Abstract:Purpose: To report a rare case of delayed dislocation of a novel posterior chamber phakic intraocular lens into the vitreous cavity, which was successfully treated by a reformed technique. Case PresentationA 29-year-old female received Ejinn phakic refractory lens (EPRL) implantation to correct her high myopia. Spontaneous dislocation into the vitreous cavity occurred 26-months post-operatively without traumatic history. Pars plana vitrectomy combined with cutting the EPRL into two equal pieces was performed to remove the dislocated EPRL. ConclusionDislocation into the vitreous cavity of EPRL can be successfully and easily removed by our reformed technique. Concerns about zonules-related complications pre-operatively, intraoperatively, and post-operatively must be raised in the practice of EPRL implantation.

Abstract:We report a case of Descemet's membrane detachment after inadvertent intrastromal injection of hyaluronic acid. Surgical removal was attempted with minimal but slow improvement. Near-complete resolution occurred with subsequent conservative management within 6 weeks.

Abstract:Rationale: Spontaneous absorption of lenses or cataracts is rare. However, lens capsule attachment to the endothelium combined with corneal decompensation can still occur. Patient concerns: An 81-year-old male presented with left eye pain and bulbar conjunctival injection for 6 months. Diffuse corneal edema and inferior bullous lesions were observed by slit-lamp microscopy. Following examination with swept-source optical coherence tomography, we could clearly identify a membrane structure adherent to the corneal endothelium, as well as a lens not in situ. In vivo confocal microscopy found decreased corneal endothelial density of 745 +/- 46 cells per mm(2) in the left eye. Diagnosis: Lens dislocation and spontaneous absorption, combined with corneal decompensation were diagnosed. Interventions: Surgical removal of the membrane structure combined with anterior vitrectomy was performed. Outcomes: The patient's symptoms were partly relieved. However, the corneal endothelial decompensation could not be entirely reversed. In vivo confocal microscopy verified that corneal endothelium was in situ and the density was not significantly changed in the left eye. Lessons: This case study reports a rare dislocation and spontaneous absorption of lens without any trauma or subsequent surgery. Moreover, it demonstrates corneal endothelial decompensation due to the lens capsule adhering to the corneal endothelium. Timely intervention is required to remove the dislocated lens and prevent complications.
Traumatic Lens Dislocation in an Eye with Anterior Megalophthalmos

Abstract:Anterior megalophthalmos is a rare, bilateral, nonprogressive, hereditary, congenital disorder characterized by the enlargement of all anterior segment structures of the eye, with megalocornea, iris atrophy, and zonular abnormalities. We report a case of an 8-year-old male who presented to the emergency department with a history of visual loss after a blunt ocular trauma to the left eye. The patient presented with markedly enlarged corneas and deepened anterior chambers bilaterally. Best-corrected visual acuity (BCVA) was hand motion in the left eye. An additional examination revealed multiple anterior segment abnormalities, leading to the diagnosis of megalophthalmos and lens dislocation in the anterior chamber. The patient underwent a lensectomy and anterior vitrectomy in the left eye. At six months postoperatively, the BCVA was 20/200 in the left eye. Lens dislocation in patients with megalocornea is rare. Cataract surgery in these patients requires attention to the zonular abnormalities and lens enlargement, resulting in increased rates of intraoperative and postoperative complications. Ophthalmologists should be able to diagnose this rare disorder and manage the associations and complications.

Abstract:A 27-year-old woman had foggy vision and photophobia since 10 months after implantation of implantable collamer lens (ICL STAAR Surgical AG, Nidau, Switzerland) with evidence of corneal decompensation and no cataract formation. Descemet membrane endothelial keratoplasty in phakic eyes is challenging, considering presence of posterior chamber phakic intraocular lens (IOL), decreasing the space available in anterior chamber to manoeuvre the graft. Need of inferior peripheral iridotomy in presence of central hole technology in ICL depends on the dynamics of full chamber air bubble. At 8 months, vision had improved to 20/20 and normal IOP with well-attached graft, stable phakic IOL and clear lens.

Abstract:Aims/Purpose: A full‐thickness corneal laceration can occur following blunt trauma and is an ocular emergency that requires decisive immediate, short and long‐term management. We describe an unusual case of blunt trauma leading to dehiscence of an old cataract wound with traumatic expulsive Iridodialysis associated with vitreous prolapse to create awareness among ophthalmologists on key diagnostic features and patient specific management for this condition. Methods: An 84 year old female presented to the eye emergency department following accidental trauma to her right eye. Ocular examination revealed a 3 mm corneal laceration that was just away from the limbus which was compatible with the incision used for prior cataract surgery over 10 years prior. There was signs of expulsive Iridodialysis as well as vitreous prolapse from this wound with associated ruptured posterior capsule, inferior IOL subluxation and pupil block angle closure. Results: Immediate management included excision of prolapsed vitreous and iris from the corneal laceration, placement of bandage contact lens and an eyeshield, as well as topical and systemic antibiotic therapy, topical steroids and IOP control. Within 3 days, the corneal laceration remained sealed following removal of the bandage contact lens and surgery was performed which included pars plana vitrectomy, IOL repositioning and corneal suturing. Conclusions: We report this case to emphasize the importance of careful anterior segment examination in cases of suspected globe rupture to identify the site of the opening and to always consider old surgical wounds. Once confirmed, no pressure should be placed on the eye to reduce the risk of further extravasation of intraocular contents.

Abstract:Background Femtosecond laser-assisted cataract surgery (FLACS) has grown in popularity among ophthalmologists as a novel technique. However, descemet membrane detachment (DMD) began to be found as the complication after FLACS. We report a case of serious DMD following FLACS due to the inappropriate incision design. Case presentation An 85-year-old man with apparent cornea arcus senilis underwent femtosecond laser-assisted cataract surgery in his right eye. A biplanar model was chosen for the main incision. A serious descemet membrane detachment (DMD) occurred at the end of phacoemulsification, which was connected with the main incision. However, the surgeon confused it with the transient swelling of corneal endothelium, and did not treated DMD timely. DMD was confirmed by anterior segment optical coherence tomography (AS-OCT) at the postoperative 1-month follow-up. Eventually DMD was resolved by intracameral perfluropropane (C3F8) gas injection. Conclusions This case suggests that a careful incision separation and a triplanar incision design in FLACS may reduce the incidence of DMD in cataract surgery.
Dislocated Sömmering Ring with Decompensated Cornea Presenting 14 Years After Lens Aspiration and Anterior Vitrectomy: A Case Report and Literature Review

Abstract:BACKGROUND A Sömmering ring is a post-cataract opacity (PCO) formed due to the adhesion of anterior capsule edges to the posterior capsule. It is mostly associated with aphakic cases but has also been reported in pseudophakic eyes. It becomes clinically important when dislocation occurs. The purpose of this manuscript was to report a case of anterior dislocation of the Sömmering ring 14 years after lensectomy for congenital cataract. CASE REPORT We describe a 20-year-old man who presented to the emergency room with pain, photophobia, redness, and blurry vision in the right eye. The patient was known to have bilateral amblyopia and had undergone bilateral lens aspiration and anterior vitrectomy without intraocular lens (IOL) implantation 14 years prior to presentation. On examination, the intraocular pressure was normal, the conjunctiva had moderate diffuse injection, and the cornea had 2+ diffuse edema with bullae and Descemet folds. The anterior chamber was deep with semilunar-shaped solid white material consistent with the Sömmering ring in the anterior chamber. On dilated fundus examination, the view was hazy, and B-scan ultrasonography indicated highly reflective material in the vitreous cavity and a flat retina. CONCLUSIONS Sömmering ring in the anterior chamber can be inflammatory and especially damaging to the corneal endothelium. If this complication occurs, timely intervention in the form of surgical removal is crucial to prevent endothelial cell loss and corneal decompensation. A thorough cleaning of the cortical matter after lens removal during primary surgery can reduce the risk of secondary formation of PCO. Implantation of IOL in myopic eyes can be considered to reduce the risk of Sömmering ring formation in the future and its complications.
First report of traumatic globe rupture after femtosecond laser-assisted penetrating keratoplasty

Abstract:Purpose: To describe the first reported case of traumatic globe rupture after femtosecond laser-assisted penetrating keratoplasty. Methods: A 21-year-old Hispanic man suffered a corneal laceration after blunt trauma leading to a visually significant corneal scar and aphakia. Results: The patient underwent femtosecond laser-assisted penetrating keratoplasty using a zigzag-shaped wound with secondary intraocular lens placement. Eighteen months after surgery, he suffered another blunt trauma resulting in inferior dehiscence of his inferior penetrating keratoplasty wound, loss of uveal tissue, and extrusion of the scleral-fixated posterior chamber intraocular lens. The patient underwent repair of the wound dehiscence. Conclusions: Despite the theoretical advantages conferred by a shaped graft-host junction, a shaped penetrating keratoplasty wound created with femtosecond laser can still be subject to dehiscence from trauma.
Recognizing and Managing Bullous Descemet Detachment Secondary to Accidental Hydroseparation During Phacoemulsification/Cataract Surgery by Relaxing Descemetotomy

Abstract:Purpose: To report a new technique called relaxing Descemetotomy for treatment of bullous Descemet detachment (BDD) secondary to accidental hydroseparation of Descemet membrane (DM) during stromal hydration in cataract surgery. Methods: A clear corneal keratome entry was created close to the limbus extending inward to create a relaxing cut (ab externo relaxing Descemetotomy) on taut DM, thus creating an egress route for supra-Descemetic fluid (SDF). This was followed by pneumodescemetopexy to drain SDF internally. Results: Three patients with a history of unsuccessful pneumodescemetopexy and with planar or mildly convex separation of DM without break on anterior segment optical coherence tomography (ASOCT) underwent this procedure. All had successful reapposition of DM clinically and on ASOCT. All showed resolution of stromal edema and improved uncorrected and best-corrected visual acuity postoperatively. Conclusions: Rarely stromal hydration performed with an irrigating cannula positioned too close to the posterior stroma can result in hydroseparation of DM creating BDD, seen intraoperatively as a fluid wave propagating across the posterior aspect of the cornea. Large folds, free mobility, and DM tear classically seen with rhegmatogenous Descemet detachment are not seen clinically or on ASOCT in BDD. Pneumodescemetopexy alone cannot resolve BDD because without a DM tear, SDF cannot evacuate. Combining relaxing Descemetotomy with pneumodescemetopexy allows SDF to drain internally and Descemet detachment to resolve. This technique has numerous advantages over classical venting incisions in terms of ease, preferable limbal location, larger incision size, absence of complications such as visual axis scars, irregular astigmatism, epithelial ingrowth, infectious keratitis, etc.

Abstract:Sodium hyaluronate 1% was first introduced to ophthalmic surgery in 1979 from Drs. Stegmann and Miller during cataract surgery [1]. Ophthalmic viscoelastic devices (OVDs) are widely used in anterior segment surgery, among others, in order to maintain anterior chamber depth intraoperatively and to protect the corneal endothelium [2]. Since the first use of sodium hyaluronate 1%, other viscoelastic agents have been developed and introduced into the operating theater, such as combinations of natrium hyaluronate with chondroitin sulfate and natrium hyaluronate with lidocaine hydrochloride [3], [4]. Intraoperative anterior segment ocular coherence tomography (i-AS-OCT) was first introduced in 2005 and offers the ability to assess the structures of anterior segment in real-time during surgery [5]. Of great importance is the noninvasive nature of OCT as well as the easy use of the device, which is often integrated in the surgical microscope [6]. We present here a case of iatrogenic Descemet membrane detachment caused by an accidental injection of an OVD into the cornea during complicated cataract surgery. DSAEK for implantable collamer lens dislocation and corneal decompensation 6 years after implantation

Abstract:A 39-year-old woman with a history of high myopia underwent uneventful implantation of phakic posterior chamber implantable collamer lenses (ICLs) 6 years earlier in both eyes with a visual acuity of 20/20 bilaterally. The patient presented as an emergency with sudden decreased vision in her right eye after blunt trauma. Slit-lamp examination showed a partially dislocated ICL in the anterior chamber, associated with ocular hypertension. It was decided to treat her with topical ocular hypotensive agents and surgical repositioning of the ICL. There was a progressive loss of endothelial cells and decreased visual acuity. Descemet stripping automated endothelial keratoplasty was needed to correct the endothelial failure. This case presents a potential complication of the ICL several years after implantation, and should be considered in these types of procedures.

Abstract:Capsulotomy with neodymium-doped yttrium--aluminum-garnet (Nd:YAG) laser is an effective treatment for posterior capsule opacification following cataract surgery. A wide opening of the posterior capsule associated with the ruptured anterior hyaloid can cause anterior chamber vitreous prolapse. Two patients who developed angle-closure glaucoma associated with vitreous prolapse following Nd:YAG laser posterior capsulotomy were successfully treated with antiglaucoma medication and peripheral iridotomies. Patient identification for potential risk factors and a careful postoperative follow-up are essential to avoid these serious complications.

Post-traumatic posterior giant retinal tear and macular hole associated retinal detachment

Abstract:Background: Blunt trauma to the eye can present with varied manifestations involving both the anterior and posterior segments of the eye. Giant retinal tear (GRT) following trauma occurs most commonly at the equatorial region or anterior to the equator. GRT posterior to the equator is rare. Purpose: To demonstrate the successful management of a post-traumatic posterior GRT and full-thickness macular hole (MH) associated retinal detachment (RD). Synposis: A 21-year-old-male presented with sudden diminution of vision in the right eye (RE) following blunt-trauma with cricket ball. RE vision at presentation was hand movement close to face. Anterior segment of RE revealed pupillary sphincter tear, posterior synechiae and posterior subcapsular cataract (PSC). RE fundus revealed a posterior-GRT, full thickness MH, mild vitreous haemorrhage and rhegmatogenous RD. He was managed with pars plana vitrectomy, encircling scleral band, perfluorocarbon liquid-assisted flattening of GRT, internal limiting membrane peeling, and endotamponade. Post-operatively the retina was attached, MH was closed and the patient achieved an ambulatory vision of 1/60. Highlights: This video demonstrates the successful management of a posterior-GRT and MH associated RD. Removal of adherent hyaloid from the long anterior flap of posterior GRT, peeling of ILM from temporal narrow mobile strip of retina (which has a risk of radial extension of GRT edges) and manoeuvring in suboptimally dilated pupil are illustrated in this video. Management of TraumaticCrystalline Lens Subluxation and Dislocation

Abstract:Many systemic or localized ocular conditions can lead to the weakening of the zonules that support the intraocular lens, subsequently leading to lens subluxation or complete dislocation into the vitreous cavity. Some of these conditions include pseudoexfoliation syndrome, Marfan syndrome, Weill-Marchesani syndrome, intraocular neoplasia, aniridia, trauma, microspherophakia, and iatrogenic causes such as prior vitrectomy or filtration procedures. Traumatic zonular dialysis is a major cause of lens subluxation and dislocation. Blunt trauma may cause transient shortening of the eye in the anterioposterior direction causing equatorial expansion, which can stretch or break the zonules. In a similar manner, perforating and penetrating injury of the eye can damage the zonules directly or indirectly. Patients may develop traumatic cataract or be symptomatic purely from the subluxed lens. Symptoms of a subluxed or dislocated lens include hyperopic or myopic shift, severe astigmatism, monocular diplopia, glare, and functional aphakia. The routine phacoemulsification techniques used today for cataract surgery may not be suitable in cases with weak zonular support. Here we review the current management strategies for traumatic crystalline lens subluxation and dislocation.

Abstract:Epithelial ingrowth is a rare but potentially sight-threatening complication caused by the invasion of corneal or conjunctival epithelial cells into the eye during ocular surgeries. DMEK is emerging as a widely used surgery for endothelial keratoplasty with its improved safety profile. We describe a case of epithelial ingrowth in the graft-host interface after uneventful DMEK associated with vitreous prolapse in the anterior chamber.

PurposeTo report a challenging Descemet Membrane Endothelial Keratoplasty (DMEK) case, complicated by intraoperative aqueous misdirection and spontaneous anterior chamber fibrin reaction.MethodsA 70-year-old female affected by corneal edema due to Fuchs endothelial dystrophy underwent a triple procedure (cataract extraction - IOL implantation - DMEK surgery) in her left eye. This report illustrates the management of the intraoperative complications of aqueous misdirection syndrome and anterior chamber fibrin reaction.ResultsDespite the optimal management of the posterior pressure and the thorough removal of the fibrinous reaction during the case, the DMEK graft was not completely unfolded and centred at the end of the surgical procedure. Nonetheless, the patient showed good long-term anatomical and functional recovery: at the last follow-up (2 years after surgery), central corneal thickness was 526 μm with a best corrected visual acuity of 20/25 and an endothelial cell density of 1112 cell/mm2.ConclusionEarly recognition and prompt management of intraoperative aqueous misdirection syndrome and anterior chamber fibrin reaction during DMEK surgery is essential to ensure good functional and anatomical outcomes.

Abstract:Objectives: To describe the clinical and Scheimpflug imaging features in cases of anterior megalophthalmos (AM). Methods: Retrospective record review was performed for patients with AM who presented between June 2017 and May 2018. Clinical history, slit lamp examination, Scheimpflug imaging indices (Pentacam-HR, Oculus, GmbH), dilated fundus examination and treatment records were reviewed. Results: The study included eight eyes of four male patients (mean age 6.5 years, range 4-10 years). Corrected distance visual acuity ranged from finger counting to 20/80. The mean horizontal corneal diameter, central corneal thickness, steep keratometry, flat keratometry, anterior chamber (AC) angle, AC depth, maximum pupil diameter and axial length were 13.8 ± 0.5 mm, 538.7 ± 68.9 µm, 42.8 ± 1.6 D, 41.4 ± 0.9D, 47.0 ± 4.2 degree, 3.8 ± 0.3 mm, 3.9 ± 0.1 mm, and 24.9 ± 0.9 mm, respectively. Posterior dislocation of crystalline lens, vitreous degeneration and rhegmatogenous retinal detachment (consequent of retinal dialysis/atrophic hole/lattice with hole) were noted in seven, eight and five eyes, respectively. Pigment dispersion glaucoma was noted in both eyes of one patient, which was managed with topical anti-glaucoma medication. Vitrectomy with silicone oil tamponade was successful in retinal reattachment in all three eyes that underwent surgery. Conclusion: Scheimpflug imaging helps in demonstrating the corneal and anterior segment pathological changes in AM. The disease extends to involve the zonules, vitreous and retina as well. Ophthalmologists should be able to identify this disorder, recognise and manage the associations and complications.
