Abstract:In diffuse optical tomography (DOT) and spectroscopy (DOS) using handheld probes, tissue curvature can cause bad fiber-to-tissue contact. Understanding and minimizing image artifacts caused by these coupling errors would significantly improve DOT and DOS image quality. In this work, we utilized Monte Carlo simulations and experiments with gelatin-Intralipid phantoms to systematically study the influence of source or detector (optode) coupling errors. Optode coupling errors can increase the amplitude and decrease the phase of the measured diffuse reflectance, creating artifacts in the reconstructed absorption maps, such as hot spots on the edges. We propose an outlier removal algorithm that can correct these image artifacts, and we demonstrate its performance using simulations, phantom experiments, and breast patient data acquired with bad probe contact due to a dense or small breast. Further, we designed and implemented a new resistance-type thin-film force sensor array that provides real-time optode coupling feedback and guides the outlier removal to minimize optode coupling errors. Our approaches and study results have significant implications for reducing image artifacts arising from handheld probes, which are commonly used with mobile and wearable DOT and DOS devices. SU‐FF‐I‐39: A Computer Simulation Platform for the Optimization of Breast Tomosynthesis System

Abstract:Purpose: In breast tomosynthesis there are tradeoffs between optimization of resolution, noise and acquisition speed with a given glandular dose. Our purpose is to provide a simulation platform to investigate any plausible configuration for a tomosynthesis system, and find an optimal combination of these parameters. Method and Materials: Simulated projections of a slanted thin tungsten wire placed in different object planes are modified according to a detector'sMTF (binned or not binned). In addition, the focal spot blur (FSB), which depends on the location of the wire, the system geometry, the source‐detector movement speed and exposure time, is also incorporated into the projections. Then a maximum likelihood (ML) algorithm is used for 3D reconstruction. The in‐plane and in‐depth MTF were determined. To evaluate the noise performance, simulated noiseless projections of calcification and tumor in uniform breast tissue were modified with real detector's NPS and for a given detector noise and dose. The signal to noise ratio (SNR) of the reconstructed images is calculated with different configurations, e.g. view number and angle, pixel binning and FSB. Results: The FSB is determined by the exposure time of each view at a fixed gantry travel speed. For SID = 65cm and exposure time ≤ 80ms per view, the 2×2 binning causes more degradation for the in‐plane resolution than the FSB and reconstruction blur. The in‐depth resolution can be improved by increasing the number of views and the span of view angle. However the SNR will be degraded because with the lower exposure per view (total dose constant), the detector performance is degraded by electronic noise. Conclusion: Many design parameters need to be considered for optimizing a tomosynthesis system. We provide a simulation platform to predict system performance with different configurations in advance. It will be helpful for practical implementation of breast tomosynthesis.
Reducing image artifact in diffuse optical tomography by iterative perturbation correction based on multiwavelength measurements

Abstract:Ultrasound (US) guided diffuse optical tomography has demonstrated great potential for breast cancer diagnosis, treatment monitoring, and chemotherapy response prediction. Optical measurements of four different wavelengths are used to reconstruct unknown optical absorption maps, which are then used to calculate the hemoglobin concentration distribution of the US visible lesion. Reconstructed absorption maps are prone to image artifacts from outliers in measurement data from tissue heterogeneity, bad coupling between tissue and light guides, and motion by patient or operator. We propose an automated iterative perturbation correction algorithm to reduce image artifacts based on the structural similarity index (SSIM) of absorption maps of four optical wavelengths. The initial image is estimated from the truncated pseudoinverse solution. The SSIM was calculated for each wavelength to assess its similarity with other wavelengths. An absorption map is repeatedly reconstructed and projected back into measurement space to quantify projection error. Outlier measurements with highest projection errors are iteratively removed until all wavelength images are structurally similar with SSIM values greater than a threshold. Clinical data demonstrate statistically significant improvement in image artifact reduction.
Optimal Probing of Optical Contrast of Breast Lesions of Different Size Located at Different Depths by US Localization.

Abstract:We report a frequency domain optical tomography system utilizing three RF modulation frequencies, which are optimized for probing breast lesions of different size located at different depths. A real-time co-registered ultrasound scanner is used to provide on-site estimation of lesion size and location. Based on the lesion information, an optimal light modulation frequency can be selected, which may yield more accurate estimates of lesion angiogenesis and hypoxia. Phantom experiments have demonstrated that a high modulation frequency, such as 350Mhz, is preferable for probing small lesions closer to the surface while a low modulation frequency, such as 50Mhz, is desirable for imaging deeper and larger lesions. A clinical example of a large invasive carcinoma is presented to demonstrate the application of this novel technique.
Benign versus malignant breast masses: optical differentiation with US-guided optical imaging reconstruction

Abstract:Purpose: To investigate prospectively the feasibility of using optical tomography with ultrasonographic (US) localization to differentiate malignant from benign breast masses and to compare optical tomography with color Doppler US. Materials and methods: The study was approved by the local internal review board committee and by the Human Subjects Research Review Board of Army Medical Research and Materiel Command. Signed informed consent was obtained, and the study was HIPAA compliant. Between May 2003 and March 2004, 65 consecutive women (mean age, 51 years; age range, 24-80 years) with 81 breast lesions underwent US-guided biopsy and were scanned with a combined imager. The hand-held probe, which consisted of a centrally located US transducer surrounded by near-infrared sensors, was used to simultaneously acquire coregistered US images and optical data. The lesion location obtained at US was used to guide optical imaging reconstruction. Light absorption was measured at two wavelengths. From these measurements, tumor angiogenesis was assessed on the basis of calculated total hemoglobin concentration. A Student t distribution was used to calculate the statistical significance of mean maximum and mean average hemoglobin concentrations obtained in malignant and benign lesion groups, and P < .001 was considered to indicate a statistically significant difference. Results: Biopsy results revealed eight early stage invasive carcinomas (malignant group) and 73 benign lesions (benign group). The mean maximum and mean average hemoglobin concentrations in the malignant group were 122 micromol/L +/- 26.8 (+/- standard deviation) and 88 micromol/L +/- 24.5, respectively. The mean maximum and mean average hemoglobin concentrations in the benign group were 55 micromol/L +/- 24.8 and 38 micromol/L +/- 17.4, respectively. Both the maximum and average total hemoglobin concentrations were significantly higher in the malignant group compared with the benign group (P < .001). When a maximum hemoglobin concentration of 95 micromol/L was used as the threshold value, the sensitivity, specificity, positive predictive value, and negative predictive value of optical tomography were 100%, 96%, 73%, and 100%, respectively, and the sensitivity, specificity, positive predictive value, and negative predictive value of color Doppler US were 63%, 69%, 19%, and 94%, respectively. Conclusion: Findings indicate that optical tomography with US localization is feasible for differentiating benign and early stage malignant breast lesions.

Abstract:Ultrasound (US)-guided diffuse optical tomography (DOT) has demonstrated potential value for screening and treatment monitoring of breast cancers. However, in clinical cases, the chest wall, bad probe-tissue contact, and tissue heterogeneity can create image artifacts, causing misinterpretation of lesion images. In the current work, realistic and flexible threedimensional numerical breast phantoms were generated using the Virtual Imaging Clinical Trials for Regulatory Evaluation (VICTRE) tools developed by U.S. Food and Drug Administration (FDA). By selecting physical attributes and tissue optical properties, the VICTRE breast phantoms were, for the first time, adopted in DOT for in silico studies. Monte Carlo simulations were conducted to generate the forward data. Edge artifacts (hot spots on the edge of the regions of interests) were found on the reconstructed images when there was a mismatch between the lesion-side breast and the contralateral reference-side breast. We propose a fully automated, connected components analysis-based algorithm that can remove these edge artifacts and improve lesion reconstruction.

Abstract:Diffuse optical breast imaging utilizes near-infrared (NIR) light propagation through tissues to assess the optical properties of tissues for the identification of abnormal tissue. This optical imaging approach is sensitive, cost-effective, and does not involve any ionizing radiation. However, the image reconstruction of diffuse optical tomography (DOT) is a nonlinear inverse problem and suffers from severe illposedness due to data noise, NIR light scattering, and measurement incompleteness. An image reconstruction method is proposed for the detection of breast cancer. This method splits the image reconstruction problem into the localization of abnormal tissues and quantification of absorption variations. The localization of abnormal tissues is performed based on a well-posed optimization model, which can be solved via a differential evolution optimization method to achieve a stable reconstruction. The quantification of abnormal absorption is then determined in localized regions of relatively small extents, in which a potential tumor might be. Consequently, the number of unknown absorption variables can be greatly reduced to overcome the underdetermined nature of DOT. Numerical simulation experiments are performed to verify merits of the proposed method, and the results show that the image reconstruction method is stable and accurate for the identification of abnormal tissues, and robust against the measurement noise of data.

Abstract:This study proposed diffuser-aided diffuse optical imaging (DADOI) as a new approach to improve the performance of the conventional diffuse optical tomography (DOT) approach for breast imaging. The 3-D breast model for Monte Carlo simulation is remodeled from clinical MRI image. The modified Beer-Lambert's law is adopted with the DADOI approach to substitute the complex algorithms of inverse problem for mapping of spatial distribution, and the depth information is obtained based on the time-of-flight estimation. The simulation results demonstrate that the time-resolved Monte Carlo method can be capable of performing source-detector separations analysis. The dynamics of photon migration with various source-detector separations are analyzed for the characterization of breast tissue and estimation of optode arrangement. The source-detector separations should be less than 4 cm for breast imaging in DOT system. Meanwhile, the feasibility of DADOI was manifested in this study. In the results, DADOI approach can provide better imaging contrast and faster imaging than conventional DOT measurement. The DADOI approach possesses great potential to detect the breast tumor in early stage and chemotherapy monitoring that implies a good feasibility for clinical application.

Abstract:Shallow lesions less than 1.5-cm deep are frequently seen in breast patients when they are scanned in reflection geometry. Two boundary conditions are compared for imaging shallow lesions, and a new probe design is introduced. A partial reflection boundary condition is suitable for imaging shallow lesions less than 1.0-cm deep; whereas an absorption boundary condition is desirable for imaging lesions more than 1.5-cm deep. Our new probe design incorporates either a partial reflection boundary or an absorption boundary based on a priori knowledge of lesion depth provided by coregistered real-time ultrasound images. An angled source is introduced to further improve the illumination of the region between 1.0- to 1.5-cm depths. Simulation, phantom, and freshly excised mouse tumor experiments demonstrate that targets located at different depths can be uniformly reconstructed. A clinical example is given to demonstrate the utility of this new approach for optimally probing lesions located at different depths.
A Study On Optical Modulation Signal And Tissue Displacement In Ultrasound Modulated Optical Tomography

Abstract:Ultrasound modulated optical tomography (UOT) is a hybrid technique which combines optical contrast with ultrasound resolution and has shown some potential for early cancer detection, functional and molecular imaging. However, one current problem with this technique is the weak optical modulation signal strength and consequently low Signal-to-Noise Ratio (SNR). In this study, the effect of increasing the amplitude of the ultrasound induced particle displacement on the UOT signal is investigated using a Monte Carlo simulation tool. The simulation software was validated against those reported in the literature and good agreement was achieved. The simulated amplitude of particle displacement was varied from 0.1 to 500nm. The results showed a significant increase in UOT signal with particle displacement for low displacements, followed by saturation when displacement increased beyond a certain level. Further simulations were performed to investigate the saturation by changing the optical wavelength from 400nm to 600nm. The results show that the UOT signal saturates at lower particle displacements for smaller wavelengths. This suggests that the phase variations along a photon path can be large enough to cause cancellation as particle displacement increases. This study is part of ongoing efforts to improve the SNR of UOT through using the large particle displacements created by high amplitude ultrasound and radiation forces.

Abstract:By mapping the mechanical properties of tissue, elastography can improve identification of breast cancer. On the macro-scale, ultrasound elastography and magnetic resonance elastography have emerged as effective clinical methods for the diagnosis of tumors. On the micro-scale, optical coherence elastography (OCE) shows promise for intraoperative tumor margin assessment during breast-conserving surgery. Whilst several OCE studies have demonstrated strong potential, the mechanical models used require the assumption of uniaxial stress throughout the sample. However, breast tissue is heterogeneous and contains compressible features (e.g., ducts and blood vessels) and collagen-rich fibrotic features (e.g., stroma). This heterogeneity can invalidate the assumption of uniaxial stress and reduce the accuracy of OCE, often making it challenging to interpret images. Here, we demonstrate a new variant of OCE based on mapping the Euler angle, i.e., the angle between the principal compression and the loading axis induced by tissue heterogeneity, which removes the assumption of uniaxial deformation. This is enabled by a hybrid three-dimensional (3-D) displacement estimation method that combines phase-sensitive detection and complex cross-correlation, providing access to the 3-D displacement and 3-D strain tensor on the micro-scale. We demonstrate this new OCE technique through experiments on phantoms and 10 fresh human breast specimens. Through close correspondence with histology, our results show that mapping the Euler angle provides additional contrast to both optical coherence tomography and a current OCE technique in identifying cancer. Mapping the Euler angle in breast tissue may provide a new biomarker for intraoperative tumor margin assessment.

Abstract:Breast cancer has become one of the most common and lethal cancers in women worldwide. Ultrasound tomography (UST) is supposed to be an imaging method for early diagnosis of breast cancer. In the implementation of UST, operators usually need to compromise between the image quality and the complexity of scanning. In order to explore the relationship between them, a preliminary UST imaging system is designed and introduced in this paper. It uses two co-planar 128-element piezoelectric ultrasonic transducers as transmitter and receiver respectively, each of which can rotate freely in the horizontal plane. Based on this prototype, a series of experiments with different scanning patterns are designed, the corresponding signal completeness are calculated, and the imaging results are compared.

Abstract:Breast cancer is a vital global health concern, necessitating the development of advanced imaging methodologies for early and accurate detection. Although several standard approaches are available, due to their low sensitivity, inability to recognize cancer in denser breasts, radiation exposure, and high cost, they paved way for future research. This paper introduces a novel approach by utilising the dual modality for breast cancer detection by integrating the Electrical Impedance Tomography (EIT) and Ultrasound Tomography (UT) methods. A simulation-based study using Finite Element Modelling (FEM) is used to solve the forward problem of both modalities using COMSOL Multiphysics software. Here, 16 Ag/Agcl electrodes and 16 ultrasonic piezoelectric transducers were positioned alternately in a 2D circular breast model. Patient-specific FEM meshes enhance the accuracy of EIT and ultrasound imaging. The fusion of these modalities offers a promising avenue for early and precise breast cancer detection, with the potential to transform diagnostics and improve patient health care.

Abstract:Ultrasound (US)-guided diffuse optical tomography (DOT) has demonstrated potential value for breast cancer diagnosis and treatment response assessment. However, in clinical use, the chest wall, poor probe-tissue contact, and tissue heterogeneity can all cause image artifacts. These image artifacts, appearing commonly as hot spots in the non-lesion regions (edge artifacts), can decrease the reconstruction accuracy and cause misinterpretation of lesion images. Here we introduce an iterative, connected component analysis-based image artifact reduction algorithm. A convolutional neural network (CNN) is used to segment co-registered US images to extract the lesion location and size to guide the artifact reduction. We demonstrate its performance using Monte Carlo simulations on VICTRE digital breast phantoms and breast patient images. In simulated tissue mismatch models, this algorithm successfully reduces edge artifacts without significantly changing the reconstructed target absorption coefficients. With clinical data it improves the optical contrast between malignant and benign groups, from 1.55 without artifact reduction to 1.91 with artifact reduction. Abstract:BACKGROUND: Ultrasound computed tomography (USCT) is a promising technique for improving the detection of breast cancer. Image quality of USCT has a major impact on the breast cancer diagnosis. OBJECTIVE: This paper investigates the combination of variational mode decomposition (VMD) and coherent factor method for USCT image quality enhancement. METHODS: The signals can be decomposed into multiple intrinsic mode functions (IMFs) sifting through the frequency by VMD method. Refactoring the remaining IMFs, spatio-temporally smoothed coherence factor (STSCF) beamforming method is applied to reconstructed data for USCT. RESULTS: The validation of combination the VMD and STSCF is described through the breast phantom experiment and in vivo experiments. The evaluation indicators such as contrast ratio (CR), contrast to noise ratio (CNR) and signal to noise ratio (SNR) have been better improved in the experimental results. For the breast phantom, the proposed method gives a higher resolution and the better contrast properties for the hyperechoic cyst. The borders of cysts and tumors in the breast phantom can be distinguished clearly. For volunteer breast experiments, artifacts are removed more efficiently while the clutters are suppressed simultaneously. CONCLUSION: The combination of VMD and STSCF can further reduce the noise and suppress the side lobes.

Probing physiology and molecular function using optical imaging: applications to breast cancer

Abstract:The present review addresses the capacity of optical imaging to resolve functional and molecular characteristics of breast cancer. We focus on recent developments in optical imaging that allow three-dimensional reconstruction of optical signatures in the human breast using diffuse optical tomography (DOT). These technologic advances allow the noninvasive, in vivo imaging and quantification of oxygenated and deoxygenated hemoglobin and of contrast agents that target the physiologic and molecular functions of tumors. Hence, malignancy differentiation can be based on a novel set of functional features that are complementary to current radiologic imaging methods. These features could enhance diagnostic accuracy, lower the current state-of-the-art detection limits, and play a vital role in therapeutic strategy and monitoring.
Diffuse Optical Measurement of Blood Flow in Breast Tumors

Abstract:Blood flow contrast between tumor and normal tissues in patients with malignant and benign breast cancer was measured by diffuse optical correlation methods. The measurements were carried out with a hand-held optical probe that was manually scanned over the tumor-bearing breast. Increased blood flow was observed in tumor regions relative to healthy tissue, and control subjects did not exhibit significant blood flow heterogeneity. The measurements introduce a new optical contrast for diffuse optical mammography.
