Survival after Perihilar Cholangiocarcinoma Resection —development of a Prognostic Nomogram and Comparison of Three Prognostic Schemes

Abstract:Introduction: Disease-specific nomograms have been proposed as a better means to predict long term survival for individual patients. We sought to externally validate the performances of the 7th, 8th edition American Joint Committee on Cancer (AJCC) staging system, Memorial Sloan Kettering Cancer Center (MSKCC) nomogram, and Buettner's nomogram for perihilar cholangiocarcinoma (PHC), as well as identify factors to improve the prediction of prognosis for patients with PHC. Method: 359 patients who underwent surgery for PHC between 2013 and 2018 were identified using a multi-center database. The predictive power of the AJCC staging system and nomogram were assessed by concordance index. Result: Using the AJCC 7th edition staging system, we found that the 5-year disease free survival for stages I, II, III, and IVa was 39%, 21%, 23% and 28%, In comparison, when the 8th edition staging system was used, the 5-year overall survival was 39% and 21% in stages I and II, resulting in 20%, 12%, and 12% in stages IIIa, IIIb, and IIIc, respectively. 1∼6% patients in stages IVa and IVb survived 5 years. The AJCC 8th edition staging system, MSKCC nomogram, and Buettner's nomogram had a slightly better discriminatory ability with a concordance index of 0.65, 0.62, and 0.66 respectively compared with 0.62 for the 7th edition. A revised nomogram based on age, N status, differentiation, and margin status performed better (C-index 0.69). The calibration plot of the revised PHC nomogram demonstrated good calibration Conclusion: The proposed nomogram and 8th AJCC staging to predict DFS after curative intent resection of PHC had a better prognostic accuracy than the 7th AJCC staging system.
