Abstract:Optical cochlear implant is an emerging applied technology of stimulating auditory neurons by using pulsed laser light as a stimulus to evoke neural activity instead of electrical currents. Laser, stimulation has more potential in accuracy and high frequency resolution than electrical stimulation. In this paper we demonstrated a fiber laser system (980nm) to irradiate on guinea pig cochlear in vivo for the first time. The pulsed laser stimulation successfully evoked auditory nervous impulse, similar to acoustic stimulation. And the performance under different laser pulse intensity and pulse width is further tested and discussed. The results show that the 980nm pulsed laser stimulation with microsecond level width is effective and safe to evoke auditory response. Photoacoustic or photothermal effects maybe the main mechanism. In the end, we put forward some prospects of the applied technology in cochlear implant.

Abstract:Transient electrical impulses are conventionally used to elicit physiological responses in excitable tissues. While electrical stimulation has many advantages, it requires an electrode–tissue interface, exhibits relatively low spatial selectivity and always produces a “stimulus artifact”. Recently, it has been shown that pulsed, low-energy infrared laser light can evoke nerve, muscle and sensory responses similar to those induced by traditional electrical stimulation in a contact-free, damage-free, artifact-free and spatially selective manner. However, the effect of transient infrared laser light on neurotransmission in the CNS is still largely unknown. Here, we tested the effect of infrared laser light on GABAergic neurotransmission. We recorded spontaneous inhibitory postsynaptic currents (sIPSCs) from cultured rat cortical neurons prior to and after infrared laser stimulation. Using transient infrared laser light, we either stimulated the neuronal soma that had axonal projections to the recorded neuron or directly stimulated the axons that projected to the recorded neuron. Optical stimulation led to enhanced amplitude, decreased decay time constant and increased frequency of sIPSCs. These alterations of sIPSC properties produced by optical stimulation were specifically mediated by GABAA receptors and caused by the transient laser light per se since no exogenous substances such as caged compounds were used. These data show that optical stimulation using transient infrared laser light can alter GABAergic neurotransmission and demonstrate that it may be an alternative approach to electrical stimulation in studying GABAergic function.

Abstract:This study has investigated the feasibility of 980-nm low-energy pulsed near-infrared laser stimulation to evoke auditory responses, as well as the effects of radiant exposure and pulse duration on auditory responses. In the experiments, a hole was drilled in the basal turn of the cochlea in guinea pigs. An optical fiber with a 980-nm pulsed infrared laser was inserted into the hole, orientating the spiral ganglion cells in the cochlea. To model deafness, the tympanic membrane was mechanically damaged. Acoustically evoked compound action potentials (ACAPs) were recorded before and after deafness, and optically evoked compound action potentials (OCAPs) were recorded after deafness. Similar spatial selectivity between optical and acoustical stimulation was found. In addition, OCAP amplitudes increased with radiant exposure, indicating a photothermal mechanism induced by optical stimulation. Furthermore, at a fixed radiant exposure, OCAP amplitudes decreased as pulse duration increased, suggesting that optical stimulation might be governed by the time duration over which the energy is delivered. Thus, the current experiments have demonstrated that a 980-nm pulsed near-infrared laser with low energy can evoke auditory neural responses similar to those evoked by acoustical stimulation. This approach could be used to develop optical cochlear implants.

Abstract:There has been much research in recent years on pulsed laser stimulation to evoke auditory nerve responses.However,the mechanism is not yet fully understood.This paper describes use of the simulation software COMSOL Muhiphysics to build a three-dimensional model to study both the photoacoustic and photo-thermal effect induced by the pulsed laser stimulation of the cochlea,combined with in vivo experiments on guinea pigs to analyze the mechanisrr.The simulations show that pulsed lasers cause a temperature rise,but also trigger a photoacoustic effect with the compound action potentials induced in guinea pigs deafened acutely by drugs.The results demonstrate that the response from optical stimulation is more likely the superposition of photoacoustic and photo-thermal effects.

Abstract:Background: Infrared laser stimulation has been proposed as an innovative method to elicit an auditory nerve response. Most studies have focused on using long-wavelength infrared (>980nm) pulsed lasers with high water absorption coefficients. This paper sought to assess whether a short-wavelength laser (465nm) with an absorption coefficient as low as 10−3cm−1 would activate the auditory nerve and studied its potential mechanism. Method: Optical compound action potentials (OCAPs) were recorded when synchronous trigger laser pulses stimulate the cochlea before and after deafening, varying the pulse durations (from 800μs to 3600μs) and the amount of radiant energy (from 18.05mJ/cm2 to 107.91mJ/cm2). A thermal infrared imager was applied to monitor the temperature change of the guinea pig cochlea. Results: The results showed that pulsed laser stimulation at 465nm could invoke OCAPs and had a similar waveform compared to the acoustical compound action potentials. The amplitude of OCAPs had a positive correlation with the increasing laser peak power, while the latency of OCAPs showed a negative correlation. The imager data showed that the temperature in the cochlea rose quickly by about 0.3∘C right after stimulating the cochlea and decreased quickly back to the initial temperature as the stimulation ended. Conclusions: This paper demonstrates that 465-nm laser stimulation can successfully induce OCAPs outside the cochlea, and that the amplitude and latency of the invoked OCAPs are highly affected by laser peak power. This paper proposes that a photothermal effect might be the main mechanism for the auditory nerve response induced by short-wavelength laser stimulation.

Abstract:The last decade has seen discussions about applying infrared laser stimulation to brain–machine interfaces, such as cochlear implants, due to the capability of activating spatially selected neural populations without introducing exogenous agents to tissues. We established transtympanic laser stimulation, infrared laser irradiation of the cochlea from the outer ear, to reduce the invasiveness of surgical implantation by leveraging its contactless feature. Laser stimulation induces auditory perception in an intensity-dependent manner. Furthermore, the simultaneous combination of auditory and laser stimulation results in greater auditory perception than either auditory or laser stimulation alone. These findings suggest that laser stimulation can evoke and potentially enhance auditory perception, holding promise for implementation in auditory prostheses. Our work lays the groundwork for integrating transtympanic laser stimulation into auditory prostheses.

Abstract:Laser can stimulate sound wave, it's possible for fiber laser to evoke neural activity safely and produce the auditory perception, this acts as the electrode role in cochlear implant. Moreover, the laser is characterized by definite direction and optical signal between adjacent fibers don't readily interfere. Therefore, optical fiber cochlear implant based on optoacoustic effect is promising. The design and preparation of an experimental system based on optoacoustic effect is introduced in this paper. The selection of laser source and the control methods of optical signal were discussed, and testing results of the fiber optoacoustic effect, including the time domain waveforms and spectrograms of the evoked optoacoustic sound were given. Also, a preliminary discussion for the extended fiber array system applied in artificial fiber cochlea as an alternative to electrodes stimulation was progressed. © (2013) Trans Tech Publications, Switzerland.
Neuromodulation with Submillimeter Spatial Precision by Optoacoustic Fiber Emitter

Abstract:New techniques to non-invasively modulate neural activities with high precision are constantly being sought in neuroscience. Ultrasound is a promising neuromodulation modality due to its non-invasive potential. However, conventional piezo-based ultrasound neuromodulation using a transducer usually have a spatial resolution of more than a few millimeters, limiting research on specific brain area or neural circuits. Here, a spatially confined optoacoustic emitter is developed for high-precision neuromodulation, allowing neurons to be activated at submillimeter spatial precision. By designing a candle soot-PDMS layered optoacoustic fiber tip (CPOF) with a diameter of 350 mu m, we demonstrate the generation of local ultrasound waves with a peak frequency of 3.5 MHz. The ultrasound generated by CPOF can directly activate cortical neurons in culture within a radius of 450 mu m around the fiber tip, generating intracellular calcium transients. The CPOF promises to be a novel technique for high-precision neural stimulation and for investigating the mechanism of neurostimulation.

Abstract:The discovery that a pulsed laser could trigger an auditory neural response inspired ongoing research on cochlear implants activated by optical stimulus rather than by electrical current. However, most studies to date have used visible light (532 nm) or long-wavelength near-infrared (>1840 nm) and involved making a hole in the cochlea. This paper investigates the effect of optical parameters on the optically evoked compound action potentials (oCAPs) from the guinea pig cochlea, using a pulsed semiconductor near-infrared laser (980 nm) without making a hole in the cochlea. Synchronous trigger laser pulses were used to stimulate the cochlea, before and after deafening, upon varying the pulse duration (30-1000 mu s) and an amount of radiant energy (0-53.2 mJ/cm(2)). oCAPs were successfully recorded after deafening. The amplitude of the oCAPs increased as the infrared radiant energy was increased at a fixed 50 mu s pulse duration, and decreased with a longer pulse duration at a fixed 37.1 mJ/cm(2) radiant energy. The latency of the oCAPs shortened with increasing radiant energy at a fixed pulse duration. With a higher stimulation rate, the amplitude of the oCAPs' amplitude decreased. (C) The Authors. Performance analysis of the beam shaping method on optical auditory neural stimulation in vivo

Abstract:Previous research has shown that infrared neural stimulation (INS) could be an alternative approach to evoke auditory neural activities. The laser beam property of the fiber output is a considerable aspect of INS, and the corresponding effects on auditory responses in vivo deserve further discussions. The paper presents a beam-shaped infrared laser stimulation method of auditory nerves. Pulsed 980-nm fiber-coupled laser systems were used as the radiant sources. The gradient reflective index (GRIN) lens was added at the port of the optical fiber as a beam shaping structure. The laser spot sizes and properties between the beam-shaped output and the bare fiber output were preliminarily analyzed by a laser beam profiler. And further experiments in vivo with four deafened adult guinea pigs were conducted. Optically evoked auditory brainstem responses (OABRs) of the animal samples were recorded and compared under the two output configurations. The results show a decrease of the beam divergence compared to a bare output fiber, and the INS with a beam shaping design evokes above 13 % increase on OABR amplitudes than the bare fiber output, especially when enlarging the distance between the optical fiber and the nerve tissue. The beam shaping design can enhance the effect of INS for evoking auditory nerves, and it could be an optimized design in the future implementation of optical cochlear implants.

Abstract:Objectives: We provide an appraisal of recent research on stimulation of the auditory system with light. In particular, we discuss direct infrared stimulation and ongoing controversies regarding the feasibility of this modality. We also discuss advancements and barriers to the development of an optical cochlear implant. Methods: This is a review article that covers relevant animal studies. Results: The auditory system has been stimulated with infrared light, and in a much more spatially selective manner than with electrical stimulation. However, there are experiments from other labs that have not been able to reproduce these results. This has resulted in an ongoing controversy regarding the feasibility of infrared stimulation, and the reasons for these experimental differences still require explanation. The neural response characteristics also appear to be much different than with electrical stimulation. The electrical stimulation paradigms used for modern cochlear implants do not apply well to optical stimulation and new coding strategies are under development. Stimulation with infrared light brings the risk of heat accumulation in the tissue at high pulse repetition rates, so optimal pulse shapes and combined optical/electrical stimulation are being investigated to mitigate this. Optogenetics is another promising technique, which makes neurons more sensitive to light stimulation by inserting light sensitive ion channels via viral vectors. Challenges of optogenetics include the expression of light sensitive channels in sufficient density in the target neurons, and the risk of damaging neurons by the expression of a foreign protein. Conclusion: Optical stimulation of the nervous system is a promising new field, and there has been progress toward the development of a cochlear implant that takes advantage of the benefits of optical stimulation. There are barriers, and controversies, but so far none that seem intractable. Level of evidence: NA (animal studies and basic research).

Abstract:Combined electric and acoustic stimulation has proven to be an effective strategy to improve hearing in some cochlear implant users. We describe an acoustic microactuator to directly deliver stimuli to the perilymph in the scala tympani. The 800 µm by 800 µm actuator has a silicon diaphragm driven by a piezoelectric thin film (e.g., lead-zirconium-titanium oxide or PZT). This device could also be used as a component of a bimodal acoustic-electric electrode array. In the current study, we established a guinea pig model to test the actuator for its ability to deliver auditory signals to the cochlea in vivo. The actuator was placed through the round window of the cochlea. Auditory brainstem response (ABR) thresholds, peak latencies, and amplitude growth were calculated for an ear canal speaker versus the intracochlear actuator for tone burst stimuli at 4, 8, 16, and 24 kHz. An ABR was obtained after removal of the probe to assess loss of hearing related to the procedure. In some animals, the temporal bone was harvested for histologic analysis of cochlear damage. We show that the device is capable of stimulating ABRs in vivo with latencies and growth functions comparable to stimulation in the ear canal. Further experiments will be necessary to evaluate the efficiency and safety of this modality in long-term auditory stimulation and its ability to be integrated with conventional cochlear implant arrays.
Effect of Shorter Pulse Duration in Cochlear Neural Activation with an 810-Nm Near-Infrared Laser

Abstract:Optical neural stimulation in the cochlea has been presented as an alternative technique to the electrical stimulation due to its potential in spatially selectivity enhancement. So far, few studies have selected the near-infrared (NIR) laser in cochlear neural stimulation and limited optical parameter space has been examined. This paper focused on investigating the optical parameter effect on NIR stimulation of auditory neurons, especially under shorter pulse durations. The spiral ganglion neurons in the cochlea of deafened guinea pigs were stimulated with a pulsed 810-nm NIR laser in vivo. The laser radiation was delivered by an optical fiber and irradiated towards the modiolus. Optically evoked auditory brainstem responses (OABRs) with various optical parameters were recorded and investigated. The OABRs could be elicited with the cochlear deafened animals by using the 810-nm laser in a wide pulse duration ranged from 20 to 1000 μs. Results showed that the OABR intensity increased along with the increasing laser radiant exposure of limited range at each specific pulse duration. In addition, for the pulse durations from 20 to 300 μs, the OABR intensity increased monotonically along with the pulse duration broadening. While for pulse durations above 300 μs, the OABR intensity basically kept stable with the increasing pulse duration. The 810-nm NIR laser could be an effective stimulus in evoking the cochlear neuron response. Our experimental data provided evidence to optimize the pulse duration range, and the results suggested that the pulse durations from 20 to 300 μs could be the optimized range in cochlear neural activation with the 810-nm-wavelength laser.

Abstract:Objective: The purpose of this study was to evaluate the effects of fiberoptic collimation technique on auditory neural stimulation in the cochlea with 808 nm wavelength lasers. Background data: Recently, the pulsed near-infrared lasers in the 800-1000 nm wavelength range have been investigated as an emerging technique to trigger auditory neural response in the cochlea. A laser beam divergence in the optical stimulation pathway exists, which may affect stimulation efficiency and spatial selectivity. Methods: The fiberoptic collimation technique was proposed for cochlear neuron stimulation, and the C-lens element was designed as the collimation structure. The spiral ganglion cells in deafened guinea pigs' cochlea were irradiated with collimated and uncollimated near-infrared lasers. Optically evoked auditory brainstem response (OABR) under the two laser output modes were recorded. Results: Laser with the collimation technique evoked an average 58% higher OABR amplitude than the uncollimated laser output. In addition, the collimated laser setup consumed on average 35.2% of laser energy compared with the uncollimated laser when evoking the same OABR amplitude. Conclusions: The fiberoptic collimation technique improved stimulation efficiency and reduced stimulating energy consumption in near-infrared neural stimulation in cochlea. The positive effects of laser collimation technique could benefit further research in optically based cochlear implants.
Graded optogenetic activation of the auditory pathway for hearing restoration

Abstract:Optogenetic control of neural activity enables innovative approaches to improve functional restoration of diseased sensory and motor systems. For clinical translation to succeed, optogenetic stimulation needs to closely match the coding properties of the targeted neuronal population and employ optimally operating emitters. This requires the customization of channelrhodopsins, emitters and coding strategies. Here, we provide a framework to parametrize optogenetic neural control and apply it to the auditory pathway that requires high temporal fidelity of stimulation. We used a viral gene transfer of ultrafast targeting-optimized Chronos into spiral ganglion neurons (SGNs) of the cochlea of mice. We characterized the light-evoked response by in vivo recordings from individual SGNs and neurons of the anteroventral cochlear nucleus (AVCN) that detect coincident SGN inputs. Our recordings from single SGNs demonstrated that their spike probability can be graded by adjusting the duration of light pulses at constant intensity, which optimally serves efficient laser diode operation. We identified an effective pulse width of 1.6 ms to maximize encoding in SGNs at the maximal light intensity employed here (∼35 mW). Alternatively, SGNs were activated at lower energy thresholds using short light pulses (<1 ms). An upper boundary of optical stimulation rates was identified at 316 Hz, inducing a robust spike rate adaptation that required a few tens of milliseconds to recover. We developed a semi-stochastic stimulation paradigm to rapidly (within minutes) estimate the input/output function from light to SGN firing and approximate the time constant of neuronal integration in the AVCN. By that, our data pave the way to design the sound coding strategies of future optical cochlear implants.

Abstract:Infrared neuron stimulation is regarded as an innovative approach for stimulating cochleae in animals while the exact mechanism still remains unknown. In this paper, we studied compound action potentials of guinea pig cochleae with chronic or acute deafness. We recorded optical compound action potentials and analyzed stretched cochlear preparations by fluorescence microscopy. Photoacoustic signals were measured by hydrophone and microphone, respectively. In our experiment, we observed a switch response effect in vitro and in vivo experiments. Therefore, we proposed photoacoustic effect could invoke auditory response in infrared neuron stimulation.
