P1‐131: Identification of MAPT, GRN and C9ORF72 Mutations in Chinese Patients with Frontotemporal Dementia

Abstract:Frontotemporal dementia (FTD) is the second most common form of presenile dementia after Alzheimer disease (AD). It is a genetic heterogeneous neurodegenerative disorder. To date, there are at least eight causative genes identified in patients with FTD. Among them, variants of MAPT, GRN and C9orf72 genes are associated with the major cause of FTD. However, no comprehensive analyses of variants in these three common genes are conducted in Chinese population. A total of 70 patients with FTD were recruited in this study. According to clinical diagnosis criteria, 43 patients (68.6%) had behavioral variant FTD (bvFTD) as their initial clinical diagnosis, with the next common phenotype being progressive nonfluent aphasia (PNFA) (n=8, 11.4%) and semantic dementia (SD) (n=6, 8.6%). In addition, 3 patients (4.3%) presented clinical and electrophysiological evidence of MND before or after FTD occurred, which were defined as FTD-MND; 10 patients (14.3%) were accompanied with parkinsonism syndrome, which were enrolled in FTD- parkinsonism group. We then screened of all exons in MAPT, GRN and GGGGCC repeats in C9orf72 in this cohort of 70 patients with FTD from China. Two novel mutations in MAPT (c.530A>T, p.D177V; c.1537C>G; p.P313A) were respectively identified in a sporadic PNFA case and a PNFA family, and one previously reported mutation in MAPT (c.837T>G, p.N279K) was found in an FTD-parkinsonism family. With regard to GRN, a known nonsense mutation (c.898C>T p.Q300Term) was identified in a sporadic bvFTD. In addition, we found a bvFTD case and a FTD-MND family carrying the GGGGCC pathogenic repeat in C9orf72. The prevalence of MAPT, GRN and C9orf72 mutations varied in our cohort. Among familial FTD cases, variants in MAPT, GRN and C9orf72 were respectively responsible for 16.7%, 0 and 8.3%. In contrast, each of these three genes could account for 1.7% of sporadic FTD cases. This is the first cohort report of common causative gene screen in patients with FTD in Chinese population. Our findings indicate that MAPT, GRN and C9orf72 are the common variant genes in patients with FTD in Mainland China.
