Pancreatic Vascular Pathologies and Islet Amyloid in Type 2 Diabetes Mellitus

Abstract:Introduction: Islet amyloid is a histopathological hallmark of type 2 diabetes mellitus (T2DM) whereas vascular pathologies enhance the risk of T2DM. The aim of this study was to explore the association of pancreatic vascular pathologies with islet amyloid.Methods: We included 30 T2DM autopsy cases with islet amyloid (DA+) in comparison with 30 T2DM cases (DA-) and 60 nondiabetic subjects (ND) free from islet amyloid. Pancreatic vascular pathologies were demonstrated by histopathological stains with hematoxylin-eosin, periodic acid-schiff and CD34 monoclonal antibody. Islet amyloid deposits were identified by Thioflavin T staining. Pearson correlation coefficients were calculated to explore the association between islet amyloid deposits and pancreatic vascular pathologies.Results: Pancreatic vascular pathologies included atherosclerosis of pancreatic interlobar arteries, intra-islet and pre-islet hyaline arteriosclerosis of small arteries, arterial calcification, atheroembolism, and increased islet capillary density, which were more prevalent in diabetic patients with islet amyloid than nondiabetic controls (T2DM: 91.7%; ND: 68.3%; P < 0.001). Furthermore, patients in DA+ group (100.0%) had a significantly higher prevalence of PVPs than patients in DA- group (83.3%) and ND group (68.3%). Finally, the severity of islet amyloid deposits was linearly correlated with that of interlobar arteriosclerosis (r = 0.603, p < 0.01) and intra-islet capillary density (r = 0.734, p < 0.01) in the DA+ group.Conclusions: Pancreatic vascular pathologies are closely associated with the development of islet amyloid and type 2 diabetes.Funding Statement: This work was supported by a grant from the National Natural Science Foundation of China (81471054, 81860155) and the Innovation Project of Guangxi Graduate Education (JGY2015128).Declaration of Interests: The authors have declared that no conflict of interests exists.Ethics Approval Statement: Informed consent was obtained before autopsy.
