Clinical diagnosis and surgical management of intracranial arachnoid cysts

Abstract:Objective: To study the mechanism, diagnostic method and therapeutic principle of intracranial arachnoid cysts (IAC). Methods: Analysis of clinical manifestation of 21 patients with IAC confirmed by CT, MRI and pathology, their features of image diagnosis and results of their surgical treatment and followup studies. Results: 1-5 year followup studies showed that the symptoms were improved in 17 patients (17/21) and IAC decreased in size (19/21) to some extent after operation . Conclusion: The morbidity of IAC in infants and children was fairly high and accounted for 52.3% of the total cases. The IAC lesion was located most commonly in the sylvian fissure of middle cranial fossa and occipital lobe. CT and MRI were the most reliable method of diagnosis. If the patient with IAC develops apparent symptoms, especially raised intracranial pressure, surgical intervention is indicated. Operation performed under the microscope is preferred. Recurrence of IAC was often associated with no effective communication between IAC and subarachnoid, and no prompt drainage of blood cerebralspinal fluid after operation. 
