Abstract:Endovascular therapy brings a breakthrough progress in treatment of ischemic patients with acute large artery occlusion. Recent years studies have shown that endovascular therapy is more highly time-dependent than intravenous thrombolysis in terms of clinical outcome. To reduce reperfusion time delay is important for the management and development of endovascular therapy. Optimization of emergency rescue work flow, utilization of rapid and effective imaging evaluation system and development of fast recanalization method of thrombectomy will be helpful in clinical promotion and application of endovascular therapy in treatment of acute ischemic stroke.
Interventional technique for recanalization of chronic total renal artery occlusion

Abstract:Objective: To evaluate the interventional technique for recanalization of chronic total occlusion (CTO) of renal artery. Methods: Twelve patients with CTO of renal artery were treated with interventional procedures (percutaneous transluminal angioplasty, percutaneous transluminal angioplasty and stenting, and intra-arterial thrombolysis). Therapeutic effect of interventional procedures was assessed with angiography, the technical success rate was evaluated, and clinical benefits during following-up was assessed. Results: Revascularization was obtained in 10 patients, the success rate was 83.33%, which also got clinical improvement. Conclusion: Revascularization of renal artery CTO can be done with interventional technique, benefiting for these patients.
Superselective Transcatheter Renal Artery Embolization for the Treatment of Hemorrhage from Non-Iatrogenic Blunt Renal Trauma: Report of 16 Clinical Cases

Abstract:Objective: To explore the therapeutic efficacy and outcome of superselective transcatheter renal artery embolization for the treatment of hemorrhage from non-iatrogenic blunt renal trauma (BRT). Methods: Sixteen patients who received superselective transcatheter renal artery embolization for non-iatrogenic BRT hemorrhage between January 2003 and December 2012 were reviewed retrospectively. Spring steel coils with gelatin sponge particles were used to embolize branches of the renal artery in 15 patients with injuries to the segmental or distal renal arteries; superselective internal iliac artery branch embolization was used to occlude hemorrhage from the branch of the renal artery in two patients with pelvic fracture complicated with internal iliac artery branch laceration; and balloon catheter occlusion was used to embolize the branch of the renal artery in one patient with renal artery trunk laceration. Results: Embolization was achieved successfully in a one-stop procedure in all cases. The patient who received balloon catheter occlusion for renal artery trunk laceration was transferred immediately to surgery for emergency nephrectomy. Another patient died of intracranial trauma 1 day after surgery, although macroscopic hematuria disappeared at the time. Macroscopic hematuria disappeared within 1 day after surgery in the other 14 patients. Follow-up visits at times ranging from 6 months to 9 years after the procedure showed normal renal function without evidence of complications in all surviving patients. Conclusion: Superselective transcatheter renal artery embolization is an effective minimally invasive therapy for the treatment of BRT hemorrhage.

Abstract:The aim of the present study was to determine the accuracy of multi-section computed tomographic angiography (CTA) for the diagnosis of renal arterial h=emorrhage in comparison with digital subtraction angiography (DSA) and the effect of diagnostic outcome on therapeutic patient management. A retrospective analysis was conducted of the records of all patients undergoing DSA for acute renal hemorrhage between August 2005 and June 2014. A total of 74 patients were identified in whom CTA was performed prior to the DSA. DSA detected a renal arterial lesion in 68 cases and no bleeding was demonstrated in 6 cases. CTA correctly identified the bleeding site in 59 of the 68 DSA-positive cases. There were 14 patients with negative CTA results that underwent DSA after 2-10 days because of persistent hematuria, and 9 of them were found to have positive DSA results. The sensitivity, specificity, positive predictive value and negative predictive value of CTA for the detection of arterial lesions in the context of acute renal hemorrhage were 0.868, 0.833, 0.983 and 0.357, respectively. All patients with bleeding sites identified by DSA underwent superselective embolization in a single session. Five patients underwent a secondly embolization for recurrent hemorrhage. These results indicate that CTA is an accurate technique for the detection of major arterial hemorrhage and supports its use in the initial examination of these patients.

Abstract:Objective: To describe our experience with the treatment of renal artery aneurysms (RAAs) and renal arteriovenous fistulas (RAVFs) by transcatheter techniques with special consideration given to indications, technical options, and complications.Methods: Over the last 7 years (2004-2011), endovascular treatment of nine RAAs and six RAVFs in 15 patients (11 women; mean [standard deviation] age, 42 [15] years; range, 18-75 years) was retrospectively reviewed. Seven aneurysms and six arteriovenous fistulas were treated with coil embolization. One aneurysm was treated with the stent graft, and the other aneurysm was treated with coil embolization combined with stent graft. Electronic medical charts were reviewed, and demographic, clinical, procedural, and follow-up data were analyzed.Results: The lesion was asymptomatic in seven patients and symptomatic in eight patients, including ruptures in two patients. The most common comorbidity and associated risk factor was hypertension (n = 8). The technical success rate was 100%. There was no periprocedural mortality or major complications. The only complication was postembolization syndrome in nine patients. Mean clinical follow-up was 24.7 months, and mean imaging follow-up was 16.3 months. During the imaging follow-up, partial renal infarcts were detected in six patients, with no evidence of renal insufficiency. No recurrence was observed.Conclusions: At our institution, endovascular therapy represents the first-line treatment of RAAs and RAVFs. Postembolization syndrome and segmental renal infarcts are common events but were not found to be clinically significant. (J Vasc Surg 2013;57:765-70.)
Interventional treatment of the complications occurring after renal transplantation: a clinical study

Abstract:Objective To evaluate the safety and effectiveness of multi-mode interventional therapy for complications occurring after renal transplantation.Methods The clinical data of 26 patients with complications occurring after renal transplantation were retrospectively analyzed.Vascular and non-vascular complications were treated with different interventional techniques,and the curative effects were analyzed.Results Vascular complications were observed in 19 patients and non-vascular complications were seen in 7patients.The technical success rate of interventional therapy was 100％.One week after the treatment,the total creatinine level (SCr) was significantly improved,which decreased from preoperative (372.7±295.5)μmol/L to postoperative (184.3±138.4) μmol/L (P＜0.001).No severe complications occurred.Further analysis indicated that no statistically significant differences in patient's general condition,kidney donor source,anastomosis method existed between vascular intervention group and non-vascular intervention (P＞0.05).However,the onset time of vascular complications was markedly earlier than that of non-vascular complications (1.8± 1.4 months vs.118.3 ±54.4 months),the difference was statistically significant (P＜0.001).During the follow-up period lasting for 4-55 months (mean of 18.6 months),3 patients developed recurrence of complications;interventional therapy had to be carried out in 2 patients and their creatinine level returned to normal after treatment,and transplanted renal artery embolization had to be performed in the other patient as whose pseudoaneurysm became enlarged.Conclusion For the treatment of complications occurring after renal transplantation,interventional therapy is less-invasive,rapidly-effective and safe,this technique can timely and effectively improve the renal function and save the transplanted kidney.
Chimney Stent Graft Technique for Endovascular Repair of Penetrating Atherosclerotic Ulcers of Abdominal Aorta with Bilateral Common Iliac Artery Aneurysms and Ectopic Right Renal Artery Stenosis

Abstract:Ectopic renal artery (RA), a rare congenital anomaly, presents a challenge in open surgery and endovascular repair for abdominal aortic lesions. To preserve the abnormal main blood supply and renal function complicates the procedures requiring careful imaging measurement and well-designed therapeutic strategy. We report on a 68-year-old man with abdominal aortic atherosclerotic ulcers, bilateral common iliac artery aneurysms, and right ectopic RA stenosis that was successfully managed with antegrade chimney technique, resulting in well-excluded lesions and well-reconstructed renal blood supply without any complication. This endovascular procedure may be useful for repair of abdominal aortic lesions with ectopic RA in high-risk surgical patients.
Thrombus aspiration as a bailout procedure during percutaneous renal angioplasty

Abstract:Purpose: To present a case in which thrombus aspiration, urokinase, and abciximab were used to recanalize a sudden acute thrombotic occlusion of the right renal artery during percutaneous renal angioplasty. Case report: A 72-year-old man with severe arterial hypertension, impaired renal function, and peripheral artery disease was referred for interventional renal revascularization of a proximal stenosis of the right renal artery. Predilation was unsuccessful, and stent placement was followed by immediate occlusion of the distal renal artery, probably due to dislocation of a mural thrombus. Since intra-arterial administration of urokinase (300,000 IU) was ineffective, thrombus aspiration was performed using the 7-F guiding catheter. After successful removal of the thrombus, abciximab was given intravenously. Control angiograms showed recanalization of the stented segment and patency of the distal renal arteries, an outcome confirmed 8 months later by duplex ultrasound. Conclusions: As demonstrated in our case, thromboembolic complications can be rapidly and successfully treated on the table by combined measures, such as catheter thrombus extraction and pharmacological strategies.
Catheter-directed thrombolysis to treat juxtarenal aortic occlusion

Abstract:To present our initial experience with Catheter-directed thrombolysis (CDT) using urokinase for the treatment of juxtarenal aortic occlusion. This was a retrospective study of 14 patients with juxtarenal aortic occlusion treated with CDT for 2-6 days between March 2012 and March 2015 at the Department of Vascular Surgery of People's Hospital of Peking University. All patients had an occlusion of the abdominal aorta adjacent to the origin of the renal arteries and were diagnosed by computed tomography. All patients were classified as TransAtlantic Inter-Society Consensus II (TASC D). Successful thrombolysis was defined as a reduction of >50% of the thrombus volume. There were nine men (64%) and five women (36%). Mean age was 58.3 +/- 9.4 years. The main symptom was claudication in eight patients (57%), rest pain in four (29%), and ulceration in two (14%). The duration of disease was 30.8 +/- 24.7 months (range, 1-72 months). CDT lasted 6 days in 12 patients, 2 and 3 days in the other 2 patients. The use of CDT was successful in all 14 patients. Three patients complained of transient pain, but it did not need intervention. No patient experienced perioperative visceral embolization. Limb ischemia was significantly relieved in all patients. At 1 year of follow-up, two patients presented asymptomatic re-occlusion. CDT could be considered in the treatment of juxtarenal aortic occlusion.

Abstract:Objective: To describe the experience in endovacular treatment of juxtarenal aortic occlusions. Methods: The clinical data of 14 patients with juxtarenal aortic occlusion admitted from March 2012 to November 2014 were retrospectively analyzed. Of the patients, 9 cases were male and 5 cases were female; ages ranged from 42 to 71 years; 8 cases presented with intermittent claudication, 5 cases manifested as rest pain, and one case had toe gangrene, and the course of disease ranged from one month to 72 months. All patients received covered stent implantation atfer thrombolysis via a multi–side-hole catheter. Results: Interventional procedures were successfully completed in all patients. The thrombi in the abdominal aorta disappeared after catheter-directed thrombolysis in all patients except two cases, in whom little mural thrombi remained. All patients received covered kissing stent placement, after which, their symptoms of limb ischemia were signiifcantly relieved. Follow-up period ranged from one month to 26 months, with average of 14.8 months, and the patency rate was 78.6%. Conclusion: hTrombolytic therapy combined with covered stent placement is a safe and effective endovascular treatment method for juxtarenal aortic occlusion.
Analysis Of The Etiology And The Strategy Of Diagnosis And Treatment Of Acute Renal Infarction

Abstract:Objective To investigate the etiology and treatment strategy of acute renal infarction by analyzing the clinical characteristics, treatment and prognosis of patients with acute renal infarction. Methods We retrospectively reviewed the medical records of 32 patients diagnosed as acute renal infarction from March 2012 to March 2015 at Peking University Third Hospital. Results Among the 32 patients with acute renal infarction the most common causes were history of malignancy (18.7%), atherosclerosis (15.6%) and cardiogenic (9.4%). The most common locations of the acute renal infarction was upper pole of the left kidney. The common symptoms and signs included Lumbago (84.4%) and renal pain (93.8%). The common abnormalities in laboratory tests included elevated N%(71.9%), elevated LDH (71.9%), proteinuria (75.0%) and hematuria (65.6%). Typical enhanced CT manifestations appeared in all the 32 cases of patients with acute renal infarction. The time from presentation to receiving computer tomography examination was (6.34±4.55)h. All the patients received anticoagnlation and (or) interventional therapy, and symptoms were relieved in all the patients. Conclusion Without a clear etiology, the diagnosis of acute renal infarction is often delayed or unrecognized because of its non-specific presentation and the rarity of the disease. The early diagnosis of acute renal infarction needs timely abdominal enhanced CT examination in the emergency department. Anticoagulant therapy and (or) intervention therapy is beneficial.
Percutaneous Transluminal Angioplasty Combined with Thrombolysis for Acute Thrombosis in Arterio-Venous Fistula and Graft

Abstract:OBJECTIVE:To evaluate the efficacy of ultrasound or fluoroscopic-guided percutaneous transluminal angioplasty (PTA) combined with thrombolysis for the treatment of acutely thrombosed arteriovenous fistula (AVF) or grafts (AVG).METHODS:One hundred and ninety-two hemodialysed patients, in whom the thrombosed arterio-venous AVF or AVG developed less than 72 h and there were no contraindications for thrombolysis and PTA, underwent PTA combined with thrombolysis therapy in Sir Run Run Shaw Hospital of Zhejiang University from October 2014 to October 2017. Under ultrasound and/or fluoroscopic guidance, balloon catheter was introduced to thrombosis sites along a guide wire. Then the balloon was inflated and normal saline mixed with urokinase and heparin was injected for thrombolysis. After blood flow was restored, angioplasty was performed on vascular stenosed sites.RESULTS:A total of 274 endovascular interventional operations were performed for 192 patients. The procedure success rate was 98.2%, clinical success rate was 93.8%and complication rate was 1.46%. The post-intervention primary patency rates for AVF group were 87.4%, 76.7%and 63.9%at 3, 6 and 12 months, respectively; while the post-intervention secondary patency rates were 93.7%, 91.6%and 83.0%, respectively. The post-intervention primary patency rates for AVG group were 60.7%, 51.5%and 43.1%at 3, 6 and 12 months, while the post-intervention secondary patency rates were 82.7%, 77.1%and 70.8%, respectively.CONCLUSIONS:PTA combined with thrombolysis is an effective and safe therapeutic option for AVF and AVG thrombosis, which would prolong hemodialysis access and save vascular resources for hemodialyzed patients.
Management of Iatrogenic Renal Arteriovenous Fistula and Renal Arterial Pseudoaneurysm by Transarterial Embolization

Abstract:The purpose of this study was to evaluate the efficacy and safety of transarterial embolization (TAE) for iatrogenic renal arterial pseudoaneurysm and arteriovenous fistula at our center. Our retrospective analysis included 27 patients who received TAE for iatrogenic renal arterial pseudoaneurysm and arteriovenous fistula between January 2006 and January 2016. Data on demographics, type of minimally invasive renal procedures, clinical manifestation, imaging features, embolization procedure, and perioperative details were collected. The technical and clinical success rates were analyzed. Furthermore, the changes in serum creatinine and eGFR before and after embolization were recorded and compared by t test. The median time between iatrogenic renal injury and TAE was 3 days (range, 0-110 days), with most patients (24/27, 88.9%) receiving TAE within 14 days. Only 1 patient was diagnosed with renal artery pseudoaneurysm 110 days after laproscopic partial nephrectomy. The technical and clinical success rates were 100% and 96.3%, respectively, with 1 patient requiring a second embolotherapy at the third postoperative day. No other patient required additional endovascular or surgical intervention due to recurrent hemorrhage. The mean serum creatinine before TAE was 92.8 +/- 25.3 mu mol/L and after TAE, 96.1 +/- 27.7 mu mol/L (P=.095). The eGFR of pre-and postembolization was 75.2 +/- 26.5 mL/min/1.73m(2) and 72.5 +/- 26.2mL/min/1.73m(2) (P=.16). No severe complications were observed during follow-up. This retrospective review demonstrated that TAE for the treatment of iatrogenic renal artery pseudoaneurysm and/or arteriovenous fistula was safe and associated with high technical and clinical success rate.

Abstract:Renovascular hypertension (RVH) is a primary cause of secondary hypertension, primarily driven by the activation of the renin–angiotensin–aldosterone system activation. Recently, growing studies suggested accessory renal artery (ARA) might also contribute to RVH. However, the treatment of ARA‐related hypertension and whether to take interventional treatment lack consensus. Herein, we report two cases of ARA‐related hypertension in our hospital. Imaging studies of both patients showed ARA stenosis. One patient had ARA occlusion well‐compensated through tortuous collateral branches, achieving normal blood pressure by medical treatment alone. The other patient had ARA stenosis coexisted with main renal artery stenosis, and revascularization of both arteries led to a significant postoperative reduction in blood pressure. A literature review was conducted to summarize overall treatment strategies for ARA‐related hypertension and clarify the relationship between ARA and hypertension. Recent research supported an association between ARA and hypertension. While medical therapy remains the first‐line treatment for ARA‐related hypertension, interventional procedures should be considered for patients whose blood pressure remains uncontrolled despite conservative management.

Abstract:Objective To evaluate a combination of interventional treatment and surgical exploration for acute lower limb ischemic disease.Methods We reviewed 42 cases admitted from July 2007 to January 2010,all patients complained pain,paralysis,pulselessness,pallor and paresthesia.After Fogarty thrombectomy angiography was taken in DSA room.Patients with angiostenosis greater than 50％ were then managed by interventional treatment(CDT,PTA,Stenting).Results Lives were saved in all patients,40 lower limbs were saved,and 2 patients received below knee amputation.The amputation rate was 4.76％.Dorsal or(and)posterior tibial artery of foot was felt in 33 patients,symptoms significantly improved.The other 7 patients still had painful and paralysis on the diseased limb.Conclusions The interventional treatment and surgical operation in acute lower limb ischemic disease is safe and result is satisfactory,which can improve the long-term patency and salvage rate of the lower limb.

Abstract:RATIONALE:Renal aneurysm is a rare disease with the atypical symptoms and mostly diagnosed by imaging modalities. Endovascular therapy is a one of treatment methods.PATIENT CONCERNS:A retrospective analysis of 32 patients with renal artery aneurysm, from June 2010 to May 2016 in our hospital, was made in our study. All of them underwent therapy, and the effects and perioperative characteristics of it were analyzed.DIAGNOSIS:32 patients were taken contrast-enhanced CT and diuretic renal dynamic imaging to evaluate the state of illness.INTERVENTIONS:The preoperative blood creatinine, perioperative hemoglobin and relief of hypertension were performed.OUTCOMES:The preoperative blood creatinine was slightly increasing, while the perioperative hemoglobin was slightly decreasing. The relieving hypertension was performed in 9 of patients.LESSONS:Endovascular therapy is a mature and feasible treatment method. There is little effect on hemoglobin and renal function in postoperation, while it can also decrease the blood pressure.

Abstract:BACKGROUND Arteriovenous fistulas (AVFs) are used to provide vascular access for hemodialysis in patients with end-stage renal failure. However, stenosis and thrombosis can compromise long-term AVF patency. The objective of this study was to evaluate catheter thrombolysis with percutaneous transluminal angioplasty (PTA), using a trans-brachial approach, for acutely thrombosed AVFs. MATERIAL AND METHODS This retrospective study examined 30 cases of AVF thrombosis treated between January 1, 2015 and January 1, 2017. All patients received transcatheter thrombolysis with PTA using a trans-brachial approach. AVF patency was assessed after 6 months. RESULTS Thrombolysis with PTA was performed at 2 to 72 h after diagnosis of AVF occlusion due to acute thrombosis, and AVF patency was restored in all patients. After 6 months, the primary and secondary patency rates were 76.7% and 93.3%, respectively. For type I stenosis, primary patency was achieved in 10 of 16 patients (62.5%) and secondary patency was achieved in 14 of 16 patients (87.5%). For type II stenosis, primary patency was achieved in 13 of 14 patients (92.9%) and secondary patency was achieved in 14 of 14 patients (100%). Comparing type I and II stenosis, a significant difference was detected in the rates of primary patency (odds ratio=0.909, 95% confidence interval 0.754-1.096, P=0.049), but not secondary patency (P=0.178), after 6 months. CONCLUSIONS Our study provides preliminary evidence that catheter-directed thrombolysis with PTA using a trans-brachial approach can achieve high patency rates when used to treat acutely thrombosed AVFs.

Abstract:Objective To analyze the interventional therapeutic effects in renal artery stenosis(RAS) disease.Methods Forty-five cases of RAS from March 2003 to March 2008 were admitted in our hospital.Fifty-three renal arteries were found stenosis or obstructed.Of those 11 arteries received percutaneous transluminal angioplasty(PTA) treatment,and other 42 arteries received PTA and stent planting treatment.The cases were followed up in blood pressure,serum creatinine and renal artery colour Doppler for 9 to 48 months.Results All the interventional operation of 45 cases were successful.After operation,the hypertension disappeared in 9 cases,improved in 28 cases,and the renal function improved in 12 cases.The effective rate of hypertension improving was 100% in fihromuscular dysplasia and takayasu disease group,which was much higher than 65.1% of atherosclerotic group(P<0.05).Eighteen arteries were followed up by colour Doppler,and found restenosis in 7 cases.Conclusion The operation of PTA and PTA/stent provides a micro-trauma,safe and effective choice for therapy of RAS disease.
