Abstract:Background and AimsDuodenal perforation caused by foreign bodies (FBs) is very rare but is an urgent emergency that traditionally requires surgical intervention. Several case reports have reported the successful endoscopic removal of duodenal perforating FBs. Here we aimed to evaluate the safety and efficacy of endoscopic management of duodenal perforating FBs in adults.MethodsBetween October 2004 and October 2022, 12,851 patients with endoscopically diagnosed gastrointestinal FBs from four tertiary hospitals in China were retrospectively reviewed. Patients were enrolled if they were endoscopically and/or radiographically diagnosed with duodenal perforating FBs.ResultsThe incidence of duodenal total FBs and perforating FBs was 1.9% and 0.3%, respectively. Thirty-four patients were enrolled. Endoscopic removal was achieved in 25 patients (73.5%), and nine patients (26.5%) received surgery. For the endoscopic group, most perforating FBs were located in the duodenal bulb (36.0%) and descending part (28.0%). The adverse events included 3 mucosal injuries and 1 localized peritonitis. All patients were cured after conventional treatment. In the surgical group, most FBs were lodged in the descending part (55.6%). One patient developed localized peritonitis and one patient died of multiple organ failure. The significant features of FBs requiring surgery included FB over 10 cm, both sides perforation, multiple perforating FBs and massive pus overflow.ConclusionEndoscopic removal of duodenal perforating FBs is safe and effective, and can be the first choice of treatment for experienced endoscopists. Surgical intervention may be required for patients with FBs over 10 cm, both sides perforation, multiple perforating FBs, or severe infections. image

Abstract:Foreign body ingestion is a common problem encountered at gastroenterology clinics and emergency rooms which can cause serious complications. Usually, foreign bodies are directly visible with flexible endoscopes and can be readily removed. However, when foreign bodies migrate into the deeper tissue of the esophagus, surgery is typically required. There is currently no consensus regarding the best treatment. In this report, we present two cases in which fish bones embedded in the submucosal and muscularis propria of the esophagus were successfully removed via endoscopic submucosal dissection (ESD). Both patients were discharged without any complications.
The Safety and Efficacy of Endoscopic Submucosal Fenestration in the Treatment of Completely Embedded Upper Gastrointestinal Foreign Body

Abstract:BACKGROUND:Upper gastrointestinal foreign body ingestion is a common digestive tract emergency, of which completely embedded ones were challenging for most endoscopists. We aim to evaluate the efficacy and safety of endoscopic submucosal fenestration in the treatment of completely embedded upper gastrointestinal foreign bodies. METHODS:From December 2018 to December 2021, 19 patients with completely embedded upper gastrointestinal foreign bodies who underwent endoscopic submucosal fenestration in Zhongshan Hospital, Fudan University were included. The safety, efficacy, and outcome were retrospectively reviewed. RESULTS:Among the 19 patients, 15 foreign bodies were embedded in the esophagus, 3 located in the gastric wall, and 1 located in the duodenal bulb. The foreign bodies were successfully managed in 12 cases, and 7 failed after attempts of repeated exploration. Two cases confirmed completely traversing into the mediastinum were successfully removed after transfer to surgery. One case had retrieval of a foreign body in a half-year examination. Till now, 3 failed patients had great relief of symptoms and only one patient claimed occasional thoracodynia. Of note, there were neither serious adverse events, nor long-term complications during the follow-up. CONCLUSION:In disposing of foreign bodies completely embedded in the upper gastrointestinal tract, ESF is a safe and effective alternative to surgery.
Delayed Endoscopic Removal of Sharp Foreign Body in the Esophagus Increased Clinical Complications

Abstract:Foreign bodies impaction in the esophagus is a common clinical emergency. The aim of this study was to investigate the clinical features of foreign body ingestion, and to analyze the risk factors of complications during the endoscopic procedure. From 18 general hospitals in Zhejiang Province in China, 595 patients who underwent gastroscopic removal of ingested foreign bodies were prospectively recruited. Patient characteristics, clinical features, foreign body features, clinical outcomes, and complications were documented. The most common types of foreign body in the esophagus were sharp objects (75.9%), including fish bones (34.0%), chicken bones (22.1%), and fruit nuclei (17.1%). The remaining types were non-sharp objects (24.1%), including food bolus (14.6%). Most objects were lodged in the proximal esophagus (75.9%). Foreign body-related complications occurred in 63 patients (10.5%), including hemorrhage (5.0%), perforation and infection (5.5%). The complication rate was increased by 4.04- and 8.48-fold when endoscopic retrieval was performed after impacted for over 24 and 48hours, respectively, after impaction, as compared with within 12hours. Logistic regression analysis revealed that the patients with sharp objects developed more complications than those with non-sharp ones (odds ratio, 2.85; 95% confidence interval, 1.08-7.50; P=.034). However, complications were unrelated with the location in the esophagus or length of foreign bodies (P>.05). Sharp objects were the most frequently ingested foreign bodies in the esophagus in China. The prevalence of complications was increased in the patients with long foreign body retention time (>24 hours) and sharp objects. Sharp foreign bodies in the esophagus are recommended to be removed within 24hours.
Endoscopic Removal Of Large, Rare Rectosigmoid Foreign Body With Sengstaken-Blakemore Tube

Abstract:A 36-year-old man presented to our hospital with a 1-day history of abdominal bloating after inserting a carrot into his rectum. The carrot did not pass spontaneously, and manual extraction was unsuccessful. The patient reported no anorectal or abdominal pain. Abdominal computed tomography confirmed a cone-shaped, low-density foreign body (length, 14 cm; maximum diameter, 4 cm) in the rectosigmoid junction with no evidence of bowel perforation. Emergency colonoscopy revealed the distal part of the carrot in the rectum. After switching to a 5.0-mmdiameter transanal gastroscope and insertion along the gap between the foreign body and bowel wall, liquid paraffin was injected by catheter into the lumen, surrounding the foreign body. A 5.6-mm-diameter Sengstaken-Blakemore tube was then inserted into the transanal endoscope and smeared with liquid paraffin. Both devices were then slowly advanced forward beyond the carrot. The gastric and esophageal balloons were successively inflated (up to 200 and 150 ml, respectively). Finally, the endoscope was gently pulled and the foreign body (carrot) was fully extracted without complications.

Abstract:Background and Aim: Traditionally, surgery is the only choice for esophageal foreign bodies (FBs) penetrating into the mediastinum or cervical para-esophageal space. Recent progress in endoscopic techniques and devices has made it possible to perform endoscopic procedures in the mediastinum. Here, we describe a novel method to remove the entirely embedded esophagus-penetrating FBs through an endoscopic approach. Methods: Patients who underwent endoscopic removal of entirely embedded esophagus-penetrating FBs were prospectively enrolled between December 2018 and June 2020. All procedures were performed by using five major steps: localization of the FB; mucosal incision; wound debridement, myotomy, and FB exposure; FB extraction; and closure of the esophageal wound. Results: A total of five cases of entirely embedded esophagus-penetrating FBs were evaluated, two in children and three in adults. All FBs were successfully removed and extracted using a therapeutic endoscope and a hook knife or a hybrid knife and a grasping forceps as accessories. The average procedure time was 19.0 +/- 12.4 min (range 10-40 min). The major surgical procedure was avoided in all patients. The average length of hospital stay was 3.8 +/- 2.5 days (range 2-8 days). There were no severe short-term adverse events after all procedures. During the 1 month and 3 months follow-up, no patient developed long-term adverse events including stenosis of the esophagus and fistula formation. Conclusions: Endoscopic approach was a safe and effective way to remove entirely embedded esophagus-penetrating FBs.
Endoscopic Closure of a Duodenal Fistula Caused by a Deformable Foreign Body

Abstract:A previously healthy 84-year-old man presented at the emergency department with severe, intermittent, middle-abdominal, back-referred colic pain. The patient reported that the symptoms started 1 month before arrival and progressively aggravated but did not improve with cephalosporins. The pain initially occurred during sleep and was relieved when changing positions. Neither fever, nausea, vomiting, diarrhea, abdominal distension, nor constipation was presented. He had no other medical history. His family and travel history were noncontributory. Physical examination was unremarkable except for upper abdominal tenderness and rebound tenderness. Initial laboratory work, including complete blood count, C-reactive protein, comprehensive metabolic panel, lipids, pancreatic lipase, urine routine, and stool routine, were within normal limits except for slightly decreased hemoglobin level (127 g/L; lower limit of normal, 131 g/L). The abdominal ultrasound examination identified a possible proximal aneurysm of the superior mesenteric artery with thrombosis. A computed tomography scan of the abdomen with intravenous contrast showed a tubular, progressively enhancing mass-like lesion adjacent to the superior mesenteric artery and seemly communicating with the intestinal lumen, and a thickened intestinal wall with blurred edges (Figure 1A–C). The erect double-contrast barium radiograph suggested a descending duodenal diverticulum (Figure 1D). The upper endoscopic evaluation revealed a stalk-shaped foreign body in the horizontal part of the duodenum. It was lodged in the horizontal part of the duodenum and surrounded by purulent liquid, forming a duodenal fistula (Figure 1E). After an 10-cm stalk was extracted, it was observed that the fistula was communicating with the abdominal cavity (Figure 1F). Subsequently, the fistula was fully closed using several clips (Figure 1G). A naso-intestinal tube was then inserted into the jejunum to provide nutrient support combined with intravenous antibiotics and a proton-pump inhibitor after the endoscopic surgery. After further questioning of the patient’s history, the patient confessed to consuming Dendrobium officinale before the symptom onset, which is a herbaceous plant generally dried and curled into a clump for convenient storage and used as a tonic herb in traditional Chinese medicine that is often used to make tea or medicinal food (Figure 1I–K). The dried Dendrobium officinale clump that the patient had swallowed happened to be stuck in the horizontal part of the duodenum; it swelled and stretched when contacting gastric contents, leaving the indigestible stem that finally caused a fistula. Five weeks after the procedure, the patient gained a good recovery without any complications.

Abstract:A 37-year-old male suffered from abdominal pain and distention for 8 hours after a huge long foreign body was inserted through anorectum by self-masturbation. The colonoscope with the polypectomy snare griped the foreign body was pulled out slowly along with the dragged foreign body. However, when we reached at 20 cm from the anal margin, it got stuck because of the turn point in the sigmoid colon. Different methods were tried to remove the foreign body. At last, We asked the patient to turn to supine position, take deep breath and defecation. More air was insufflated to make the colon cavity larger to facilitate removal, and slowly the colonoscope dragged the foreign body (35 cm in length) out from the anus successfully. No bleeding or perforation occurred during the procedure. Although the foreign body was huge, effort was made to help the patient avoid taking operation.

Abstract:A 69-year-old woman was diagnosed with a duodenal adenoma near major duodenal papilla during cancer screening examination (Figure 1A). Therefore, endoscopic mucosal resection (EMR) was proposed to remove the duodenal lesion. Unfortunately, satisfactory visualization of the duodenal lesion was not obtained during gastroscopic operation. Unexpectedly, duodenoscopy provided optimal visualization of the duodenal lesion. Consequently, the "sandwich method" using duodenoscopy-gastroscopy-duodenoscopy was successfully performed to remove the challenging duodenal lesion. Firstly, the duodenoscopy was used to create a submucosal bleb through injecting saline containing 0.3 % indigo carmine. Subsequently, the gastroscopy with a transparent capwas used to remove the duodenal lesion with en bloc resection. Then, the duodenoscopy was reused to close the mucosal defect. Finally, pathologic examination showed a tubule-villous adenoma. The patient was recovered uneventfully, and discharged 2 days later.
Sonographic and CT Imaging Features of Intestinal Perforation from a Pill and Packing

Abstract:Rationale: Sharp foreign bodies such as toothpicks or chicken bones can lead to intestinal perforation. Small intestinal perforation secondary to foreign body ingestion is usually manifested as an acute abdomen without a history of trauma. Here we describe the diagnosis and treatment of a case of small bowel perforation caused by an ingested pill and its outer packing. Patient concerns: An 84 years old male patient complained of right lower abdominal pain for 4 days and the pain was becoming progressively worse. Diagnoses: The patient, who has Alzheimer's disease, mistakenly took the pill (oxiracetam) without removing the outer packaging. This resulted in perforation of the small intestine. Interventions: During the ultrasound examination, the scanning physician discovered that the abnormal sonographic findings present could not be explained by the leading diagnosis of perforation of the small intestine at the time. This led the physician to suspect small bowel perforation secondary to a foreign body. The subsequent computerized tomography (CT) examination further confirmed the ultrasound findings. Outcomes: Emergency laparotomy was performed and the foreign body was removed. After the surgical procedure, the patient resumed anti-inflammatory treatment (Cefoxitin sodium 2000 mg tid) and rehydration therapy (Sodium Chloride Solution 100 mL tid). Lessons: Because ingestion of foreign bodies of this type is relatively rare, when patients cannot provide an accurate history, diagnosis can be quite difficult. In this paper, the imaging features associated with intestinal perforation secondary to foreign body ingestion on ultrasound and CT are described. This series of events demonstrate how imaging findings can guide and alter a clinician's decision-making.
Successful Treatment of Fish Bone Perforation of the Duodenum with Hepatic Abscess Formation and Literature Review

Abstract:Symptomatic gastrointestinal foreign bodies are a common clinical event. Some patients can develop complications, such as gastrointestinal obstruction, perforation etc., and rarely the secondary hepatic abscess can also occur. A 45-year-old male was admitted to hospital with abdominal pain for 1 month and high fever for 6 days. He was diagnosed with hepatic abscess formation due to perforation of the digestive tract by a foreign body. This diagnosis was confirmed when the patient underwent surgery and the foreign body was identified as a fish bone. The fish bone was removed by laparotomy and the patient recovered uneventfully. Based on this case and the currently available literature, we recommend that endoscopic or surgical treatment should be initiated as soon as possible after detection of symptomatic gastrointestinal foreign bodies. Such intervention is important in reducing the risk of perforation of the digestive tract, as well as hepatic abscess formation.
Safety of endoscopic resection for duodenal subepithelial lesions with wound closure using clips and an endoloop: an analysis of 68 cases

Abstract:Background: Endoscopic removal of a duodenal lesion is still considered to be a challenging procedure that can be fraught with potentially serious complications, specifically perforation or delayed bleeding. This study was to assess the safety of endoscopic resection for duodenal subepithelial lesions (SELs) with wound closure using clips and an endoloop. Methods: From October 2010 to July 2015, a total of 68 consecutive patients with duodenal SELs were treated with endoscopic resection with wound closure using clips and an endoloop. The main outcome measures considered were the incidence of complete resection, perioperative perforation, delayed perforation, delayed bleeding, residual lesions, and lesion recurrence. Results: Complete resection was successfully achieved for all 68 patients. The median lesion size was 1.7 cm. The median procedure time was 62 min. The mean hospital stay was 5.5 days. During the procedure, five patients developed perioperative perforations (7.4 %) and no patients developed delayed bleeding, delayed perforation, or other serious complications. The five patients with perioperative perforations recovered after conservative treatment. The perioperative perforation rate was significantly higher for lesions originating in the muscularis propria layer (18.2 %) than in the submucosal layer (2.2 %; p < 0.05). No residual or recurrent lesions were detected during the follow-up period (median: 27 months). Conclusions: Endoscopic resection with wound closure using clips and an endoloop is an effective and reasonably safe therapeutic method for treating/removing duodenal SELs when managed by an experienced endoscopic team, and it can provide an alternative treatment option for patients with duodenal SELs.

Abstract:As a common clinical emergence, esophageal foreign body can lead to esophageal perforation followed by severe complications including aortic injury, mediastinal abscess and airway obstruction, leading to a high rate of mortality. Therefore, fast and effective diagnosis and treatment are of great necessity. In this case, holmium laser-assisted gastroscopy was adopted to remove the foreign body incarcerated in the esophagus, allowing patients to avoid traumatic and costly surgeries. It is a supplement to traditional methods of foreign body removal. The new combination tried in this report can bring development and innovation inspiration to the development of endoscopic technology.

Abstract:Background and Aims Endoscopic resection is a feasible treatment for gastrointestinal extraluminal tumors, but remains a challenging procedure with limited data. In this study, we assessed the safety and efficacy of endoscopic resection for extraluminal tumors in the upper gastrointestinal tract. Methods From May 2016 to December 2021, 109 patients undergoing endoscopic resection for extraluminal tumors in the upper gastrointestinal tract were retrospectively included. Clinicopathological characteristics, procedure-related parameters, adverse events (AEs), and follow-up outcomes were analyzed. Results The en bloc tumor resection rate was 94.5% and the en bloc retrieval rate was 86.2%. Statistical analysis revealed tumor size ≥3.0 cm and irregular shape as significant risk factors for piecemeal extraction. Resection time and suture time were 46.8±33.6 and 20.6±20.1 min, respectively. Large tumor size was significantly associated with a longer procedure duration. Five patients (4.6%) experienced major AEs, including recurrent laryngeal nerve injury, hydrothorax, major bleeding, local peritonitis, duodenal leakage, and repeat endoscopic surgery for tumor extraction. Minor AEs occurred in 13 patients (11.9%). Irregular tumor shape and tumor location (duodenum) were significantly associated with AE occurrence. Mean postoperative hospital stay was 4.7±3.3 days. No recurrence or metastasis was observed during the mean follow-up period of 31.8±15.2 months. Conclusion Endoscopic resection is a safe and feasible therapeutic approach for upper gastrointestinal extraluminal tumors. Tumor size, shape, and location impact the difficulty and safety of the procedure. Endoscopic resection of duodenal tumors is also feasible but associated with an increased risk of AEs, compared with tumors in other locations.

Distinct foreign bodies predispose to particular locations of impaction in the gastrointestinal tract, commonly meat boluses in the esophagus above a preexisting esophageal stricture or ring in adults and coins in children. Several other groups are at high risk of foreign body impaction, mentally handicapped individuals or those with psychiatric illness, abusers of drugs or alcohol, and the geriatric population. Patients with foreign body ingestion typically present with odynophagia, dysphagia, sensation of having an object stuck, chest pain, and nausea/vomiting. The majority of foreign bodies pass through the digestive system spontaneously without causing any harm, symptoms, or necessitating any further intervention. A well-documented clinical history and thorough physical exam is critical in making the diagnosis, if additional modalities are needed, a CT scan and diagnostic endoscopy are generally the preferred modalities. Various tools can be used to remove foreign bodies, and endoscopic treatment is safe and effective if performed by a skilled endoscopist.

Abstract:BACKGROUND When an anorectal foreign body is found, its composition and shape should be evaluated, and a timely and effective treatment plan should be developed based on the patient's symptoms to avoid serious complications such as intestinal perforation caused by displacement of the foreign body. CASE SUMMARY A 54-year-old male was admitted to our outpatient clinic on June 3, 2023, due to a rectal foreign body that had been embedded for more than 24 h. The patient reported using a glass electrode tube to assist in the recovery of prolapsed hemorrhoids, however, the electrode tube was inadvertently inserted into the anus and could not be removed by the patient. During hospitalization, the patient underwent surgery, and the foreign body was dragged into the rectum with the aid of colonoscopy. The anus was dilated with a comb-type pulling hook and an anal fistula pulling hook to widen the anus and remove the foreign body, and the local anal symptoms were then relieved with topical drugs. The patient was allowed to eat and drink, and an entire abdominal Computed tomography (CT) and colonoscopy were reviewed 3 d after surgery. CT revealed no foreign body residue and colonoscopy showed no metal or other residues in the colon and rectum, and no apparent intestinal tract damage. CONCLUSION The timeliness and rationality of the surgical and therapeutic options for this patient were based on a literature review of the clinical signs and conceivable conditions in such cases. The type, material and the potential risks of rectal foreign bodies should be considered.

Abstract:RATIONALE:Accidental ingestion of a foreign body is common in daily life. But the hepatic migration of perforated foreign body is rather rare.PATIENT CONCERNS:A 37-year-old man presented with a history of vague epigastric discomfort for about 2 months.DIAGNOSIS:A diagnosis of the foreign body induced hepatic inflammatory mass was made based on abdominal computed tomographic scan and upper gastrointestinal endoscopy.INTERVENTIONS:The patient underwent laparoscopic laparotomy. During the operation, inflammatory signs were seen in the lesser omentum and segment 3 of liver. B- Ultrasound guided excision of the mass (in segment 3) was performed. Dissecting the specimen revealed a fish bone measuring 1.7 cm in length.OUTCOMES:The patient recovered uneventfully and was discharged on day 5 after surgery.LESSONS:This study shows the usefulness of endoscopy for final diagnosis and treatment in foreign body ingestion. Early diagnosis and decisive treatment in time are lifesaving for patients with this potentially lethal condition.

Abstract:Background and Aims: Endoscopic resection is a feasible treatment for GI extraluminal tumors but remains a challenging procedure with limited data. In this study, we assessed the safety and efficacy of endoscopic resection for extraluminal tumors in the upper GI tract. Methods: From May 2016 to December 2021, 109 patients undergoing endoscopic resection for extraluminal tumors in the upper GI tract were retrospectively included. Clinicopathologic characteristics, procedure-related parameters, adverse events (AEs), and follow-up outcomes were analyzed. Results: The en-bloc tumor resection rate was 94.5% and en-bloc retrieval rate 86.2%. Statistical analysis revealed tumor size >= 3.0 cm and irregular shape as significant risk factors for piecemeal extraction. Resection time and suture time were 46.8 +/- 33.6 minutes and 20.6 +/- 20.1 minutes, respectively. Large tumor size was significantly associated with a longer procedure duration. Five patients (4.6%) experienced major AEs, including recurrent laryngeal nerve injury, hydrothorax, major bleeding, local peritonitis, duodenal leakage, and repeat endoscopic surgery for tumor extraction. Minor AEs occurred in 13 patients (11.9%). Irregular tumor shape and tumor location (duodenum) were significantly associated with AE occurrence. Mean postoperative hospital stay was 4.7 +/- 3.3 days. No recurrence or metastasis was observed during the mean follow-up period of 31.8 +/- 15.2 months. Conclusions: Endoscopic resection is a safe and feasible therapeutic approach for upper GI extraluminal tumors. Tumor size, shape, and location impact the difficulty and safety of the procedure. Endoscopic resection of duodenal tumors is also feasible but associated with an increased risk of AEs compared with tumors in other locations.
