Pediatric Intracranial Ependymoma—a Single-Center Clinical Analysis and Literature Review

Abstract:Background: Pediatric intracranial ependymoma (EPN) is an important central nervous system neoplasm, and has a high recurrence rate. The data on Chinese children with EPN are limited. This study sought to explore the characteristics of pediatric EPN at a single center in China. Methods: The clinical characteristics and outcomes of children with EPN, who had been admitted to the Children's Hospital of Soochow University from October 2011 to February 2021, were retrospectively analyzed. Results: In total, 33 children were enrolled in this study (male to female ratio: 2:1). The patients had an average age of 3 years and 9 months, and 42.4% of patients were younger than 3 years. Only 1/3 of the patients presented with neurological symptoms. In relation to location,6cases were supratentorial, and 27 cases were infratentorial. In terms of histologic grading, 8 cases were Grade II, 20 cases were Grade III, and 5 cases were Grade II or III. Follow-up data were available for 28 patients; 16 patients had tumor recurrence. As of April 1th, 2021, 12 patients had no recurrence at the time of the last follow-update. Six patients had a follow-up time >36 months, and 3 patients had a follow-up time >5 years. Of the 18 patients who underwent complete resection, 8 had tumor recurrence. After recurrence, 8 patients underwent reoperation, and 4 patients received adjuvant radiotherapy and chemotherapy. Conclusions: We described the clinical features of Chinese children with EPN based on our institutional experience. Most cases of recurrence occurred within 24 months of surgery. Adjuvant radiotherapy and chemotherapy might facilitate disease control after resection at recurrence.
