patients were recruited in this study, and all patients were performed perfusion CT before and after TIPS surgery within 2 wk. Quantitative parameters of CT perfusion, including liver blood volume (LBV), liver blood flow (LBF), hepatic arterial fraction (HAF), spleen blood volume (SBV) and spleen blood flow (SBF), were measured and compared before and after TIPS, and the quantitative parameters between clinically significant PH (CSPH) and non-CSPH (NCSPH) group were also compared. Then the correlation of CT perfusion parameters with HVPG were analyzed, with statistical significance as P < 0.05.RESULTS: For all 24 PH patients after TIPS, CT perfusion parameters demonstrated decreased LBV, increased HAF, SBV and SBF, with no statistical difference in LBF. Compared with NCSPH, CSPH showed higher HAF, with no difference in other CT perfusion parameters. HAF before TIPS showed positive correlation with HVPG (r = 0.530, P = 0.008), while no correlation was found in other CT perfusion parameters with HVPG and Child-Pugh scores.CONCLUSION: HAF, an index of CT perfusion, was positive correlation with HVPG, and higher in CSPH than NCSPH before TIPS. While increased HAF, SBF and SBV, and decreased LBV, were found after TIPS, which accommodates a potential non-invasive imaging tool for evaluation of PH.

The clinical value of dual-energy CT-based iodine quantification parameters in the evaluation of hemodynamics and liver function in patients with cirrhosis

Abstract:Objective:To assess the clinical value of dual-energy CT (DECT) parameters including arterial iodine fraction (AIF) and portal venous blood flow iodine content (PVIC) in the evaluation of hemodynamics and liver function in patients with cirrhosis.Methods:A total of 55 cases with liver cirrhosis (study group) and 26 cases without hepatic disease (control group) were enrolled in the study.All cases underwent upper abdomen CT scan.The enhancement scan protocol of gemstone spectral imaging (GSI) including arterial phase,portal venous phase and delayed phase were performed with DECT scanner (GE Discovery CT 750 HD).Iodine concentration was measured on material decomposition of iodine base figure.Region of interest (ROI) was selected in left lateral lobe,left medial lobe and right lobe of liver respectively and mean liver parenchyma iodine concentration of arterial phase (Ia) and portal venous phase (Ip) were calculated.The arterial iodine fraction (AIF=Ia/Ip) and the portal venous blood flow iodine content (PVIC≈ Ip-Ia) were calculated.AIF and PVIC were compared by single factor analysis,and the difference between groups was analyzed in pairs.The correlation among AIF,PVIC and CTP grade,score were evaluated by Spearman's correlation analysis.Results:Liver cirrhosis group (Child-Pugh B,C) and control group were statistically different in AIF (F 68.650,P＜0.01).AIF of Child-Pugh B,C were higher than that of control group [(31.70±5.07) ％,(47.74±5.75) ％ vs (21.06±4.29) ％].AIF was positively correlated with CTP grade in patients with liver cirrhosis (r1 =0.780,r2 =0.848,P＜ 0.01).Liver cirrhosis group (Child-Pugh A,B,C) and control group were different in PVIC with statistically significance (F=14.949,P＜0.01).PVICs of Child-Pugh A,B,C were lower than that of control group [(1.72±0.51),(1.37±0.38),(0.94±0.27) vs (2.08±0.54)mg/mL]s,and showed an declining trend with statistically significance.PVIC was negatively correlated with CTP grade score in patients with liver cirrhosis (r1 =-0.608,r2 =-0.548,P＜ 0.01).Conclusion:The change of AIF and PVIC reflects a relative decrease of portal venous blood flow quantity (Child-Pugh A,B,C),even presenting a gradual decrease with the degree of cirrhosis,and an increase of hepatic artery blood flow quantity in patients with hepatic cirrhosis (Child-Pugh B,C).AIF and PVIC were correlated to CTP grade,which can be an important reference in the evaluation of liver function reserve.
Quantitative assessment of portal hypertension and its effect on the peripheral organs by CT plain scanning

Abstract:Objective To investigate the value of CT plain scanning in the quantitative assessment of portal hypertension and its effect on the peripheral organs.Methods A respective analysis was made on the CT scan data of 86 people,of whom 45 were patients with portal hypertension (the portal hypertension group) and 41 were normal adults (the control group).Close observation was made on the CT values of the portal right and left branch junction,the abdominal aorta in the same scanning layer,the pancreas,the double kidneys and the spleen of the 2 groups.The patients in the portal hypertension group were classified in accordance with liver function,then,measurements and statistical analyses were made in the CT values of the above corresponding parts.Results The CT values of the portal right and left branch junction,the abdominal aorta in the same scanning layer,the pancreas,the double kidneys and the spleen were respectively (36.97 ± 5.81),(41.06 ± 7.22),(42.64 ± 6.07),(33.18 ± 4.20) and (45.29 ± 4.60) HU,while those of the control group were respectively (43.92 ± 5.71),(47.19 ± 5.80),(50.06 ± 7.80),(36.12 ± 4.50) and (52.38 ± 4.95) HU.The CT value of the above corresponding parts regarding liver function classification (A,B and C) were respectively (39.57 ±(38.67 ±7.35),(37.33 ±4.09),(33.46 ±3.97),(28.23 ±4.13),(48.66 ±4.38),(45.03 ±4.43) and(40.12 ±5.32) HU.Variance analysis showed that there were statistical differences in the CT values of the portal vein,abdominal aorta,pancreas,double kidneys antd spleen between the portal hypertension group and the normal group (P ＜ 0.05).There was also statistical significance in the above corresponding parts regarding liver function classification (A,B and C).Conclusion In case of portal hypertension,the CT valtues of the portal vein,the abdominal aorta,the pancreas,the double kidneys and the spleen would decrease,andt the bigher the liver function classification,the lower the CT values,irndlicating that portal hypertension had certain effects on the tuction of peripheral organs and hemodynamics of aorta.
Contrast-enhanced ultrasound for the characterization of portal vein thrombosis vs tumor-in-vein in HCC patients: a systematic review and meta-analysis

Abstract:Objectives Portal vein thrombosis (PVT) is a common complication of liver cirrhosis. However, differentiation of thrombosis and tumor-in-vein (TIV) may be challenging. Contrast-enhanced ultrasound (CEUS) is an excellent method for detection of vascularization and could help in the distinction. We performed a systematic review and meta-analysis for evaluating the diagnostic value of CEUS in differentiating between PVT and TIV in hepatocellular carcinoma (HCC) patients. Methods PubMed, Embase, Cochrane Library, and Web of Science were searched up to the 5th of May 2019. The study quality was assessed by QUADAS-2 tool. Pooled sensitivity and specificity were calculated by the bivariate random effect model and hierarchical summary receiver-operating characteristic (SROC) curve was plotted. Results Seven studies including 425 participants were analyzed after screening 986 articles searched from databases. The pooled sensitivity and specificity of CEUS in diagnosing TIV were 0.94 (95%CI, 0.89–0.97) and 0.99 (95%CI, 0.80–1.00), respectively. The area under the curve (AUC) of SROC curve was 0.97 (95%CI, 0.95–0.98). The pooled sensitivity and AUC were consistent across all the subgroups of different subject numbers, country, study design, CEUS contrast agents, and diagnostic criteria. Conclusions CEUS is highly efficient in differentiating TIV from PVT and is an alternative or a substitute for CT and/or MRI. MRI-based Liver Vascular Morphology and Surface Nodularity Quantification for Assessment of Portal Hypertension

Abstract:Hepatic venous pressure gradient (HVPG) measurement is the standard technique of assessing portal pressure. Portal hypertension is mainly caused by increased intrahepatic resistance to portal blood flow related to structural changes, including fibrosis and vascular distortion. Here, we used dynamic contrast-enhancement (DCE) MRI to quantify segmented liver contours and vascular morphology in evaluating portal pressure. Our study showed the MRI measurements correlated well with HVPG measurement. A non-invasive prediction model combining liver vascular morphology and liver surface nodularity (LSN) can potentially be used to assess clinically significant portal hypertension (CSPH).
Application of Hepatic Transit Time and Shear Wave Velocity in Assessing Portal Pressure in Patients with Cirrhotic Portal Hypertension

Abstract:The aim of the study described here was to explore the value of hepatic transit time (HTT) and shear wave velocity (SWV) in diagnosing cirrhotic portal hypertension. Fifty-seven patients had undergone esophagogastric varicose vein embolization (group OBS) and 50 healthy controls (group CON) were retrospectively compared with respect to HTT (arterial–hepatic vein [HA-HVTT], portal vein–hepatic vein [PV-HVTT], liver parenchyma–hepatic vein [PA-HVTT]) and SWV to analyze their efficacy in diagnosing cirrhotic portal hypertension. The correlations between SWV/HTT and free portal pressure (FPP) in group OBS were also analyzed. Compared with group CON, group OBS had a shorter HTT and faster SWV. The area under the curve (AUC) of PV-HVTT (0.93) was higher than those of HA-HVTT (0.75) and PA-HVTT (0.64), the AUCs of PV-HVTT (0.93, threshold 7.9 s) and SWV (0.91, threshold 2.0 cm/s) did not statistically differ (z = 0.35, p = 0.73). HTT and FPP in group OBS had a negative correlation. In conclusion, HTT and SWV can be used to diagnose cirrhotic portal hypertension without difference in diagnostic efficacy, and HTT is more meaningful for assessing the changes in portal pressure.

Abstract:Objective To investigate the feasibility of non-invasive quantitative evaluation of portal pressure (Pp) by contrast enhanced ultrasonography (CEUS).Methods 18 portal hypertension patients (PTH group) were performed with CEUS within one week before splenectomy and pericardial devascularization,and 20 healthy volunteers as controls were recruited in this study (control group).Hepatic-right kidney sectionwas chosen to calculate the area under curve of portal vein/hepatic artery (Qp/Qa) and the perfusion intensity of portal vein/hepatic artery (Ip/Ia) through time intensity curves (TIC) of liver parenchyma generated from CEUS images.Pp was measured by intra-operative mesenteric vein catheter,and the correlation betweenPp and Qp/Qa,Ip/Ia were analyzed by Pearson correlation test.Results The levels of Qp/Qa and Ip/Ia in the PTH group were 2.28 ± 0.66 and 0.35 ± 0.14 respectively,which were both significantly declined than that in the controlgroup (5.72 ± 3.69 and 1.97 ± 0.17).In the PTH group,the correlation coefficient were-0.747 and-0.617,and the linear regression equations were Y =-83 X + 5.013 andY =-15X + 0.837,which indicated that Qp/Qa and Ip/Ia had significant correlation with Pp.Conclusions CEUS parameters,including Qp/Qa and Ip/Ia,are significantly correlated to Pp in portal hypertension patients,which indicate that CEUS could be a new non-invasive clinical method for evaluating Pp.

Abstract:Objective To analyze the possibility of assessing portal hypertension in patients with decompensated liver cirrhosis via contrast-enhanced ultrasonography and common serum markers.Methods Fifty-four patients with decompensated liver cirrhosis were divided into two groups according to hepatic venous pressure gradient (HVPG):HVPG＜12 mmHg (1 mmHg=0.133 kPa) and HVPG ≥12 mmHg.The non-invasive index of routine blood test,liver function,coagulation function,Child-pugh score (CPS) and the results of contrast-enhanced ultrasound such as hepatic artery arrival time (HAAT),hepatic vein arrival time (HVAT)、portal vein arrival time (PVAT),hepatic artery to henatic vein arrive transmit time (HA-HVTT) and portal vein to hepatic vein arrive transmit time (PV-HVTT) were assessed by univariate analysis and multivariate Logistic regression analysis,and then were used to generate a diagnostic model.The receiver operating characteristic curve was also used for analysis.Results The non-invasive model is Y =-0.217 × PV-HVTT + 1.526 × CPS-7.097.When the area under ROC curve (AUROC) was ≥0.857 and the best cutoff value was ≥0.631,and the sensitivity and specificity in judging HVPG≥ 12 mmHg were 87.5 ％ and 78.6％,respectively.Conclusions The model composed of PV-HVTT and CPS could be used to assess portal hypertension.

Abstract:Objective To evaluate the color Doppler ultrasonography and contrast enhanced ultrasound (CEUS) findings of congenital intrahepatic portosystemic venous shunts (IPSVS).Methods Nineteen patients of congenital IPSVS were examined with color Doppler ultrasonography and CEUS.All patients were confirmed by CT angiography.The hepatic artery arrival time (HAAT),portal vein arrival time (PVAT),and hepatic vein arrival time (HVAT) on CEUS were recorded.The interval time between hepatic artery arrival time and hepatic vein arrival time (HA-HVTT) and the interval time between portal vein arrival time and hepatic vein arrival time (PV-HVTT) were calculated.Results The types of IPSVS between portal and systemic veins were based on Park's classification.Color Doppler ultrasonography showed abnormal communication between the portal vein branch and the hepatic veins,duplex Doppler ultrasonography showed abnormal spectral pattern from the portal vein such as undulating,triphasic waveform mimicked that of the hepatic vein.CEUS demonstrated abnormal communication between portal vein branch and hepatic vein.HVAT,HA-HVTT,and PV-HVTT were shorter statistically in congenital IPSVS group than those in cirrhosis and normal groups.Conclusions Congenital IPSVS is a rare vascular abnormality that is usually asymptomatic.Color Doppler ultrasonography is a useful tool for diagnosis of congenital IPSVS.CEUS provides helpful data for the diagnosis and differential diagnosis of congenital IPSVS.
Changes in Spectral CT Iodine Densities before and after TIPS in HBV-related liver cirrhosis: A Preliminary Study

Abstract:Abstract Objectives To compare changes in spectral CT iodine densities of hepatic parenchyma and vessels before and after transjugular intrahepatic portosystemic shunt (TIPS) in hepatitis B virus (HBV)-related liver cirrhosis. Materials and Methods Twenty-five patients with HBV-related liver cirrhosis who received TIPS for gastroesophageal varices bleeding were recruited. Each patient underwent three phases contrast CT before and after TIPS within 4 weeks, with the raw data reconstructed at 1.25-mm-thick slices. Iodine density (in milligrams per milliliter) was measured on iodine-based material decomposition image. Multiple regions of interest (ROIs) in liver parenchyma, aorta and portal vein were selected from three slices of images. Portal vein trunk was set as the central one, and mean liver parenchymal iodine densities from arterial phase (AP), venous phase (VP) and equilibrium phase (EP) were recorded. Quantitative indices of iodine density (ID), including normalized ID in liver parenchyma for arterial phase (NIDLAP), ID of liver parenchyma for venous phase (IDLVP), ID of portal vein in venous phase (IDPVP) and liver arterial iodine density fraction (AIF), were measured and compared before and after TIPS. Results Based on Child-Pugh stage, 4, 12 and 9 patients were classified as grade A, B, and C, respectively. Liver volume was comparable before and after TIPS (1110.5±287.4 vs. 1092.0±276.3, P = 0.28). After TIPS, ID was decreased in aorta (146.0±34.5 vs. 120.9± 30.7, P<0.01) whereas increased in liver parenchyma at arterial phase, as demonstrated by IDAP (9.3±3.1 vs. 13.4±4.4 mg/mL) and AIF (0.40±0.11 vs. 0.58 ± 0.11, P<0.01). For venous or equilibrium phase, quantitative indices remained stable (23.1±4.5 vs. 23.0±5.3, 19.8±4.1 vs. 19.4±4.6) mg/mL (Ps>0.05). For portal vein, ID and NID were increased after TIPS (23.1±11.7 vs. 36.5±13.0, 16.4±8.5 vs. 31.8±12.8) (P<0.01). No positive correlation between iodine density and preoperative Child-Pugh score was observed. Conclusions Based on iodine density measurement, spectral CT as a noninvasive imaging modality may assess hemodynamic changes before and after TIPS in HBV-related liver cirrhosis.

Preliminary Study of Spectral CT Dynamic Contrast-enhanced Imaging on the Diagnosis of Liver Cirrhosis

Abstract:Purpose:To evaluate the usefulness of spectral CT dynamic contrast-enhanced imaging for the diagnosis of liver cirrhosis.Methods:Thirty-five subjects underwent spectral CT of the liver.Among them,the control group consisted of 10 subjects who were patients without hepatic disease and the other 25 subjects were patients with liver cirrhosis proved clinically.Parameters which included CT value with 70 keV,iodine concentration,water concentration and effective atomic number(Eff-Z) in arterial phase and portal phase were measured and compared between the cirrhosis group and control group on segment Ⅰ to Ⅷ according to Couinuad classification.Results:Cirrhotic liver parenchyma showed mild heterogeneous enhancement in arterial phase.The CT value of segment Ⅱ and Ⅳ of cirrhotic liver(77±9HU and 79±11HU) tended to decrease significantly when compared with control group(71±7HU and 72±7HU).There was no significant difference in the CT value on other segments and in iodine concentration,water concentration and Eff-Z between liver cirrhosis group and control group on segment Ⅰ to Ⅷ(P0.05).Cirrhotic liver parenchyma showed homogeneous enhancement in portal phase.The CT value,iodine concentration and Eff-Z tended to decrease significantly when compared with control group on segment Ⅰ to Ⅷ.However,there was no significant difference in water concentration between liver cirrhosis group and control group.P0.05 was considered to be of statistical significance.Conclusion:Spectral CT especially the spectral parameters in portal phase are different with parameters in arterial phase,which are helpful for the diagnosis of liver cirrhosis to a certain degree.

Abstract:_Objective:The purpose of this report was to investigate the haemodynamics of portal hypertension of he-patic cirrhosis by use of spectral CT to evaluate the risk of variceal bleeding.Methods:Forty-one patients with hepatic cir-rhosis were selected into our study underwent three-phase spectral CT scans and analyzed retrospectively.The iodine con-centrations for hepatic and splenic parenchyma on arterial and portal vein phases were measured using the iodine-based ma-terial decomposition images.The left gastric vein index (GLI)was obtained by dividing the iodine concentration of the left gastric vein from that of the portal vein.The diameters of main portal vein and spleen vein were also measured.The compar-isons of spectral parameters between hypertension and non-hypertension group,variceal and non-variceal group,bleeding and non-bleeding group were analyzed and ROC curves were drawn based on recorded data.Results:The GLI was positively cor-related with the spleen vein diameter and the correlation coefficient was 0.358 (P= 0.035).The GLI was (0.99±0.26)in the variceal group and (0.78±0.22)in the non-variceal group with significant difference (P<0.05).The GLI was (1.01± 0.21)in the bleeding group and (0.83±0.28)in the non-bleeding group with significant difference (P<0.05).The cut-off value of GLI was 0.87.The area under ROC curve was 0.71.The sensitivity and specificity for predicting EVB was 82.4% and 65%,respectively.Conclusion:The GLI can be useful to predict the risk of esophageal variceal haemorrhage.

Abstract:BACKGROUND: Hepatic venous pressure gradient (HVPG) is the gold standard for diagnosis of portal hypertension (PH). However, its use can be limited because it is an invasive procedure. Therefore, it is necessary to explore a non-invasive method to assess PH.AIM: To investigate the correlation of computed tomography (CT) perfusion of the liver with HVPG and Child-Pugh score in hepatitis B virus (HBV)-related PH.METHODS: Twenty-eight patients (4 female, 24 male) with gastroesophageal variceal bleeding induced by HBV-related PH were recruited in our study. All patients received CT perfusion of the liver before transjugular intrahepatic portosystemic stent-shunt (TIPS) therapy. Quantitative parameters of CT perfusion of the liver, including liver blood flow (LBF), liver blood volume (LBV), hepatic artery fraction, splenic blood flow and splenic blood volume were measured. HVPG was recorded during TIPS therapy. Correlation of liver perfusion with Child-Pugh score and HVPG were analyzed, and the receiver operating characteristic curve was analyzed. Based on HVPG (> 12 mmHg vs ≤ 12 mmHg), patients were divided into moderate and severe groups, and all parameters were compared.RESULTS: Based on HVPG, 18 patients were classified into the moderate group and 10 patients were classified into the severe group. The Child-Pugh score, HVPG, LBF and LBV were significantly higher in the moderate group compared to the severe group (all P < 0.05). LBF and LBV were negatively associated with HVPG (r = -0.473, P < 0.05 and r = -0.503, P < 0.01, respectively), whereas splenic blood flow was positively associated with hepatic artery fraction (r = 0.434, P < 0.05). LBV was negatively correlated with Child-Pugh score. Child-Pugh score was not related to HVPG. Using a cutoff value of 17.85 mL/min/100 g for LBV, the sensitivity and specificity of HVPG ≥ 12 mmHg for diagnosis were 80% and 89%, respectively.CONCLUSION: LBV and LBF were negatively correlated with HVPG and Child-Pugh scores. CT perfusion imaging is a potential non-invasive quantitative predictor for PH in HBV-related liver cirrhosis.
