Abstract:Background: Children are vulnerable to the respiratory effects of air pollution, and their lung function has been associated with long-term exposure to low air pollution level in developed countries. However, the impact of contemporary air pollution level in developing countries as a result of recent efforts to improve air quality on children's lung function is less understood. Methods: We obtained a cross-sectional sample of 617 schoolchildren living in three differently polluted areas in Anhui province, China. 2-year average concentrations of air pollutants at the year of spirometry and the previous year (2017–2018) obtained from district-level air monitoring stations were used to characterize long-term exposure. Forced vital capacity (FVC), forced expiratory volume in 1 second (FEV 1 ), and forced expiratory flow between 25 and 75% of FVC (FEF 25−75 ) were determined under strict quality control. Multivariable regression was employed to evaluate the associations between air pollution level and lung function parameters, overall and by demographic characteristics, lifestyle, and vitamin D that was determined by liquid chromatography tandem mass spectrometry. Results: Mean concentration of fine particulate matter was 44.7 μg/m 3 , which is slightly above the interim target 1 standard of the World Health Organization. After adjusting for confounders, FVC, FEV 1 , and FEF 25−75 showed inverse trends with increasing air pollution levels, with children in high exposure group exhibiting 87.9 [95% confidence interval (CI): 9.5, 166.4] mL decrement in FEV 1 and 195.3 (95% CI: 30.5, 360.1) mL/s decrement in FEF 25−75 compared with those in low exposure group. Additionally, the above negative associations were more pronounced among those who were younger, girls, not exposed to secondhand smoke, non-overweight, physically inactive, or vitamin D deficient. Conclusions: Our study suggests that long-term exposure to relatively high air pollution was associated with impaired lung function in children. More stringent pollution control measures and intervention strategies accounting for effect modification are needed for vulnerable populations in China and other developing countries.

Indoor Air Pollution and Lung Cancer in China

Abstract:Although indoor air pollution has been suggested as a risk factor for lung cancer, it has not been fully studied in Chinese population. This study aims to investigate associations between cooking, solid fuel usage, environmental tobacco smoking exposure, housing characteristics and lung cancer in an urban Chinese population, with a focus on nonsmoking women. We conducted a case-control study in Taiyuan, China, consisting of 399 lung cancer cases and 466 healthy controls, of which 164 cases and 218 controls were female non-smokers. Indoor PM concentrations, including PM1, PM2.5, PM7, PM10 and TSP, were measured using a particle mass monitor. Unconditional logistic regression models were used to calculate odds ratios (ORs) and 95% confidence intervals after adjusting for age, education level, annual personal income and packyears of smoking (when the analyses included smokers). Among non-smoking women, lung cancer was strongly associated with multiple sources of indoor air pollution, including heavy exposure to ETS at work (aOR=3.65), high frequency of cooking (aOR=3.30), and solid fuel usage for cooking (aOR=4.97) and heating (aORcoal stove=2.00, aORheatable brick bed =8.1). Housing characteristics related to poor ventilation, including single-story houses, less window area, no separate kitchen, no kitchen ventilator and rarely having windows open, are associated with lung cancer, with aORs varying between 1.8-3.0. Indoor mean PM levels in the study population are much higher than WHO guidelines for ambient PM. Mean PM2.5 concentration was 74ug/m , and PM10 was 232ug/m . PM levels in winter are strongly correlated with solid fuel usage for cooking and heating, as well as use of ventilators. PM1 levels in cases’ houses are more than 3 times higher than that in controls’ houses. Our results suggest that exposure to indoor air pollution may play an important role in the development of lung cancer among Chinese female non-smokers. Both reducing pollutant emission and improving ventilation may be preventive measures in control of PM levels inside the house, in order to reduce the development of lung cancer.

Abstract:Background: China has been facing nationwide air pollution at unprecedented high levels primarily from fossil-fuel combustion in the past decade. However, few studies have been conducted on the adverse effect of severe air pollution on lung development in school-age children. Methods: Using wellness check and air pollution data from 2014 to 2017, we conducted a retrospective analysis of lung development in 21 616 school-age children from Shijiazhuang and Qingdao from North China with severe vs mild air pollution. Linear mixed effects model was performed to assess the effect of air pollution on forced vital capacity (FVC) growth. Results: Exposure to severe air pollution was associated with a dramatic reduction in annual FVC growth rate (-71.3 mL, p< 0.001). In addition, every 10 μg/m3 increase in annual PM2.5 level was associated with a reduction of annual FVC growth by 12.2 mL ( p< 0.001). Sex discrepancy (boys vs girls) in FVC growth was greater in Qingdao (35.4 mL/year, 95% CI: 26.0 to 44.7) than in Shijiazhuang (19.8 mL/year, 95% CI: 9.3 to 30.3) (p for interaction=0.063). Exposure to indoor coal- or wood-burning stove heating (-79.4 mL, p< 0.001) and secondhand smoke at home (-59.3 mL, p= 0.003) were inversely associated with FVC growth. Conclusion: Our study raised serious alarm over the threat of severe air pollution to lung development in school-age children. Sex discrepancy in lung development was reduced dramatically in heavily polluted area.

Abstract:BACKGROUND Although evidence suggests a weak association of single environmental risk factors with asthma and allergy risk in children, the influence of a combination of multiple environmental pollutants and household air pollution in the relationship of asthma and allergy remains unclear. We aimed to assessed the association of environmental composite quality index scores with asthma and allergy. METHOD Environmental composite quality indices were collected using the questionnaire that contains asthma and allergy, Chinese residential living habits, and outdoor environment items. Multivariable odds ratios (ORs) and 95% confidence intervals (CIs) were estimated using the binary logistic regression model after adjusting the most known asthma or allergy risk factors. RESULTS In this cross-sectional study, a total of 1636 girls and 1446 boys were included in the final analyses. Higher environmental quality index score was correlated with higher risk of asthma, eczema, and rhinitis. Comparing the top (≥5) to bottom group (0-2), the multivariable ORs were 2.47 (95% CI, 1.41-4.33, P trend = 0.004) for asthma, 1.51 for eczema (95% CI, 1.18-1.93; P trend = 0.002) and 1.83 for rhinitis (95% CI, 1.12-3.00; P trend = 0.017). Similar findings were observed in the stratified analyses. CONCLUSION The study showed that there is a harmful association between environmental quality and asthma with allergic diseases in children living in northeast China, and further research was needed to confirm the findings in other areas and clarify the specific mechanism.

Abstract:Background: Few studies have evaluated the effects of ambient air pollution exposure on lung function, especially in areas with high air pollution levels. Objectives: To investigate the associations of annual concentrations of particulate matter with diameters < 2.5 mu m (PM2.5) and nitrogen dioxide (NO2) with adult lung function in Shanghai, China. Methods: We included 5276 permanent residents aged >= 20 years. Annual residential exposure to PM2.5 and NO2 was estimated by validated satellite-based and land use regression models, respectively. The effects of PM2.5 and NO2 on lung function were estimated separately using multivariable linear regression, adjusting for potential confounders. Results: Higher exposure to PM2.5 and NO2 was significantly associated with lower forced vital capacity (FVC), inspiration capacity (IC), and vital capacity (VC). An increase of 10 mu g/m(3) in the annual average PM2.5 exposure was associated with a 45.83 ml (95% CI: -82.59, -9.07) lower FVC, 1.36 (95% CI: -2.42, -0.29) lower FVC of % predicted (FVC%pred), 121.98 ml (95% CI: -164.38, -79.57) lower IC, and 89.12 ml (95% CI -124.94, -53.3) lower VC. For NO2, an increase of 10 mu g/m3 in the annual average concentration was associated with 26.65 ml (95% CI: -46.29, -7.00) lower FVC, 0.70 (95% CI: -1.27, 0.13) lower FVC%pred, 65.26 ml (95% CI: -87.76, -42.76) lower IC, and 45.88 ml (95% CI: -65.03, -26.73) lower VC. The estimated effects on FEV1 were -10.25 ml (95% CI: -40.92, 20.42) and -0.29% (95% CI: -1.40, 0.82) per 10 mu g/m(3) increase in PM2.5 and -0.74 ml (95% CI: -17.13, 15.65) and 0.01% (95% CI: -0.58, 0.61) per 10 mu g/m(3) increase in NO2, which were not statistically significant. Stratified analysis showed that the estimated effects of PM2.5 were greater in the healthy subgroup than the COPD patients. Obese individuals were more susceptible to adverse effects of PM2.5 and NO2 on lung function. Education level showed no or only weak evidence of modification of the associations between air pollution and lung function. Conclusion: In this study, long-term exposure to ambient air pollutants was significantly associated with impaired lung function, presenting as restrictive ventilatory patterns.

Abstract:No prior cohort studies exist in China examining the association of outdoor air pollution with mortality. We studied 70,947 middle-aged men and women in the China National Hypertension Survey and its follow-up study. Baseline data were obtained in 1991 using a standard protocol. The follow-up evaluation was conducted in 1999 and 2000. Annual average air pollution exposure between 1991 and 2000, including total suspended particle (TSP), sulfur dioxide (SO2) and nitrogen oxides (NOx), were estimated by linking fixed-site monitoring data with resident zip code. We examined the association of air pollution with mortality using proportional hazards regression model. We found significant associations between air pollution levels and mortality from cardiopulmonary diseases and from lung cancer. Each 10μg/m3 elevation of TSP, SO2 and NOx was associated with a 0.9% (95%CI: 0.3%, 1.5%), 3.2% (95%CI: 2.3%, 4.0%), and 2.3% (95%CI: 0.6%, 4.1%) increased risk of cardiovascular mortality, respectively. We found significant effects of SO2 on mortality after adjustment for TSP. Conclusively, ambient air pollution was associated with increased cardiopulmonary and lung cancer mortality in China. These data contribute to the scientific literature on long-term effects of air pollution for high exposure settings typical in developing countries.

We aimed to examine the association between long-term exposure to outdoor air pollution and mortality in a nationally representative cohort in China. Methods: We studied 98,904 middle-aged men and women in the China National Hypertension Survey and its follow-up study. Baseline data on the participants'demographic characteristics, medical history, lifestyle-related risk factors, blood pressure, and body weight were obtained in 1991 with the use of a standard protocol. The follow-up evaluation was conducted in 1999 and 2000. Annual average air pollution exposure between 1991 and 2000, including total suspended particle (TSP), sulfur dioxide (SO2), and nitrogen oxides (NOx), were estimated by linking fixed-site monitoring data with resident zip code. We examined the association of air pollution with mortality, using proportional hazards regression models, with adjustment for a wide range of individual risk factors. Results: We found significant associations between air pollutants (TSP, SO2, and NOX) and mortality from all nonaccidental causes and from cardiopulmonary diseases and lung cancer. Each 10 μg/m3 elevation of TSP, SO2, and NOx was associated with a 0.2% (95% CI: −0.1%, 0.5%), 1.9% (95% CI: 1.4%, 2.3%), and 1.4% (95% CI: 0.4%, 2.4%) increased risk of all-cause mortality, respectively. We found significant effects of gaseous pollutants (SO2 and NOx) on mortality after adjustment for TSP. Our analysis also provided preliminary, but not conclusive, evidence that females, never-smokers, and obese people might be more vulnerable to air pollution than males, current or past smokers, and people who are not obese. Conclusion: Our study generated the first prospective evidence of the association between air pollution and mortality in China. These data contribute to the scientific literature on long-term effects of air pollution for high exposure settings typical in developing countries.

Abstract:Abstract Background Increasing evidence is appearing that ozone has adverse effects on health. However, the association between long-term ozone exposure and lung function is still inconclusive. Objectives To investigate the associations between long-term exposure to ozone and lung function in Chinese young adults. Methods We conducted a prospective cohort study among 1594 college students with a mean age of 19.2 years at baseline in Shandong, China from September 2020 to September 2021. Lung function indicators were measured in September 2020 and September 2021, including forced vital capacity (FVC), forced expiratory volume in 1 s (FEV1), forced expiratory flow at the 25th, 50th, and 75th percentile of the FVC (FEF25, FEF50, and FEF75) and mean flow rate between 25% and 75% of the FVC (FEF25-75) were measured. Daily 10 km×10 km ozone concentrations come from a well-validated data-fusion approach. The time-weighted average concentrations in 12 months before the lung function test were defined as the long-term ozone exposure. The associations between long-term ozone exposure and lung function indicators in Chinese young adults were investigated using a linear mixed effects model, followed by stratified analyses regarding sex, BMI and history of respiratory diseases. Results Each interquartile range (IQR) (8.9 µg/m3) increase in long-term ozone exposure were associated with a -204.3 (95% confidence interval (CI): -361.6, -47.0) ml/s, -146.3 (95% CI: -264.1, -28.4) ml/s, and − 132.8 (95% CI: -239.2, -26.4) ml/s change in FEF25, FEF50, and FEF25-75, respectively. Stronger adverse associations were found in female participants or those with BMI ≥ 24 kg/m2 and history of respiratory diseases. Conclusion Long-term exposure to ambient ozone is associated with impaired small airway indicators in Chinese young adults. Females, participants with BMI ≥ 24 kg/m2 and a history of respiratory disease have stronger associations.

Abstract:Although China has achieved great progress in ambient pollution reduction in the past two decades, it is still one of the few countries with the worst air pollution levels in the world. In addition to lifestyle choices such as smoking, diet and exercise, exposure to outdoor air pollution is being considered a major determinant of cardiovascular disease in the Chinese population. Numerous epidemiological studies on air pollution and cardiovascular mortality have been conducted in China, using time-series, case-crossover, or cross-sectional designs. The increased cardiovascular mortality risks observed in the Chinese population are similar in magnitude, per amount of pollution, to the risks found in other parts of the world. However, the importance of these increased cardiovascular risks is greater than in North America or Europe, because the air pollution in China is at much higher levels in general and the Chinese population accounts for more than one fourth of the world’s total. There has been no air pollution cohort study in China examining the long-term effects of air pollution, nor any published Chinese data assessing the relation between air pollution and cardiovascular morbidity and sub-clinical indicators. Future research in China should focus on a prospective analysis of association between air pollution and cardiovascular disease and the likely underlying pathophysiologic links.
The Association Between High Ambient Air Pollution Exposure and Respiratory Health of Young Children: A Cross Sectional Study in Jinan, China

Abstract:There is growing concern about health effects of high air pollution in less-developed countries. Children represent a population at increased risk for air pollution-related respiratory conditions. This study investigated the relationship between high ambient air pollution exposure and respiratory health of young schoolchildren. From 2014 to 2016 in Jinan, China, 2532 primary school children in grades three to five from two different schools with different levels of air pollution were included in the study. Levels of ambient air pollution exposure including PM10, PM2.5, NO2, SO2, CO and O-3 were measured continuously at the two schools. A questionnaire about children's respiratory health was conducted every year. Among them, about 150 randomly selected children also performed lung function tests two times a year at the beginning of November and middle of December. Annual average exposure levels of PM2.5 (66.8-79.1 vs 90.0-107.7 mu g/m(3)), PM10 (129.5-177.3 vs 198.1-218.6 mu g/ m(3)), NO2 (45.3-53.2 vs 45.0-56.2 mu g/m(3)), SO2 (29.8-56.5 vs 40.5-80.3 mu g/m(3)), CO (1.3-1.5 vs 1.4-1.7 mg/m(3)) and O-3 (84.8-120.2 vs 61.1-128.1 mu g/m(3)) in the heavy pollution primary school were significantly higher than the light one. The higher air pollution exposure was related to increased prevalence of respiratory diseases of young children in the last year, especially allergic rhinitis. The increased odds of lung function impairment associated with exposure to higher air pollution, could be up to 171.5% (aOR = 2.715; 95% CI = 1.915-3.849) for PEF < 75% predicted in 2014. However, after short-termexposure for 1.5 month or a week, paired comparison for parameters of the same child showed different results. The association between high ambient air pollution exposure and respiratory health of young children is closely related to exposure time and dose and may be fluctuate and complex. Abstract:The associations of long-term exposure to various constituents of fine particulate matter (≤2.5 μm in aerodynamic diameter, PM2.5) air pollution with lung function were not clearly elucidated in developing countries. The aim was to evaluate the associations of long-term exposure to main constituents of PM2.5 with lung function in China. This is a nationwide, cross-sectional analysis among 50,991 study participants from the China Pulmonary Health study. Multivariable linear regression models were used to obtain differences of forced expiratory volume in 1 s (FEV1), forced vital capacity (FVC), FEV1/FVC, peak expiratory flow (PEF), and forced expiratory flow at 25-75% of exhaled FVC (FEF25-75%) associated with an interquartile range (IQR) change of PM2.5 or its constituents. Residential annual PM2.5 levels varied from 26 μg/m3 to 92 μg/m3 (average: 53 μg/m3). An IQR increase of PM2.5 concentrations was associated with lower FEV1 (19.82 mL, 95% CI: 11.30-28.33), FVC (17.45 mL, 95% CI: 7.16-27.74), PEF (86.64 mL/s, 95% CI: 59.77-113.52), and FEF25-75% (31.93 mL/s, 95% CI: 16.64-47.22). Black carbon, organic matter, ammonium, sulfate, and nitrate were negatively associated with most lung function indicators, with organic matter and nitrate showing consistently larger magnitude of associations than PM2.5 mass. This large-scale study provides first-hand epidemiological evidence that long-term exposure to ambient PM2.5 and some constituents, especially organic matter and nitrate, were associated with lower large- and small- airway function.

Abstract:China’s economy has developed rapidly in the recent two decades. Economic development is usually linked with increase in energy consumption and consumption emissions, which in turn leads to worsening of air quality. Due to the adoption of various control measures, the ambient air quality in a number of large cities in China has actually improved. The ambient air TSP and SO2 levels in China have been decreasing in the last decade. However, ambient air NOx level has been increasing due to the increased number of motor vehicles. Coal has been and is still the major source of energy in China. Ambient air pollution in large cities has changed from the conventional coal combustion type to the mixed coal combustion/motor vehicle emission type. A series of epidemiological studies on air pollution and health effects ranging from mortality, morbidity to functional changes have been conducted in China. The results showed that ambient air pollution had acute and chronic effects on mortality, morbidity, hospital admissions, clinical symptoms, lung function changes, etc. The exposure–response relationship between air pollutants and daily mortality, morbidity, hospital admissions, and lung function has been established accordingly.

Abstract:BACKGROUND: Studies on the association of greenness with respiratory health are scarce in developing countries, and previous studies in China have focused on only one or two indicators of lung function.OBJECTIVE: The study aims to evaluate the associations of residential greenness with full-spectrum lung function indicators and prevalence of chronic obstructive pulmonary disease (COPD).METHODS: This nationwide cross-sectional survey included 50,991 participants from the China Pulmonary Health study. Lung function indicators included four categories: indicators of obstructive ventilatory dysfunction (FEV1, FVC and FEV1/FVC); an indicator of large-airway dysfunction (PEF); indicators of small-airway dysfunction (FEF25-75% and FEV3/FEV6); and other indicators. Residential greenness was assessed by the Normalized Difference Vegetation Index (NDVI). Multivariable linear regression models and logistic regression models were used to analyze associations of greenness with lung function and COPD prevalence.RESULTS: Within the 500 m buffer, an interquartile range (IQR) increase in NDVI was associated with higher FEV1 (24.76 mL), FVC (16.52 mL), FEV1/FVC (0.38), FEF50% (56.34 mL/s), FEF75% (33.43 mL/s), FEF25-75% (60.73 mL/s), FEV3 (18.59 mL), and FEV6 (21.85 mL). However, NDVI was associated with lower PEF. In addition, NDVI was significantly associated with 10% lower odds of COPD. The stratified analyses found that the associations were only significant in middle-young people, females, and nonsmokers. The associations were influenced by geographic regions.CONCLUSIONS: Residential greenness was associated with better lung function and lower odds of COPD in China. These findings provide a scientific basis for healthy community planning.

Abstract:Background: China has experienced increasingly severe levels of air pollution in the past decades, yet studies on the health effects of air pollution in China at a national study level, remain limited. This study assess the sub-chronic effect of ambient air pollution on respiratory mortality in the 32 largest Chinese cities.Methods: We employ two-way fixed effects panel data analysis and monthly air pollution and mortality panel data. We estimate associations between monthly respiratory mortality and air pollution; pollution is defined as particulate matter with aerodynamic diameter <10 mu m. We adjust for city characteristics, seasonality (monthly effects), and weather conditions (precipitation and temperature). We examine the associations between monthly injury mortality and air pollution to check for robustness.Results: The results show positive and statistically significant associations of air pollution with respiratory mortality. During the study period (2006-2010) a 10 mu g/m(3) increase in monthly PM10 concentration is associated with a 1.05% (95% Cl, 0.08-2.04%) increase in adult respiratory mortality rate. The air pollution effect is the most salient in northern cities (with central heating system) during the cold season (October-April); a 10 mu g/m(3) increase in monthly PM10 concentrations is associated with a 1.62% (95% Cl, 0.22-3.46%) increase in the elderly respiratory mortality rate. There is no statistically significant association between the young adult respiratory mortality and air pollution.Conclusions: The elderly respiratory mortality rate in China is positively and statistically significantly associated with air pollution. The effect is largest in northern cities during cold months when coal is burned for heating. (C) 2014 Elsevier Inc. All rights reserved.
The effects of indoor and outdoor air pollution on the prevalence of adults' respiratory diseases in four Chinese cities: a comparison between 2017-2018 and 1993-1996

Abstract:Background: Over the past decades, both ambient and household air pollution have changed in several aspects, including the emission sources and the concentrations of pollutants, in many Chinese cities. It is unknown whether these changes are associated with changes in health conditions, especially given changes in other factors due to rapid economic growth. Methods: Two cross-sectional surveys were conducted in two periods spanning more than twenty years (1993-1996 vs. 2017-2018) in four Chinese cities of Chongqing, Wuhan, Lanzhou, and Guangzhou. Data were collected regarding adults' respiratory disease, smoking status, education, occupation, and household characteristics. Ambient air pollution data were obtained for each study. We first used logistic regression models to construct the district-specific adjusted disease prevalences. In the second stage, first-difference regression models were employed to examine whether the change in respiratory diseases prevalences was associated with the change in outdoor air pollution and indoor air pollution surrogates. Results: A total of 7,557 and 9,974 households were participating in Period 1 (1993-1996) and Period 2 (2017-2018), respectively. Compared to Period 1, we found substantial reductions in the ambient air pollution concentrations, and a suggestive improvement in cooking-related indoor air pollution in Period 2. We observed decreases in the district-specific covariate-adjusted prevalences of both asthma and chronic bronchitis among participants, with an average reduction of 3.6% (range: 0.0% to 24.3%). From Period 1 to Period 2, one percent decrease in the proportion of cooking with coal was associated with a 19.0% (95% CI, 0.96-37.04%) decrease in the prevalence of males' chronic bronchitis and a 1.86% (0.69-3.04%) increase in the prevalence of females' asthma. Little evidence was observed regarding the potential health benefits associated with the decreases in ambient air pollution levels. Conclusions: The substantial reduction in household use of coal for cooking might be an important contributor to the decrease in adults' respiratory disease prevalence from 1993-1996 to 2017-2019 in four Chinese cities. Changes in this indoor air pollution source, along with other risk factors for respiratory diseases, may have masked respiratory health benefits associated with reductions in outdoor air pollution levels.
