Abstract:This study used event-related potentials (ERPs) to assess effects of low-level prenatal lead exposure on auditory recognition memory in 2-month-old infants. Infants were divided into four groups according to cord-blood lead concentration: (1) <2.00 μ g/dL, (2) 2.00-2.99 μ g/dL, (3) 3.0-3.7 μ g/dL, and (4) ≥3.7 μ g/dL. The first group showed the normally expected differences in P2, P750, and late slow wave (LSW) amplitudes elicited by mothers' and strangers' voices. These differences were not observed for one or more ERP components in the other groups. Thus, there was electrophysiological evidence of poorer auditory recognition memory at 2 months with cord-blood lead ≥2.00 μ g/dL.

Abstract:Background- Lead (Pb) exposure during childhood has been associated with numerous deficits in neurobehavioral, motor, and cognitive functions. The effects of Pb on sensory function are less well-studied, and only in animals. The rapid increase in visual acuity (VA) in infancy reflects increasing myelination to a large degree and overall maturation of the central nervous system. VA might thus be sensitive to effects of Pb. Aim- To investigate the effect of early life Pb exposure on infant grating VA. Methods- Pb was measured using AAS in umbilical cord blood and infant blood at 9 months and 18 months in a cohort of healthy, full-term infants born in rural Fuyang County near Hangzhou, China between 2008-2011. VA was measured in infants using Teller Acuity Cards at 6 weeks (N=354), 9 months (N=330), and 18 months (N=285) and assigned as the narrowest grid the infant fixated on. Multivariate linear regression was used to evaluate relationships between cord, 9 month, or 18 month Pb and VA at 6 weeks, 9 months, or 18 months. Results- Means for Pb were 3.1, 4.4, and 3.6 µg/dL, for cord, 9 months, and 18 months, respectively. 9-month VA was lower in infants with higher concurrent blood Pb at 9 months. In adjusted models, VA decreased by 0.12 units (95%CI: 0.02-0.22) per every 1 µg/dL increase in concurrent blood Pb (p=0.02). Further analysis using quartiles revealed a highly linear association (ß= -1.41, -1.03, -0.26, for the upper three quartiles vs. the lowest one, respectively; p-trend=0.0001). No significant associations were observed at the other time points. Conclusions- VA at 9 months was lower in infants with higher concurrent blood Pb. The observed inverse association between VA and blood Pb at 9 months suggests that early life exposure to Pb may adversely affect myelination and visual system development.
Prenatal Exposure to Multiple Pesticides is Associated with Auditory Brainstem Response at 9months in a Cohort Study of Chinese Infants.

Abstract:Background: Pesticides are associated with poorer neurodevelopmental outcomes, but little is known about the effects on sensory functioning.Methods: Auditory brainstem response (ABR) and pesticide data were available for 27 healthy, full-term 9-month-old infants participating in a larger study Of early iron deficiency and neurodevelopment. Cord blood was analyzed by gas chromatography-mass spectrometry for levels of 20 common pesticides. The ABR forward-masIdng.condition consisted of a click stimulus (masker) delivered via ear canal transducers followed by an identical stimulus delayed by 8, 16, or 64 milliseconds (ms). ABR peak latencies were evaluated as a function of masker-stimulus time interval. Shorter wave latencies reflect faster neural conduction, more mature auditory pathways, and greater degree of myelination. Linear regression models were used to evaluate associations between total number of pesticides detected and ABR outcomes. We considered an additive or synergistic effect of poor iron status by stratifying our analysis by newborn ferritin (based on median split).Results: Infants in the sample were highly exposed to pesticides; a mean of 4.1 pesticides were detected (range 0-9). ABR Wave V latency and central conduction time (CCT) were associated with the number of pesticides detected in cord blood for the 64 ms and non-masker conditions. A similar pattern seen for CCT from the 8 ms and 16 ms conditions, although statistical significance was not reached. Increased pesticide exposure was associated with longer latency. The relation between number of pesticides detected in cord blood and CCT depended on the infant's cord blood ferritin level. Specifically, the relation was present in the lower cord blood ferritin group but not the higher cord blood ferritin group.Conclusions: ABR processing was slower in infants with greater prenatal pesticide exposure; indicating impaired neuromaturation. Infants with lower cord blood ferritin appeared to be more sensitive to the effects of prenatal pesticide exposure on ABR latency delay, suggesting an additive or multiplicative effect (C) 2016 Elsevier Ltd. Prenatal Maternal Stress in Relation to the Effects of Prenatal Lead Exposure on Toddler Cognitive Development

Abstract:Objective: To evaluate the effects of maternal lead exposure during pregnancy on toddler cognitive development and the potential effect modification by maternal stress.Methods: We conducted a prospective birth-cohort study in Shanghai from 2010 to 2012 and investigated 225 mother-infant pairs. The mothers were recruited in mid-to-late pregnancy and children were followed up until 24-36 months old. A self-administered Symptom Checklist-90-Revised Scale (SCL-90-R) was used to assess maternal emotional stress during pregnancy. Maternal whole blood lead levels were measured during gestational weeks 28-36. The toddlers' cognitive levels were assessed using the Gesell Development Scale. Multiple linear regression models were established to explore the main effects of prenatal lead exposure on toddlers' cognitive abilities and the modifying effects of maternal stress. Covariate information was collected through interviews, questionnaires and medical records.Results: The mean maternal blood lead concentration was 3.30 (95%CI: 3.05, 3.57) mu g/dL. After adjusting for relevant confounders, no significant associations of maternal blood lead concentrations with toddlers' cognitive levels were observed in all five domains of the Gesell scale (P>0.05). However, the interaction between prenatal maternal blood lead and stress was significant in the domains of adaptive behavior, language and social behavior. When stratified by maternal stress levels, compared with non-significant associations (P>0.05) among low (P1-P75) prenatal stress group, adverse associations between maternal blood lead concentrations (log10-transformed) and toddlers' cognitive levels were observed among high (P75-P100) prenatal stress group in the domains of language (beta =-33.82, 95%CI:-60.04,-7.59), social behavior (beta = -41.00, 95%CI:-63.11,-18.89) and adaptive behavior (beta =-17.93, 95%CI:-35.83,-0.03).Conclusion: Prenatal maternal stress may exacerbate the deleterious effects of prenatal exposure to lead on toddler cognitive development. Prenatal and Postnatal Lead Exposure and Cognitive Development of Infants Followed over the First Three Years of Life: A Prospective Birth Study in the Pearl River Delta Region, China

Abstract:Purpose: Our pilot studies showed that there was a significant relationship between low cord blood levels and scores of neonatal behavioral neurological assessment. The study was further to probe the adverse cognitive effects induced by low-level lead exposure during prenatal and postnatal period.Method: Totally 362 mothers with their infants located the PRD, Guangdong, China participated in the study during their stay in these center: 141 in the high lead group [umbilical-cord blood lead levels (UCBLLs) >= 3.92 mu g/dl] and 102 in the low lead group (UCBLLs <= 1.89 mu g/dl). The other 137 subjects failed to complete the study for a variety of reasons. Blood Lead levels (BLLs) were measured by atomic absorption spectrophotometry, equipped with a graphite furnace. The developmental functioning of infants and children was assessed with BSID-II. The children's birth outcome and the rest of information were obtained from their medical records or a comprehensive questionnaire from their parents, which contained demographic characteristics, lifestyle, mother's IQ and environmental lead sources, etc.Results: Of 380, 243 newborns (63.95%) had complete data collection for all variables included at 6, 12, 24 and 36 months of age. The mean UCBLLs for high and low lead group were 5.63 +/- 0.32 mu g/dl and 1.35 +/- 0.26 mu g/dl, respectively. Significant inverse associations have been found between the UCBLLs and the MDI and the PDI. The associations might attenuate over subsequent years. BLLs at 24 months were significantly associated, in an inverse direction, with MDI at 24 and 36 months. The observed trend of cognitive deficit beginning at 6 months of life might persist, and even develop over the coming years. A positive significant effect of home nurture environment was observed on MDI scores at 12, 24, 36 months of age and PDI scores at 24 and 36 months of age.Conclusion: Our study demonstrates that prenatal and postnatal lead exposure as low as 5 mu g/dl has an adverse effect on neurodevelopment, best arrested by measuring UCBLLs and BLLs at 24 months of age, and suggest a reference for a blood lead critical value below 5 mu g/dL. The collective evidence indicate that low lead exposure must be addressed appropriately by health policy makers and argues for an improvement of home nurture environment, i.e., reduce the burden of Pb on children and, strengthen the training of cognitive ability. Abstract:Background: Occupational studies suggest that exposure to organophosphate insecticides (OPs) can lead to vision or hearing loss. Yet the effects of early-life exposure on visual and auditory function are unknown. Here we examined associations between prenatal OP exposure and grating visual acuity (VA) and auditory brainstem response (ABR) during infancy. Methods: 30 OPs were measured in umbilical cord blood using gas chromatography tandem mass spectrometry in a cohort of Chinese infants. Grating visual acuity (VA) (n = 179-200) and auditory brainstem response (ABR) (n = 139-183) were assessed at 6 weeks, 9 months, and 18 months. Outcomes included VA score, ABR wave V latency and central conduction time, and head circumference (HC). Associations between sensory outcomes during infancy and cord OPs were examined using linear mixed models. Results: Prenatal chlorpyrifos exposure was associated with lower 9-month grating VA scores; scores were 0.64 (95% CI: -1.22, -0.06) points lower for exposed versus unexposed infants (p = 0.03). The OPs examined were not associated with infant ABR latencies, but chlorpyrifos and phorate were both significantly inversely associated with HC at 9 months; HCs were 0.41 (95% CI: 0.75, 0.6) cm and 0.44 (95% CI: 0.88, 0.1) cm smaller for chlorpyrifos (p = 0.02) and phorate (p = 0.04), respectively. Conclusions: We found deficits in grating VA and HC in 9-month-old infants with prenatal exposure to chlorpyrifos. The clinical significance of these small but statistically significant deficits is unclear. However, the disruption of visual or auditory pathway maturation in infancy could potentially negatively affect downstream cognitive development.
Prenatal Naled and Chlorpyrifos Exposure is Associated with Deficits in Infant Motor Function in a Cohort of Chinese Infants.

Abstract:Background: Organophosphate insecticides (OPs) are used worldwide, yet despite nearly ubiquitous exposure in the general population, few have been studied outside the laboratory. Fetal brains undergo rapid growth and development, leaving them susceptible to long-term effects of neurotoxic OPs. The objective here was to investigate the extent to which prenatal exposure to OPs affects infant motor development.Methods: 30 OPs were measured in umbilical cord blood using gas chromatography tandem mass spectrometry in a cohort of Chinese infants. Motor function was assessed at 6-weeks and 9-months using Peabody Developmental Motor Scales 2nd edition (PDMS-2) (n = 199). Outcomes included subtest scores: reflexes, stationary, locomotion, grasping, visual-motor integration (V-M), composite scores: gross (GM), fine (FM), total motor (TM), and standardized motor quotients: gross (GMQ), fine (FMQ), total motor (TMQ).Results: Naled, methamidophos, trichlorfon, chlorpyrifos, and phorate were detected in = 10% of samples. Prenatal naled and chlorpyrifos were associated with decreased 9-month motor function. Scores were 0.55, 0.85, and 0.90 points lower per 1 ng/mL increase in log-naled, for V-M (p = 0.04), FM (p = 0.04), and FMQ (p = 0.08), respectively. For chlorpyrifos, scores were 0.50, 1.98, 0.80, 1.91, 3.49, 2.71, 6.29, 2.56, 2.04, and 2.59 points lower for exposed versus unexposed infants, for reflexes (p = 0.04), locomotion (p = 0.02), grasping (p = 0.05), V-M (p < 0.001), GM (p = 0.007), FM (p = 0.002), TM (p < 0.001), GMQ (p = 0.01), FMQ (p = 0.07), and TMQ (p = 0.008), respectively. Girls appeared to be more sensitive to the negative effects of OPs on 9-month motor function than boys.Conclusions: We found deficits in 9-month motor function in infants with prenatal exposure to naled and chlorpyrifos. Naled is being aerially sprayed to combat mosquitoes carrying Zika virus, yet this is the first nonoccupational human study of its health effects. Delays in early-motor skill acquisition may be detrimental for downstream development and cognition.
Low-Level Prenatal Lead Exposure and Neurobehavioral Development of Children in the First Year of Life: A Prospective Study in Shanghai

Abstract:We used a prospective study design to assess the effects of prenatal low-level lead exposure on the development of urban, inner-city children in Shanghai. Umbilical cord blood samples were consecutively collected from 605 live newborns. Two hundred and fifty-seven samples were excluded from the study due to clotting. Lead levels were determined on 348 cord blood samples. The geometric mean was 9.2 μg/dl. Based on their cord blood lead levels, infants were classified into two exposure groups: 104 in a relatively low lead group (lead levels ≤30 percentile), and 104 in a relatively high lead group (lead levels ≥70 percentile). Seventy-five subjects failed to complete the study, and 133 babies were included in the final cohort: 69 babies in the high lead group and 64 in the low lead group. At 3, 6, and 12 months, the Bayley Scales of Infant Development were administered and capillary blood lead levels were measured. Detailed information was obtained on a wide range of variables relevant to infant development. At all three ages, the Mental Development Index (MDI) scores, adjusted for confounders, were inversely related to the infants' cord blood lead levels. The difference of the mean adjusted MDI scores between low and high lead groups was 3.4 at 3 months, 6.3 at 6 months, and 5.2 at 12 months of age. These differences were statistically significant at all time points. No significant association between cord blood lead levels and the Psychomotor Development Index (PDI) scores was detected at all three visits after adjustment for confounders. Postnatal lead levels were unrelated to concurrent developmental status. We conclude that prenatal low-level lead exposure, which is relatively common in Shanghai, is associated with an adverse developmental impact on children through the first year of life.
Lead exposure through gestation-only caused long-term learning/memory deficits in young adult offspring

Abstract:Numerous observations in clinical and preclinical studies indicate that the developing brain is particular sensitive to lead (Pb)'s pernicious effects. However, the effect of gestation-only Pb exposure on cognitive functions at maturation has not been studied. We investigated the potential effects of three levels of Pb exposure (low, middle, and high Pb: 0.03%, 0.09%, and 0.27% of lead acetate-containing diets) at the gestational period on the spatial memory of young adult offspring by Morris water maze spatial learning and fixed location/visible platform tasks. Our results revealed that three levels of Pb exposure significantly impaired memory retrieval in male offspring, but only female offspring at low levels of Pb exposure showed impairment of memory retrieval. These impairments were not due to the gross disturbances in motor performance and in vision because these animals performed the fixed location/visible platform task as well as controls, indicating that the specific aspects of spatial learning/memory were impaired. These results suggest that exposure to Pb during the gestational period is sufficient to cause long-term learning/memory deficits in young adult offspring.

Abstract:Importance:Although the effects of lead (Pb) exposure on neurocognition in children have been confirmed, the individual associations of prenatal Pb exposure and its interaction with genetic factors on cognitive developmental delay (CDD) in children remain unclear.Objective:To investigate the association of prenatal Pb exposure and its interaction with genetic factors with CDD risk.Design, Setting, and Participants:Women in Wuhan, China, who had an expected delivery date between March 2014 and December 2017, were recruited for this prospective cohort study. Children were assessed for cognitive development at approximately 2 years of age (March 2016 to December 2019). Maternal venous blood, cord blood, and venous blood from children were collected in a longitudinal follow-up. Data analysis was performed from March 2022 to February 2023.Exposure:Prenatal Pb exposure, and genetic risk for cognitive ability evaluated by polygenic risk score constructed with 58 genetic variations.Main Outcomes and Measures:Cognitive developmental delay of children aged approximately 2 years was assessed using the Chinese revision of the Bayley Scale of Infant Development. A series of multivariable logistic regressions was estimated to determine associations between prenatal Pb exposure and CDD among children with various genetic backgrounds, adjusting for confounding variables.Results:This analysis included 2361 eligible mother-child pairs (1240 boys [52.5%] and 1121 girls [47.5%]; mean [SD] ages of mothers and children, 28.9 [3.6] years and 24.8 [1.0] months, respectively), with 292 children (12.4%) having CDD. Higher maternal Pb levels were significantly associated with increased risk of CDD (highest vs lowest tertile: odds ratio, 1.55; 95% CI, 1.13-2.13), adjusting for demographic confounders. The association of CDD with maternal Pb levels was more evident among children with higher genetic risk (highest vs lowest tertile: odds ratio, 2.59; 95% CI, 1.48-4.55), adjusting for demographic confounders.Conclusions and Relevance:In this cohort study, prenatal Pb exposure was associated with an increased risk of CDD in children, especially in those with a high genetic risk. These findings suggest that prenatal Pb exposure and genetic background may jointly contribute to an increased risk of CDD for children and indicate the possibility for an integrated strategy to assess CDD risk and improve children's cognitive ability.
Prenatal and Early Childhood Exposure to Lead and Repeated Measures of Metabolic Syndrome Risk Indicators From Childhood to Preadolescence

Abstract:Background: Exposure to lead (Pb) during the early life stages has been associated with the development of metabolic syndrome (MetS). Longitudinal studies of Pb exposure in critical developmental windows in children are limited. Methods: Our study included 601 mother-child dyads from the PROGRESS (Programming Research in Obesity, Growth, Environment and Social Stressors) birth cohort. Blood lead levels (BLLs) were assessed during the second and third gestational trimesters, in cord blood at delivery, and at ages 1, 2, and 4 years. Bone lead levels in the patella and tibia were assessed at 1 month postpartum and evaluated in separate models. To account for cumulative exposure (prenatal, postnatal, and cumulative), we dichotomized the BLLs at each stage visit and determined the following: "higher" if a BLL was at least once above the median (HPb) and "lower" if all BLLs were below the median (LPb). We analyzed fasting glucose, HbA1c, triglycerides (TGs), total cholesterol (TC), high-density lipoprotein cholesterol (cHDL), low-density lipoprotein cholesterol (cLDL), body mass index, waist circumference (WC), body fat percentage, and systolic (SBP) and diastolic blood pressure (DBP) at two study visits between 6 and 12 years of age and created cutoff points based on the clinical guidelines for each indicator. Mixed effects models were used to analyze each outcome longitudinally for each BLL score, adjusting for child's sex, size for gestational age, child's age, maternal parity, mother's age, and socioeconomic status. Results: We observed associations for HPb exposure and TC in all stages (OR = 0.53, 95%CI = 0.32-0.86) and postnatally (OR = 0.59, 95%CI = 0.36-0.94) and for prenatal HPb and TGs (OR = 0.65, 95%CI = 0.44-0.95). HPb at all stages was associated with WC (OR = 0.27, 95%CI = 0.08-0.86), BMI (OR = 0.33, 95%CI = 0.11-0.99), SBP (OR = 0.53, 95%CI = 0.32-0.85), and DBP (OR = 0.57, 95%CI = 0.34-0.95). Pb levels in the patella were associated with cHDL (OR = 1.03, 95%CI = 1.00-1.07) and those in the tibia with TGs (OR = 0.95, 95%CI = 0.91-0.99). Conclusion: Early life exposure to Pb may alter early indicators of MetS. A follow-up of these children will allow for more definition on the impact of longer-term exposures.
Associations of Prenatal Lead and Mercury Coexposure with Neurodevelopment

Abstract:Background: Humans are exposed to multiple chemicals, but most toxicologic studies focus on only a single agent, such as Lead, mercury or Manganese. So it can't reflect the truth. Aims: we prospectively examined whether coexposure to lead and mercury causes neurodevelopmental deficiencies that are more severe than expected based on effects of exposure to each metal alone. Methods: Approximately 300 pregnant women who gave birth in hospitals during October 2008 and October 2009 were investigated and contributed a cord blood sample at the time of delivery for lead and mercury determination. Children at the age of 18 months (16 months 15 days to 19 months 15 days) were followed up by the Bayley Scales of Infant Development–III. And a questionaire survey was conducted to analyse related factors affecting 18-month-old children’s physical growth and neurodevelopment. Results: Mean (±SD) cord blood concentrations at delivery were 45.70±19.54 µg/L for lead and 2.35±1.49 µg/L for mercury. Factorial design technique didn’t show any association of coexposure with Cognitive (F=0.231, P=0.631), Language (F = 2.175, P = 0.142), or Motor (F =0.058, P=0.810) development. Conclusions: We didn’t observe evidence of synergism between lead and mercury, whereby lead toxicity was not increased among children with high mercury coexposure.

Abstract:Objective: To assess associations of single and combined exposures to lead and stress during different stages of pregnancy with offspring neurodevelopment.Methods: We measured prenatal lead (maternal blood-lead in early-pregnancy and umbilical-cord-blood-lead) and maternal stress levels in Shanghai-Birth-Cohort from 2013 to 2016. Maternal stress was assessed using Center-for-Epidemiological-Studies-Depression-Scale and Self-Rating-Anxiety-Scale during mid-pregnancy. The Ages-Stages-Questionnaires-3 (at 6/12-months-of-age) and Bayley-III (at 24-months-of-age) were both used to assess neurodevelopment. Results: A total of 2132 mother-child pairs with both prenatal lead and stress measurements were included. The geometric-means of blood-lead in early-pregnancy and cord-blood-lead were 1.46 mu g/dL and 1.33 mu g/dL, respectively. Among the study women, 1.89 % and 0.14 % were screened positive for depression and anxiety. Adjusting for related confounders, the combined exposures had stronger adverse associations with offspring social-emotional skills than single exposures; and the combined exposure in early-pregnancy was associated with greater neurodevelopmental differences than combined exposure around-birth, especially in social-emotion at 24 months-of-age [beta (95 %CI): 10.48(-17.42,-3.54) vs. 5.95(-11.53,-0.36)]. Conclusions: Both single and combined prenatal exposures to lead/stress impaired infant neuro-development, and the effects of combined exposure may be more profound than single exposures. Combined exposure in early pregnancy may be associated with worse neurodevelopmental outcomes than combined exposure around birth, especially in social-emotional development.

Abstract:There is considerable evidence that prenatal lead exposure is detrimental to child cognitive and socio-emotional development. Further evidence suggests that the effects of prenatal lead on developmental outcomes may be conditional upon exposure to social stressors, such as maternal depression and low socioeconomic status. However, no studies have examined associations between these co-occurring stressors during pregnancy and neonatal brain volumes. Leveraging a sample of 101 mother-infant dyads followed beginning in mid-pregnancy, we examined the main effects of prenatal urinary lead levels on neonatal lateralized brain volumes (left and right hippocampus, amygdala, cerebellum, frontal lobes) and total gray matter. We additionally tested for moderations between lead and depressive symptoms and between lead and family income relative to the federal poverty level (FPL) on the same neurodevelopmental outcomes. Analyses of main effects indicated that prenatal lead was significantly ( ps < 0.05) associated with reduced right and left amygdala volumes ( βs = −0.23- -0.20). The testing and probing of cross-product interaction terms using simple slopes indicated that the negative effect of lead on the left amygdala was conditional upon mothers having low depressive symptoms or high income relative to the FPL. We interpret the results in the context of trajectories of prenatal and postnatal brain development and susceptibility to low levels of prenatal lead in the context of other social stressors.

Abstract:Human &Experimental Toxicology, Volume 43, Issue , January-December 2024. Lead (Pb) has become a major environmental contaminant. There are several ways in which lead can enter the human body and cause toxic effects on human health. This review focuses on the impact of lead toxicity at prenatal and early life stages and its effect on neurodevelopment. Lead exposure to the developing foetus targets foetal neural stem cells. Hence, it has detrimental effects on developing neural and glial cells, adversely influencing cognition and behaviour. Lead has a profound influence on the movement of calcium ions (Ca2+), which can be attributed to most of the mechanisms by which lead affects neurodevelopment. There is no known safe threshold of lead exposure for children. Lead can affect foetal neurodevelopment leading to various neurological disorders, and neurotoxic effects on behavioural and cognitive outcomes. In this review, we discuss prenatal and early-life lead exposure, its mechanism, and consequences for neurodegenerative disorders such as Alzheimer's disease and Parkinson's disease in later stages of life. This review further highlights the importance of lead exposure during pregnancy and lactation periods as well as early development of the child in understanding the extent of lead-induced neurological damage to the foetus/children and the associated future risks.

Abstract:Purpose: Our pilot studies showed that there was a significant relationship between blood lead levels of women at the first trimester and scores of neonatal behavioral neurological assessment (NBNA). This study went further (1) to determine particular neurotoxicity during a specific trimester, (2) to analyze "safe" levels of Pb in neonates, and (3) to identify influencing factors for prenatal Pb exposure.Method: A total of 415 mothers with newborn located Shenzhen, Guangdong, China participated in the study: 219 in the high lead group [blood lead levels (BLLs) at first trimester >= 4.89 mu g/dl and 196 in the low lead group [BLLs <= 1.96 mu g/dl]. The maternal BLLs at each stage of pregnancy and delivery were measured by atomic absorption spectrophotometry, equipped with a graphite furnace. The developmental functioning of newborns was assessed with NBNA in 3 days. The children's birth outcome and the rest of information was obtained from their medical records or a comprehensive questionnaire from their parents, which contained demographic characteristics, lifestyle, IQ occupation and influencing factors for lead exposure during and before first trimester, etc.Results: Of 415 newborns, 332 (80.00%) had complete data collection for all variables at four-stage follow-up. The maternal mean BLL at first trimester for 332 newborns was 3.98 +/- 1.15 mu g/dl (0.38 - 15.86 mu g/dl) and the geometric mean (GM) was 3.63 +/- 0.35 mu g/dl (95%Cl: 2.98-4.32 mu g/dl). In total, about 4.82% of newborns had maternal BLLs > 10 mu g/dl. Significant inverse associations have been found between the maternal BLLs at the first trimester and the NBNA scores (P < 0.05). Drinking milk and supplements of Ca, Fe, or Zn are protective factors of high BLLs (OR = 0.363, P < 0.05).Conclusion: Our study demonstrate that fetal lead exposure as low as 5 mu g/dl has an adverse effect on neurodevelopment, most expressed during the first trimester and best arrested by measuring maternal BLLs. The collective evidence indicates that screening and intervention after the first trimester may be too late to prevent the fetal neurotoxic effects. (C) 2014 Elsevier Inc. All rights reserved.

Abstract:Background and aim: To assess the current exposure levels of lead and stress during pregnancy, and compared the effects of prenatal combined exposure of lead and stress during different stages of pregnancy on the neurodevelopment of offspring. Methods: A total of 5270 mother-child pairs were assessed in the Shanghai Birth Cohort, maternal blood lead in early pregnancy and umbilical cord blood lead were measured. Assessments of prenatal maternal stress, the Center for Epidemiological Studies-Depression Scale and the Self-Rating Anxiety Scale during mid-pregnancy, postnatal maternal stress using the Edinburgh Postnatal Depression Scale were conducted, and groups of prenatal single exposure to lead/stress and combined exposure were therefore formed. The Ages-Stages-Questionnaires (at 6- and 12-months-of-age) and the Bayley-III (at 24-months-of-age) were performed to assess the offspring neurodevelopment. Results: The geometric means of maternal blood lead in early pregnancy and umbilical cord blood lead were 1.45 μg/dL and 1.53 μg/dL, respectively. A total of 1.86% and 0.18% of the mothers were screened positive for depression and anxiety. Adjusting for related confounders, the prenatal combined exposure in early pregnancy was associated with more deficits in infant communication skills and social-emotional skills, especially in social-emotional at 24-month-old (P=0.016); the prenatal combined exposure around birth was adversely associated with infant neurodevelopment levels in multiple domains. Conclusions: The prenatal combined exposure to lead and stress may affect offspring neurodevelopment more profoundly (especially on social-emotional development), and the combined exposure in early pregnancy may induce more deficits in offspring social-emotional development than the combined exposure around birth. Abstract:Objective To explore the effects of prenatal exposure to stress and lead on spatial learning and memory development in rats. Methods All 32 Sprague-Dawley (SD) pregnant rats were divided randomly into 4 groups, 8 per group in line with the Random Number Table. The four groups were: no maternal stress, no Pb exposure (NS/C) ; non-maternal stress, Pb exposure (NS/L), maternal stress, no Pb exposure (S/C), and maternal stress plus Pb exposure (S/L). The spatial learning and memory ability, the serum corticosterane level both pre and post-testing of 30-day old offsprings, and lead concentration in hippocampns were tested by means of Morris Water Maze, radioimmunoassay and Inductively Coupled Plaswa Mass Spectrometry(ICP-MS). Results The residence time of male and female in NS/L was ( 16. 08±3.41 ) s, ( 15.72 ± 3.33 ) s, which were significantly shorter than NS/L (25.42 ± 4. 76) s, (24. 55± 4. 43 ) s and S/C ( 20. 96±3.45 ) s, ( 20. 65 ±2. 98 ) s, and significant difference was observed in the joint exposure effect( F = 5. 478, P < 0. 05 ). The effect of the joint exposure was significant on pest-testing serum eorticosterone. The hippocampus lead concentrations of NS/L and S/L were (0. 4378±0. 1041 ) μg/g and ( 0. 4679±0. 1243 ) μg/g without significant differences ( F = 0. 298, P 0. 05). Conclusion Prenatal joint exposure to restraint stress and lead might increase the effects of single exposure on the spatial learning and memory ability and serum corticosterone level of offsprings, and the joint influence on cortieosterone level might be one of the reasons of further impairment of learning and memory.
