Abstract:As the world's second-largest economy, China is going on suffering from environmental pollution, especially for ambient air pollution, which has become a major threat to public health; public awareness of the detrimental effects of air pollution on health is increasing-particularly in relation to haze days. Considering the nonlinear relationship of ambient air pollution exposure and health impacts, and the differences in specific sources of air pollution with those in North America and Europe, conducting health impact assessments of ambient air pollution in China has thus become an urgent task for public health practitioners. Systematic review of the health effects of exposure to ambient air pollution from quantitative studies conducted in Chinese could provide vital information for epidemiology-based health impact assessments and the implementation of a national environmental protection policy.

Abstract:China’s economy has developed rapidly in the recent two decades. Economic development is usually linked with increase in energy consumption and consumption emissions, which in turn leads to worsening of air quality. Due to the adoption of various control measures, the ambient air quality in a number of large cities in China has actually improved. The ambient air TSP and SO2 levels in China have been decreasing in the last decade. However, ambient air NOx level has been increasing due to the increased number of motor vehicles. Coal has been and is still the major source of energy in China. Ambient air pollution in large cities has changed from the conventional coal combustion type to the mixed coal combustion/motor vehicle emission type. A series of epidemiological studies on air pollution and health effects ranging from mortality, morbidity to functional changes have been conducted in China. The results showed that ambient air pollution had acute and chronic effects on mortality, morbidity, hospital admissions, clinical symptoms, lung function changes, etc. The exposure–response relationship between air pollutants and daily mortality, morbidity, hospital admissions, and lung function has been established accordingly.
Systematic Review of Chinese Studies of Short-Term Exposure to Air Pollution and Daily Mortality

Abstract:Health effects attributable to air pollution exposure in Chinese population have been least understood. The authors conducted a meta-analysis on 33 time-series and case-crossover studies conducted in China to assess mortality effects of short-term exposure to particulate matter with aerodynamic diameters less than 10 and 2.5 μm (PM10 and PM2.5), sulfur dioxide (SO2), nitrogen dioxide (NO2), ozone (O3) and carbon monoxide (CO). Significant associations between air pollution exposure and increased mortality risks were observed in the pooled estimates for all pollutants of interest. In specific, each 10 μg/m(3) increase in PM2.5 was associated with a 0.38% (95% Confidence Interval, CI: 0.31, 0.45) increase in total mortality, a 0.51% (95% CI: 0.30, 0.73) in respiratory mortality, and a 0.44% (95% CI: 0.33, 0.54) in cardiovascular mortality. When current annual PM2.5 levels in mega-Chinese cities to be reduced to the WHO Air Quality Guideline (AQG) of 10 μg/m(3), mortality attributable to short-term exposure to PM2.5 could be reduced by 2.7%, 1.7%, 2.3%, and 6.2% in Beijing, Shanghai, Guangzhou and Xi'an, respectively. The authors recommend future studies on the nature of air pollution concentration and health effect relationships in Chinese population to support setting stringent air quality standards to improve public health.
Systematic Review and Meta-Analysis of the Adverse Health Effects of Ambient PM2.5 and PM10 Pollution in the Chinese Population.

Abstract:INTRODUCTION:As the largest developing country, China has some of the worst air quality in the world. Heavy smog in January 2013 led to unprecedented public concern about the health impact of exposure to particulate matter. Conducting health impact assessments of particulate matter has thus become an urgent task for public health practitioners. Combined estimates of the health effects of exposure to particulate matter from quantitative reviews could provide vital information for epidemiology-based health impact assessments, but estimates for the Chinese population are limited.METHODS:On December 31, 2013, we systematically searched the PubMed, Web of Science, and China National Knowledge Infrastructure databases using as keywords names of 127 major cities in Mainland China, Hong Kong, and Taiwan. From among the 1464 articles identified, 59 studies were manually screened. Random-effects or fixed-effects models were used to combine their risk estimates, the funnel plots with Egger test were performed to evaluate the publication bias and Meta regression were run to explore the association between exposure to particulate matter with aerodynamic diameters less than 10 and 2.5 µm (PM10 and PM2.5) and the resulting health effects by the Comprehensive Meta Analysis.RESULTS:In terms of short-term effects, the combined excess risks of total non-accidental mortality, mortality due to cardiovascular disease, and mortality due to respiratory disease were 0.36% (95% confidence interval [95%CI]: 0.26%, 0.46%), 0.36% (95%CI: 0.24%, 0.49%), and 0.42% (95%CI: 0.28%, 0.55%), for each 10 μg/m(3) increase in PM10. A 10 μg/m(3) increase in PM2.5 was associated with a 0.40% (95%CI: 0.22%, 0.59%) increase in total non-accidental mortality, a 0.63% (95%CI: 0.35%, 0.91%) increase in mortality due to cardiovascular disease, and a 0.75% (95%CI: 01.39%, 1.11%) increase in mortality due to respiratory disease. For constituent-specific mortality, increases of 0.40-3.11% were associated with an increase of 10 ng/m(3) for nickel in PM. The summary estimate ranges of hospital utilization were 0.08% ~ 0.72% and -0.58% ~ 1.32% for a 10 μg/m(3) increase in PM10 and PM2.5. In terms of long-term effects, a 10 μg/m(3) increase of PM10 corresponded to 23-67% increase in the risk of mortality.CONCLUSION:Short exposures to PM10 and PM2.5 are associated with increases in mortality, but evidence of constituent-associated health effects, long-term effects and morbidity in China is still inadequate.

Abstract:Ambient particulate matter (PM) pollution in China continues to be a major public health challenge. With the release of the new WHO air quality guidelines in 2021, there is an urgent need for China to contemplate a revision of air quality standards (AQS). In the recent decade, there have been an increase in epidemiological studies on PM in China. A comprehensive evaluation of such epidemiological evidence among the Chinese population is central for revision of the AQS in China and in other developing countries with similar air pollution problems. We thus conducted a systematic review on the epidemiological literature of PM published in the recent decade. In summary, we identified: (1) Short-term and long-term PM exposure increase mortality and morbidity risk without a discernible threshold, suggesting the necessity for continuous improvement in air quality; (2) The magnitude of long-term associations with mortality observed in China are comparable with those in developed countries, whereas the magnitude of short-term associations are appreciably smaller; (3) Governmental clean air policies and personalized mitigation measures are potentially effective in protecting public and individual health, but need to be validated using mortality or morbidity outcomes; (4) Particles of smaller size range and those originating from fossil-fuel combustion appear to show larger relative health risks; and (5) Molecular epidemiological studies provide evidence for the biological plausibility and mechanisms underlying the hazardous effects of PM. This updated review may serve as an epidemiological basis for China's AQS revision and proposes several perspectives in designing future health studies.

Abstract:Decades of gross domestic product-focused economic development has not only made China the world's second largest economy, but has also led to massive environmental problems, including air pollution, which has been shown to lead to multiple adverse health outcomes. This review paper aims to contribute a better understanding regarding the interactive relationships between ambient air pollution and public health in China, based on a literature review from international journals, public data from government agencies and relevant reports from international organisations. Various studies have been conducted regarding air pollution and public health in China and their outcomes, which are valuable for further investigations and policymaking. However, current knowledge regarding air pollution and public health is still inadequate and has become a key barrier to supporting recommendations for further policymaking. This is despite the fact that China has launched relevant laws, regulations and standards and built about 106 national monitoring sites. Finally, two recommendations in particular are proposed: to enable public participation and improving the executive power of the government for reducing air pollution and to improve public health in China.

Abstract:Ambient air pollution in China has worsened following dramatic increases in industrialization, automobile use and energy consumption. Particularly bothersome is the increase in the PM2.5 fraction of pollutants. This fraction has been associated with increasing rates of cardio-respiratory disease in China and elsewhere. Ambient pollutant levels have been described in many of China’s cities and are comparable to previous levels in southern California. Lung cancer mortality in China has increased since the 1970s and has been higher in men and in urban areas, the exact explanation for which has not been determined. The estimation of individual risk for Chinese citizens living in areas of air pollution will require further research. Occupational cohort and case-control designs each have unique attributes that could make them helpful to use in this setting. Other important future research considerations include detailed exposure assessment and the possible use of biomarkers as a means to better understand and manage the threat posed by air pollution in China.

Abstract:Although China has achieved great progress in ambient pollution reduction in the past two decades, it is still one of the few countries with the worst air pollution levels in the world. In addition to lifestyle choices such as smoking, diet and exercise, exposure to outdoor air pollution is being considered a major determinant of cardiovascular disease in the Chinese population. Numerous epidemiological studies on air pollution and cardiovascular mortality have been conducted in China, using time-series, case-crossover, or cross-sectional designs. The increased cardiovascular mortality risks observed in the Chinese population are similar in magnitude, per amount of pollution, to the risks found in other parts of the world. However, the importance of these increased cardiovascular risks is greater than in North America or Europe, because the air pollution in China is at much higher levels in general and the Chinese population accounts for more than one fourth of the world’s total. There has been no air pollution cohort study in China examining the long-term effects of air pollution, nor any published Chinese data assessing the relation between air pollution and cardiovascular morbidity and sub-clinical indicators. Future research in China should focus on a prospective analysis of association between air pollution and cardiovascular disease and the likely underlying pathophysiologic links.
The Ambient Particular Matter Pollution Situation and Its Health Effect in China

Abstract:Particular matter is the main pollutant for the ambient environment in China.Some PM 10 pollution index data were analyzed for four different cities.Exposure-response between PM 10 , PM 2.5 and daily mortality of inhabitants in China was discussed.Epidemiological literature of PM 10 , PM 2.5 and daily mortality were extracted using Endnote, and the relationship between PM 10 , PM 2.5 and daily mortality of inhabitants using meta-analysis method.The results show that the air pollution index average value is 92.8, 74, 67.9 and 75.1 for Beijing, Shanghai, Guangzhou and Chongqing, respectively.It is more serious for PM 10 pollution in Beijing than other three cities.The daily mortality of inhabitants increase by 0.29% (95% CI: 0.21%-0.38%)and 0.56% (95% CI: 0.40%-0.72%),as the concentration of PM 10 , PM 2.5 increase a certain degree (10µg/m³).The results of meta-analysis can provide the scientific basis for particular matter pollution control.The related control measurements should be made to reduce PM pollution.

Abstract:During the rapid economic development in China, ambient air pollutants in major cities, including PM10 (particulate matter with aerodynamic diameter < or =10 microm) and SO2 have been reduced due to various measures taken to reduce or control sources of emissions, whereas NO2 is stable or slightly increased. However, air pollution levels in China are still at the higher end of the world level. Less information is available regarding changes in national levels of other pollutants such as PM2.5 and ozone. The Chinese Ministry of Environmental Protection (MOEP) set an index for "controlling/reducing total SO2 emissions" to evaluate the efficacy of air pollution control strategy in the country. Total SO2 emissions declined for the first time in 2007. Chinese epidemiologic studies evidenced adverse health effects of ambient air pollution similar to those reported from developed countries, though risk estimates on mortality/morbidity per unit increase of air pollutant are somewhat smaller than those reported in developed countries. Disease burden on health attributable to air pollution is relatively greater in China because of higher pollution levels. Improving ambient air quality has substantial and measurable public health benefits in China. It is recommended that the current Chinese air quality standards be updated/revised and the target for "controlling/reducing total SO2 emissions" be maintained and another target for "reducing total NO2 emissions" be added in view of rapid increase in motor vehicles. Continuous and persistent efforts should be taken to improve ambient air quality.

Abstract:The rapid economic growth in China is coupled with a severe ambient air pollution, which poses a huge threat to human health and the sustainable development of social economy. The rapid urbanization and industrialization over the last three decades have placed China as one of countries with the greatest disease burden in world. Notably, the prevalence rate of chronic noncommunicable diseases (CND), including respiratory diseases, CVD, and stroke, in 2010 reaches 16.9%. The continuous growth of the incidence of CND urgent needs for effective regulatory action for health protection. This study aims to evaluate the impact of rapid urbanization on status of ambient air pollution and associated adverse health effects on the incidence and the burden of CND and risk assessment. Our findings would be greatly significant in the prediction of the risk of ambient air pollution on CND and for evidence-based policy making and risk management in China.

Abstract:Background Rapid economic and social development in China has resulted in severe air pollution and consequent adverse impacts on society. The health effects of air pollution have been widely studied. Methods Using information from the China Health and Retirement Longitudinal Study (CHARLS) database, we established a hierarchical linear model combining pollution and socioeconomic and psychosocial variables to examine the effects of air pollution on public health in China. Local air pollution was characterized in multiple dimensions. Results The relationship of health to its determinants greatly differed between Eastern and Central/Western China. Higher education, higher income level, better life satisfaction, and long-term marriage were significantly associated with better health status among Chinese. In addition, regional healthcare resources were positively associated with the health of residents. As indicated by the hierarchical model with health as dependent variable, in Central/Western China, longest duration of good air quality in spring/summer was positively associated with health (estimated coefficient = 0.067, standard error = 0.026), while the mean Air Quality Index (AQI) in autumn/winter was inversely associated with health (estimated coefficient = 0.082, standard error = 0.031). Good air quality in the current study is defined as daily average AQI less than 35. Conclusions Duration (in days) of acceptable air quality was particularly important for improving public health. Future policies should target increased duration of good air quality while managing air pollution by controlling or decreasing severe air pollution.

Abstract:Increasing air pollution could undermine human health, but the causal link between air pollution and eye and ear health has not been well-studied. Based on four-week-level records of eye and ear health over 1991-2015 provided by the China Health and Nutrition Survey, we estimate the causal effect of air pollution on eye and ear health. Using two-stage least squares estimation, we find that eye or ear disease possibility rises 1.48% for a 10 μg/m3 increase in four-week average PM2.5 concentration. The impacts can last about 28 weeks and will be insignificant afterward. Females, individuals aged 60 years and over, with high exposure environments, relatively poor economic foundations, and low knowledge levels are more vulnerable to such negative influences. Behavioral channels like more smoking activities and less sleeping activities could partly explain this detrimental effect. Our findings enlighten how to minimize the impact of air pollution and protect public health.
Fine particulate matter and cardiorespiratory health in China: A systematic review and meta-analysis of epidemiological studies

Abstract:This review aimed to systematically summarize the epidemiological literature on the cardiorespiratory effects of PM2.5 published during the 13(th) Five-Year Plan period (2016-2020) in China. Original articles published between January 1, 2016 and June 30, 2021 were searched in PubMed, Web of Science, the China National Knowledge Internet Database and Wanfang Database. Random- or fixed-effects models were used to pool effect estimates where appropriate. Of 8558 records identified, 145 met the full eligibility criteria. A 10 mu g/m(3) increase in short-term PM2.5 exposure was significantly associated with increases of 0.70%, 0.86%, 0.38% and 0.96% in cardiovascular mortality, respiratory mortality, cardiovascular morbidity, and respiratory morbidity, respectively. The specific diseases with significant associations included stroke, ischemic heart disease, heart failure, arrhythmia, chronic obstructive pulmonary disease, pneumonia and allergic rhinitis. The pooled estimates per 10 mu g/m(3) in-crease in long-term PM2.5 exposure were 15.1%, 11.9% and 21.0% increases in cardiovascular, stroke and lung cancer mortality, and 17.4%, 11.0% and 4.88% increases in cardiovascular, hypertension and lung cancer incidence respectively. Adverse changes in blood pressure, heart rate variability, systemic inflammation, blood lipids, lung function and airway inflammation were observed for either short-term or long-term PM2.5 exposure, or both. Collectively, we summarized representative exposure-response relationships between short- and long-term PM2.5 exposure and a wide range of cardiorespiratory outcomes applicable to China. The magnitudes of estimates were generally smaller in short-term associations and comparable in long-term associations compared with those in developed countries. Our findings are helpful for future standard revisions and policy formulation. There are still some notable gaps that merit further investigation in China. Abstract:Airborne particulate matter presents a serious health threat to human beings, but in China there have until now been few epidemiological studies, especially regarding the impact of ambient fine particulate matter (PM2.5). This study explored first the temporal and spatial characteristics of ambient airborne PM2.5 in China, 2013. Mortality, respiratory diseases, cardiovascular diseases, and chronic bronchitis were then evaluated as four health endpoints attributed to PM2.5. The results showed that the average annual PM2.5 concentration was 72.71 μg/m3; the PM2.5 concentration was below 35 μg/m3 for only 6% of the time, for the whole year. In terms of the PM2.5 concentration, January (133.10 μg/m3) and December (120.19 μg/m3) were the most polluted months, whereas July (38.76 μg/m3) and August (41.31 μg/m3) were the least polluted months. The most highly polluted areas were concentrated in North China. In terms of the health endpoints attributable to PM2.5, there were 763,595 mortality, 149,754 cardiovascular diseases, 446,035 respiratory diseases, and 2,389,035 chronic bronchitis cases. Results were very important to clarify the current PM2.5 pollution situation and the health impact of PM2.5 in China. And also provided a reference for the assessing damage caused by PM2.5 pollution.

Abstract:This study systematically reviewed scientific evidence linking ambient air pollution to physical activity and sedentary behavior in China. A keyword and reference search was conducted in PubMed, Web of Science, and the Cochrane Library. Predetermined selection criteria included—study designs: interventions or experiments, retrospective or prospective cohort studies, cross-sectional studies, and case-control studies; subjects: people of all ages; exposures: specific air pollutants and/or overall air quality; outcomes: physical activity and/or sedentary behavior; and country/area: mainland China. Ten studies met the selection criteria and were included in the review. Six adopted a cross-sectional design and the remaining four adopted a prospective cohort design. Four studies assessed a specific air pollutant namely particulate matter with diameter <2.5 μg/m3 (PM2.5), whereas the remaining six focused on overall air quality, defined using air quality indexes. Decline in overall air quality and increase in PM2.5 concentration were found to be associated with reduced daily/weekly duration of outdoor leisure-time and/or transportation-related physical activity such as walking but increased duration of daytime/nighttime sleeping among Chinese residents. In contrast, evidence linking overall air quality and PM2.5 concentration to sedentary behavior remains mixed and inconclusive. In conclusion, preliminary evidence indicates that ambient air pollution impacts Chinese residents’ daily physical activity-related behaviors. Future studies adopting objective measures of physical activity and a longitudinal or experimental study design are warranted to examine the impact of air pollution on sensitive sub-populations such as children, older adults and people with pre-existing conditions, and in locations outside China.

Abstract:Household air pollution prevails in rural residences across China, yet a comprehensive nationwide comprehending of pollution levels and the attributable disease burdens remains lacking. This study conducted a systematic review focusing on elucidating the indoor concentrations of prevalent household air pollutants-specifically, PM2.5, PAHs, CO, SO<sub>2</sub>, and formaldehyde-in rural Chinese households. Subsequently, the premature deaths and economic losses attributable to household air pollution among the rural population of China were quantified through dose-response relationships and the value of statistical life. The findings reveal that rural indoor air pollution levels frequently exceed China's national standards, exhibiting notable spatial disparities. The estimated annual premature mortality attributable to household air pollution in rural China amounts to 966 thousand (95% CI: 714-1226) deaths between 2000 and 2022, representing approximately 22.2% (95% CI: 16.4%-28.1%) of total mortality among rural Chinese residents. Furthermore, the economic toll associated with these premature deaths is estimated at 486 billion CNY (95% CI: 358-616) per annum, constituting 0.92% (95% CI: 0.68%-1.16%) of China's GDP. The findings quantitatively demonstrate the substantial disease burden attributable to household air pollution in rural China, which highlights the pressing imperative for targeted, region-specific interventions to ameliorate this pressing public health concern.

Abstract:Background: Ambient fine particulate matter (PM2.5) pollution is currently a most severe and worrisome environmental problem in China. However, current knowledge of the health effects of this pollution is insufficient.Objectives: This study aims to provide an overall understanding regarding the long-term mortality effects of current PM2.5 pollution in China and the potential health benefits of realizing the goals stipulated in the ongoing action plan of Air Pollution Prevention and Control (APPC) and the targets suggested by the WHO.Methods: Three typical causes and all-cause of PM2.5-related mortality were considered. The log-linear exposure-response function was adopted, and a meta-analysis was used to determine the exposure-response coefficients, based on newly available data in China and abroad.Results: In the 74 leading cities of China, approximately 32% of the reported deaths, with a mortality rate of 1.9%, were associated with PM2.5 in 2013, in which deaths from cardiovascular, respiratory and lung-cancer causes accounted for 20% of the reported deaths, with a mortality rate of 1.2%. The regional difference is remarkable for the mortalities and proportions of the different causes. If the PM2.5 concentration goals of the APPC plan, the first interim and the guideline targets of the WHO could be achieved, the PM2.5-related all-cause mortality would be reduced by 25%, 64% and 95%, respectively, compared with that of 2013.Conclusions: PM2.5 pollution in China has incurred great health risks that are even worse than those of tobacco smoking. The health benefits of the APPC plan could be outstanding, although there is still great potential to improve future air quality. (C) 2016 Elsevier B.V. All rights reserved.
