Abstract:Stanford Type A aortic dissection is a life-threatening disease. A 46-year-old female patient with Stanford Type A aortic dissection was successfully treated by placing a stent-graft into the ascending aorta via femoral artery. No complication was found immediately after the operation. Bentall operation was performed to treat the development of severe aortic insufficiency 21 months after the stent-grafting procedure. Literature review was done to discuss the possibility of using endoluminal stent placement to treat Stanford Type A aortic dissection.
Virtual Implantation of Stent-graft by Finite Element Simulation and Its Applications in Endovascular Treatment Planning for B Type Aortic Dissection

Abstract:Thoracic endovascular aortic repair has been widely applied to treat Stanford Type B aortic dissection. However, retrograde type A dissection can occur as a complication after thoracic endovascular repair for Stanford type B aortic dissection. In order to investigated the possible mechanical reasons of the new entry occurring when stent grafts were implanted into the true lumen of one type B aortic dissection, a framework of virtual implantation of stent-graft by using finite element simulations was developed in this paper. The animal experiments were adopted to verify the finite element simulation of stent-graft implantation. Moreover, the manufactured stent-grafts were implanted virtually into the true lumen of the model of patient-specific aortic dissection with difference configurations such as oversizing ratio, anchoring position, stent struct. The following conclusions are summarized: (a) the radial force plays a more dominant role than the elastic recoil support in the deployed stent-graft. Excessive oversizing ratio may lead to the insufficient expansion of stent graft and retrograde aortic dissection; (b) the anchoring position of the stent-graft has a clear effect on the stress of the aorta, and the bare stent and thinner nickel-titanium alloy ring generate the higher stress at the aorta wall; (c) Comparing the three manufactured stent-grafts: Talent, Valiant and cTag, the later two have the better deployed geometries and better adherence to the artery wall.
Development of new endovascular stent-graft system for type B thoracic aortic dissection with finite element analysis and experimental verification

Abstract:Endovascular repair of the thoracic aorta with self-expanding stent-grafts has been emerging as a less invasive alternative treatment compared with conventional open surgeries. Despite the promising efficacy and safety of endovascular stent grafting, the stent-graft failure remains a major concern in terms of stent migration, device fatigue, and the risk of endoleaks. Challenges associated with the stent-grafts involve optimized geometrical structure, lifetime fatigue resistance, and adequate radial support. In this work, a novel endovascular stent-graft system is developed specially for the treatment of Stanford type B thoracic aortic dissections (TAD). Numerical study with finite element analysis (FEA) was utilized to evaluate the mechanical behaviors of the individual stent component. Results of the simulation were validated by experimental tests. Based on the systematic analysis of the parametric variations, a final stent-graft system was developed by the selection and arrangement of the individual stent components, targeting an optimal performance for treatment of TAD. The optimized solution of the stent-graft system was tested in clinical trials, showing advantageous therapeutic efficacy.
Numerical Modeling and Simulations of Type B Aortic Dissection Treated by Stent-Grafts with Different Oversizing Ratios

Abstract:Retrograde type A dissection after thoracic endovascular aortic repair has been a major drawback of endovascular treatment. This study investigated the biomechanical mechanism of stent-graft-induced new lesions after implantation and analyzed the relationship between radial force and spring-back force of the stent-graft when it was implanted virtually under different oversizing ratios. Based on the computed tomography angiography images, a three-dimensional geometric model of a patient-specific aortic dissection was established. The stent was designed in CAD software and the stent-graft implantation procedure under different oversizing ratios was simulated in the finite element analysis software. Implantation simulations were performed six times for each stent-graft model under 0%, 3%, 6%, 9%, 12%, and 15% oversizing ratios and the peak stress of the aorta was compared among groups. It was observed that the peak stress of the aorta was located where the proximal bare stent interacted with aortic wall and its value was increased by 62.2% from 0% to 15% oversizing ratio. The conclusions are reached that the long-term higher stress in the aortic wall may lead to the emergence of new lesions in these areas, and the radial force plays a key role in the formation of a new entry in the real aorta model.
Surgical treatment patterns and clinical outcomes of type B aortic dissection involving the aortic arch

Abstract:Objective: In the present report, we have described the outcomes of endovascular repair, hybrid arch repair, and open surgical repair for type B dissection involving the aortic arch (B1-2, D). Methods: Cases of endovascular repair, hybrid arch repair, and open surgical repair performed between January 2015 and December 2019 for aortic dissection designated as B1-2, D by the Society for Vascular Surgery/Society of Thoracic Surgeons classification were retrospectively analyzed. The primary end point was all-cause mortality at follow-up. The secondary end points included early mortality, early morbidities, and aortic-related late events. Kaplan-Meier curves were created to analyze survival from all-cause mortality and freedom from aortic-related late events in the endovascular, hybrid, and open groups. Propensity score matching and stratification (stratified by proximal dissection extension: B1, D and B2, D) were performed as sensitivity analyses to compare the outcomes among the three treatment patterns after controlling for major confounders. Results: The present study included 151 patients (men, 79.5%; mean age, 47.3 ± 10.5 years), with 72 (47.7%) in the endovascular group, 46 (30.5%) in the hybrid group, and 33 (21.8%) in the open group. No significant difference was noted in early mortality between the endovascular, hybrid, and open groups (1.4% vs 2.2% vs 3.0%; P = .791). The incidence of early endoleak was significantly greater (33.3% vs 13.0% vs 6.1%; P = .002) and the incidence of renal function deterioration was less (4.2% vs 26.1% vs 24.2%; P = .001) after endovascular repair vs hybrid arch repair and open surgery. After a median follow-up of 40.0 months (range, 0-84.0 months), no significant differences were found in all-cause mortality (5.6% vs 4.3% vs 3.0%; P = 1.0), aortic-related late events (16.7% vs 15.2% vs 12.1%; P = .834), or late endoleak (9.7% vs 4.3% vs 6.1%; P = .630) after endovascular, hybrid, and open surgery. The propensity score matching and stratification analyses displayed consistent outcomes for early mortality, all-cause mortality, and aortic-related late events among the three groups. Conclusions: The mid- to long-term outcomes after endovascular repair, hybrid arch repair, and open surgical repair for type B dissection involving the aortic arch (B1-2, D) were favorable and comparable in selected patients. Extensive experience and multidisciplinary teamwork are prerequisites for individualized strategies for repair of B1-2, D.

Abstract:Acute type A aortic dissection remains a deadly and elusive condition, with risk factors such as hypertension, bicuspid aortic valves, and genetic predispositions. As existing guidelines for surgical intervention based solely on aneurysm diameter face scrutiny, there is a growing need to consider other predictors and parameters, including wall stress, in assessing dissection risk. Through our research, we aim to elucidate the biomechanical underpinnings of aortic dissection and provide valuable insights into its prediction and prevention. We applied finite element analysis (FEA) to assess stress distribution on a rare dataset comprising computed tomography (CT) images obtained from eight patients at three stages of aortic dissection: pre-dissection (preD), post-dissection (postD), and post-repair (postR). Our findings reveal significant increases in both mean and peak aortic wall stresses during the transition from the preD state to the postD state, reflecting the mechanical impact of dissection. Surgical repair effectively restores aortic wall diameter to pre-dissection levels, documenting its effectiveness in mitigating further complications. Furthermore, we identified stress concentration regions within the aortic wall that closely correlated with observed dissection borders, offering insights into high-risk areas. This study demonstrates the importance of considering biomechanical factors when assessing aortic dissection risk. Despite some limitations, such as uniform wall thickness assumptions and the absence of dynamic blood flow considerations, our patient-specific FEA approach provides valuable mechanistic insights into aortic dissection. These findings hold promise for improving predictive models and informing clinical decisions to enhance patient care.

Abstract:Based on the research of geometry model,finite element model,material model,boundary conditions and solution control of coronary stent,coronary stent expansion process simulation analysis is made using finite element method,and experimentally validated to the analysis results.It shows that the mechanics model by finite element method can simulate and analyze the real expansion process of coronary stent well under the condition of ignoring the action between sacculus and blood vessel.The coronary stent expansion rate and dog bone rate made by this method reaches 1.31% and 3.66% respectively,which meets the analysis results very well.So this method provides technical basis for new construction coronary stent.

Abstract:Introduction: Treatment of residual aortic dissection following ascending arch repair remains clinically challenging. We propose a novel technique to stabilize the aortic arch and create a proximal seal zone by placement of a bare metal stent into the arch prior to thoracic endovascular aortic repair (TEVAR). Using finite element analysis and computational fluid dynamics, we assess if this technique achieves the desired technical result. Methods: We analyzed two patients at our institution, both of whom presented with residual arch and descending aortic dissection after ascending aortic repair. Pre-operative CT angiograms were segmented to create patient-specific 3D geometrical models of the dissected aorta. A tetrahedral mesh was derived from this model and subsequently used in a computational simulation of stent deployment in the aortic arch using finite element analysis. We then evaluated computational fluid dynamics of the stented aorta. Results: In silico deployment of an uncovered stent in the dissected aortic arch along the centerline of the true lumen was successfully achieved using our computational model of stent crimping and expansion. The model produced realistic results when compared to intra-operative fluoroscopic imaging. Minimal arterial damage was observed based on the stress and strain of the aortic wall. Computational fluid analysis demonstrated the presence of flow to the carotid and subclavian arteries, with areas of increased turbulence at the ostia as expected (Figure 1). Figure 1. Conclusions: Our in silico model shows feasibility of aortic arch stabilization using an uncovered stent to create a proximal seal zone for TEVAR while maintaining blood flow to branch vessels. Expected surgical outcomes can be computationally modeled using patient-specific geometries to allow for pre-operative planning and surgical risk assessment.

Three-Dimensional Printing to Guide the Application of Modified Prefenestrated Stent Grafts to Treat Aortic Arch Disease

Abstract:Background: The aim of this study was to summarize the experience and outcomes of total endovascular repair of aortic arch disease using three-dimensional (3D) printing to guide the application of modified prefenestrated/branched stent grafts. Patients and methods: From April 2018 to March 2019, 17 patients with aortic arch disease were treated in our department. Five patients had an aortic arch aneurysm and 12 had undergone an aortic arch dissection. Thirteen men and 4 were women, with an average age of 57.82 ± 10.47 years. Preoperatively, a 3D-printed model of the aorta was made according to computed tomography data. Then, under the guidance of the 3D-printed aortic model, modified prefenestrated/branched stent grafts were prepared, and the diameter of the stent grafts was reduced intraoperatively by a physician for total endovascular repair. Aortic computed tomography angiography was performed 3 and 6 months after the surgery. Results: All procedures were completed in one stage with no conversions to sternotomy. Among all 17 patients, the operation was successful in 16. One patient was treated with a chimney graft and a stent graft fenestrated in situ because of distortion of the stent. The success rate of the technique was 94.18%. The average operation time was 4.18 ± 1.57 hr, and no patients died. No neurologic complications, such as cerebral infarction or paraplegia, were observed during the follow-up period. Conclusions: Three-Dimensional printing can be used to help guide the treatment of aortic arch disease using modified prefenestrated/branched stent grafts. This minimally invasive total treatment technique is accurate, allows quick recovery, and has a low complication rate. The short-term follow-up data show the safety and reliability of the method; however, further research and development are needed.

Biomechanics and Modeling in Mechanobiology

Abstract:Abstract Thoracic endovascular aortic repair (TEVAR) has been accepted as the mainstream treatment for type B aortic dissection, but post-TEVAR biomechanical-related complications are still a major drawback. Unfortunately, the stent-graft (SG) configuration after implantation and biomechanical interactions between the SG and local aorta are usually unknown prior to a TEVAR procedure. The ability to obtain such information via personalised computational simulation would greatly assist clinicians in pre-surgical planning. In this study, a virtual SG deployment simulation framework was developed for the treatment for a complicated aortic dissection case. It incorporates patient-specific anatomical information based on pre-TEVAR CT angiographic images, details of the SG design and the mechanical properties of the stent wire, graft and dissected aorta. Hyperelastic material parameters for the aortic wall were determined based on uniaxial tensile testing performed on aortic tissue samples taken from type B aortic dissection patients. Pre-stress conditions of the aortic wall and the action of blood pressure were also accounted for. The simulated post-TEVAR configuration was compared with follow-up CT scans, demonstrating good agreement with mean deviations of 5.8% in local open area and 4.6 mm in stent strut position. Deployment of the SG increased the maximum principal stress by 24.30 kPa in the narrowed true lumen but reduced the stress by 31.38 kPa in the entry tear region where there was an aneurysmal expansion. Comparisons of simulation results with different levels of model complexity suggested that pre-stress of the aortic wall and blood pressure inside the SG should be included in order to accurately predict the deformation of the deployed SG.

Abstract:Background: Stent grafting for treatment of type B aortic dissection has been extensively used. However, the difference in the long-term clinical outcome between patients with chronic versus acute type B aortic dissection remains unknown. This study aimed to analyze the difference in long-term clinical outcome after endovascular repair for patients with chronic (>= 2 weeks) versus acute (<2 weeks) type B aortic dissection.Methods: Between May 2000 and June 2011, a total of 174 patients with type B aortic dissection (56 chronic, 118 acute) treated by endovascular repair were studied prospectively. Follow-up three-dimensional computed tomography scanning and aortoangiography were scheduled at 3-6 months after the index procedure. Propensity score matching was used to compare the difference in the endpoint between the two groups.Results: The procedure-related event rate was 18.6% in the acute group and 5.4% in the chronic group (P = 0.021), but this difference became nonsignificant after propensity score matching. At the end of follow-up (mean 2.49 years), overall and aorta-related mortality was 11.0% and 7.6%, respectively, in the acute group, and was not significantly different from that in the chronic group (3.6% and 3.6%, P = 0.148 and P = 0.506, respectively). Both false and true lumina showed significant remodeling over time, with >93% complete false-lumen thrombosis. Untreated tear and type I endoleak were predictors of clinical events during follow-up.Conclusion: Comparable long-term clinical results were achieved in patients with chronic or acute type B aortic dissection after implantation of a stent graft.
Review and mid-term follow-up of Stanford type B aortic dissections treated by endovascular placement of stent-grafts

Abstract:Objective To introduce the experience of treating Stanford type B aortic dissections by endovascular implantation of stent-graft. Methods We retrospectively studied 70 patients who had been treated form 2000 to 2007, combing with their preoperative imaging examination, DSA during the procedure and postoperative follow-up imaging data. We observed the anatomical features and the complications of stem implantation such as endoleak, migration and fracture of stent-graft during the follow-up and tried to figure out the high risk factors of the complications. Results The patients had 2. 66 lacerations in average, and the distance between the first laceration and left subclavian was 27. 61 mm. Three cases underwent left common carotid artery or left subclavian artery bypass, while 2 cases underwent superior mesenteric artery (SMA) and renal artery bypass before the placement of stent-graft. Sixteen left subclavian arteries were covered simultaneously without bypass. Ten cases (14.3%) occurred endoleak after the procedure, but complications such as misplacement, migration, rupture or conversion to open surgery were not observed. Sixty cases (85. 7%) were treated technical successfully. Followed up between 2 weeks and 72 months, 5 cases were observed type I endoleak and 3 cases acceptted reintervention. This study indicates that endoleak is associated with more tear number, larger aortic diameter and lesser curvature side. Conclusions The endovascular repair is a safe, efficient and feasible therapy to aortic dissection. The creative clinic practice is a breakthrough in the treatment of patients with anatomical limits such as short neck, branch artery evolved, low tear and narrow profile artery. Although we have widen the indication safely, the long term results remain to be further observed.

Abstract:Numerical simulation of stent deployment is very important to the surgical planning and risk assess of the interventional treatment for the cardio-cerebrovascular diseases. Our group developed a framework to deploy the braided stent and the stent graft virtually by finite element simulation. By using the framework, the whole process of the deployment of the flow diverter to treat a cerebral aneurysm was simulated, and the deformation of the parent artery and the distributions of the stress in the parent artery wall were investigated. The results provided some information to improve the intervention of cerebral aneurysm and optimize the design of the flow diverter. Furthermore, the whole process of the deployment of the stent graft to treat an aortic dissection was simulated, and the distributions of the stress in the aortic wall were investigated when the different oversize ratio of the stent graft was selected. The simulation results proved that the maximum stress located at the position where the bare metal ring touched the artery wall. The results also can be applied to improve the intervention of the aortic dissection and the design of the stent graft.
Treatment of Stanford Type A Aortic Dissection with Triple Pre-Fenestration, Reduced Diameter, and Three-Dimensional-printing Techniques: A Case Report

Abstract:BACKGROUND A 63-year-old female was diagnosed with acute Stanford type A aortic dissection. The patient had pain in the chest and back for 1 wk. The computed tomography angiography (CTA) showed Stanford type A aortic dissection (Myla type III aortic arch). The intimal tear was located at the top of the aortic arch and retrograded to the ascending aorta. CASE SUMMARY Preoperatively, a three-dimensional (3D)-printed model of the aortic arch was made according to CTA data. Then, under the guidance of the 3D-printed aortic model, a pre-fenestrated stent-graft was customized, and the diameter of the stent-graft was reduced intraoperatively by surgeons. 3D printing, triple pre-fenestration, and reduced diameter techniques were used during the surgery. The CTA examinations were performed at the 3(rd )mo and 1st year after the surgery; the results showed that the aortic dissection was repaired without endoleak, and all three branches of the aortic arch remained unobstructed. CONCLUSION Applying the triple pre-fenestration technique for aortic arch lesions was feasible and minimally invasive in our case. The technique provides a new avenue for thoracic endovascular aortic repair of Stanford type A aortic dissection.

Endovascular repair in type A aortic dissection: Anatomical candidacy for currently manufactured stent grafts and conceptual valve-carrying devices for an Endo-Bentall procedure

Abstract:Objectives: Endovascular treatment has been suggested as an alternative for open surgery for type A aortic dissection, but current devices have severe anatomical limitations. This study assesses the computed tomography-based anatomical suitability of currently manufactured stent grafts as well as 2 embodiments of valve-carrying devices. Methods: In a retrospective single-centre cohort of 200 consecutive ascending/arch operations between 2009 and 2018, a total of 112 patients with type A aortic dissections were identified and evaluated for endovascular candidacy based on the locations of the entries, the landing zone diameters/lengths and the supra-aortic vessel origins according to the anatomical instructions for use criteria of 6 commercially available (tubular, branched or fenestrated) stent grafts. Two suggested valve-carrying devices with inner branches or fenestrations for the coronary arteries and branches for the supra-aortic vessels were also evaluated. Results: The anatomical feasibility for commercial stent grafts ranged from 4% to 21%. The main limitations were proximal landing zone diameter (considering oversizing <15%), length due to dilatation and an entry too close to the sinotubular junction. For the valve-carrying conduits, anatomical feasibility was between 31% and 80%, with the main limiting factors being the diameter of the aortic annulus and its distance to the coronary arteries. Conclusions: The anatomical applicability of currently manufactured stent grafts for the treatment of type A aortic dissection is limited mainly by the absence of a suitable proximal landing zone in the ascending aorta and might substantially be improved by anchoring in the aortic annulus using a valve-carrying device that uses either fenestrations or branches for the coronary arteries.

Abstract:This work explores the use of an embedded computational fluid dynamics method to study the type B aortic dissection. The use of the proposed technique makes it possible to easily test different intimal flap configurations without any need of remeshing. To validate the presented methodology, we take as reference test case an in vitro experiment present in the literature. This experiment, which considers several intimal flap tear configurations (number, size and location), mimics the blood flow in a real type B aortic dissection. We prove the correctness and suitability of the presented approach by comparing the pressure values and waveform. The obtained results exhibit a remarkable similarity with the experimental reference data. Complementary, we present a feasible surgical application of the presented computer method. The aim is to help the clinicians in the decision making before the type B aortic dissection surgical fenestration. The capabilities of the proposed technique are exploited to efficiently create artificial reentry tear configurations. We highlight that only the radius and center of the reentry tear need to be specified by the clinicians, without any need to modify neither the model geometry nor the mesh. The obtained computational surgical fenestration results are in line with the medical observations in similar clinical studies.
