Learn more.Copy URL Share a linkShare onFacebookTwitterLinkedInRedditWechat Dear Editor, Recently, we successfully developed a radiolabeled probe targeting integrin α6 for the positron emission tomography (PET)/magnetic resonance (MR) imaging of pancreatic ductal adenocarcinoma (PDAC) and its precancerous lesion, pancreatic intraepithelial neoplasia (PanIN). Most pancreatic cancers are PDAC.1 Standard approaches for detecting PDAC at early stage are lacking with most diagnosed patients at advanced stage.2 Tumor-targeted molecular imaging significantly improved the tumor-detection rate,3 which may also enhance the early diagnosis of PDAC. Considering the genetic complexity of PDAC, a PDAC-specific target with highly adaptability, sensitivity, and specificity is required. We selected integrin α6 as the target of PDAC as it was overexpressed in pancreatic cancer cells4, 5 and cancerous tissue of patients with PDAC4, 6, 7 with an overexpression rate up to 92%.4 Increased integrin α6 levels were associated with stronger PDAC cell invasiveness5 and poorer prognosis for patients.6, 7 Furthermore, the integrin α6β4 dimer was overexpressed in PanIN even at a stage as early as PanIN 1A.4 Here, we confirmed that integrin α6 was overexpressed in human PDAC (Figure 1A,B) and increased integrin α6 had poorer prognosis for PDAC patients (Figure 1C). Recently, a peptide CRWYDENAC (dubbed RWY) targeting integrin α6 has been identified8 and employed for nanotherapeutics of nasopharyngeal carcinoma8 and for PET imaging of hepatocellular carcinoma by our team.9 Here, we used the Ala scanning mutagenesis to modify the RWY peptide. R, W, and Y were the three key amino acids within the RWY peptide, since their replacement by an alanine reduced the cellular binding ability of this peptide, both in vitro and in vivo. Interestingly, the alanine substitution of E resulted in the CRWYDANAC (dubbed S5) peptide with an approximately 1.5-fold enhanced tumor binding ability (Figure 1D,E). The higher binding affinity of S5 peptide to tumor cells was further confirmed in vitro by flow cytometry (Figure 1F,G) and in vivo in PDAC mouse model using optical imaging (Figure 1H,I). The S5 peptide had the highest blocking effect on Cy5-RWY. F and G, Sw1990 cells were incubated with Cy5-RWY and Cy5-S5 at 37°C for 30 min, and the integrin α6-binding affinities of Cy5-RWY and Cy5-S5 were analyzed by flow cytometric analysis (F); the fluorescence intensity of the S5 group presented as relative (1.41 ± 0.03) fold over RWY, indicating a higher tumor cell-binding ability of S5. H and I To further investigate the in vivo tumor-binding ability of the S5 peptide, Cy5-S5, Cy5-RWY, and Cy5-CG7C (negative control) were synthesized and used in NIRF. In the in vivo NIRF imaging of mice bearing subcutaneous Sw1990-luciferase xenografts, Cy5-S5 exhibited higher fluorescence intensity in tumor location (black arrow: the xenograft) (H); the fluorescence intensity of Cy5-S5 presented as relative fold over RWY (2.64 ± 0.09) was significantly higher than that of Cy5-RWY (1.00 ± 0.10, P = .000) (mean ± SD, ANOVA in (D) and (E) and Student's t-test in (G) and (I) were used for statistical analysis. *P < .05, **P < .01, ***P < .001. NS, not significant) The recently introduced PET/MR imaging technique displays certain advantages in tumor imaging and has potential applications in the imaging of early pancreatic cancer.10 Thus, we synthesized the radiotracer 18F-NOTA-PEG4-S5 (18F-S5 for short) for the PET/MR imaging (Figure S1A). 18F-S5 was stable in serum (Figure S1B), and it showed ideal biodistribution characteristics in vivo that the uptake of 18F-S5 in tumor tissues was the highest among all collected tissues and organs except for the kidney (Figure S1C,D, Table S1). The convenience of 18F-S5-PET/MR imaging on PDAC and PanIN was proved in four mice models. Approximately 3.7 × 106 Bq (100 μCi) of 18F-S5 was intravenously injected to a mouse for PET imaging, and 2.5 mmol/kg of gadolinium-diethylenetriamine pentaacetic acid was injected for enhanced MR imaging. After 120 min postinjection of 18F-S5, PET/MR imaging was obtained by using a hybrid 3.0T PET/MR scanner (uPMR 790; UIH, Shanghai, China). The PDAC-targeting effect of 18F-S5 was preliminary examined in a subcutaneous Sw1990-luciferase tumor-bearing mouse model, where the tumor-to-muscle ratio reached 3.62 ± 0.25 in unblocked mice and significantly decreased to 1.62 ± 0.05 when the binding was blocked by nonradiolabeled S5 (Figure S2A-D). Similar results were observed in the orthotopic Sw1990-luciferase mouse model with a tumor-to-muscle ratio of 3.48 ± 0.62 (Figure S2E-G). In the genetically engineered KPC mice (LSL-KrasG12D/+; Pdx1-Cre; Trp53fl/fl) with spontaneously developed PDAC, a routine MR scan showed no abnormal signal in pancreas on T1 weighted images, and the tumor only presented as local moderate enhancement on the enhanced MR scans (Figure S3A). However, in the PET/MR imaging, 18F-S5 produced high PET signals in the possible location of the tumor (Figure 2A). The uptake of 18F-S5 in tumor was quantified as 4.34 ± 1.16 %ID/g (a percentage of the injected dose per gram) (n = 5) and the tumor-to-muscle ratio was 4.32 ± 0.65 (Figure 2B). The pancreas of the KPC mouse displayed morphological changes compared with the pancreas of a healthy mouse (Figure 2C). The overexpression of integrin α6 in the spontaneously developed PDAC tissue was confirmed using immunohistochemistry (Figure 2D). In addition, 18F-S5-PET/MR imaging could also detect metastatic tumors in KPC, where 18F-S5 produced high PET signals in both of the PDAC lesions and the metastatic tumors in the abdominal cavity (Figure S4A-D). Furthermore, 18F-S5-PET/MR imaging could detect precancerous lesions of PDAC in the KC mice (LSL-KrasG12D/+; Pdx1-Cre) with spontaneous developed PanIN (Figure 2E), which was undetectable when enhanced MR imaging was used alone (Figure S3B). 18F-S5 showed a high accumulation in the possible lesion site, as the quantified uptake was 2.99 ± 0.52 %ID/g and the PanIN-to-muscle ratio was 3.35 ± 0.43 (Figure 2F). The pancreas of the KC mouse displayed morphological changes as its outer surface was rougher (Figure 2G ) than that of the normal pancreas, as shown in Figure 2C. Immunohistochemical analysis confirmed the formation of PanIN lesions in the pancreas and increased integrin α6 expression in lesion tissues (Figure 2H). The etiology of NPC has been linked to genetic susceptibility, Epstein-Barr virus (EBV) infection, and environmental factors. Accumulated evidence including multiple genome-wide association studies (GWASs) has revealed robust genetic predisposition of NPC. However, GWAS-identified genetic variants collectively account for only 8.2% of NPC heritability [1]. The underlying inherited predisposition is largely undetermined. The strongest genetic signal for NPC consistently hits the human leukocyte antigen (HLA) region on 6p21 [2]. However, the highly polymorphic nature and complicated long-range linkage disequilibrium (LD) in the HLA region particularly obscure the causal variants driving the association. In addition, most genetic variants located in introns or intergenic regions. The causal genes mediating genetic effects on NPC risk have rarely been ascertained by GWAS alone. Recently, transcriptome-wide association study (TWAS) has been proposed as an attractive approach to identify novel gene-trait associations and prioritize causal genes for complex traits [3]. By integrating GWAS and gene expression data, TWAS can effectively and economically assess associations between genetically predicted gene expression levels and disease risks in large populations. Hence, using the cis-regulated expression in addition to genetic variants to explore NPC susceptibility genes could be promising and reasonable for mechanistic and functional inference. Nevertheless, neither public data of nasopharyngeal tissue were available, nor TWAS for NPC had been conducted. Herein, we integrated genome and transcriptome data of 89 nasopharyngeal tumor tissues and investigated the associations between predicted gene expression levels and NPC risk using multicenter GWAS data involving 4506 NPC cases and 5384 cancer-free subjects (defined as controls) from South China. Given the close relationship between EBV infection and NPC, a cis-regulated expression weight matrix from EBV-transformed lymphocytes (n = 117) in the GTEx project was used for further evaluation. Study populations and detailed methodology are described in the Supplementary file of methods. We predicted the expression levels of 2505 and 2411 genes in the GWAS population by constructing the models for the prediction of gene expression in nasopharyngeal tissues (NP models) and EBV-transformed lymphocytes (lymphocyte models), respectively (Supplementary Table S1), and 377 genes overlapped (Supplementary Figure S1). Thirty-three genes were associated with NPC at a Bonferroni-corrected threshold, and all were located in the HLA region (Figure 1A). Among them, 11 of 13 previously reported genes were replicated. Our results were consistent with the studies focusing on the HLA region in South China [4, 5], where most of the reported genes available in TWAS were replicated. The predicted expression levels of ZFP57 (NP models), MICA (both models), and HLA-C (lymphocyte models) were significantly higher in cases than in controls, while the expression levels of MOG, HCG27, HLA-DQB1, HLA-H, HLA-U (NP models), HLA-F (both models), HLA-A, and HLA-DRB1 (lymphocyte models) were lower in cases than in controls. The two overlapping genes showed similar associations with NPC (HLA-F: Z score = -10.28 and -8.95; MICA: Z score = 7.82 and 6.60, for NP and lymphocyte models, respectively) (Supplementary Table S2). Interestingly, half of the previously reported genes belonged to HLA class I. Most of them showed lower levels of predicted expression in cases than in controls, possibly because EBV transcripts in NPC tumors were involved in the inhibition of HLA class I gene expression [6]. It is rational to assume that the low expression levels of these genes may affect the anti-EBV immune response in presenting peptides to cytotoxic T cells, facilitating immune evasion of tumor cells or EBV-mediated oncogenic action. Among them, the predicted expression levels of 9 genes were significantly higher in NPC cases than in controls, including HLA-DOB, HCG4B, RPL23AP1, HLA-J in NP models and HCG4, CCHCR1, STK19, C4B, IFITM4P in lymphocyte models, while 13 other genes showed significantly lower expression levels in cases than in controls, including HCP5, ZSCAN23, HCG4P11, HCG4P7, MICD, MICB-DT, SNHG32 in NP models and NOTCH4, C4A, HCG22, POU5F1, MICE, HLA-S in lymphocyte models (Figure 1A). We performed conditional analyses to determine whether the associations between predicted gene expression levels and NPC were influenced by the GWAS signals. After conditioning on the respective GWAS index SNP, the associations for HLA-DOB, NOTCH4, ZSCAN23, STK19, C4B, HLA-J, HLA-S, and MICB-DT remained significant. After conditioning on all previously reported SNPs, NOTCH4, HCG4, HCG22, POU5F1, HCG4B, HCG4P11, MICB-DT, STK19 and IFITM4P remained significant. It indicated that their associations were partially independent of the GWAS signals (Supplementary Table S3). Due to the complicated structure with high LD and co-expression networks in the HLA region, we conducted fine-mapping analyses to prioritize the causal genes. Using posterior inclusion probability (PIP) analysis, we prioritized 7 causal genes: MICA, HLA-DQB1, HLA-DOB, ZSCAN23, HCG27, MICD, and HLA-U. HLA-DOB, ZSCAN23, and MICD were newly identified as NPC susceptibility genes (Supplementary Table S4). Furthermore, we conducted expression quantitative trait locus (eQTL) analyses to identify whether the genetic variants could influence the expression levels of these genes. We found that individuals with relevant risk SNPs (the GWAS index SNPs) exhibited higher expression of HLA-DQB1, MICA, MICD and HLA-U, or lower expression levels of ZSCAN23, HCG27, and HLA-DOB. These results indicated that the risk alleles affected the expression levels of the causal genes (Figure 1B). Two HLA class II genes (HLA-DQB1 and HLA-DOB) were prioritized as causal genes. Both genes were associated with other virus-associated cancers, such as cervical cancer [7]. A comprehensive TWAS exploring genetic susceptibility for antiviral immune response using 7924 subjects from the UK Biobank cohort revealed that the genetic determinants for EBV infection were predominantly located on HLA class II genes. The most significant signals associated with the antibody level of BamHI Z EBV replication activator (ZEBRA) hit HLA-DQB1 [8]. HLA-DOB may impact viral clearance capacity and persistent infection of hepatitis B virus (HBV) and hepatitis C virus (HCV) [9]. Since EBV reactivation with elevated EBV DNA load or antibodies was observed at the preclinical phase of NPC, we hypothesized that HLA class II genes, especially HLA-DQB1and HLA-DOB, participate in the early stage of NPC tumorigenesis by influencing EBV infection. Besides, some identified pseudogenes, such as IFITM4P [10], may function by regulating their parental genes. However, their biological mechanisms remain unclear, and further researches are needed. Gene Ontology (GO) enrichment analysis confirmed that TWAS-identified genes (354 genes with P < 0.05) were enriched in the pathways of cell-mediated immune response, antigen processing and presentation (Figure 1C). Similarly, the top pathways annotated with the Kyoto Encyclopedia of Genes and Genomes (KEGG) database focused on infection of herpes simplex virus type 1, human T-cell leukemia virus type 1, EBV, and autoimmune disorders such as graft-versus-host disease (Figure 1D). In summary, using a TWAS approach, we corroborated the central role of HLA genes in NPC susceptibility. Apart from HLA class I genes, we propose critical roles of HLA class II genes and other nonclassical HLA genes. Seven genes, including HLA-DQB1 and HLA-DOB, were prioritized as causal genes. Recent evidence indicated that these genes are pivotal in the metastable equilibrium between host and virus. Our findings provide additional evidence for a better understanding of the genetic etiology of NPC and clues to further advance this field.
