Abstract:Congenital hand difference is caused by abnormal embryonic development of the limb and represents one of the most prevalent congenital birth defects worldwide. Using the new classification system proposed by Oberg, Manske and Tonkin (OMT) and endorsed by the International Federation of Societies for Surgery of the Hand, congenital hand differences are classified into malformations, deformations, and dysplasias and syndromes. Malformations are subdivided into abnormal development of proximal-distal, radial-ulnar (anterior-posterior), dorsal-ventral, and unspecified axis. We introduce here the state-of-the-art surgical treatment for thumb duplication and syndactyly. The surgical principle, timing, procedures, and postsurgical management are described for each condition.

Abstract:Objective To investigate the clinical features and treatment outcome of complex congenital syndactyly,Methods Forty-seven cases of congenital complex syndactyly were involved in the study,There were 19 cases of syndactyly associated with brachydactyly, 7 cases of acrosyndactyly, 6 cases of syndactyly associated with polydactyly, 4 cases of Poland syndrome, 2 cases of mitten hand, 2 cases of Apert syndrome, 2 cases of syndactyly associated with defect of digits, 1 case of syndactyly associated with cleft hand, and 4 cases of other types,Separation of the syndactylous digits was completed by a single surgical procedure in 21 cases and by multiple surgical procedures in 26 cases,In the meantime, some of the associated hand deformities were treated surgically,Results Average postoperative follow-up period was 16 months,83% of patients' parents were satisfied with the overall function of the hand,60% were satisfied with the appearance of the hand,Of 234 separated fingers, 34 had scar tissue contracture , web scar adhesion and lateral deviation deformity,All complications needed surgical treatment,Conclusion Complex syndactyly is a complicated congenital hand deformity,It involves more than one kind of hand deformity and compromises hand function and appearance severely,Separation of the digits can greatly improve the function,However, more work needs to be done to improve appearance of the hand.

Abstract:We report on a patient with a partial deletion on the short arm of chromosome 18 (del 18p), who presented with dysmorphic features and delayed developmental milestones as well as with a patent ductus arteriosus (PDA) and pulmonary valve stenosis (PS). Several forms of congenital heart disease (CHD) are found in about 10% of patients with del (18p), but coexisting PDA and PS have not been reported. Del (18p) must be considered in patients with characteristic phenotypic abnormalities and congenital heart disease, including a combination of PDA and PS.

Abstract:Syndactyly is one of the most common congenital hand differences, characterized with certain degrees of fusion of adjacent digits in soft tissues or bones. It can be associated with polydactyly, camptodactyly, brachydactyly or symphalangism. Diversity of manifestations and incomplete penetration of heritage make the diagnosis and treatment lack of a clear routine. The management of syndactyly was based on the individual experience over the past years with shortage of the knowledge in genetics and surgical strategy. Therefore, to standardize the treatment and update the surgical procedure in syndactyly, to decrease the huge waste of medical resources, the Hand Surgery Branch of Chinese Medical Association, Chinese Medical Doctor Association and Chinese Association of Rehabilitation Medicine convened an expert panel specialized in syndactyly with 10 famous domestic medical colleges and affiliated hospitals, to discuss and formulate about the spectrum of diagnosis and treatment. The consensus statement on diagnosis and clinical management of syndactyly was established for clinical references.
Progress in the studies on surgical treatment of congenital syndactyly of the hand

Abstract:Congenital syndactyly is one of the most common congenital hand anomalies, and syndactyly release is one of the most common procedures in surgery of congenital hand diseases. The goal of syndactyly release is to create a web space close to normal in order to improve the appearance and function of the finger and incur the minimum number of long-term morbidity. This article reviews the domestic and foreign literatures published in recent years and summarizes the progress in the treatment of congenital syndactyly. ( 1 ) Operation timing of syndactyly release. Different choices for operation timing have been proposed according to the types of the congenital syndactyly and the surgical techniques applied. Technique for single-stage, more than 2-digit release has obvious advantages with preoperative delineation of the vascular anatomy of the hand. ( 2 ) Web space reconstruction is the most important stage in the surgical treatment of syndactyly, and several dorsal metacarpal flaps have been described for syndactyly correction in recent years. To evaluate and compare the results of different dorsal flap techniques, postoperative complications and web-space appearance have been taken into account. ( 3 ) Separation of the digits and defect covering. The use of skin grafts is time consuming, and a variety of dorsal flaps and other techniques have been used to avoid complications related to skin grafts. And it is very necessary to emphasize and control the indications in the application of these methods.
Molecular Genetics and Genetic Diagnosis of Split-Hand/Foot Malformation

Abstract:Split-hand/foot malformation(SHFM)is a severe congenital malformation with hand/foot crack,syndactyly and hypoplasia of phalange and metacarpal. SHFM, characterized by defect or dysplasia of central rays and fusion of other rays, occurs as an isolated trait or a part of congenital anomaly syndrome. By far, 7 chromosomal loci associated with isolated SHFM have been confirmed, i.e., SHFM1-6 and SHFM/SHFLD. SHFM1, SHFM3, SHFM4 and SHFM5 are related with chromosomal rearrangement,including genomic small-scale deletion and duplication.SHFLD mainly relates to duplication of chromosome section. SHFM2 and SHFM6 have low frequency and the mechanism is not yet clear. In this paper, the gene mutations,frequency,test method,corresponding genetic counseling and their mechanism were all reviewed.

Abstract:Hemangioma and vessel deformities (including vascular malformations and lymphatic vessel deformity) are very common congenital or acquired lesions and are visible in any site of the body surface. About 60% of them are distributed in the head and neck, about 20% of them are distributed in the four limbs, and the proportion of hand lesions is not explicitly reported.

Abstract:Objective To investigate the clinical features and treatment principles of unilateral congenital upper limb myohypertrophy.Methods A retrospective review of the cases of unilateral congenital upper limb myohypertrophy treated by the author from May 1992 to October 2012 was conducted.There were 12 cases with 7 males and 5 females.Patients' age averaged 5.5 years (range,3 to 17 years).They had the following clinical and morphological characteristics:congenital; unilateral hypertrophy of the upper extremity on both the flexion and extension side; more severe involvement of the hand than the forearm and upper arm with splayed fingers and broad hand; hyperabduction of the thumb towards radial and palmar side with a hypermegalia web space; ulnar/radial deviation and flexion deformity at the metacarpophalangeal joints (similar to windblown hand deformity) most prominent in the index and middle fingers.When primary or secondary macromelia is excluded,clinical diagnosis of unilateral congenital upper limb myohypertrophy could be made based on the above characters.Final diagnosis depended on the intraoperative findings of multiple aberrant intrinsic and extrinsic muscles with abnormal origin/insertion and orientation,and increased volume.All these 12 patients had functional impairment of the hand and went through 1 to 4 operations which included coarctation of the thumb web space,debulking of the thenar and hypothenar eminence,excision of aberrant muscles,stabilization of the metacarpophalangeal joint central slip,metacarpal osteotomy,skin release and dermatoautoplasty at the volar aspect of the metacarpophalangeal joint.Intraoperatively,the abnormal conditions of muscles in the operation area were carefully explored.These surgical procedures were carried out individually or simultaneously when up to 3 of them were combined at the same time.Results All the patients were followup by the surgeon for an average of 2.5 years.The radialhyperabduction of the thunb was reduced by an average of 25°.22 joints were underwent operations to correct lateral deviation,and 4 had recurrence.The osteotomy healed in all 5 cases.Flexion deformity of the metacarpophalangeal joints was improved by an average of 12°.60％ of the parents were satisfied with the appearance of the limb while 40％ were not.As for functions of the operated limb,50％ of the parents were satisfied versus 50％ dissatisfied.Conclusion Unilateral congenital upper limb myohypertrophy is a rare congenital anomaly with independent morphological characteristics.The appearance and function of the hand are severely affected,mainly attributed to congenital aberrant intrinsic and extrinsic muscles with abnormal insertions and orientation,and increased volume.Surgical corrections can partially improve the appearance and function of the hand.

Abstract:Congenital heart disease (CHD) is a type of birth defects due to the abnormal development of heart and blood vessels during embryonic stage. Studies indicate that the etiology of CHD is complicated. Genetic and epigenetic mechanisms including chromosomal abnormalities, gene mutations, nucleic acid modifications, non-coding RNAs may play important roles in CHD. At present, genetic mechanisms such as chromosome abnormality and gene mutation have been widely used in the diagnosis and treatment of clinical diseases. However, the application of genetic and epigenetic modification in diagnosis and treatment of CHD still need further research. This paper reviews the relationship between chromosomal abnormality, gene mutation, copy number variation, epigenetic modification and the occurrence of CHD, which may provide a basis for further exploring the early diagnosis and individualized therapy of CHD.

Abstract:Limb bud is composed of mesodermal mesenchyma or mesodermal center and one layer of dermal layer on the external surface, and it is the initial phase of upper limb development. The development of limb bud is the result of gradual changes of various signaling molecular mechanisms in three-dimensional space, and the three axes are proximo-distal, anteroposterior, and dorsoventral, which correspond to the shoulder-finger direction, thumb-little finger direction, and forearm-palm direction in limb morphology. In order to guarantee the correct development and growth of limb bud, three categories of cell groups are of vital importance: they are apical ectodermal ridge (AER) at the lateral side of the limb bud, the progress zone (PZ) at the medial side of the limb bud, and the zone of polarizing activity (ZPA) at the posterior side of the limb bud. The signaling molecules generated by the cells in these regions decide the growth directions of the adjacent cells so that the normal growth and development of upper limb is maintained (Fig. 15.1).

Abstract:Objective To review and analyze the clinical morphologic characteristics of the hand deformity in Poland syndrome,and establish a new classification system and treatment strategy. Methods Sixty-three cases with Poland syndrome treated by the authors between February 2000 and August 2011 were evaluated retrospectively.The clinical morphologic characteristics of the hand deformity were systemically analyzed.Based on the severity degree of morphologic deformity,the Poland hand was classified into typical type and atypical type.Typical type was sub-classified into type Ⅰ,type Ⅱ,type Ⅲ,type Ⅳ and type Ⅴ,and atypical type was sub-classified into type Ⅰ,type Ⅱ and type Ⅲ.The deformed hands in each type were also summarized with respect to growth and development,syndactyly,X-ray features and impairment of functions.56 cases of them were treated surgically under the guidance of the new classification,and a new strategy for surgical treatment was established. Results A follow-up of 10.5 months on average (range from 6 to 18 months) postoperatively showed that the success rate of interdigital syndactyly welease was 100％.The major postoperative problems included scar contracture and web adhesion.Currently there were still no ideal methods for treatment of associated anomalies(such as short fingers,hypoplasia of forearm and upper arm,aplasia of digits,breast hypoplasia).Conclusion The hand deformities in Poland syndrome are complicated with multiple manifestations.The new classification system covers clinical and morphologic characteristics comprehensively,and offers a basis for options of surgical treatment of the deformed hand.The associated anomalies still need to be treated in the future.

Abstract:Anomalies of the thumb development are not uncommon and may be associated with a number of syndromes also. These anomalies range from total absence to duplication. Reconstructive surgery for the creation of an opposable thumb is the most rewarding aspect of Hand Surgery and also the most challenging. Classification systems have been modified for better description. A number of procedures have been introduced to improve the functionality of the hand in anomalies and age old concepts are undergoing a metamorphosis to further this. A brief description of the common conditions and their treatment are discussed here and highlighted by selected clinical cases.

Phenotype‐to‐Genotype Description of Prenatal Suspected and Postnatal Discovered Upper Limb Anomalies: A Retrospective Cohort Study

Prenatal Diagnosis

Abstract:Objective To evaluate phenotype and genotype characteristics of fetuses and children with upper limb anomalies. Method Retrospective cohort study of a prenatal and postnatal cohort with upper limb anomalies from January 2007 to December 2021 in a Fetal Medicine Unit. Prenatally on ultrasound suspected upper limb anomalies, such as transverse and longitudinal reduction defects, polydactyly, and syndactyly, and postnatally identified children referred to the Congenital Hand Team were evaluated separately. Results The prenatal group included 199 pregnancies: 64 transverse and 19 longitudinal reduction defects, 103 polydactylies, and 13 cases with syndactyly. The majority of cases with longitudinal reduction defects (n = 10, 52.6%), polydactyly (n = 62, 60.2%), and syndactyly (n = 10, 76.9%) were non‐isolated, as opposed to transverse reduction defects, which were generally isolated (n = 41, 64.1%). The postnatal cohort included 362 children with upper limb anomalies with 49 transverse and 22 longitudinal reduction defects, 226 polydactylies, and 65 syndactylies. Chromosomal or monogenic abnormalities were identified in 76/199 (38.2%) cases of the prenatal cohort and in 31/362 (8.6%) cases of the postnatal cohort. Conclusion Prenatal identification of minor defects of the digits is a challenge, with more postnatal than prenatal cases. The majority of cases with isolated anomalies in both groups had no underlying chromosomal or monogenic cause, nor were they associated with a syndrome, as compared to the non‐isolated cases. Conducting structural anomaly scans and genetic counseling are crucial to assess the risk of genetic abnormalities.

Abstract:Congenital ptosis, a birth defects presents at birth or by 1 year of age, is characterized by the drooping of the upper eyelid. Either in isolation (nonsyndromic) or with many different systemic disorders (syndromic). The estimated prevalence of ptosis (congenital and acquired) ranges from 0.79 to 1.99 per 10,000 people in different populations, and it is more prevalent in males. The underlying pathogenesis of congenital ptosis is myogenic and neurogenic, related to the development of muscles and nerves. Although most cases are sporadic, there are familial transmission characteristics, including autosomal dominant, recessive mode, and X-linkage inheritance patterns. Moreover, some forms are due to chromosomal aberrations and mutations and deletions in mitochondrial DNA. Genes involved in simple congenital ptosis (SCP) are ZFHX4 and COL25A1. The clinical aspects of various syndromes involving congenital ptosis are partly caused by single-gene mutations. However, the pathogenesis of congenital ptosis is not fully understood. We review the reported epidemiology, genetics, and clinical features of congenital ptosis and associated syndromes here.
Clinical Classification and Treatment Strategy of Congenital Skin Bridge Syndactyly

Abstract:Objective To review and summarize the clinical characteristics of congenital skin bridge syndactyly,and offer a clinical classification system and surgical treatment strategy.Methods A retrograde review of the congenital constriction band syndrome cases treated between February 2002 and August 2011 was conducted.A total of 16 cases with skin bridge syndactyly were identified.Based on the morphologic characteristics of inter-digital web space in fingers with skin bridge syndactyly,they were classified into type Ⅰ and type Ⅱ as web spaces were intact or not.Type Ⅰ and type Ⅱ were sub-classified into type ⅠA,type ⅠB,type ⅠC and type ⅡA,ⅡB,ⅡC,respectively,based on the relative position between the ring band and skin bridge.Treatment methods of different types of the skin bridge syndactyly were analyzed,and a treatment strategy was suggested.Results The mean follow-up time was 8.6 months (range,3 to 18 months).All of the finger separations and Z-plasty procedures were successful.75％ of the parents were satisfactory with hand function,and 31.3％ with hand appearance postoperatively.Conclusion The new classification system covers clinical and morphologic characteristics,and offers a basis for options of surgical treatment with congenital skin bridge syndactyly.Functional improvement was much better than aesthetic improvement.
Clinical presentations and treatment strategy of associating deformities in Poland syndrome

Abstract:Objective To study and review the morphological characteristics of and treatment strategy for malformations other than hand and pectoralis major deformities in Poland syndrome.Methods Ninety-one cases of Poland syndrome treated from February 2000 to April 2014 were retrospectively reviewed.The clinical morphological characteristics of malformations other than hand and pectoralis major deformities were systemically analyzed,and the treatment methods of various malformations were also reviewed and summarized.Results Eighty-five out of the 91 cases had associating deformities other than the hand and chest.Seventy-one cases had 2 or more than 2 additional deformities.A total of 12 types of deformity existed,the top three being nipple,rib and clavicle deformities.Conclusion Morphological changes of patients with Poland syndrome are complex.Besides deformities in the hands and pectoralis major,there are deformities in other parts of the body,which can seriously affect the appearance and function.Hand surgeons should have a more comprehensive understanding of the morphological manifestations of Poland syndrome and thorough physical examination of the patient to make an individualized treatment plan teamed with relevant departments and based on gender and age of the patient.

Abstract:Polydactyly is one of the most common hereditary limb malformations, involving additional digits on the hands and/or feet, which is a very attractive model to appreciate clinical and genetic heterogeneity. A high level of heterogeneity in polydactyly has been identified in different regions. However, such data of the medical literatures for Asian populations are relatively limited. This study was intended to shed light on the phenotypic manifestations of polydactyly in the recruited Chinese population and to characterize the medical literature on this condition. A total of 459 well-characterized polydactyly cases from Shanghai Children's Medical Center were recruited. Their phenotypes, inheritance patterns, and clinical heterogeneity were obtained from clinical medical records. It was found that 4.8% of cases were familial and 95.2% were sporadic. The proportions of preaxial and postaxial polydactyly types were 74.7% and 25.3%, respectively. In preaxial polydactyly, type I formed the overwhelming majority (95.9%). Among the postaxial polydactyly cases, type A was most prevalent at 69.8% and type B was witnessed in 30.2% of cases. Familial and sporadic polydactyly patients mainly had unilateral presentations. A total of 583 limbs with additional digits were recorded in the 459 subjects. Upper limb involvement was more common than lower, and right hand involvement was more common than left for preaxial polydactyly, and lower limb involvement was more common than upper in postaxial polydactyly. This cohort added useful clinical/epidemiological information to the polydactyly literature in the Chinese population and highlighted its marked clinical heterogeneity.

Abstract:Objective To review the clinical and radiological characteristics of patients with congenital fusion of the 4th and 5th metacarpal bones,and propose a new classification to provide better guidelines for the treatment of the deformity.Methods Based on the experience of 11 cases (16 hands) treated by the authors,the new classification takes into account according to the shape of the deformity and the appropriate treatment methods for each group.Under the new classification scheme,all patients were divided into 3 groups,according to the inter-metacarpal angle between the 4th and 5th metacarpals,and each group was further subdivided into 2 sub-groups according to the length of the fifth finger ray.Results All 16 hands could be classified according to this new classification,including 2 cases of group Ⅰ (both Ⅰ A),9 cases of group Ⅱ (Ⅱ A in 8 cases,ⅡB in 1 case),and 5 cases of group Ⅲ (Ⅲ A in 4 cases,Ⅲ B in 1 case).Patients in different groups received appropriate treatment according to the treatment guidelines proposed by the new classification scheme.It was also possible to classify all the cases illustrated in the literature according to this scheme.Conclusion The new classification could not only classify the deformity according to different morphological characteristics,but also provide general guidelines for deformity correction and a useful scheme for comparison of treatment outcomes among different studies.
Syndromic Involvement of Patients Presenting With Congenital Upper Limb Anomalies: An Analysis of 4,317 Cases

Abstract:Purpose This study investigated the patterns of syndromic involvement for patients with congenital upper limb anomalies (CULAs). We hypothesize that patients with CULAs will present with predictable syndromic patterns. Methods This retrospective study queried the multicenter Congenital Upper Limb Differences (CoULD) Registry. Of the 4,317 patients enrolled, 578 (13%) reported one or more syndromes. Syndromes were confirmed to be recognized by the Online Mendelian Inheritance in Man. Demographics were reviewed and compared with the full CoULD registry group. Syndromes reported by five or more patients were examined to determine the type of CULA according to Oberg/Manske/Tonkin classifications. Results Of the 578 children with one or more reported syndromes, 517 had Online Mendelian Inheritance in Man recognized syndromes (cohort A), In cohort A, 58 syndromes were each represented by a single patient within the registry. Forty-eight syndromes in cohort A were reported by two or more patients, which accounted for 461 of the total patients with reported syndromes. However, VACTERL and Poland syndromes were the most commonly reported syndromes. Patients with CULAs and syndromes frequently exhibited bilateral involvement (61%), compared with the entire CoULD group (47%) and other orthopedic (50%) and medical conditions (61%) compared with the entire CoULD group (24% and 27%, respectively). Additionally, they exhibited a lower frequency of family history of a congenital orthopedic condition (21%) or a family member with the same CULA (9%) compared with the entire CoULD group (26% and 14%, respectively). Conclusions Associated syndromes were recorded in 578 patients (13%) in the CoULD registry as follows: 58 syndromes represented by a single patient, 48 by 2 or more patients, and 23 syndromes by 5 or more patients. Rare syndromes that are only represented by a single patient are more likely to be unknown by a pediatric hand surgeon, and consultation with a geneticist is advised.
