Echocardiography in the Diagnosis and Follow-Up of Patients with Pulmonary Arterial Hypertension Associated with Congenital Heart Disease

Abstract:Transthoracic echocardiography is the first-line imaging modality in pulmonary arterial hypertension associated with congenital heart disease (PAH–CHD). Echocardiography provides detailed structural and haemodynamic assessment allowing the detection of pulmonary hypertension, as well as previously undiagnosed congenital heart defects. Beyond the assessment of pulmonary pressures, the echocardiographic examination is able to provide invaluable information on right ventricular adaptation to the high afterload, changes in right ventricular geometry, ventriculo–ventricular interaction and valve function. Echocardiography also provides us with prognostic information in pulmonary arterial hypertension associated with congenital heart disease. Serial echocardiographic assessment allows monitoring of the efficacy of advanced therapies and follow-up of our patients. Recently developed advanced echocardiographic techniques, such as 3D and speckle-tracking imaging, are promising for PAH–CHD patients.
