Abstract:The 13th World Congress of the International Hepato-Pancreato-Biliary Association was held from 4th to 7th September 2018 in Geneva,Switzerland.Thousands of wellknown specialists and scholars from 96 countries around the world were invited for the great event.The congress aimed to explore the latest achievements of diagnosis and treatment of hepatobiliary and pancreatic diseases from both the clinical and basic perspective.In this article,authors reviewed and analyzed the up-to-date research information and combined clinical researches of the author team,in order to share the experience and achievements in the field of hepato-pancreato-biliary surgery with colleagues and provide new information and inference for optimization of diagnosis and treatment in this field.

Abstract:Abstract The incidence of pancreatic cancer has been rising worldwide, and its clinical diagnosis and treatment remain a great challenge. To present the update and improvements in the clinical diagnosis and treatment of pancreatic cancer in recent years, Chinese Pancreatic Association, the Chinese Society of Surgery, Chinese Medical Association revised the Guidelines for the Diagnosis and Treatment of Pancreatic Cancer in China (2014) after reviewing evidence-based and problem-oriented literature published during 2015–2021, mainly focusing on highlight issues regarding diagnosis and surgical treatment of pancreatic cancer, conversion strategies for locally advanced pancreatic cancer, treatment of pancreatic cancer with oligo metastasis, adjuvant and neoadjuvant therapy, standardized processing of surgical specimens and evaluation of surgical margin status, systemic treatment for unresectable pancreatic cancer, genetic testing, as well as postoperative follow up of patients with pancreatic cancer. Forty recommendation items were finally proposed based on the above issues, and the quality of evidence and strength of recommendations were graded using the Grades of Recommendation, Assessment, Development, and Evaluation system. This guideline aims to standardize the clinical diagnosis and therapy, especially surgical treatment of pancreatic cancer in China, and further improve the prognosis of patients with pancreatic cancer.

Quantitative Imaging in Medicine and Surgery

Abstract:Following tremendous economic progress, society in China is also undergoing fundamental changes, as is the healthcare system. Currently the training of Chinese young doctors and their future work placement are all undergoing re-structuring. We compiled some thoughts and opinions on the topic of 'should clinicians in China engage in research?', and publish them as a special report in this issue of Quantitative Imaging in Medicine and Surgery (QIMS). The contributors included some editorial members of this journal, and a few personal friends. Besides a few minor linguistic corrections, opinions from the contributors have not been edited, as we want authors' to write their own independent views. However, it is possible there is a selection bias of the contributors of this paper; more likely those who are interested in the medical research are selected and therefore the views of the contributors may not be generalizable. To compare the structure and funding of China with other countries, authors from UK, The Netherlands, France, and USA are also invited.

Abstract:Pancreatic cancer is a highly malignant, fast progressive digestive system neoplasm with poor prognosis, and the incidence has increased significantly in the recent years. The treatment mode of pancreatic cancer has been transformed from surgery-first approach to multidisciplinary approach including surgery, chemotherapy, radiotherapy and other comprehensive treatment modalities, accompanying with significant improvement in clinical efficacy. During the last decade, the molecular mechanism of pancreatic cancer has been explored in depth. Indeed, we had a clearer understanding of the disease, which provides clues for translation of basic research into clinical practice. This review summarized recent high quality studies on pancreatic cancer including pathogenic factor, advances in basic research, and clinical treatment.

Abstract:Pancreatic cancer is one of the most malignant digestive system tumors. In the recent decade, the effect of diagnosis and treatment of pancreatic cancer has improved due to the renewal of treatment concept and the popularization of effective treatment. However, the overall efficacy of pancreatic cancer is still dismal and the 5-year survival rate is only about 10%. Further improving the diagnosis and treatment of pancreatic cancer is the top priority of oncology research and clinical practice. Based on past clinical and scientific research experience, the authors have proposed ten hot spots and future directions for a reference, which focusing on early prevention, early diagnosis, and early treatment of pancreatic cancer, molecular typing and precise treatment, new drug development and regimen combination, surgical technology and strategy change, model establishment and database development, as well as innovation of traditional Chinese medicine and breakthrough of treatment concept. A breakthrough in the diagnosis and treatment of pancreatic cancer in the next ten years is raising hope, when doctors can truly prevent and control pancreatic cancer.

Abstract:With the increase of morbidity and mortality, pancreatic cancer will not only become one of the leading causes of death in China, but also a major public health problem in the world. New onset cases of pancreatic cancer all around the world are expected to increase year by year. The prognosis of patients with pancreatic cancer is poor. The 5-year survival rate is only about 6%. The major reason for the poor prognosis is the lack of typical clinical symptoms in the early stage of disease, which makes it difficult to detect and control its relevant recurrence and metastasis. Moreover, lack of effective chemotherapy drugs and resistance to existing chemotherapy drugs are also important reasons for its dismal status. Radical resection surgery is still the most effective treatment. However, even after radical resection surgery, early recurrence and distant metastasis are still the main problems that bother patients. This article reviews recent high quality studies on pancreatic cancer including epidemiological investigation, advances in basic research, and clinical treatment.

Abstract:Pancreatic cancer is one of the more dismal gastrointestinal tumors due to the obscure symptoms in the early stage of disease, rapid progression and insensitivity to chemotherapy. The 5-year survival rate is only approximately 9% for patients with pancreatic cancer. In the recent years, with the change in dietary structure and life style as well as the improvement of clinical detection rate, the incidence of pancreatic cancer is increasing annually, and it severely undermines the health of people. However, the advance in molecular biology provides more potential targets for the treatment of pancreatic cancer. Besides, well-conducted clinical trials continue to translate more theoretical innovations from bench to bedside. These both can improve the prognosis of pancreatic cancer eventually. The present review summarized the important findings in pancreatic cancer research in 2020.

Abstract:Pancreatic carcinoma has the poorest prognosis in all the digestive cancers,and the management remains challenging.Nowadays the treatment strategy of pancreatic carcinoma has been changed from "surgery first" into the mode of multi-disciplinary team (MDT) with surgery as a leading and crucial position.By historical review,some issues related to the strategy of borderline resectable pancreatic carcinoma,including value of neoadjuvant therapy,perioperative complications,laparoscopic pancreaticoduodenectomy were discussed and commentated.The breakthrough for managing pancreatic carcinoma will be based on early diagnosis and the development of biologic targeting therapy.The surgeons' concept should constantly advance with the times,and the strategy pattern should be converted from local to system,from morphology to biology,and from surgery to oncology.

Abstract:The International Summit Forum in Colorectal and Anal Surgery was recently held in Shanghai at the Xinhua Hospital under the direction of Professor Long Cui on 4–5 July 2014 with specially invited overseas speakers including Professors Francis Seow-Choen (Singapore) and Andrew Zbar (Israel). Professor Seow-Choen spoke about the rise of novel technologies for colorectal surgery in Singapore, focusing on laparoscopic and robotic surgeries and on the new strategy of reverse-TME and hybrid NOTES resections There were also talks on the local Chinese experience with laparoscopic-assisted resection (Drs. Zhen Minhua and Zhongmin, Shanghai), robotic colorectal resection (Dr. Xu Jianming, Shanghai), NOTES (Dr. Fu Tao, Xian) and hand-assisted surgery (Dr. Lin Jianjiang, Hangzhou). Professor Zbar discussed the technique of transperineal sonography along with novel 3D post-processing endoanal ultrasonographic technology in the delineation of complex fistula-in-ano and for the diagnosis of perianal Crohn’s disease. In addition, Dr. Wang Honxiang (Shanghai) outlined the clinical approach to complicated cryptogenic fistula-in-ano. There were further presentations focusing on rectal cancer management (Drs. Liu Menhua, Beijing, and Yao Liqing, Shanghai) with the consideration of the comprehensive care of advanced (T4) rectal cancer (Dr. Fu Chuangang, Shanghai) and excellent presentations of the experience with and outcome of selective inter-sphincteric rectal resection (Dr. Du Jinlin, Jinhua) and extralevator rectal resection (the ELAPE procedure; Dr. Wang Zhenjun, Shenyang). Additional special presentations included a tactical approach to the diagnosis and management of Crohn’s disease (Dr. Gu Yunfei, Liaonin) and a similar evaluation of strategies in the treatment of severe constipation (Dr. Liu Baohua, Beijing). Two further special presentations were made by Professor Long Cui (Shanghai) concerning planning considerations for impending molecular and genetic coloproctological research (given China’s considerable resources and facilities for tissue banking) and by Professor Wang Jianping (Guangzhou) who at the start of the meeting outlined the future direction of colorectal surgery in China. The principles highlighted by Professor Wang aim to standardize colorectal resectional techniques across China and to develop a nationwide cooperative colorectal cancer network which will foster clinical research. The meeting was of high quality and excellently organized with an expanded emphasis on minimally invasive approaches along with the definition of national strategic protocols for the management of inflammatory bowel disease, complex perirectal sepsis and functional bowel disorders. The future of Chinese coloproctology seems assured with a plan spearheaded by Professor Long Cui and his colleagues which will enhance Chinese-led research and will strengthen international collaboration. It is certainly worth placing this congress in one’s calendar so as to gain a greater insight into the evolution of Chinese coloproctology and to foster scientific and publishing partnerships. J. Abstract:Abstract Pancreatic cancer is an increasingly common cause of cancer mortality with a tight correspondence between disease mortality and incidence. Furthermore, it is usually diagnosed at an advanced stage with a very dismal prognosis. Due to the high heterogeneity, metabolic reprogramming, and dense stromal environment associated with pancreatic cancer, patients benefit little from current conventional therapy. Recent insight into the biology and genetics of pancreatic cancer has supported its molecular classification, thus expanding clinical therapeutic options. In this review, we summarize how the biological features of pancreatic cancer and its metabolic reprogramming as well as the tumor microenvironment regulate its development and progression. We further discuss potential biomarkers for pancreatic cancer diagnosis, prediction, and surveillance based on novel liquid biopsies. We also outline recent advances in defining pancreatic cancer subtypes and subtype-specific therapeutic responses and current preclinical therapeutic models. Finally, we discuss prospects and challenges in the clinical development of pancreatic cancer therapeutics.

Abstract:Surgical resection is the most effective local treatment for removing tumor burden in the comprehensive treatment of pancreatic cancer.However,the effect is limited by both malignant behavior and anatomical location.From the view of the"paying attention to the overall,more than concerning about the local",scientific view of the relationship among the surgery and other treatment helps us to have a correct understanding of the multidisciplinary comprehensive treatment of pancreatic cancer,also contributes to the formulation of clinical measures.Here we firstly re-think over a number of definitions associated with the treatment of pancreatic cancer,then re-understand the significance of comprehensive treatment of the pancreatic malignancy,and finally try to look ahead the future development of pancreatic cancer.
