Abstract:Background. The low incidence of thymic carcinoma has precluded the development of randomized clinical trials, and present knowledge is based on small retrospective studies. We performed this single-center retrospective analysis to evaluate the clinical characteristics, treatments, and prognosis in patients with pathologically confirmed thymic carcinoma.& para;& para;Methods. Data regarding clinicopathologic characteristics, treatment protocols, toxicities, and survival were collected from 135 patients who attended our institution between January 1980 and January 2010. Survival was assessed using the Kaplan-Meier method. Univariate and multivariate analyses were performed using the log-rank test and Cox proportional hazards model.& para;& para;Results. The 135 patients (88 men) were with a median age of 48 years, and 123 patients were diagnosed with Masaoka stage III to IV disease. R0 resection was performed in 35 patients. Treatment comprised radiotherapy in 121 patients and chemotherapy in 60. The median follow-up time was 12.5 years. At 5 and 10 years, local-regional relapse free survivals were 81.4% and 54.4%, overall survivals were 42.2% and 15.4%, progression-free survivals were 29.7% and 8.0%, and distant metastasis-free survivals were 35.9% and 25.6%, respectively. R0 resection was the only independent prognosticator of overall survival, progression-free survival, and distant metastasis-free survival in univariate and multivariate analyses.& para;& para;Conclusions. Thymic carcinoma was frequently diagnosed at Masaoka stage III to IV with a poor prognosis. Surgical resection is still the predominant treatment. Radiotherapy may increase local-regional relapse free survival with mild toxicities in advanced-stage patients. (C) 2017 by The Society of Thoracic Surgeons

Abstract:Objective As thymic carcinoma is a rare neoplasm,imformation regarding its result of treatment and possible prognostic factors are limited.We retrospectively analyzed 51 patients with pathologically confirmed thymic carcinoma treated at the Cancer Hospital,Fudan University.Methods Fifty-one patients with pathologically confirmed thymic carcinoma were classified according to the Masaoka satging criteria: stage Ⅱ 5 patients,stage Ⅲ 34,stage ⅣA 5,and stage ⅣB 7.Nineteen patients underwent gross total resection,23 had subtotal resection,and 4 had biopsy alone.Irradiation dose ranged from 2250?cGy to 7000?cGy,with a median of 5586?cGy.Twenty-seven patients also received chemotherapy,including endoxan,adriamycin,cisplatin,5-Fu or vincristine.The data of clinical and follow-up were analysed.The overall survival,and local control rate were calculated by Kaplan-Meier method.Prognosis factors were analyzed by Cox regression model.Results The overall 5-year survival rate was 55%.The 5-year local control rate was 71%.Univariate statistical analysis showed the Masaoka stage showed significance prognostic factors.Conclusions With an overall 5-year survival rate of 55%,this result shows that surgery plus postoperative radiotherapy is a model and reasonable treatment for thymic carcinoma.
Long-Term Survival After Surgical Treatment of Thymic Carcinoma: A Retrospective Analysis from the Chinese Alliance for Research of Thymoma Database

Abstract:Background Thymic carcinoma is a type of rare and highly malignant tumor that originates from the thymic epithelium. Treatment and prognosis of thymic carcinoma remain controversial. We retrospectively analyzed survival data from a large-sample multicenter database in China. Methods The Chinese Alliance for Research of Thymoma constructed a retrospective database of patients with thymic epithelial tumors, which enrolled 1930 patients from January 1996 to August 2013, including 329 with thymic carcinomas. In this study, we analyzed clinical, pathologic, and treatment information, measured long-term survival rates, and identified relevant prognostic factors. Results Of 329 patients, R0 resection was performed in 211 (57.7 %), R1 in 34 (9.2 %), and R2 in 84 (22.5 %). The 3-, 5-, and 10-year survival rates were 78.3, 67.1, and 47.9 %, respectively. In univariate analysis, early Masaoka-Koga stage, R0 resection, and postoperative radiotherapy were associated with better overall survival. Early Masaoka-Koga stage and postoperative radiotherapy were also associated with disease-free survival. In multivariate analyses, R0 resection, Masaoka-Koga stage, and postoperative radiotherapy were significant prognostic factors of survival. Conclusions Complete resection is the preferred primary treatment for thymic carcinoma. R0 resection, early Masaoka-Koga stage, and postoperative radiotherapy are significant predictors of improved survival.

Abstract:Background. Thymic squamous cell carcinomas are rare malignancies with limited information regarding results of surgery therapy. The objective of the current study was to evaluate their clinical and pathologic features and possible prognostic factors. Methods. From February 2003 to December 2010, 105 consecutive surgical patients were enrolled in this study with pathologic confirmed thymic squamous cell carcinoma in Shanghai Chest Hospital. Clinical and pathologic data were retrospectively reviewed. Survival analysis was performed using the Kaplan-Meier and log rank tests. Multivariate analysis was performed using the Cox regression model. Results. The squamous cell subtype made up 73.4% of the thymic carcinomas and 16.1% of total thymic tumors. Four patients (3.8%) presented with myasthenia gravis. The 5-year overall survival rate and the disease-free survival rate were 59.5% and 57.6%, respectively. On multivariate analysis, only the completeness of resection (hazard ratio 3.692; 95% confidence interval: 1.827 to 7.464; p = 0.001) was found to have significant impact on overall survival. Completeness of resection (hazard ratio 2.361; 95% confidence interval: 1.226 to 4.547; p = 0.010) and chemotherapy (hazard ratio 0.307; 95% confidence interval: 0.134 to 0.704; p = 0.005) were associated with disease-free survival. Conclusions. Complete resection not only predicts better overall survival but also extends disease-free interval after resection of thymic squamous cell carcinoma. Our results provide additional evidence supporting an alternative clinical stage specific for thymic carcinoma. (C) 2013 by The Society of Thoracic Surgeons

Abstract:Background: Thymic carcinomas (TCs) and thymic neuroendocrine tumors (TNETs) are aggressive cancers with poor survival outcome and limited investigation. This study is to investigate clinicopathologic features on TC and TNET patients’ prognosis of a large cohort. Materials and Methods: The Surveillance, Epidemiology, and End Results database were used to identify a total of 362 TC and TNET patients with documented clinicopathologic features we investigated. The characteristics and overall survival of the TC and TNET patients were studied. Results: Two hundred and forty TC and 122 TNET patients were identified. For the entire cohort of TC and TNET, histologic type (P < 0.001), tumor size (P = 0.015), Masaoka-Koga stage (P = 0.008), regional node positive (P = 0.004), surgery of primary site (P < 0.001), lymph node surgery (P = 0.013), and chemotherapy (P = 0.001) were considered as significant clinicopathologic features that could affect prognosis of TC and TNET patients in univariate analysis. More importantly, histologic type (P < 0.001), regional nodes positive (P = 0.03) and surgery of primary site (P < 0.001) were able to independently predict overall survival of those patients. In addition, for the cohort of TC, we found that regional nodes positive (P = 0.034) and surgery of primary site (P = 0.001) could be independent predictors of TC patients’ survival. Conclusion: Regional nodes detection is essential for TC and TNET patients. Surgery of primary site is the preferred primary treatment for those patients.
Analysis of treatment and prognosis of 73 thymic carcinoma patients

Abstract:Objective To define the clinical characteristics, treatment method and prognosis of thymic carcinoma. Methods From June 1962 to July 2003, 73 such patients were analyzed retrospectively. All were confirmed histopathology as thymic carcinoma, including 45 males and 28 females. Masaoka's system was used for staging(stage Ⅱ 1.4%, Ⅲ 47.9% and Ⅳ 50.7%). Forty patients(55%) were treated by surgery: total resection in 6 patients(15%), subtotal resection in 21 , and exploration only 13. Seventy patients(96%) were given radiotherapy with median of 60Gy. Chemotherapy was instincted in 30 patients(32%) with median of 4 cycles. Survival rate was calculated and the survival curve plotted by Kaplan-Meier method. The median follow-up time was 14 years. Results The overall 5-, 10-year survival rate was 31%, and 19%, respectively, with median survival time of 35.1 months. The 5-year survival rate was 45%, and 17% in stage Ⅲ, and Ⅳ, respectively(P=0.002). Five patients alive of 6 patients treated by total resection, 52% and 14% in those treated by subtotal resection and explored, unoperated groups, respectively(P=0.003). The 5-year survival rate in patients treated by chemotherapy was 28% and 33% in those not treated by chemotherapy(P=0.671). Incidence of radiation pericarditis and radiation pneumonitis was 7% and 9%, respectively. Thirteen patients(36%) failed in 36 patients with stage Ⅱ,Ⅲ lesions: 4 patients by local relapse, and 7 by distant metastasis, and 2 by both local recurrence and distant metastasis. Conclusions The prognosis of thymic carcinoma is poor. Masaoka staging and type of tumor resection are significant prognosis factors. The complication of radiotherapy was mild. The role of chemotherapy is unclear.
Long-Term Follow-Up And Prognostic Factors For Advanced Thymic Carcinoma

Abstract:The aim of this study was to evaluate the long-term survival outcomes in patients with advanced thymic carcinoma and identify prognostic factors influencing the survival.We retrospectively analyzed 90 consecutive patients with pathologically confirmed advanced thymic carcinoma (Masaoka III and IV) in our institute, from December 2000 to 2012. Age, sex, clinical characteristics, laboratory findings, Masaoka and tumor node metastasis staging, pathologic grade, and treatment modalities were analyzed to identify prognostic factors associated with the progress-free survival (PFS) and the overall survival (OS) rates. Statistical analysis was conducted using SPSS, version 19.0 (SPSS, Inc, Chicago, IL).A total of 73 (81.1%) male and 17 (18.9%) female patients participated in the study. The median follow-up time was 75 months (range, 20-158 months). The 5-year PFS and OS rates were 23.6% (95% confidence interval [CI], 14.6%-33.8%) and 35.7% (95% CI, 25.1%-46.4%), respectively. The multivariate Cox regression model analysis showed that factors improving the PFS were the normal lactate dehydrogenase (LDH) level (P < 0.001), Masaoka III stage (P = 0.028), and radiotherapy (RT) (P < 0.001). The LDH (P < 0.001), T stage (P < 0.001), and the pathologic grade (P = 0.047) were independently prognostic of OS.Long-term follow-up of the advanced thymic carcinoma showed poor outcomes of PFS and OS. LDH, Masaoka stage, and RT affected the PFS, and LDH, T stage, and pathologic grade seemed to affect the OS. Establishing a better staging system for predicting outcomes would be warranted.
Clinicopathological Analysis of 241 Thymic Epithelial Tumors-Experience in the Shanghai Chest Hospital from 1997-2004.

Abstract:BACKGROUND:To assess the correlation of WHO histological classification of thymomas and thymic carcinomas (TCs) with prognosis in recently treated patient cohort compared to a historical one from a single institution.METHODS:Retrospective review of clinical charts and histological sections of 241 patients treated during 1997-2004. Univariate and multivariate analysis of associations between risk factors including gender, age, tumor size, myasthenia gravis, WHO histological subtype, Masaoka stage, resection status, (neo-)adjuvant therapies, and survival.RESULTS:The 5-year overall survival (OS) of A, AB, B1, B2, B3 thymomas and TCs patients was 100%, 100%, 94%, 80%, 94% and 45%. Five-year progression-free survival (PFS) was 100%, 96%, 78%, 80%, 78% and 39%, respectively. The 5-year OS of patients with Masaoka stage I, II, III and IV thymomas and TCs was 96%, 89%, 59% and 50%. (Neo-)adjuvant therapies were administered more often than in the historical cohort. Tumor-related death mainly occurred in patients with stage III, IV and B2, B3 thymomas and TCs. By univariate analysis, gender, tumor size, myasthenia gravis (MG) status, histotype, Masaoka stage, resection status and treatment were associated with OS. By multivariate analysis, histological subtype, Masaoka stage, and (neo-)adjuvant therapy were revealed as independent prognostic indicators.CONCLUSIONS:WHO histological subtype, Masaoka stage and (neo-)adjuvant treatment have remained independent determinants of OS in patients with thymomas and TCs. Compared with the historical cohort during 1969-1996, prognosis of patients with B2, B3 thymomas has improved, which may be partly due to the increased use of adjuvant therapies. Prognosis of patients with TCs remained unsatisfactory, suggesting that neoadjuvant treatment should be tested to improve survival.

Abstract:OBJECTIVES:Thymic carcinoma (TC) and thymic carcinoid (TCD) are aggressive thymic epithelial neoplasms with a poor prognosis. Due to rarity, little is known about their comparative clinical characteristics, treatment outcomes and patterns of relapse.METHODS:A retrospective cohort study was performed on 287 patients with TC and 56 patients with TCD who were treated at the Shanghai Chest Hospital between February 2003 and April 2014. Patient demographics, tumour stage, treatment, pathologic findings and postoperative outcomes were compared between the two tumour types using both multivariable Cox regression analysis and propensity-matched analysis.RESULTS:Compared to patients with TC, significantly more patients with TCD were male, had larger tumours, and displayed a greater proportion of lymph node metastases. However, overall survival was similar (60.7% 5-year survival for TC, 80.7% for TCD, P = 0.159), as was disease-free survival (41.1% 5-year survival for TC, 37.6% for TCD, P = 0.696) and patterns of relapse. Multiple Cox regression analysis identified younger patients [hazard ratio (HR) 1.018; 95% confidence interval (CI) 1.000-1.035; P = 0.047], more completeness of resection (HR 1.424; 95% CI 1.105-1.836; P = 0.006), adjuvant radiotherapy (HR 0.455; 95% CI 0.276-0.751; P = 0.002), and no adjuvant chemotherapy (HR 1.799; 95% CI 1.017-3.183; P = 0.044) as independent factors predicting better overall survival. Completeness of resection (HR 1.258; 95% CI 1.022-1.548; P = 0.031) and TNM stage (HR 1.479; 95% CI 1.107-1.977; P = 0.008) were independent predictors of disease-free survival. Propensity matching produced 46 patients in each group and no significant difference on overall survival or disease-free survival was found.CONCLUSIONS:Patients with TCD have discrete features but share a similar clinical course to those with TC. The importance of complete resection in both of these thymic malignancies is emphasized. Further investigation at multiple centers with the longer follow-up data is required to substantiate our conclusion.

Abstract:Thymomas are rare, and information regarding their surgical outcomes and possible prognostic factors is limited. In this study, we aimed to determine the clinicopathological characteristics of thymoma and estimate independent predictors of both overall and disease-free survival in thymoma patients. We carried out a retrospective review of the clinicopathological characteristics and prognostic factors in 761 consecutive patients with pathologically confirmed thymoma treated in Shanghai Chest Hospital between January 2001 and December 2011. Survival was calculated using the Kaplan-Meier method and evaluated with log-rank tests. Multivariable analysis was performed using the Cox regression model. Complete follow-up information was available for 544 patients. The overall survival rate was 92.8% at 5 years and 90.5% at 10 years. The 5- and 10-year disease-free survival was 87.9 and 82.1%, respectively. On multiple Cox regression analysis, the Masaoka-Koga clinical stage [odds ratio (OR), 2.057; 95% confidence interval (CI), 1.454-2.911; P < 0.01] and sex (OR, 2.244; 95% CI, 1.115-4.519; P = 0.02) were found to be independent predictors of overall survival. The Masaoka-Koga clinical stage (OR, 2.127; 95% CI, 1.487-3.042; P < 0.01) and completeness of resection (OR, 2.935; 95% CI, 1.410-6.109; P < 0.01) predicted disease-free survival. The four-tiered Masaoka-Koga clinical stage is the most important prognostic factor, predicting not only overall survival but also disease-free survival after thymoma resection. Completeness of resection predicts disease-free survival, and the World Health Organization histological classification may not have significant prognostic implications.
Clinical Outcomes and Prognosis of Recurrent Thymoma Management

Abstract:INTRODUCTION: Clinical outcomes and prognosis of recurrent thymoma are not well known because of its rarity and indolent clinical course. This study was designed to determine the clinical outcomes and prognosis of recurrent thymoma.METHODS: Between 1986 and 2009, 41 of 305 patients who underwent resection for thymoma had recurrence. We reviewed those patients retrospectively.RESULTS: Of 15 patients who underwent re-resection for recurrent thymoma, 13 patients (87%) achieved complete re-resection. Of 26 patients who did not undergo re-resection, 11 patients received chemotherapy, five received chemoradiotherapy, one received another treatment, and nine patients did not receive any treatment. World Health Organization histological types AB or B1 and complete re-resection were positive prognostic factors. The complete re-resection group had better survival after recurrence than the non/incomplete re-resection group (5-year survival rate: 90.9% versus 44.7%, p = 0.014). The complete re-resection group had comparable survival after initial resection of thymoma to the patients without recurrence (5-year, 10-year survival: 91.7%, 91.7% versus 90.7%, 86.5%, p = 0.618) and better survival compared with the non/incomplete re-resection group (5-year, 10-year survival: 91.7%, 91.7% versus 81.6%, 56.7%, p = 0.018).CONCLUSIONS: World Health Organization histologic types AB or B1 and complete re-resection are favorable prognostic factors of recurrent thymoma. In particular, complete re-resection of recurrent thymoma contributes to better survival than other management. Therefore, we suggest that long-term surveillance extended at least 20 years may be essential for early detection of recurrence to increase the chance of complete re-resection of recurrent thymoma.

Abstract:Introduction: Thymic epithelial tumors represent the most common cause of anterior mediastinal masses in adults. Among these tumors, thymomas constitute the majority, whereas thymic carcinomas are less prevalent and associated with a poorer prognosis. This study seeks to investigate the clinical and prognostic factors of these rare entities, thymoma, and thymic carcinomas. Materials and methods: We conducted a retrospective analysis on a cohort of 60 patients diagnosed with thymoma and thymic carcinoma, who were under follow-up at our clinic between January 1998 and February 2023. The clinical characteristics and prognostic factors of these patients were analyzed separately. Results: Thymomas accounted for 75% of the cases. The median age at diagnosis was 46 years in both patient groups. Masaoka stage 4 was observed in 28.9% of thymoma patients and 66.7% of thymic carcinoma patients. The median overall survival (mOS) for thymoma patients was 261.4 months, while it was 9.23 months for patients with thymic carcinoma. Curative surgery emerged as a prognostic factor significantly influencing overall survival in both thymoma and thymic carcinoma patients. Conclusion: Our study highlights the significance of Eastern Cooperative Oncology Group (ECOG) performance status and curative surgery as prognostic factors affecting overall survival in thymoma patients. In thymic carcinoma, only curative surgery was found as a prognostic factor. These findings may enhance patient care and guide personalized treatment strategies. Further investigations and prospective studies are warranted to corroborate and expand upon these results.

Methods 283 patients with pathologically confirmed thymoma were enrolled into this study. Overall survival rate and local control rate were calculated. Cox regression model was used to analyze the prognostic factors. Results The 5-year overall survival rate was 94.3%, 86.3%, 71.6%, 39.4% and 80.4% for patients with stage Ⅰ,Ⅱ,Ⅲ, Ⅳ disease and all patients, respectively. The 10-year overall survival rate was 84.3%, 75.4%, 56.6% , 29.6% and 68.4% for stage Ⅰ,Ⅱ,Ⅲ, Ⅳ disease and all patients. The Masaoka staging, resection extent, and gender were independent prognositic factors in multivariate analysis. Conclusion The Masaoka staging and resection extent are independent prognositic factors in patients with thymoma treated with multimodality therapy.
Thymic Squamous Cell Carcinoma: A Population-Based Surveillance, Epidemiology, and End Result Analysis

Abstract:Objectives Thymic squamous cell carcinoma (TSCC) is a rare neoplasm that has been sparsely cited in the literature. The aim of this study was to determine disease characteristics and prognostic factors of patients in a Surveillance, Epidemiology, and End Results (SEER) analysis. Methods Cases from 1990-2016 were retrieved from the SEER database and demographics, treatments, and survival outcomes were analyzed. Results The TSCC accounted for 72.4% of the thymic carcinomas and 7.2% of thymic tumors. The 276 patients (165 men) selected for analysis had a median age of 65 (24-85) years, and 201 patients were diagnosed with Masaoka-Koga stage III/IV. The median survival of TSCC was 59 months with a 49.0% 5-year OS rate, a better prognosis than lymphoepithelioma-like carcinoma (32.1%) and undifferentiated carcinoma (33.3%). Multivariate analysis revealed the Masaoka-Koga stage (p = 0.003) and surgical types (complete resection, incomplete resection, and none; p < 0.001) were determinants of survival. Complete resection had the best prognosis with a 72.7% 5-year OS rate. Chemotherapy was an independent protective factor (HR = 0.555, 95% CI 0.347-0.886; p = 0.014) though poor survival was showed in univariate analysis. And the survival benefit of chemotherapy was validated in PSM analysis (3-year OS rate was 77.7% with chemotherapy vs. 52.8% without chemotherapy; p = 0.014). Conclusions TSCC was frequently diagnosed in older patients with advanced Masaoka-Koga stage and had more favorable survival than other subtypes of thymic carcinomas. Complete resection is the preferred treatment. Masaoka-Koga stage and chemotherapy had a strong association with prognosis.
Thymic Epithelial Tumors: A Clinicopathologic Study of 249 Cases from A Single Institution

Abstract:The WHO histological classification for thymic epithelial tumors of 2004 edition is widely used, but its prognostic value is still controversial. In the present study we collected 249 Chinese patients with thymic epithelial tumors from West China Hospital of Sichuan University since 1999-2009 to assess the prognosis relating to tumor stages, histological classifications, MG and adjuvant therapy. There were 18 cases of type A (7.2%), 97 of type AB (39.0%), 22 of type B1 (8.8%), 63 of type B2 (25.3%), 16 of type B3 (6.5%) thymomas and 33 of thymic carcinomas (13.3%). According to the Massaoka staging, there were 107 patients in stage I (43%), 73 patients in stage II (29.3%), 50 patients in stage III (20.1%) and 19 in stage IV (7.6%). 101 patients (40.6%) complicated with MG, the incidence of MG was highest in type B3 thymomas, then in B2, none of thymic carcinomas complicated with MG. Cox regression analysis showed the Masaoka stage was the most important prognostic factor. Besides of staging, WHO histological classification was also an independent prognostic factor. The age, gender, MG and adjuvant therapy have no significant influence to the prognosis of the patients.

Abstract:Objective To explore the results of surgical treatment for thymoma,and to investigate the influence of pathological predictors on long-term survival.Methods The clinical and pathologic features of 104 cases from 2000 to 2009 underwent surgical treatment for thymoma were reviewed.The prognostic factors against postoperative survival were analyzed.Results The rate of complete resection was 92.3%.The morbidity was 3.8% and no death happened perioperatively.Thymomas were staged according to the Masaoka system as 26.9% stage Ⅰ,40.4% stage Ⅱ,28.8% stage Ⅲ,3.8% stage Ⅳa and classified according to the World Health Organization system as 3.8% type A,8.6% typeAB,22.1% type B1,43.3% type B2,17.3% type B3,5.8% type C.Mean followup was 62.3±18.5 months.The overall 1-,3- and 5-yearsurvival rates were 98.1%,88.5 and 82.3%,respectively.The Masaoka staging and the World Health Organization classification significantly influenced the long-term survival.Stage Ⅲ,Ⅳa,type B3 and type C were related with worse prognosis.Conclusions Surgical method is safe and effective in the treatment of thymoma.The clinical staging and the histologic classification influence long-term survival.

Abstract:7604 Background: In 1999 the World Health Organization (WHO) published a histologic sub-typing system for thymoma that divided it in to six classes, which included one category for thymic carcinoma (WHO C, Group 1) and five categories of invasive thymoma (WHO A, AB, B1, B2 and B3, Group 2). Methods: The SEER database was used to retrospectively identify thymic neoplasms from 2000-2010 using the six-tier classification system. Only those patients having thymic neoplasia as their only cancer and undergoing complete (R0) resection were included in the analysis. Overall survival (OS) and thymic-specific survival (TSS) were evaluated by Kaplan-Meier methods. Multivariate analysis(MVA) was performed including group, age, race, Masaoka stage, gender, and adjuvant radiation therapy as key variables. Results: 505 thymic neoplasms were identified including 96 (19%) in group 1 and 409 (81%) in group 2. Median follow-up was 38 months. For patients with early Stage(I/II) group 1 and group 2 tumors, OS at 2, 3, and 5 years was 90.5, 90.5, 67.9% for Group 1 and 98.2, 95.8, 95.8 for Group 2 respectively. Group 1 classication (p<0.0001, HR 1.82), advancing age (p=0.003, HR 1.03) and increasing stage (p=0.04, HR 1.37) were significant predictors of worse OS. TSS was excellent in the overall group with thymus-related death listed as cause of death in only 35 (6.9%) of patients. Any Group 1 and Stage III/IV Group 2 tumors had significantly worse TSS. Histologic sub-typing within Group 2 was not associated with OS, TSS or high-risk of lymph node involvement (all<5%). In early-stage Group 1 patients, 15/34 (44%) did not have a nodal exam and 6/19 (32%) had positive nodes. Conclusions: Early-stage Group 2 patients have an excellent OS and TSS may not benefit from adjuvant therapy. Trials of adjuvant therapy should be considered for advanced-stage Group 2 patients and all Group 1 patients. Thymic carcinomas may be understaged due to lack of lymph node examination and the high percentage with involved nodes.
A Recurrence Predictive Model for Thymic Tumors and Its Implication for Postoperative Management: a Chinese Alliance for Research in Thymomas Database Study

Abstract:OBJECTIVE:Our aim was to investigate appropriate postoperative management based on the risk of disease recurrence in thymic epithelial tumors after complete resection.METHODS:The Chinese Alliance for Research in Thymomas retrospective database was reviewed. Patients having stage I to IIIa tumors without pretreatment and with complete resection were included. Clinicopathologic variables with statistical significance in the multivariate Cox regression were incorporated into a nomogram for building a recurrence predictive model.RESULTS:A total of 907 cases, including 802 thymomas, 88 thymic carcinomas, and 17 neuroendocrine tumors, were retrieved between 1994 and 2012. With a median follow-up of 52 months, the 10-year overall survival rate was 89.5%. Distant and/or locoregional recurrences were noted in 53 patients (5.8%). The nomogram model revealed histologic type and T stage as independent predictive factors for recurrence, with a bootstrap-corrected C-index of 0.86. On the basis of this model, patients with T1 thymomas or T2 or T3 type A, AB, or B1 thymomas had a significantly lower incidence of recurrence (low-risk group) than those with T2 or T3 type B2 or B3 thymomas and all thymic carcinomas and neuroendocrine tumors (high-risk group) (2.7% versus 20.1% [p < 0.001]). In the high-risk group, more than half of the recurrences (55.2% [16 of 29]) were seen within the first 3 postoperative years, whereas all recurrences but one were recorded within 6 years after surgery. Recurrence occurred quite evenly over 10 postoperative years in the low-risk group.CONCLUSIONS:A 6-year active surveillance should be considered in high-risk patients regardless of adjuvant therapy. For low-risk patients, annual follow-up may be sufficient. Studies examining postoperative adjuvant therapies would be plausible in high-risk patients.
Real-world study of treatment and outcome of type B2 + B3 thymoma: The neglected part of thymoma

Abstract:Background: This study aimed to examine the treatment and prognosis of patients with type B2 + B3 thymoma and compare it with those patients with type B2 and B3 thymoma. Methods: We conducted a retrospective analysis of the results of 39 patients with type B2 + B3 thymoma, 133 patients with type B2 thymoma, and 64 patients with type B3 thymoma. The Kaplan-Meier technique was used to generate survival curves. For multivariate analysis, the Cox proportional hazard model was applied. Results: With a median follow-up of 60 months (range: 1-128 months), the percentage of patients with tumor, node, metastasis (TNM) stage III and IV disease gradually increased from 19.5% to 25.6% to 35.9% among those with histological subtypes B2, B2 + B3, and B3, respectively, p = 0.045. Twenty-three patients experienced recurrence or metastasis. The total 10-year progression-free survival (PFS) rates were 86.0% overall (85.0% in type B2, 87.2% in type B2 + B3, and 87.5% in type B3). Age, R0 resection, and Masaoka-Koga stage were found to have a significant on PFS in all patients. There was no statistically significant difference in PFS between different histotypes of thymoma, p = 0.650. PFS was predicted by R0 resection in all histotypes and by the Masaoka-Koga stage in the type B2 subgroup. Conclusion: Combining the two staging methods to guide the diagnosis and treatment of patients with B2 + B3 thymoma is recommended. R0 resection is recommended to reduce recurrence. Patients with B2 + B3 thymoma have a prognosis similar to those with a B2 thymoma or a B3 thymoma alone.
