Abstract:OBJECTIVE:To evaluate the clinical feature and therapy of rare aggressive fibromatosis in the head and neck.METHOD:Clinicopathologic data of 12 cases of aggressive fibromatosis in the head and neck were analysed. All cases underwent surgical excision, one case received radiotherapy in addition to surgery.RESULT:Six cases recurred after cutting off. One of them recurred twice no death was found following operation.CONCLUSION:Aggressive fibromatosis in the head and neck is a rare benign, non-metastatic proliferation tumor of fibrous tissue with a capacity of infiltration to surrounding structures and a tendency of local recurrence after surgical resection. Complete surgical excision was considered to be the only effective method of cure. Chemotherapy and radiotherapy can be used in unsatisfactory surgical marginal recurrence.

Abstract:OBJECTIVES:Follicular dendritic cell sarcoma (FDCS) in the head and neck is uncommon. The etiology, pathogenesis, optimal treatment, and prognosis are not well understood. In this review, we investigated these features to deepen the understanding of the disease.METHODS:We reviewed FDCS in the head and neck in the English language literature through Medline. We analyzed the clinical characteristics, pathologic features, immunophenotypic profile, and treatments of FDCS.RESULTS:Of 137 reported cases of FDCS in the head and neck region, 127 included data on age and gender. Among those, 64 were females and 63 were males. The mean age was 46 years (9-79 years). In the 106 cases with complete follow-up data, fourteen patients (13.2%) had local recurrence. Fourteen patients (13.2%) had distant metastasis. The metastatic site involved the lung, liver, adrenal, rib, vertebral body, and iliac bone. The overall 5-year survival rate was 80.0%. Compared to nodal FDCS and extranodal FDCS, the overall 5-year survival rates were 71.1 and 85.3%, respectively, with no significance (P = 0.42), 0.8% waived treatment, 46.6% received surgery alone, 30.5% received surgery combined with postoperative radiotherapy, 8.4% received surgery combined with postoperative radiotherapy and chemotherapy, 7.6% received surgery combined with postoperative chemotherapy, 3.1% received chemotherapy alone, 2.2% received chemotherapy combined with radiotherapy, 0.8% received radiotherapy alone.CONCLUSIONS:FDCS in the head and neck is rare. There was no difference in the survival between nodal and extranodal FDCS. The optimal treatment was undetermined.
Clinical Analysis of Low Grade Myofibroblastic Sarcoma in Head and Neck Regions

Abstract:Objective To investigate the clinical feature,therapy and prognosis of low grade myofibroblastic sarcoma.Methods A case of low grade myofibrolastic sarcoma in head and neck regions and the related literatures were reviewed.Results The patient received surgical treatment.Recurrences occurred at 6 months and 11months postoperation.An expand excision on the recurred lesion was performed.There was no symptom of recurrence in the 3-year follow up.Low-grade myofibroblastic sarcoma is a rare tumor with a predilection for head and neck sites,it can recur and metastasize.Conclusion Low grade myofibroblastic sarcoma exhibites aggressive growth,local recurrence,and sometimes distant metastasis.It needs excision with tumor-free margin and long time follow up.

Abstract:Objective:To investigate the clinicopathological features,therapeutic methods and therapeutic efficacy of imflammatory myofibroblastic tumor(IMT) in head and neck region.Method:Clinical data of 6 cases which were diagnosed by pathology and immmunohistochemistry from January 2004 to June 2011 were retrospectively analyzed.All cases were confirmed by pathology and immmunohistochemistry.Result:All cases were operated.All the patients were followed up for a period of 6 months to 7 years after operation.Two patients died of tumor until the last follow-up,while 2 cases lived with no evidence of disease.Conclusion:IMT is a very rare tumor in head and neck region,which often occurs in lung.The diagonosis of IMT is based on pathology and immunohistochemistry.Extended local excision with tumor-free margin is the treatment of choice.Chemotherapy and radiotherapy may not be helpful to prevent recurrence after operation.Due to high recurrence rate,long-term follow up is necessary after operation.
Analysis of Clinicopathologic Features and Prognosis in 10 Patients with Non-Epithelial Malignant Neoplasmas of the Larynx and Hypopharynx

Abstract:Objective To investigate the clinicopathologic features,diagnosis and treatment of non-epithelial malignant tumors of larynx and hypopharynx. Methods From 1979 to 2002,the clinicopathologic features,diagnosis and treatment of 10 patients with non-epithelial malignant neoplasmas of larynx and hypopharynx were retrospectively analyzed.Primary lesions of 8 cases located at larynx whose pathological types included 5 sarcomas,2 lymphomas and 1 extramedullary plasmacytoma;Lesions of 2 cases located in hypopharynx.whose pathological types included 1 primary carcinosarcoma and 1 metastatic carcinoma diffused from stomach through lymphoma.All patients were treated with surgery combined with radiotherapy and/or chemotherapy. Results 2 patients with free-neoplasm have outlived for 7 years or 8 years respectively;other 7 patients with free-neoplasm had died in 1-6 years after surgery;and 1 case lost follow-up. Conclusion Non-epithelial malignant neoplasm in larynx and hypopharynx is uncommon and has poor prognasis.Immunohistochemistry staining is helpful for the final diagnosis.The therapy must differ from different types of tumors.
Cellular Fibrous Histiocytoma: a Clinicopathologic Analysis of 27 Cases

Abstract:OBJECTIVE:To study the clinicopathologic characteristics of cellular fibrous histiocytoma (CFH) with emphasis on diagnosis and differential diagnosis.METHODS:Clinical and pathologic features were reviewed in 27 cases of CFH (encountered during the period from 2008 to 2012) along with outcome analysis. Immunophenotyping was performed with EnVision method.RESULTS:The patients included 13 males and 14 females. The age at presentation ranged from 15 to 61 years (mean, 34 years; median, 32 years). The tumor occurred in the extremities (n = 14), head and neck (n = 7), and trunk (n = 6). Histologically, the tumors were located in the dermis. Some cases showed wedge like extension into the subcutaneous adipose tissue. On high power, they consisted of dense fibroblasts and myofibroblasts. Other cell components such as psammoma-like histiocytes, hemosiderin-containing macrophages or touton-type giant cells were rare. The spindled tumor cells were arranged mostly in intersecting fascicles. Focal storiform architecture was not uncommon. In addition, a few cases showed prominent hemangiopericytoma-like pattern. There was no prominent cellular atypia but increased mitotic figures were not difficult to find. Two cases exhibited necrosis. By immunohistochemistry, the tumor cells showed variable expression of alpha smooth muscle actin. CD34 positive cells were present in some cases, but were distributed mostly in the periphery or bottom of the lesions. They were all negative for desmin, h-caldesmon, S-100 protein and cytokeratin. Follow-up in 19 cases revealed local recurrences in 5 cases and bilateral pulmonary metastases in 1 case after repeated recurrences.CONCLUSIONS:CFH is a cellular form of benign fibrous histiocytoma which has a risk for local recurrence after incomplete excision. Distant metastasis can occur in rare examples. However, there were no morphological parameters predicting the risk of recurrence or metastasis. Increased awareness of the clinocopathological features and immunophenotypes of CFH is helpful in avoiding misdiagnosing the disease as malignant tumors, especially dermatofibrosarcoma protuberans.
Atypical Fibrous Histiocytoma: a Clinicopathologic Analysis of 24 Cases

Abstract:Objective To study the clinicopathologic characteristics of atypical fibrous histiocytoma (AFH),with emphasis on differential diagnosis.Methods The clinical and pathologic features were reviewed in 24 cases of AFH (from 2007 to 2012).The follow-up data were analyzed.Immunohistochemical study using EnVision method was carried out.Results There were 10 males and 14 females with age at presentation ranging from 8 to 67 years (mean =41 years and median =39 years).The tumor occurred in the extremities (number =14),trunk (number =8) or head and neck region (number =2).Apart from one case,all were located in the dermis.The clinical appearance was similar to those of classic fibrous histiocytoma.Histologically,the tumor was characterized by various number of hyperchromatic bizarre cells scattered in the background.Mitotic figures including atypical ones were noted,especially in the more cellular areas.Immunohistochemical study showed that the tumor cells expressed vimentin.Focal positivity for alpha-smooth muscle actin was demonstrated in some cases.Follow-up information was available in 14 cases.Three of them suffered local recurrence.None of these cases had distant metastasis.Conclusions Atypical fibrous histiocytoma represents a pleomorphic variant of fibrous histiocytoma.Although the tumor exhibits worrisome features,it usually pursuits a relatively benign course.Nevertheless,rare cases may recur,especially after incomplete excision.AFH is sometimes mistaken as atypical fibroxanthoma.A distinction between the two entities is warranted as they represent two different entities.

Abstract:Background: The spectrum of pulmonary fibrohistiocytic lesions (PFL) ranges from benign fibrohistiocytoma or inflammatory pseudotumor, fibrohistiocytic type (IP), to fibrohistiocytic lesions of borderline malignant appearance (B), to unequivocal malignant fibrous histiocytoma (MFH). The authors reviewed a spectrum of PFL in an attempt to define their natural history and establish independent factors of prognostic significance. Methods: Thirty-one fibrohistiocytic lesions were analyzed by multiple clinical, gross, and microscopic factors. Prognostic factors were evaluated by univariate and multivariate Cox proportional hazards models. Results: After histopathologic review, the cases were classified into IP (15 cases), B (3 cases), and MFH (13 cases). IP tended to occur in younger patients (median age, 30 years; range, 11-64 years) than did MFH (median age, 54 years; range, 12-81 years) (P = 0.03). All 15 patients with IP and 3 patients with B survived. Local recurrence developed in 2 patients with IP and 6 patients with MFH. Distant metastases occurred in 7 patients with MFH. Eight of 13 patients with MFH died of disease, with a median survival time of 16 months (range, 6-36). For all pulmonary fibrohistiocytic lesions, univariate Cox regression analysis disclosed several statistically significant prognostic factors associated with mortality: metastasis (P = 0.0005), significant necrosis (P = 0.0015), local recurrence (P = 0.003), bizarre giant cells (P = 0.0282), mitoses greater than or equal to 3 per 50 high-power fields (P = 0.0286), stage at surgery (P = 0.002), high cellularity (P = 0.0267), and poor circumscription (P = 0.0294). Other parameters (age, sex, symptoms, laterality, location, atypical mitoses, nuclear pleomorphism, degree of fibrosis, and size) were not significant prognostic factors. By multivariate Cox regression analysis, metastasis was the most significant individual parameter (P = 0.002), overriding all other prognostic factors. With metastasis removed from the model, local recurrence (P = 0.0199) and necrosis (P = 0.037) were the other significant independent prognostic factors. Conclusions: Certain individual clinical, gross, and microscopic features have prognostic relevance in determining survival time in pulmonary fibrohistiocytic lesions.

Abstract:We thank Dr. de Carvalho Aguiar and colleagues for their insightful letter entitled “Novel compound heterozygous mutations in PRKRA cause pure dystonia,“ which revisits our recent article on recessively transmitted DYT16 dystonia. The authors used Sanger sequencing to uncover novel compound heterozygous PRKRA variants (c.230G>C [p.Cys77Ser]; c.638G>T [p.Cys213Phe]) in 2 female siblings of a nonconsanguineous Brazilian family presenting an earlyonset isolated dystonia phenotype. In our article, we described a consanguineous Polish sibpair affected by adolescence onset dystonia-parkinsonism, who harbored the recurrent homozygous PRKRA c.665C>T (p.Pro222Leu) mutation within a 1.2-Mb spanning disease haplotype shared with three DYT16 families of Brazilian origin. Now, the identification of independent disease-segregating PRKRA mutations in familial dystonia rules out the possibility that the original c.665C>T (p.Pro222Leu) variant only represents an innocuous tag single-nucleotide polymorphism, which is in linkage disequilibrium with the real causative sequence alteration borne on the common haplotype. Hence, the study by de Carvalho Aguiar and colleagues finally proves that PRKRA coding region mutations are indeed the underlying genetic defect in DYT16. Considered with another recent article reporting compound heterozygous PRKRA variants (c.665C>T [p.Pro222Leu]; c.637T>C [p.Cys213Arg]) in a sporadic infant manifesting dystonia-parkinsonism in the setting of a febrile illness, to date, a total of four different PRKRA missense substitutions affecting distinct parts of the encoded protein have been clearly implicated in DYT16. All of these appear to be either extremely rare or private, as evidenced by recently released exome data from the Exome Aggregation Consortium (ExAC), and only the c.665C>T (p.Pro222Leu) variant seems to occur in diverse ethnicities (prevalence according to ExAC: 1:3336 in Europeans, 1:2575 in African Americans, and 1:964 in Latinos). De Carvalho Aguiar and colleagues applied four different bioinformatics algorithms (PolyPhen2, SIFT, Mutation taster, and CADD) to assess the possible damaging nature of the DYT16-associated PRKRA variants and found at least one benign prediction for each variant, indicating the limited accuracy of some of these in silico tools. On the clinical level, the growing number of molecularly confirmed DYT16 cases highlights phenotypic heterogeneity, ranging from severe infantile combined dystonia syndromes to moderate adolescence onset isolated dystonia (Table 1). Also, remarkable intrafamilial variability is observed, suggesting that additional genetic or environmental disease modifiers may exist in DYT16. The most common clinical expression might be one of early-onset generalized dystonia in combination with varying degrees of parkinsonism, mandating comprehensive PRKRA genetic screening in such cases, particularly within a familial context suggestive of recessive disease transmission. Moreover, functional studies should be the logical consequence to provide insight as to how PRKRA gene mutations mediate the phenotypic effect and to develop therapeutic strategies for this debilitating condition. Autosomal-recessively inherited isolated dystonia phenotypes might be more common than previously appreciated.

Abstract:OBJECTIVE:To study the clinical, pathological and immunohistochemical features of giant cell fibroblastoma (GCF), with emphasis on its differential diagnosis and histogenesis.METHODS:Seven cases of GCF were investigated by light microscopy and immunohistochemistry.RESULTS:Six cases occurred in children, and one occurred in a 35 year-old adult (mean 9.4 years). Five were male and two were female. Clinically, all cases appeared as slowly growing painless nodules located in the dermis or subcutis of the trunk and extremities. Microscopically, the poorly circumscribed tumor was composed of a proliferation of slightly to moderately atypical spindle cells which were arranged in parallel or wavy fascicles, and embedded in a fibromyxoid to collagenous background. The pathognomonic feature consisted of irregular distributed cleft-like or sinusoid-like pseudovascular spaces lined with a discontinuous layer of pleomorphic spindle cells and multinucleate giant cells. There was transition in shape between these two cells. Immunohistochemially, both cells expressed vimentin and CD34. Follow-up information in five cases showed local recurrences in two cases.CONCLUSIONS:(1) GCF is a distinctive fibroblastic tumor of intermediate malignancy that occurs predominantly in children. Recognizing its clinical and pathological characteristics is important to avoid misdiagnosis with other lesions with similar features. (2) GCF shared clinical, immunohistochemical and cytogenetic features with its adult counterpart-dermatofibrosarcoma protuberans (DFSP). The additional coexistence of GCF and DFSP areas in some primary cases and the reciprocal transformation in recurrent tumors all suggest that they are two closely related entities, possibly representing two members of the CD34 positive dendritic neoplasms.
Analysis of the Clinicopathologic Characteristics and Prognosis of Head and Neck Lymphoma

Abstract:Statistical reports on non-Hodgkin’s lymphoma (NHL) of the head and neck combining clinical medicine with pathology are rare. To provide a basis for prognosis prediction and individualized treatment, we will investigate the clinicopathologic characteristics and prognosis of lymphoma in the head and neck region. Four hundred sixty-one patients with NHL in the head and neck region diagnosed through histological biopsy were retrospectively analyzed. Fluorescence in situ hybridization (FISH) and immunohistochemistry (IHC) were performed in all cases to evaluate the genetic status and protein expression levels. Patients were followed up by telephone. The prevalence rate of primary extranodal NHL (PENHL) in the head and neck region was 44.62% (166/372). The incidence of extranodal lymphoma accounted for 36.66% (169/461) of all head and neck lymphomas. Among the cases of PENHL of the head and neck, diffuse large B-cell lymphoma (DLBCL) (60/76, 78.95%) and extranodal NK/T-cell lymphoma, nasal type (ENKTCL) (21/24, 87.5%) were the most common subtypes originating from B-cell lymphoma (BCL) and T-cell lymphoma (TCL), respectively. The most common sites of nodal and extranodal onset were neck lymph nodes and the gastrointestinal tract, respectively. The most common and primary locations of BCL and TCL were the tonsils and nasal cavity, respectively. The 3-year survival rates of PENHL, ENKTCL, and DLBCL of the head and neck were 42%, 28.57%, and 41.67%, respectively, and the 5-year survival rates were 24%, 19.05%, and 20%, respectively. Survival analysis showed that male sex was a risk factor ( HR = 5.421 ; 95% CI, 1.164-25.267; p < 0.05 ) and that comprehensive treatment was a protective factor ( HR = 0.117 ; 95% CI, 0.025-0.545; p < 0.05 ) against extranodal DLBCL in the head and neck region. Bone marrow involvement was a risk factor for PENHL of the head and neck ( HR = 5.072 ; 95% CI, 1.17-21.991; p < 0.05 ). The purpose of this review is to show that PENHL of the head and neck with high incidence deserves more attention, and a model of multidisciplinary diagnosis and treatment should be adopted.

[Angiomatoid Fibrous Histiocytoma: Report of 5 Cases with Review of Literature].

Abstract:OBJECTIVE:To study the clinicopathologic features, immunophenotype and differential diagnosis of angiomatoid fibrous histiocytoma (AFH).METHODS:The clinicopathologic features of 5 cases of AFH were analyzed. Immunohistochemical study was carried out and the literature was reviewed.RESULTS:There were a total of 3 males and 2 females. The average age of patients was 21.4 years old. The average duration of symptoms was 13 months. The patients primarily presented with a slowly enlarging painless deep dermal or subcutaneous mass. The mass was located in the head and neck region in 3 cases, elbow in 1 case and foot in 1 case. The patients underwent complete resection of the tumor, with no adjuvant chemotherapy and/or radiotherapy given. During a period of follow up for 10 to 29 months, all of them had no recurrence or distant metastasis. Gross examination showed that the tumor was well-circumscribed and had a grey-colored cut surface, with focal hemorrhagic cystic changes. The average tumor dimension was 1.9 cm. Histologically, the tumor was composed of histiocytoid or spindly cells arranged in nodular pattern. Fibrillary neuropil-type intercellular material was identified in all cases and a fibrous pseudocapsule surrounded by lymphocytes and plasma cells was demonstrated in 3 cases. Immunohistochemical study showed that all of them were positive for vimentin and negative for S-100 protein, pan-cytokeratin, CD34 and CD31. Three of the cases expressed desmin and CD68. Two cases were epithelial membrane antigen and CD99-positive.CONCLUSIONS:AFH is a rare tumor of intermediate malignant potential. Definitive diagnosis requires thorough histologic examination and clinical correlation. Immunohistochemistry is also helpful for diagnosis and differential diagnosis. Wide local excision with post-operative follow up is the main modality of treatment.

Abstract:Caveolin-3 is the principal structural protein of caveolae membrane domains in striated muscle cells. Caveolin-3 mRNA and protein expression are dramatically induced during the differentiation of C2C12 skeletal myoblasts, coincident with myoblast fusion. In these myotubes, caveolin-3 localizes to the sarcolemma (muscle cell plasma membrane), where it associates with the dystrophin-glycoprotein complex. However, it remains unknown what role caveolin-3 plays in myoblast differentiation and myotube formation. Here, we employ an antisense approach to derive stable C2C12 myoblasts that fail to express the caveolin-3 protein. We show that C2C12 cells harboring caveolin-3 antisense undergo differentiation and express normal amounts of four muscle-specific marker proteins. However, C2C12 cells harboring caveolin-3 antisense fail to undergo myoblast fusion and, therefore, do not form myotubes. Interestingly, treatment with specific p38 mitogen-activated protein kinase inhibitors blocks both myotube formation and caveolin-3 expression, but does not affect the expression of other muscle-specific proteins. In addition, we find that three human rhabdomyosarcoma cell lines do not express caveolin-3 and fail to undergo myoblast fusion. Taken together, these results support the idea that caveolin-3 expression is required for myoblast fusion and myotube formation, and suggest that p38 is an upstream regulator of caveolin-3 expression.
Fibromatosis of the head and neck: morphological, immunohistochemical and clinical features.

Abstract:The term fibromatosis indicates a proliferation of well-differentiated fibroblasts and myofibroblasts that develops in the soft tissue. This tumor has been considered locally aggressive because of the infiltrative growth pattern, but does not metastasize. Its occurrence in the head and neck region is not common, and very sporadically it may occur in the oral cavity or jaw bones. Two cases of adult fibromatosis are described here; one involving the submucosa of the middle hard palate and the other the subcutaneous area of the frontal bone. Tumor growth was rapid and bone involvement occurred in one case. Biopsies were taken and in both cases histological examination showed a tumor consisting of fibroblasts within a collagenous stroma, with rare atypia and mitoses. The patients underwent surgical resection with wide surgical margins; excised material was analyzed microscopically and a diagnosis of fibromatosis was rendered. Immunohistochemistry was positive for actin, vimentin and desmin, and negative for S-100, CD34, the progesterone receptors (PR)-A and PR-B and the estrogen receptors (ER) a and ER beta. Follow-up at 5 years after surgery found both patients to be progression free. The clinical, histological and immunohistochemical features were analyzed to better characterize this form of the disease, that very rarely involves the oral cavity.
Epithelioid and Clear Cell Solitary Fibrous Tumors: Clinicopathologic, Immunohistochemical, and Molecular Genetic Study of 13 Cases

Abstract:Solitary fibrous tumors (SFTs) are ubiquitous soft tissue neoplasms known for their protean histology and potentially aggressive behavior. Although most cases are composed of a monotonous proliferation of spindle cells, some tumors show unusual cytologic features. We have studied 13 SFTs that were characterized by a predominant population of round epithelioid cells with abundant eosinophilic cytoplasm and clear cell changes. The tumors occurred in 8 women and 5 men, aged 36 to 80 years (mean=63 y), and were located within the orbit (3), lower extremity (3), retroperitoneum (2), abdominal cavity (2), and superficial soft tissues of the neck, pelvis, and pubis (1 each). The tumors measured from 3.5 to 24.5 cm. Using a risk assessment system, 6 cases were stratified as low-risk tumors; 3 of these showed no evidence of recurrence or metastases from 6 to 18 years, and 1 tumor in the orbit recurred and led to the patient's demise. Five cases were of intermediate risk; clinical follow-up showed no evidence of recurrence or metastases from 3 to 4 years in 3 patients, and 1 patient suffered a recurrence 4 years after diagnosis. Two cases were high risk; 1 patient died after 1 year and the second patient experienced local recurrence at 4 years. Immunohistochemical studies showed nuclear positivity for STAT6 in 10 cases. CD34 immunohistochemistry was positive in 11 cases. A NAB2::STAT6 rearrangement was present in all cases. Epithelioid and clear cell SFT should be considered in the differential diagnosis of soft tissue neoplasms with epithelioid and clear cell morphology.
Clinicopathological Analysis of Myxofibrosarcoma of the Head

Abstract:Purpose To study the clinicopathological characteristics and to discuss the differential diagnoses of myxofibrosarcoma(MFS) of the head. Methods The clinical and pathological feature of 8 cases of MFS was evaluated.Among which,6 cases were immunohistochemically studied. Results There were 6 male and 2 female patients,aged between 12 to 58 years(mean 34 years).Five tumors occurred in the nasal cavity or paranasal sinuses,2 in the ear regions,and 1 was located in the orbit.Clinically,most patients presented with nasal obstruction,rhinorrhagia,headache,exophthalmos or a mass in the ear regions.CT scan or MRI usually showed a soft tissue tumor with invasion in the adjacent tissues,associated with bone erosion or destruction.Histologically,the tumors were mainly composed of spindle to stellate-shaped cells deposited in a prominent myxoid matrix.In some cases,hyperchromatic pleomorphic cells and multivacuolated pseudolipoblasts were present. Immunohistochemically,the neoplastic cells were diffusely positive for vimentin and one case expressed α-SMA focally.Follow-up information of 7 patients showed recurrences in two cases at 1 and 2 months after local excision respectively and both died of the disease. Conclusions Myxofibrosarcoma of the head is an aggressive soft tissue sarcoma that has a high rate of local recurrence.Combined modality is recommended to control the relapse of the disease.

Abstract:OBJECTIVE:To assess the therapeutic strategies and risk factors of malignant fibrous histiocytoma of soft tissue.METHODS:The 78 cases with malignant fibrous histiocytoma of soft tissue treated at Muscular Skeletal Tumor Center of People's Hospital, Peking University from December 1999 and October 2010 were retrospected. Univariate and multivariate analyses were performed to determine the probable risk factors including sex, age, tumor location, tumor size and so on.RESULTS:All 60 cases were followed up ranged from 6 to 131 months (medium 35.5 months). The 1-, 3-, 5-year overall survival rate was 84.9%, 72.9% and 56.9% respectively. Local recurrence rate is 33.3% (20 cases) with a median time of 11.5 months (1 to 72 months) and metastatic rate is 15.0% (9 cases) with a median time of 7 months (1 to 26 months). Univariate analysis indicated that condition of presentation (primary case or recurrence case), tumor size and surgical margin were significantly related to survival rate (all P < 0.05, Kaplan-Meier Log-rank test), surgical margin and radiotherapy related to local recurrence rate (P value were 0.000 and 0.039 respectively), and surgical margin related to metastatic rate. Multivariate analysis showed that surgical margin was independent risk factors for survival rate (P = 0.002, OR = 5.753, 95%CI 1.904 - 17.386) and local recurrence rate (P = 0.000, RR = 0.044, 95%CI 0.010 - 0.188).CONCLUSIONS:Surgical margin was independent risk factors for survival rate and local recurrence rate. Comprehensive therapy of surgery followed by radiotherapy can improve survival rate, reduce local recurrence rate.
[Low-Grade Fibromyxoid Sarcoma: a Clinicopathological Analysis of 9 Cases].

Abstract:OBJECTIVETo study the clinicopathological features, immnophenotype and differential diagnosis of low-grade fibromyxoid sarcoma (LGFMS).METHODSThe clinical, radiological and pathological features of 9 cases of LGFMS were reviewed.RESULTSThe patients consisted of six males and three females with ages ranging from 11 to 65 years (mean 31.4 years). Clinically, most cases presented as slowly growing painless masses located in the extremities, trunk and neck. Two cases had a history of a rapid recent enlargement. Three cases presented with recurrent diseases after incomplete resection. Ultrasound showed heterogeneous signal. Precontrast CT and T1-weighted MRI showed a nodular mass of low to isodensity, while contrast-enhanced CT and T2-weighted MRI demonstrated contrasting zonal areas of hypo/hyperintensity. The tumors measured 3 to 16 cm (mean 7.7 cm), with a fibrous to myxoid appearance on cut surface. Histologically, they were composed of alternating collagenous and myxoid areas. A transition between the two areas could be identified. Giant rosette-like structures were observed in 3 cases. The center of the giant rosettes was composed of eosinophilic collagen, which showed centrifugal in arrangement. There was also gradual transition between the giant rosettes and the fibromyxoid component. On high magnification, the tumor was composed of ovoid to spindle-shaped cells with hyperchromatic nuclei. Nuclear atypia was inconspicuous. The tumor cells were arranged mostly in interlacing fascicles or whorls. Vessels were not prominent and necrosis was absent. Immunohistochemically, the tumor cells showed unique staining for vimentin, consistent with a fibroblastic differentiation.CONCLUSIONSLGFMS is a distinctive low grade fibroblastic sarcoma of young adults. Recognizing the characteristics of the rare entity may help to avoid misdiagnosis. Wide local excision is recommended to avoid local recurrences.

Abstract:Abstract BACKGROUND Chondromyxoid fibromas (CMFs) are benign tumors that occur rarely in the skull base. OBJECTIVE To conduct a preliminary exploration of the diagnosis and treatment of cranial CMFs. METHODS A retrospective analysis of 19 cases of CMFs in the base of the skull between 2009 and 2014 in our hospital was conducted. The clinical manifestations, imaging characteristics, pathology, treatment strategies, and outcomes were examined. RESULTS The study cohort included 7 women (36.8%) and 12 men (63.2%), and symptom duration ranged from 1 mo to 5 yr. Of the 19 intracranial CMF cases examined, 15 (78.9%) conformed with the diagnostic criteria for extracranial CMF. Resection operations yielded subtotal removal of 13 tumors (68.4%) and partial removal of 6 tumors (31.6%). Postoperative pathological analysis demonstrated that the tumors were characterized by spindle-shaped or stellate cells arranged in a myxoid matrix without mitoses or permeation. Follow-up (range 2-7.3 yr; mean, 4.4 ± 1.7 yr) revealed that symptoms improved postoperatively in 15 cases (78.9%), were maintained in 2 cases (10.5%), and worsened in 2 cases (10.5%). Imaging follow-up revealed that residual tumors were stable in 18 cases (94.7%) and enlarged in 1 case (5.3%). CONCLUSION An accurate diagnosis should involve comprehensive consideration of clinical, radiological, and pathological features. The treatment strategy for CMFs consists of maximizing tumor removal while protecting adjacent key structures. Postoperative stereotactic radiotherapy is appropriate for residual tumors.
